Pull bilge check valve, for inspection Anchor chain gauging Collision bulkhead gauge to test metal thickness

Halon cylinders forming part of a fixed fire extinguishing system must be visually inspected for damage, corrosion, or deterioration every year and replaced if found unserviceable. Halon cylinder was found to be deteriorated requiring maintenance and bottom cylinder support was broken. 46 CFR 147.65(b)

Each fire main piping valve must meet applicable requirements of part 128. The middle engineering passageway fire hydrant shutoff valve was seized and the handle broken in to pieces. Replace valve and handle.

The control system shall be designed for automatic shutdown of the engine with actuation of audible and visible alarms at shutdown. The visible malfunction indicator shall indicate what condition caused the shutdown and remain visible until reset. Vessel had several alarms and shutdowns that were inoperaable when testing NVIC 1-78 approved automation test proceedures. Vessel is required to be fully manned in the engineering department until all automation testing has been completed satisfactorily by ABS or attending Marine Inspector.

The Convention certificates must be on board the vessel and readily available for examination at all times. Due to automation issues, ABS class and other documents were not onboard and completed. Vessel needs to have all claasification and conventional certificates onboard and completed before COI can be issued.

Multiple Hatches and QAWTDs needed adjusted and gasket replaced, including ones leading to engine room.

Multiple Fuel Hoses need replaced 1) Hose on bow thruster engine needs replaced and chaffing gear 2) Port Auxillary Engine (Liquid Mud Pump)PTO requires two new fuel hoses 3) Port Generator require new lube oil hose and fuel hose 4) STBD generator requires fuel hose and lube oil hose 5) STBD main requires new fuel return hose 6) multiple cotton jacket hoses were found that need replaced with hoses meeting SEA-J 1942 approval

VSL had STBD liferaft improperly stowed, painter line was disconnected and hydrostatic release was upside down.

GMDSS daily log was not being kept on the VSL. 47 CFR 80.1065

Oil record book enteries for monthly OWS checks were not being logged.

STBD service generator was inoperable.

Fire pump gauge on bridge was inoperable.

Navigation lights at for the primary starboard and aft anchor station were inoperable.

Magnetic Compass and Gyro Compass had a deviation of 16 degrees.

Vessel is in process of completing ABS Load Line Survey. Required vessel to provide satisfactory report of survey once complete.

1. To be completed prior to 28Oct2010: Provide on board drug and alcohol awarness training IAW 49CFR 16.401. Currently no one is aware of the last time training was conducted.

2. To be completed prior to 28Oct2010: Compass card missing, provide on board current deviation card.

3. To be completed prior to 28Oct2010: Replace the EPIRB, currently antenna broken. Prior to getting vessel underway.

Vessel needs some sort of Eng. Asst Alarm in [name withheld].

Provide up to date copy of the Navigation Rules.

Provide (2) blank covers for two eletrical panels in the engine room.

Provide audible or visual signal for the sound powered phone in the engine room, powered by the emergency power source.

Label all valves on the bilge manifold.

SUBMIT FOR APPROVAL AUTOMATION TEST PROCEDURES AS PER NVIC 1-78.

PROVIDE DRUG HOTLINE PHONE NUMBER AND MAKE AVAILABLE TO THE CREW.

PROVIDE REQUIRED ALCOHOL BREATH TESTING DEVICE AS PER 46 CFR 4.06-20.

REPLACE BOTH PORT AND STARBOARD TAILSHAFTS; HEAVILY PITTED IN WAY OF SHAFT LOGS.

PROVIDE GENERAL ALARM AND HALON DISCHARGE SIREN WARNING SIGNS IN WAY OF MACHINERY SPACE.

TWO FIREMAN'S SUITS MUST BE PROVIDED IN ACCORDANCE WITH 46CFR 96.35.

PROVE PROPER OPERATION OF STBD M/E LOW LUBE OIL PRESSURE ALARM AND OVERSPEED TRIP SHUTDOWN.

INSTALL FIRE PUMP DISCHARGE PRESSURE GUAGE ON THE BRIDGE.

REPAIR AND PROVE PROPER OPERATION OF THE PORT ENGINE ROOM ESCAPE TRUNK EMERGENCY LIGHT.

PROVIDE ONE SERVICE LINE FOR A TOTAL OF FOUR FOR THE LINE THROWING GUN.

PROVIDE AN APPROVED TYPE EMERGENCY LIGHT TO REPLACE THE MISSING LIGHT NEAR THE STBD ESCAPE IN THE ENGINE ROOM.

MAKE PERMANENT REPAIRS TO THE UNDERDECK LONGITUDINALS AND BRACKETS IN THE STBD CHAIN LOCKER AT FRAMES 4,6, AND 8 AND REWELD FRACTURED SIDE VERTICALS IN THE PORT CHAIN LOCKER AT FRAMES 5 AND 7.

PROVE PROPER OPERATION OF THE SMOKE DETECTION SYSTEM.

PROVIDE A CORRECTED FIRE CONTROL PLAN TO HMRMS FOR REVIEW, VARIFICATION, AND APPROVAL.

PROVE PROPER OPERATION OF THE OILY WATER SEPERATOR AND THE 15PPM MONITORINGEQUIPTMENT.
