The master reported that the vessel lost its port anchor chain while conducting an anchor test at the Saipan anchorage. An ABS surveyor attended the vessel to conduct a damage survey. The surveyor reported that the outboard shear pin pad-eye lower weldment inside of the chain locker void was observed with a micro-fracture and weeping fuel oil from the contiguous fuel oil tank. The crew transferred fuel below the fracture to prevent additional seepage. ABS approved temporary repairs to the affected area so that the vessel may transit to South Korea for permanent repairs. Provide a satisfactory repair proposal to ABS for approval prior to making final repairs. Provide an ABS survey report to USCG MSU Saipan after repairs are completed to the satisfaction of the attending ABS surveyor. ICLL Ch. 1 Reg 1

Sprinklers shall be placed in an overhead position and spaced in a suitable pattern to maintain an average application rate of not less than 5 liters per square meter per minute over the nominal area covered by the sprinklers. Cargo holds #1 and #2 are loaded with containerized cargo, hazard class 1 explosive cargo, and the containers are stacked above the water sprinkler coverage area. There are no fire fighting measures for the explosive cargo stacked above the fixed sprinkler system in cargo holds #1 and #2. SOLAS 74 Ch. II-2 Reg 12(c) Class provided finding 1458.0 stating: It is recommended that this matter be resolved between the flag state and OPCO as flag state approved fire safety at new construction. Deficiency action code changed from "a" to "c" on May 7, 2025. This deficiency was appealed and is at CVC level, awaiting response. The deficiency was upheld by CVC. CVC response permits the 835 to be extended to the next dry dock. New extension date updated to the defienciency.

In all new and existing ships, fire-fighting appliances shall be kept in good order and available for immediate use at all times during the voyage. The paint locker's fixed CO2 fire extinguishing system flexible hoses are not marked with the test date and no written records were found on board for testing or renewing the hoses. SOLAS 74 Ch. II-2 Reg 15

Ventilator openings shall be provided with weather tight closing appliances. The gasket for the air escape gooseneck vent on the main deck, stbd side of centerline, at the #2 hatch, was found wasted. ICLL 1966 Ch. 1 Reg 19.4

The muster list shall specify definite signals for calling all the crew to their boat, liferaft, and fire stations, and shall give particulars of these signals. These signals shall be made on the whistle or siren and they shall be supplemented by other signals which shall be electronically operated. The emergency alarm bells in the engine room were found inoperable. The alarms do not match the muster list alarms found on board the vessel used to notify crew members of an emergency. SOLAS 74 Ch. III Reg 25(h)

The ship, whether new or existing, shall carry at least two fireman's outfits complying with the requirements of Regulation 14 of this Chapter. Administrations may require in large ships additional sets of personal equipment. In accordance with MSC/USCG MOA dated 22AUG2024, Annex III – Modification of Standards, 5.c, vessel was unable to produce two or four axes per fireman’s ensembles. SOLAS 74 Ch. II-2 Reg 52(j)(i)

The master reported that the vessel is holed in the shell plate, approximately 3/16" diameter, at the starboard transom frame 258 behind the starboard stern ladder. Crew members witnessed sea water enter the vessel through the shell plate hole. The crew moved ballast on-board to prevent additional sea water from entering the vessel. The master reported that the crew will temporarily repair the hole prior to arrival in Saipan, CNMI, where a third party ABS surveyor can attend the vessel. Provide an ABS survey report to USCG MSU Saipan after the surveyor attends the vessel. ICLL Ch. 1 Reg 1

(a) Seventeen cubic meters (600 standard cubic feet) or less of acetylene may be stowed on or below decks on any vessel. There were 12 bottles of Acetylene stored in the passageway forward of the CO2 room. Determine the volume of the bottles, and move any bottles in excess of 600 cubic feet, to a location open to the weather decks, IAW ABS 4-6-7/7.3

Main source of electrical power shall consist of at least two generating sets. Vessel has 3 ship service diesel generators, 2 of which are experiencing operability issues. #2 SSDG down due to issues with Auto Voltage Regulator (AVR) #1 SSDG experiencing issues with tripping offline while running at random times, engineers onboard troubleshooting. Vessel shall make effective repairs to restore at least two generating sets for electrical power prior to departure. Vessel authorized to shift in [name withheld] anchorage area and complete short runs in local vicinity of harbor for routine discharge ops at sea. Harbor assistance tugs shall be on standby. 74 SOLAS (1981) II-1/41.1.1

