Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel's engine overheated while sailing south of Governor's Island. Submit repair plan and conduct sea trials prior to carriage of passengers.

The design, materials, and construction of masts, posts, yards, booms, bowsprits, and standing rigging on a sailing vessel must be suitable for the intended service. Vessel suffered damage to the dolphin striker and bowsprit due to an allision with the pier in the Atlantic Basin. Submit repair plan for affected areas to the satisfaction of the OCMI. 46 CFR 177.330

Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant; and(2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant.Vessel missing the three orange smoke distress signals onboard. Vessel restricted to daylight operations until flares or approved substitutes per 180.68(c) are acquired to the satisfaction of the OCMI. 46 CFR 180.68(b)

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch. Fire pump found to be operating at pressure less than minimum. Rectify within 30 days. 46 CFR 181.10-1(c) OLD T

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Life line incorrectly attached to ring buoy grab ling. Correct within 14 days. 46 CFR 180.70(c)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible;(2) U.S. Coast Pilot or similar publication;(3) Coast Guard Light List. Vessel must replace outdated publications within 14 days. 46 CFR 184.420 (a)

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Multiple bulkhead penetrations found to not be watertight. Complete within 14 days. 46 CFR 179.320(c)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Reinstall deck plates in 2nd VOID space within 14 days. 46 CFR 176.830(a)

The means provided for pumping bilges shall be tested by operation. Forepeak bilge suction valve found seized. Repair within 14 days. 46 CFR 176.25-20(7) OLD T

[Cable and wiring must] be installed with metal supports spaced not more than 610 millimeters (24 inches) apart. Reinstall brackets located in 2nd VOID within 14 days. 46 CFR 183.330(e)

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact. distribution panel in 2nd VOID must have blanks installed and the extension cord removed from the space within 14 days. 46 CFR 183.200(c)

A battery charger intended for connection to a commericial supply voltage shall employ a transformer of the isolating type. An ammmeter that is readily visible shall be included in the battery charging circuit. Remove portable battery charger within 14 days. 46 CFR 183.05-20(f) OLD T

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following:Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Replace or repair the following items within 14 days: Foresail sheet block, main sheet, fwd throat halyard, main throat halyard, jib sheet halyard. Additionally rebed main sheet collar on boom and reinstall missing mast wedges at mast partner. 46 CFR 176.802

Replace port generator exhaust prior to next drydock.

Reinstall check valves for bilge suctions where missing prior to next drydock.

The fuel systems for the main propulsion engines... shall be checked. Remove fuel from flame impingement ball within 14 days. 46 CFR 176.25-10(5) OLD T

Fuel lines shall be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration. Support/secure fuel return tubing over the main diesel engine within 14 days. 46 CFR 182.20-40(b)(2) OLD T.

The circuit breaker, required by paragraph (c) of this section, must be interlocked with the vessel's power sources so that shore power and the vessel's power sources may not be operated simultaneously. Modify interlock on switchboard to prevent generator and shorepower from being operated at the same time within 14 days. 46 CFR 183.390(d)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Lock sewage overboard valve within 14 days. 46 CFR 159.7(c)(2)

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The valve must be operable from a safe place outside the space where the valve is installed. MI observed fuel shut off valve wiring broke during testing of remote shut off valve. Repair/replace prior to having passengers on board. 46 CFR 182.455(b)(4)

Master shall instruct each crew member, as to the duties that the crew member is expected to perform in an emergency in accordance with this part. During crew questioning for fire drill procedures MI identified inconsistencies with training and procedures. Conduct training for all crew members as required. 46 CFR 185.420

The number and identification of the items stowed inside, and their sizes(adult or child), must be marked in clearly legible letters and numbers on each container for life jackets. Letters and Numbers must be 2inches high. All life jacket containers missing the required markings, mark containers to the requirement. 46 CFR 185.604(f)

Fuel lines must be protected from damage and must meet SAE J-1942. Main Engine diesel fuel supply line was found chaffed to internal steel protective wire. Replace fuel line. 46 CFR 182.720

Fire hydrants must be provided with spanner wrenches. Aft fire station was found with out spanner wrench, provide station with wrench. 46 CFR 181.310 (b)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Main engine belt guard was loose, replace missing bolt. 46 CFR 177.960

Flexible nonmetallic hose may be used in vital and nonvital systems where permitted. Main engine raw water cooling flexible hose showing signs of wear. Replace. 46 CFR 182.720

Exhaust pipe’s watertight integrity must be maintained. MI observed salt build-up around exhaust pipe. Repair during dry-dock. 46 CFR 182.430(f)

Electrical equipment on a vessel must be installed and maintained to: minimize accidental personnel contact with energized parts. MI observed spares energized in distribution panel in E/R void space. 46 CFR 183.200 (a)

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact with energized parts. MI observed blanks in distribution panel in void 2. Repair/replace. 46 CFR 183.200(c)

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. MI observed multiple light fixtures without guards. Repair/replace. 46 CFR 183.410 (a)

