Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between bolt holes, and in line with the bolt hole pattern. MI found no stud permanently installed on Stbd Fwd Vapor control System Connection.

If twist-on type of connectors are used, the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. MI found twist-on connectors on a battery charger in the generator room.

The construction of each luminaire (lighting fixture) for a non-hazardous location must meet ANSI/UL 1598A, or IEC 60092-306:2009. MI found two aft deck weather lights to be broken and/or full of water within the lenses.

An electric apparatus must be designed, constructed, and installed to prevent any person from accidentally contacting energized parts. MI found distribution panels missing blank covering in place of breakers.

Each barge equipped with an anchor must be fitted with an operable anchoring system that includes a cable or chain, and a winch or windlass. All components of the system must be in general conformity with the standards issued by a recognized classification society. The anchor must be stowed so that it is ready for immediate use in an emergency. Vessel reported loss of anchor while located in the Mississippi River. Replace anchor and provide report from attending [name withheld] attesting to installation of new anchor and full operability of anchoring system to USCG Marine Inspector prior to departure from Sector New Orleans OCMI zone. 46 CFR 32.15-15 (e)&(f)

A pressure gage shall be installed for each pump discharge, and it shall be located at a point visible with relation to the pump controls. Pressure gage not visible at pump control room.

Metallic expansion joints found to be used for alignment of misaligned pipes. Vessel representative must show manufacturer specs to ensure compliance with installation standards.

A fitted single screen of corrosion-resistant wire of at least 30 by 30 mesh or two fitted screens, both of corrosion resistant wire, of at least 20 by 20 mesh spaced not less than one-half inch or more than 1 1/2 in apart. All cargo tank flame screens are the incorrect mesh material.

When a barge is moored and contains no oil but is not gas free: All cargo tank hatches must be clearly marked in not less than three inch lettering "Danger-Keep Out". Danger-Keep Out lettering missing from all hatch covers.

Bunker delivery notes missing during IAPP Survey.

The inspection shall be such as to insure that the vessel, as regards the structure, boilers, and other pressure vessels and their appurtenances, piping, main and auxiliary machinery, electrical installations, life-saving appliances, fire-detecting and extinguishing equipment, pilot boarding equipment, and other equipment is in satisfactory condition and fit for the service for which it is intended. #4 cargo pump prime mover increases RPMs without input.

Vessels subject to initial and periodic surveys shall have annual surveys, within 3 months either way of the certificate's anniversary date. The annual surveys shall be made by and prove satisfactory to the assigning and issuing authority prior to executing the required annual endorsements on load line certificates or exemption certificates. The scope shall be as defined in §42.09-40 and such as to ensure that the applicable load line marks are found to be correct for the condition the vessel is then in. Vessel’s load line certificate was issued by ABS on 09/24/2018 and did not receive its 2019 annual endorsement. Vessel must have an annual survey conducted in accordance with §42.09-40 to verify the certificate’s accuracy.

The equipment shall be maintained to conform with the provisions of this Annex and no changes shall be made in the equipment, systems, fittings, arrangements or material covered by the survey. The vessel has been surveyed in accordance with regulation 5 of Annex VI and carries an IAPP Certificate stating that it has an approved vapor collection system. The vessel’s vapor collection system has expired safety relief devices and pipe testing records. The vessel’s IAPP Certificate is invalid without a vapor collection system that is maintained and in compliance. MARPOL Annex VI/5.5

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present Convention to ensure that the ship in all respects will remain fit to proceed to sea without presenting an unreasonable threat of harm to the marine environment. Due to a lack of maintenance and company support, the vessel has outstanding deficiencies on multiple cargo and ballast operations equipment rendering their compliance with the provisions of their IPPC Certificate obsolete. MARPOL Annex II/8.3.1

Cargo pump relief valves shall be tested at least once each year to determine that they function satisfactorily at the pressure at which they are set to open. The cargo discharge piping of all tank vessels shall be tested at least once each year for tightness, at the maximum working pressure. Vessel’s cargo pump relief valves and cargo discharge piping are overdue for required annual testing. Vessel must maintain cargo handling equipment. 46 CFR 35.35-70

