The design of the system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire-detection system design, by a technician with qualifications as a National Institute for Certification in Engineering Technologies (NICET) level IV fire alarm engineering technician, or by an authorized classification society with equivalent experience, to comply with paragraphs (a)(1) through (6) of this section. Provide proof that the current arrangement of the fire detection system to include all newly installed components is certified and inspected by a registered professional engineer with experience in fire-detection system design, by a technician with qualifications as a National Institute for Certification in Engineering Technologies (NICET) level IV fire alarm engineering technician, or by an authorized classification society with equivalent experience. The certification letter must include verification that the fire-detection system as a whole with the new detectors installed, meets the requirements of 46 CFR 142.330(a)(1) thru (6).

Port and starboard aft voids have water ingress with no detection for cause. Vessel crew is aware of water ingress with no entry into towing vessel records. Vessel authorized transit to Joliet, IL to test, no other movements authorized. Notify OCMI when damage survey complete.

Vessel ring buoy and work vests found to be unserviceable with no entries into lifesaving maintenance or towing vessel records. Enter ring buoy and work vests into TVR (SIMEX) system for maintenance/repair.

A 40-B portable extinguisher is required in vicinity of flame locker located in starboard side storage room.

Port Bulwarks and Forward Caval(Cleat) was damaged during fleeting operations. Effect permanent repairs to return area to as built condition.

Installed electrical power sources must be capable of carrying the electrical load of the towing vessel under normal operating conditions. While operating in fleeting area in Channahon IL on Illinois river MM278, vessel lost all power due to port generator casualty. Must conduct inspection of generator, repair and conduct test of generator under load. Must submit Tech report to Coast Guard Marine Safety Unit Chicago Inspections department within 14 days. 46 CFR 143.400(b)

While running a query in Vessel Information Verification Service (VIVS)in May 0f 2018, discovered vessel was broadcasting Automatic Identification System (AIS) errors. IMT was notified in person back in June and 2 follow up emails. Vessel still reads errors as of today.Must conduct repairs to AIS within 30 days or NOV and fines will be issued. 33 CFR 164.46

Radar was inoperable.