A hatch cover must: be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Repair/replace. 46 CFR 179.360 (b)(2)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. VSL must submit sail plan and dead weight survey to MSC within 7 days. 46 CFR 176.840

Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. MI observed no marking for fuel shut off. 46 CFR 185.608

Provide third party rigging inspection report and provide information on slings being installed. 46 CFR 176.840

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Develop preventative maintenance plan. 46 CFR 176.840

If your vessel is not operated on international voyages and does not meet the conditions in paragraph (d) of this section, it must undergo a drydock and internal structural examination as follows unless it has been approved to undergo an underwater survey (UWILD) per § 176.615 of this part:(1) A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years... Marine Inspector observed that vessel did not undergo required drydock. Vessel is restricted from carriage of passengers until satisfactory drydock examination. 46 cfr 176.600

All vessels must be provided with bilge pumps in accordance with Table 182.25-10(a) Bilge pump not working in service space aft of engine room. Temporary electrical submersible installed.

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces... Several bilge alarms did not indicate the space alarming or did so only breifly. Ensure alarm indicates space continuously or on a continous cycle.

The owner and managing operator of a vessel shall ensure that the vessel complies with the laws and regulations applicable to the vessel including certificates issued by the FCC. Vessel's Bridge to Bridge and Radiotelphony certificate are expired.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Vessel has several wires that are worn or are not proper marine cables.

A hinged watertight bulkhead door must be fitted with a quick action closing devise operable from both sides of the door and indicator lights at the operating station showing whether the door is open or closed. Vessel must reinstall watertight door indicator at the operating station.

Preventative Maintenance plan was not on board. Initial rigging survey required. 46CFR176.402(c)(1) NVIC 02-16

Portable fire extinguishers shall be maintained in accordance with NFPA 10. Annual servicing of portable fire extinguishers required.

All vessels shall be provided with satisfactory arrangement for draining any water tight compartment. Multiple void spaces were unable to be pumped with bilge system. 46CFR182.25-1

A visual and audible bilge high alarm must be provided at the operating station. Audible alarm did not function.46CFR182.530(a) Rectify within 7 days

All electrical installations shall be to the satisfaction of the [name withheld] open ended wires, open junction boxes, and various other shock hazards throughout the vessel. 46CFR183.01-15(a)

Sail Requirements. Fwd stbd and port shrouds have mousing and cotter pins not installed correctly. Preventative Maintenance plan was not on board. Initial rigging survey required. 46CFR176.402(c)(1) NVIC 02-16

Life jacket must be approved in accordance with either §160.002, §160.005, or §160.055 in subchapter Q of this chapter. 5 Adult life jackets retro reflective tape was not functional.

Vessel missing light list and coast pilot publications.

Cable and wire for power and lighting circuits must: (3)- Meet 111.60-1 in subchapter J. Port generator cable, to the switchboard, is under rated and Electrical cable from switchboard to main distribution panel, in the engine room, are under rated. Rectify within 7 days. 46 CFR 183.340 (d)

Both fire hoses are chaffed. The forward hose is leaking. Installed hoses were 75 feet in length. Hoses shall be 50 feet in length. Replace both hoses. 46CFR181.15-10

Fuel shutoff valves do not shut. Demonstrate that valves can be shut from outside the space. 46CFR182.20-40(b)

Exhaust pipe was missing lagging and had a 2 inch hole from corrosion. Exhaust was leaking into E/R causing hazard for crew. 46 CFR 182.20-20

Bilge detection system would not alarm when submerged in water or when manually operated. Demonstrate proper operation. 46 CFR 182.530

Make main deck and watertight bulkheads watertight. Wire runs, pipe penetrations, and holes in main deck port side near poop deck near ladder were not watertight. 46 CFR 177.13

02 ring buoys on poop deck were not serviceable. Light for ring buoy was not in approved bracket. Lanyard for ring buoy light was deteriorated. Replace 02 ring buoys,stow light in approved bracket, and renew lanyard. 46 CFR 180.70

Isolation valve for collision void was missing handle. Install handle so crew can have positive control of the valve. 46 CFR 177.13

30 adult life jackets were no longer serviceable. Vessel restricted to carriage of 132 adults until life jackets are replaced. 46 CFR 180.71(a)

All fire extinguishers must be of an approved type. 03 brackets in saloon were not approved marine grade. 46 CFR 181.30-5

Emergency escape hatch labeling was faded. Renew labeling of all escape hatches. 46 CFR 185.606

Junction box in collision void was not covered and had exposed wires. Remove dead wires or cover junction box.46 CFR 183.01-15(a)

Aft fire hose reel was corroded. Replace hose reel. 46 CFR 181.15-5(c)

First aid kit was expired. Crew replaced with approved kit. 46CFR184.710

A vessel must carry one floating waterlight unless it is limited to daytime operations. Life buoy light was inoperable.

When, in accordance with 178.210 if this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material such that all pages are visible, at the operating station of the vessel. Vessel's stability letter was not posted.

Demonstrate appropriate sizing of starboard whisker stay turnbuckle.