Fireman's outfits & equipment shall be ready for immediate use. Noted 08 defective regulator hoses for various self-contained breathing appartuses throughout the vessel (refer to 2021 Harris Fire Servicing Report for specific SCBA serial numbers). Hoses shall be replaced with satisfactory replacements as per manufacturer recommendation. 74 SOLAS II-2/52(i)(iii)

OIL FINTERING EQUIPMENT SHALL BE OF A DESIGN APPROVED BY THE ADMINSTRATION AND SHALL BE SUCH AS WILL ENSURE ANY OILYMIXTURE DISWCHARGE INTO THE SEA AFTER PASSING THROUGH THE SYSTEM HAD AN OIL CONTENT NOT EXCEEDING 15 PARTS PER MILLION. THE VESSELS OIL FILTERING EQUIPMENT FAILED TO OPERATE AS DESIGNED.

DATA SHALL BE STAMPED DIRECTLY ON THE PRESSURE VESSELS. tHE VESSELS START AIR RECEIVER TANKS ARE MISSING DATA PLATES.

46 CFR 113.25-12(a),(c): Total of 06, general alarm signals located in interior of house and on secon flat/motor room in bow thruster space are inoperable.

SOLAS 74 III/9(a)(ii): The engine and its accessories shall be suitably enclosed to ensure operation under adverse weather conditions. Make repairs to No. 1, starboard motor air shroud cover with hole.

A safety system shall be provided to ensure that serious malfunction in machinery or boiler operations, which presents an immediate danger, shall initiate the automatic shut-down of that part of the plant and that an alarm shall be given. The electrical component that controls the low lube oil and engine over speed shutdowns for the emergency diesel generator is operating intermittently, the emergency generator operates normally despite the intermittent issue and supplies power to the ships emergency bus. Repair/replace electrical component, should the emergency generator be run for an extended period to provide emergency power prior to the repair or replacement of the component, regular rounds of the emergency generator space shall be made to monitor operations. SOLAS 74(81 Amend.) II-1/52

The function of the detection system shall be periodically tested to the satisfaction of the Administration by means of equipment producing hot air at the appropriate temperature, or smoke or aerosol particles having the appropriate range of density or particle size, or other phenomena associated with incipient fire which the detector is designed to respond. The smoke detector in the A-Deck laundry room did not activate or alarm when tested. Repair/replace smoke detector. SOLAS 74(81 Amend.) II-2/13.1.13

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. A dead ended wire was found on the starboard side main deck. Remove or isolate wire in a junction box to eliminate potential electrical hazards. SOLAS 74(81 Amend.) II-1/40.1.3

SOLAS 74 (81 ed) II-2/21 and 46 CFR 91.45-1: Repair engine room bilge foam sprinkler starboard side nozzle wasted and broken off. Upon completion of repair, bilge foam sprinkler lines must be flushed out with water and waste removed from bilge prior to next USCG visit.

SOLAS 74 (81 ed) II-2/21 and 46 CFR 91.45-1:Repair main salt water pipe for bilge foam sprinklers severely pitted forward of main engine below deck plate.

SOLAS 74 (81 ed.) II-2/21: Remove paint from pressure switch in front of paint locker room on Level 63 and prove proper operation.

SOLAS 74/78 II-1/43: It was observed that the fuel oil tank level would not supply all those services that are essential for safety in an emergency for the required period hours. Maintain EDG fuel oil tank to 95%.

ICLL 66 Reg. 1 and 46 CFR 42.09-50: Provide proof of Class approval for alteration (circular cutout) made to overhead above 63 Level in way of watertight hatch cover near machine shop.

SOLAS 74/78 II-2/20.1: Update fire control plant to show installed smoke detector in emergency diesel generator space and submit to Class for approval. Onboard ship’s approved fire control plan only shows Heat as the fire detection system for EDG space.

SOLAS 74/78 (14 cons) III 20.2: Repair aft horizontal bracket for No. 4, forward horizontal bracket for No. 6 and horizontal sheave guard for No. 3 lifeboats with wastage.
