Vessel lost all bridge control and communication with the main propulsion due to automation control/PLC alarm and disengagement. Determine cause and effect permanent repairs to the satisfaction of the cognizant OCMI/CG Marine Inspector.

The automated propulsion machinery control, monitoring and management system disengages all propulsion controls for non-critical component failures. Determine failures that cause propulsion disengagement, reprogram PLC communication after determining alarm activation vs. immediate propulsion disengagement thresholds. Thresholds should be concurred upon by the owner, operator, cognizant OCMI & Class prior to reprogramming. PSTP shall be appropriately updated to reflect any and all updates/changes to PLC. An updated copy of the PSTP should be provided to the OCMI.

Newly installed LCD80 fire detection and alarm annunciator panel is randomly going into fault mode. New annunciator panel ordered for replacement. Provide technician report attesting to repair and satisfactory testing/operation. Until proper repairs are completed, adequate testing, and CG approval are completed, a properly trained watch/roving patrol must make routine rounds of all spaces to guard against and give alarm in case of fire or other dangerous situation. Rounds by watchmen must be properly logged and presented to CG Marine Inspectors upon request. Permanent repairs shall be completed and operational testing shall be witnessed and/or approved to the satisfaction of the cognizant CG Marine Inspector.

Vessels EDG was found inoperable due to a faulty battery/charging system.

Siemens model MXL Fire Panel malfunctioning, displaying false alarms approx. every 30 minutes. System has been taken off-line. Determine cause & effect permanent repairs to the Siemens fire control monitoring & alarm panel. Provide technician report attesting to repair and satisfactory testing/operation. Alternatively: submit plans for replacement of the fire detection and alarm system. Until proper repairs (or replacement of the system), adequate testing, and CG approval are completed a properly trained watch/roving patrol must make routine rounds of all spaces to guard against and give alarm in case of fire or other dangerous situation. Rounds by watchmen must be properly logged and presented to CG Marine Inspectors upon request.

Rescue boat hull damage near keel long term temp repair has been inspected & approved, must be inspected at least weekly, any degradation shall be immediately reported to the cognizant [name withheld] permanent repairs to the hull NLT at the next scheduled school ship hull exam. Alternatively, rescue boat may be replaced, along with components/appertenances as applicable.

Found RPM gauges not working in the ECC.

Vessel reported loss of starboard propulsion. They believe the issue to be caused from corrosion in the fuse/Breaker panel. Vessel is permitted to proceed to the Bean Dock in Port Huron provided they have a tug assist and anchor watch on standby. Vessel is not to depart from their current location at anchorage until confirmation has been made with the Duty Marine Inspector and they have indicated that the vessel may proceed to the Bean Dock. Vessel is to have a service technician on-board to assess and carry out repairs to the satisfaction of the attending class surveyor or Marine Inspector. Class report and service report to be provided to Marine Inspector. Vessel to notify the local Duty Marine Inspector prior to departure from the Bean dock after repairs have been made by calling [phone withheld]. If there are any changes occur en route to the Bean Dock, call Sector Detroit Command Center to report. Cite- 46 CFR 62.25-5(b)

Rescue boat bottom hull plate damaged, determined damage to have been caused by deck stanchion inserts during vessel slewing in & boat rocking which occurred last operating season. Stanchion inserts must be modified to prevent any additional damage. Effect permanent repairs to the hull after this operating season. Alternatively, rescue boat may be replaced, along with components/appurtenances as applicable.

Fixed fire-fighting galley system is intended by the manufacturer and MSC approved plans to secure power to the stove/oven. In its current configuration, when the system is activated, power is not secured to the stove/oven. While the new breaker is on order, vessel crew may operate the stove/oven with a firm understanding that if there is a fire and they intend to activate the galley fixed system, they must attend the breaker panel and manually secure power to both the oven and stove.

Make repairs to port tail shaft/aft journal bearing and prove proper operation of port side propulsion system prior to departure from port (Toledo).