An inflatable liferaft or inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, and in accordance with servicing procedures meeting the requirements of part 160, subpart 160.151, of this chapter no later than the month and year on its servicing sticker. Liferafts required by the vessel’s COI expired on 09/05/2020. Vessel must carry liferafts that have been serviced annually. 46 CFR 160.151-57(n)

The owner, master, or person-in-charge of a tank vessel must ensure that portable and semi-portable extinguishers are inspected and maintained in accordance with NFPA 10 for annual maintenance and recharging of extinguishers. All fire protection equipment certificates onboard were expired. Annual servicing must be conducted by a certified fire extinguisher servicing agency. 46 CFR 31.10-18(a)(1)

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions and the protection of crew and the vessel from electrical hazards. Starboard accommodation passageway has broken exterior light switch and missing interior light bulb. Light switch must be replaced and lightbulb must be installed. 46 CFR 111.01-1(a)

A cargo tank of the tank vessel connected to a vapor collection system must be equipped with a permanent or portable cargo gauging device that is a closed type as defined in 46 CFR 151.15.10(c) that does not require opening the tank to the atmosphere during cargo transfer and allows the operator to determine the level of liquid in the tank for the full range of liquid levels in the tank. Vessel’s closed gauging devices are not maintained to meet the complete integrity of cargo containment. Approximately half of the devices have wasted wipers rendering the sight glass inoperable to the operator. All wasted wipers in closed gauging system must be replaced. 46 CFR 39.20-3(a)(2)

The inspection shall be such as to insure that the vessel’s auxiliary machinery equipment is in satisfactory condition and fit for the service for which it is intended. Rain stops installed at the discharge end of exhaust piping for diesel engines located on the weather deck are seized in the open condition. Protection must be provided as such to prevent the ingress of water into an internal combustion diesel engine. 46 CFR 31.10-15(b)

Cargo tanks in which Grade A, B, or C liquids are to be transported shall be fitted with either individual pressure-vacuum relief valves which shall extend to a reasonable height above the weather deck or shall be fitted with a venting system consisting of branch vent lines connected to a vent header which shall extend to a reasonable height above the weather deck and be fitted with a pressure-vacuum relief valve. The vent header system, if fitted, shall be provided with suitable connections for flushing and draining, and if desired, stop valves may be placed in the individual branch vent lines: Provided, That each such stop valve is bypassed by a pressure-vacuum relief valve. #1 and #2 vacuum tanks relief valves wasted. Replace relief valves.

The respective piping systems installed on ships and barges shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. Starboard ballast pump engine drive shaft sheared off/is missing; Port and Starboard radiators leaking on ballast pump engines.

A tank vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have— Under or around each loading manifold and each transfer connection point, a fixed container or enclosed deck area that, in all conditions of ship list or trim encountered during the loading operation, has a capacity of at least: Four barrels if it serves one or more hoses with an inside diameter of 12 inches or more, or one or more loading arms with a nominal pipe size diameter of 12 inches or more; Port and Starboard cargo manifold containment wasted.

Vessels contracted for prior to August 22, 2016, must meet the following requirements: (a) Previously installed extinguishers with extinguishing capacities smaller than as required in table 34.50-10(a) need not be replaced and may be continued in service so long as they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspection. Starboard side forward flammable locker fixed fire extinguisher wasted. There is a Portable B-40 available within the vicinity of the flammable locker.

Location of ground indicators. Ground indicators must: (a) Be at the vessel's ship's service generator distribution switchboard for the normal power, normal lighting, and emergency lighting systems; (b) Be at the propulsion switchboard for propulsion systems; and (c) Be readily accessible. Ground detection indicator middle bulb "B Phase" on main distribution panel burned out.

The respective piping systems installed on ships and barges shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. Valve handle broken on starboard midship ballast tank.

Each shore connection box must have a bottom entrance for the shore connection cable. The must provide protection to the shore connection when the connection is in use. Starboard shore connection cover has a hole in the bottom right corner.

