All life saving equipment must be tested and maintained in accordance with the minimum requirements of 191.190 of this chapter as applicable in the vessel's TSMS, if the vessel has a TSMS. Life raft has exceeded servicing date, and must be service prior to vessel's departure.

All portable fire-extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems must be inspected or tested as prescribed in this chapter. Fire-fighting equipment must be tested and serviced as applicable not later than January 29, 2021.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the 50% of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 Days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. E/R Fuel Trap / Separator flow sight glass leaking diesel fuel, creating a fire hazard. Repair leak / replace leaking material.

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. Wiring connections to STBD E/R battery bank were terminated using wing nuts and had exposed wiring in area prone to chaffing. Repair / properly connect wiring.

The owner or managing operator must implement a health and safety plan, which must document compliance with this part and include recordkeeping procedures. Found insufficient evidence of a health & safety plan meeting subchapter M being implemented. Develop and implement a health and safety plan by 9/15/2019.

The officer in charge of a navigational watch must keep the navigation assessment up-to-date to reflect changes in conditions and circumstances. At each change of the navigational watch, the oncoming officer in charge of the navigational watch must review the current navigation assessment for necessary changes. A navigation assessment entry must be recorded in the TVR, official log, or in accordance with the TSMS applicable to the vessel. The entry must include the date and time of the assessment, the name of the individual making the assessment, and the starting and ending points of the voyage or trip that the assessment covers. No conforming navigation assessments are being recorded. Solidify policy for nav assessments, and demonstrate compliance with the regulatory recordkeeping requirements by 9/15/2019.

When the towing vessel is underway, the EPIRB must be stowed in its float-free bracket with the controls set for automatic activation and be mounted in a manner so that it will float free if the towing vessel sinks. Found the EPIRB to be mounted underneath the wheelhouse deck which has approx 4-5 foot overhang, potentially inhibiting the float free arrangement of the EPIRB directly below. 46 CFR 141.380(b)
