Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the oil record book part I, so that all entries in the book appropriate to that operation are completed. PSCO identified that Chief Engineer is confusing bilge water and sludge, recording them as the same code in the oil record book, when they should be recorded as two seperate codes.

The rescue boat embarkation and launching arrangements shall be such that the rescue boat can be boarded and launched in the shortest possible time. PSCO's observed that it took over 15 minutes to start and deploy the rescue boat, thus not being able to meet the short possible time requrements.

THE ADMINISTRATION SHALL PROVIDE THE SHIP WITH AN APPROPRIATE DOCUMENT AS EVIDENCE OF COMPLIANCE OF CONSTRUCTION AND EQUIPMENT WITH THE REQUIREMENTS OF THIS REGULATION. THE VESSELS DOCUMENT OF COMPLIANCE DANGEROUS GOODS INDICATES THAT CLASS 3 AND 8 DANGEROUS GOODS ARE ONLY PERMITTED ON THE WEATHER DECK. PSCO OBSERVED CLASS 3 AND CLASS 8 DANGEROUS GOODS STORED BELOW DECK. 74SOLAS(97)II-2 REG. 54.3 17BC

A main source of electrical power of sufficient capacity to supply those services mentioned in Reg. 40.1.1 shall be provided. This main source of electrical power shall consist of at least two generator sets. A/E #1 is overheating during normal operation and incapable of being run for extended period of time. Main source of power is being supplemented by the emergency source of power to prevent overheating. 74SOLAS (04 CONS) 11-I/41.1.1

Each of the required five pumps shall have a capacity of not less than 25M3/h and each such pump shall be capable of delivering at least the two required jets of water. Alternate arrangement of portable pump could not supply adequate jets of water through 02 hoses. 17bc 74SOLAS 97COns. II-2 4.2.2

Vessel owner or operator shall conduct exercises as necessary to ensure that the plan will functioning an emergency. Vessel has not conducted SMFF or RACE exercise as required by their plan NTVRP. Must conduct according to the plan. 50c 33 CFR155.1060(a)

Generating set #2 pneumatic means of starting is not operational. Additionally, generating set #3 has excessive jacket water cooling leak. SOLAS71(14cons) I/11(a) 17b/c

01-1117 ... Not less than two of these shall also be provide with self-activating smoke signals complying with the requirements of regulation 31.3 and be capable of quick release from the navigation wings. Both life buoys on the bridge wings were tight to the crane in which they were stowed, preventing them to be quick released as designed. 74 Solas (97)III/7.1.3

02-99101 For each vessel owners/operators are required to submit an annual report for each year that they have active permit coverage. Vessel has not been submitting annual reports. Vessel master contacted company and a report was submitted during the inspection. 2013 UBP 4.4.1

SHIPS WITH LINES FOR OIL RESIDUE SHALL BE FITTED WITH A STANDARD DISCHARGE CONNECTION IN ACCORDANCE WITH THE TABLE IN REGULATION 13 OF MARPOL ANNEX I. MARPOL (2017 CONS) ANNEX I/13

THE OPERATOR OF A VESSEL WITH A CAPACITY OF 250 OR MORE BARRELS OF OIL SHALL PROVIDE TRANSFER PROCEDURES THAT MEET THE REQUIREMENTS OF THIS PART FOR TRANSFERRING TO THE VESSEL. 33CFR155.720

2515 Objective evidence discovered during an expanded ISM exam revealed the following non-conformities: the vessel failed to fully implement the requirements of the ISM Code through their SMS procedures as evident by the following deficiencies indicating that the ship and/or company are not meeting the SMS requirements. Recommend an external audit. 74 SOLAS (2014) IX/3.1 ISM Code Part A 01 - The company should ensure that each ship is appropriately manned in order to encompass all aspects of maintaining safe operations onboard. Vessel not properly manned in accordance with work/rest requirements in the STCW, as outlined in Deficiency #02. ISM Code Part A Reg. 6.2.2 02 - The company should establish procedures to ensure that personnel are given proper familiarization with their duties. Chief engineer lacked familiarization in recordkeeping requirements for the Oil Record Book Part I. Chief Engineer logged incorrect soundings for dirty oil tank #13 and bilge water tank #12 as outlined in deficiency #3. Chief engineer lacked familiarization with onboard oily water separator and indication that 15ppm sensor indicating light improperly continually stayed lit. Maintenance records indicate the equipment had no issues and chief engineer appeared to believe there is no issue with the equipment. ISM Code Part A Reg 6.3 03 – The company should establish and maintain procedures to control all documents and data relevant to the safety management system. The work/rest logs do not indicate whether the vessel is in port or at sea as the form directs to record. ISM Code Part A Reg. 11.1