On all tank vessels, the construction or conversion of which is started on or after November 10, 1936, the cargo pumps shall be designed and installed to minimize the danger of sparking. Where cargo pump shafts pierce gastight bulkheads, stuffing boxes with readily accessible gastight glands shall be provided. Cargo pump prime movers missing stuffing boxes on four (4) shafts leading to cargo pumps. Prior to conducting cargo discharge operations, vessel must provide satisfactory evidence or install required stuffing boxes to the satisfaction of the attending marine inspector.

The last 1.0 meter (3.3 feet) of vapor piping upstream of the vessel vapor connection and each end of a vapor hose must be Painted in the sequence of red/yellow/red. The width of the red bands must be 0.1 meter (0.33 foot) and the width of the middle yellow band must be 0.8 meter (2.64 feet) and labeled with the word “VAPOR” painted in black letters at least 50.8 millimeters (2 inches) high. Vapor piping is not painted correctly (i.e., "yellow/red/yellow," with "VAPOR" in white lettering. Painting must be conducted to the satisfaction of the attending marine inspector.

The tank overfill alarm required by 46 C.F.R. 39.2007(a) / 46 C.F.R. 39.2009(a)(1), must be identified with the legend “High-level Alarm” in black letters at least 50.8 millimeters (2 inches) high on a white background. Labeling must be completed to the satisfaction of the attending marine inspector.

Each item of survival equipment with an expiration date must be replaced during the annual inspection if the expiration date has passed. The two required liferafts are expired and require an annual inspection.

Objective evidence discovered during in service inspection revealed the following deficiency: The vessel failed to fully implement the requirements of the SMS as evident by def. #1. An internal audit and correcitve action within three months is required. ISM/10 ISM Code 10

Owner/operator reported that there is a possible fracture between the #5P and #6P cargo tk bulkhead. Fracture was reported approx 24" up from the #5P tk bottom outboard side. Vessel is directed to make #5P and #6P safe for entry to allow a marine inspector to conduct close up inspection of suspect areas. Inspection must be completed prior to departure.

(a) The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the following ballast water management methods: (1) Install and operate a ballast water management system (BWMS) that has been approved by the Coast Guard under 46 CFR part 162 (2) Use only water from a U.S. public water system (PWS), as defined in 40 CFR 141.2, that meets the requirements of 40 CFR parts 141 and 143 as ballast water. (i) Previously cleaned the ballast tanks (including removing all residual sediments) and not subsequently introduced ambient water; or (ii) Never introduced ambient water to those tanks and supply lines. (3) Perform complete ballast water exchange in an area 200 nautical miles from any shore prior to discharging ballast water, unless the vessel is required to employ an approved BWMS per the schedule found in §151.2035(b) of this subpart. (4) Do not discharge ballast water into waters of the United States. (5) Discharge to a facility onshore or to another vessel for purposes of treatment. Operator is on-loading BW in on COPT zone and discharging in another COPT zone. 33 CFR 151.2025

Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Raised tank top on cargo tanks 1S, 2S, 4P, 6S, 7P, 7S, 8P, and 8S have several bolts and the surrounding steel that is severely wasted. Renew as original to the satisfaction of the attending Marine Inspector.

The valves shall be of substantial construction and first class workmanship and shall be free from imperfections which may affect its serviceability. The flame screen on the P/V on the forward part of the barge was found to have a hole and needs replacing.

Each cable entering a box of fitting must be protected from abrasion and must meet the following: (a) each opening through which a conductor enters must be closed. Multiple cable runs along the cargo deck found to have wasted tubing and packing for alarm panels.

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port and a self-igniting light. Ring lifebuoy located on the bow was found improperly marked with an inoperable light.

