Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. CG requires a TPO class report attesting for cause of the loss of power and actions of correction prior to vessel departure from Sector Virginia Regulated Navigation Area (RNA).

Pressure vessels must be externally examined annually, relief valves must be tested twice every 5 years, with no more than 3 years elapsing since last test. Vessel was unable to test relief valves do to faulty pressure relief valves. Repair or replace relief valves for each pressure vessel on board that is over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure.

Vessel reported a loss of steering to Sector Delaware Bay while in the vicinity of the C&D Cannel. Vsl was in transit with loaded barge and was able to maneuver safely outside the channel and anchor near Reedy Anchorage. Vsl contacted a standby tug to assist if needed while repairs were being made to the steering system.

The vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be questioned by OCMI. Vessel broke free of mooring lines and reported damage to port side. Provide CG-2692 and receive satisfactory damaage survey by CG Marine Inspectors prior to movement of vessel. Conduct daily soundings of fuel tank and report any changes to Sector NY Command Center; provide repair plan proposal and tow plan to VTS for repair.