During inspection it was discovered that the STATE OF MICHIGAN still had unresolved comments/issues with the vessel's Periodic Safety Test Procedures. MSC Letter E-2-1701081 included a testing requirement for: "Independent Bilge Suction and Emergency Bilge Suction must be tested in accordance with 46 CFR 56.50-50(e), 56.50-50(f)(4), and 62.50-20(e)(4). Please revise the document accordingly." Submissions were made to address this comment and MSC determined that the amended PSTPs were considered satisfactory for shipboard testing as per MSC Letter E2-2002752. Sector Sault Ste Marie has allowed the vessel to be manned with one additional Licensed Engineer on top of the normal full manning requirement as per their letter dated April 25, 2019 which allowed the vessel to continue operation until its next COI exam which was May of 2020. At which point the PSTPs were required to be approved and then tested for CG Marine Inspectors. That letter is no longer valid as that COI inspection has come and gone and the issue was not resolved. The vessel may proceed on a one time transit to Traverse City and shall have the engine room manned with 3 licensed engineers at all times. PSTPs to be resolved to Sault Ste Marie's satisfaction.

The Davit installation for the rescue boat is not certified for use with the installed rescue boat. The davit's approval is for launching only SOLAS rescue boats up to 6 persons under approval series 160.156. The rescue boat that is installed is a 7 person SOLAS approved rescue boat. The fully equipped weight of the rescue boat with 7 persons is in excess of the davit's Safe Working Load (SWL). The installation shall be renewed as necessary to achieve full compliance with the regulations and full compatibility of the units. Either the Davit may be changed out to reflect a Davit approved for the current rescue boat or the rescue boat may be changed out to comply with the Davits approval and design perimeters. Full plans are to be submitted to the Coast Guard's Marine Safety Center for review and approval. Installation and testing must be overseen by a qualified Coast Guard Marine Inspector. In the interim for a period of no more than 12 months with a drop dead date of November 24th, 2021 the current installation may remain in service with the restriction that AT NO TIME MAY MORE THAN 5 PERSONS OCCUPY THE RESCUE BOAT DURING RAISING OR LOWERING EVOLUTIONS.

Submit documentation containing Design Verification Test Procedures for the machinery space and propulsion controls to the Marine Safety Center for approval and provide copy of approved version to the OCMI.

Machinery space general forward general alarm visual indicator does not flash or rotate. Correct and demonstrate proper operation.

Fuel oil day tank cross connection valve inoperative (viewed from stbd compartment below main control room). Replacement required NLT at next drydock exam.

Temp repairs completed to vnetilation ducting at forward stbd corned of steering compartment. Make permanent repairs to the same NLT at next drydock exam.

Emergency diesel generator does not have a remote means (valve) to mechanically shut off the fuel supply in case of fire in the compartment. Also see 46CFR112.05-5(b). Install and demonstrate proper operation NLT at next drydock exam.

The vessel is currently broadcasting (1) Improper Name. Must remove the TS and the space before the actual name “State Of Michigan” per the FCC registration.

1. Submit revised documentation containing Qualitative Failure Analysis and Design Verification Test Procedure for the steering control system to the Marine Safety Center for approval and provide copy of approved version to the OCMI.

Existing Fire Control Plan (FCP) must be updated to reflect current location of fire and safety equipment.

Replace cracked window pane at aft starboard area of classroom on 01 deck.

Vessel must carry one additional licensed engineer in addition to the manning shown on the COI, until the PSTP is approved and verified to the satisfaction of the OCMI, before the compliance date and prior to operating in 2018.

Several dead end circuits not ID'd or removed in wheelhouse and various cabins, several cables not properly supported through vessel, corrected during inspection.

Covers missing from several 120VAC outlet boxes, equipment junction boxes in cabins and machinery spaces, corrected during inspection.

Fire screen doors on 3 stairwells would not fully self-close, required adjustment/repair, corrected during inspection.

Submit revised FMEA, DVTP and PSTP for engineering systems automation to the MSC for approval, to reflect PLC replacements and changes: (a) Alarms and shutdown paramenters to be expressed in ranges for acceptable max/min trip points, (b)Provide acceptable protocols to testing SSDG hi jacket water temp alarms, (c) SSDG low LO pressure shut down trip point to be corrected.

Make vessel available for demonstration of newly approved revisions to automation procedures (DVTP and PSTP) in presence of the attending marine inspector.

Safety caps not in place on on valve ports on CO2 pilot/actuating cylinders and spares, corrected during inspection.

General alarm bell inoperable in engine control room, corrected during inspection.

Vessel required by 46 CFR 199.202, as passenger vessel, to carry 2 rescue boat (one on each P/S side, while only one has ever been carried. This is cleared conditionally by a 2yr exemption per CCGD9(dpi) ltr 16711 dtd 09MAY2016.

Excessive accumulations of solvents, paints and flammable liquids in small containers in Machine Shop and various small machinery rooms, presents fire hazards, to be moved to paint lockers, corrected during inspection.