In meeting these requirements the Company should ensure that: .2 any non-conformity is reported, with its possible cause, if known; .3 appropriate corrective action is taken As noted in deficiency one (1) on 06 Jul 2018 CG-835V (MISLE Activity 6469879) regarding the substandard material condition of the cargo hatches (1P, 2S, 4P, 6S, 7P, 7S, 8P, and 8S), the company failed to meet the requirements of ISM Code 10.2.2 and 10.2.3. Objective evidence in the company's failure to identify additional substandard material condition from crew inspections of the barge cargo area was noted in the lack of the ship's maintenance log entries and failure of taking appropriate corrective actions by the ship's crew or company to address the eight (8) severely wasted cargo hatches identified by the attending Coast Guard Marine Inspectors. Further evidence was noted from the Coast Guard Inspection on 05 Dec 2017 conducted by MSU Texas City. During the 05 Dec 2017 inspection, numerous cargo hatch deficiencies were identified, as noted in MISLE activity 6318874 deficiencies number four (4) and five (5). Deficiencies 4 and 5 were rectified three months later by Sector Miami on 08 MAR 2018 (MISLE Activity 6367121). During the span of those 3 to 4 months from DEC 2017 to MAR 2018, the ships crew and company did not reasonable suspect or identified the substandard condition of the remaining cargo hatches as the company only replaced the four cargo hatches (1P, 2P, 5S, and 6P) that were identified from the 05 DEC 2017 Coast Guard inspection. As noted in 10.3 the company should identify equipment and technical systems of the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems these measures should include the regular testing of standby arrangements and equipment or technical systems that not in continues use. The substandard condition of the wasted cargo hatches clearly presented a hazardous situation on board the B.NO. 240 in not being able to maintain vapor tightness.

Each outlet or junction box must be securely attached to its mounting and be affixed so as to maintain its designated degree of protection. Stbd manifold high level alarm connection into junction box is lose due to a missing nut. Replace nut, tighten down and secure to the satisfaction of the attending Marine Inspector.

Section 4.5 Part B of the Bouchard Safety Management Manual (2017 edition) prescribes the safe working practices for entry into restricted access spaces. The aft peak port space has been identified as a restricted access space and is regularly accessed for storage. There is no objective evidence to indicate that these safe work practices have been implemented prior to entry. Prior to entry, company is to provide documentation and implementation of safe working practice.

The ring, lines and letters shall be painted in white, yellow on a dark ground or in black on a light ground. They shall also be permanently marked on the sides of the vessels to the satisfaction of the assigning authority. The marks shall be plainly visible and, if necessary, special arrangements shall be made for this purpose. Both port and stbd load lines are faded and hard to read. Repaint in contrasting colors to the satisfaction of the attending Maring Inspector.

Cargo tank venting is to be be carried out in accordance with the provisions within 46 CFR 32.55-25. Cargo tank top hatches 1S, 2P & 2S were venting. Tighten hatches to prevent further venting.

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces and from galleys, main pantries and storerooms other than small service lockers shall be of "A" Class Construction. The foward "A" Class bulkhead for the galley is holed and wasted in way of the door frame. Make repairs to restore continuous boundary to the satisfaction of the attending Marine Inspector.

The posted fire control plan is no longer legible. Revise and post per the current onboard arrangements to the satisfaction of the attending Marine Inspector.

Vessel lost anchor while anchored mm66 LMR. Provide Class report attesting to anchoring arrangement post new anchor installation.

Vessel must have internal structural exam conducted by coast guard at intervals set forth in 46 CFR 31.10-21. Vessel was due for ISE on 07MAR2017. ISE must be conducted prior to departure from New Orleans.

DETERMINE CAUSE OF EXCESSIVE LEAKAGE ON CARGO PUMPS # 2, 3, 4. MAKE PERMANENT REPAIRS AND PROVE PROPER OPERATION OF CARGO PUMPS # 2, 3, 4 PRIORTO OPERATING.

PROVIDE ALL NECESSARY INFORMATION TO PROVE COMPLIANCE WITH THE BENZENE REGULATIONS.

SIDE SHELL HOLED. INTERNALS DAMAGED INCLUDING TWISTED SIDE LONGS, BENT GUSSETS, AND BUCKLED BULKHEAD INWAY OF FORWARD RAKE BULKHEAD PORT SIDE. MAKE PERMANENT REPAIRS, IN LIEU OF TEMPORARY WELD REPAIR, TO DAMAGED SIDE SHELL AND INTERNALS.
