To maintain a valid Certificate of Inspection (COI), you must complete your annual and periodic inspections within the periods specified... and your COI must be endorsed. The vessel was issued a COI on 17DEC2018 to expire 17DEC2023. An annual inspection of the vessel was due between 17SEP2019 and 17MAR2020. The vessel has failed to conduct the annual inspection within the required inspection period and therefore the COI is invalid. A new COI examination shall be conducted prior to carriage of cargo.

Vessels that operate in salt water must be inspected in accordance with the intervals set forth in table 31.10-21(a). The vessel was required to complete two (2) internal structural examinations (ISE) within a five (5) year period, with no more than three (3) years between any two examinations. The vessel completed an ISE on 31OCT2014 and 17DEC2018, with an ISE being due 31OCT2019. The vessel failed to conduct the ISE as required. An ISE shall be conducted prior to carriage of cargo.

Manned tank barges and tankships authorized to carry Grade A, B, C, D liquids at any temperature or Grade E liquids at elevated temperatures shall be provided with a combustible gas indicator suitable for determining the presence of explosive concentrations of the cargo carried. New systems were found onboard but installation was not complete and system was inoperable. Complete installation IAW manufacturer’s instructions and demonstrate proper operation to the satisfaction of the attending OCMI.