All persons who are assigned duty as officer in charge of a watch shall be provided a minimum of 10 hours of rest in any 24 hour period and 77 hours in any 7 day period. The hours of rest may be divided into no more than three periods, one of which shall be at least 6 hours in length. Parties may allow exceptions from the required hours of rest provided the rest period is not less than 70 hours in any 7 day period. Exceptions from the weekly rest period shall not be allowed for more than two consecutive weeks. Chief Engineer and 2nd engineer have received fewer than 70 hours weekly rest on multiple recorded occasions between November 2018 and March 2019. Standard workday for engineers is at least 14 hours a day (98 work hours a week, 70 rest hours), and both engineers have worked 15 hour days on multiple documented occasions without compensatory reduction in work hours later in 7 day period, leading to multiple weeks with fewer than 70 hours rest. Engineers regularly receive no single rest period of 6 consecutive hours or more a day. STCW 17 VIII/1.1 STCW 17 A-VIII/1.2-9

14102 - The Oil Record Book Part I shall be completed on each occasion whenever collection of oil residues, as well as disposal occurs. ORB soundings of bilge water tank #12 and dirty oil tank #13 remain consistent at 1.00m3 and 0.80m3 since 24November2018 which contrasts with Chief engineer’s stated use of tanks. Soundings taken during exam were approximately 10% less than previous soundings in ORB, with no record of disposal or tank-to-tank transfer. MARPOL I/17.2.3

14104 - Oil filtering equipment referred to in this regulation shall be such as will ensure that any oily mixture discharged into sea after passing through the system has an oil content not exceeding 15ppm. Vessel’s oil water separator is inoperable. MARPOL I/14.6

01117 - Every ship of 400 GT and above shall be subject to a renwal survey to ensure that the structure fully complies with the applicable requirements. IOPP issued lists Sludge Tank as only oil residue tank on board. Oil residue tanks onboard are Dirty Oil tank #13 and Bildge Water tank #12. MARPOL I/6.1.2

Machinery space openings in position 1 or 2 shall be properly framed and efficiently enclosed by steel casings of ample strength. Other openings in such casings shall be fitted with equivalent covers, permanently attached in their proper positions.Circular opening in hull starboard aft quarter approximately 5" in diameter maintains a 6 bolt manifold cover leading directly into machinery space fitted with 2 bolts and no gasket. Cover left unsecured in port. ICLL 66/88 Annex I/Reg. 17(1)

After any survey of the ship under paragraph 1 has been completed, no change shall be made in the structure, equipment,systems, fittings, arrangements or material covered by the survey, without the sanction of the Administration. The crew is using a vacuum truck hose through an opening in the hull below the deck line approximately 40' to a pipe contained in the engine space to remove sludge from the bilge and involved tank. The vessel is fitted with a standard discharge connection on IOPP. MARPOL I/6.4.2

The angular position of the rudder shall be recognizable in the steering gear compartment. The rudder angle indicator differs from the navigation bridge by 4-6 degrees. 74SOLAS 2004 CONS. CH. II-1 29.11.2 16ac

Every ship 12m or more in length overall shall display placards which notify crew of the disposal and discharge requirements of regulations 3,4,5, and 6 of Annex V. No placards posted in the vicinity of the garbage separating station. MARPOL 2011 Annex Reg. 10.1.1 40ac

Every ship 400 gross tons shall be provided with a Garbage Record book which shall be in a form specified in appendix II to Annex V. Garbage record book is not in accordance with resolution MEPC. 277(70) MARPOL 2011 Annex V Reg. 10.3 40ac

No person may use any equipment for transfer operations unless the vessel tests the equipment. The transfer pipe system from oil and fuel bunkering has not been tested at 1.5 times the maximum allowable working pressure using liquid annually. 33CFR156.170(c)(4)

9800 - The arrangements for the storage, distribution and utilization of other flammable oils shall be such as to ensure the safety of the ship and person on board. Generator #3 is leaking fuel oil at a rate of 2 drops/second. Additionally, the crew has rigged a rubber hose from the fuel supply tank located above the generator to the generator’s integral tank. Furthermore, the crew has rigged a separate rubber hose from the generator #2's fuel injection line into the bilge that leaks fuel oil at a rate of 5 drops/second. 74 SOLAS (97 Cons.) II-2/15.4

0999 All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Protective sheathing is torn as it enters the transformer box located near the cargo workshop. A separate transformer box outside of the steering gear room is broken, exposing the internal wiring to the elements. The protective sheathing on the power cord to the grinder located in the steering room is frayed exposing the wires to the elements. Lastly, the hot water pipe in the steering room is leaking condensation onto an electrical outlet located below. 74 SOLAS (97 Cons.) II-1/45.1.3

0725 Maintenance, testing and inspections shall be carried out based on the guidelines developed by the Organization. The CO2 bottles have not been hydrostatically tested within the last ten years in accordance with MSC.1/Circ.1432. Their last hydro date was February 2008. 74 SOLAS (14 Cons.) II-2/2.2.1

9800 The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. There are two holes, each one inch in diameter, in the engine room common exhaust trunk which allows exhaust to leak back into the engine space. 74 SOLAS (97 Cons.) II-1/26.1

1710 The Oil Record Book (ORB) shall be completed whenever there is collection and disposal of oil resides (sludge). The Oil Record Book does not appropriately document the total amount of sludge retained on board in tank #13. Additionally, the capacity of the tank noted in the ORB does not match the International Oil Pollution Prevention Certificate. MARPOL (11 Cons.) Annex I/Reg.17.2.3

0820 The machinery shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts. The safety shield on the #1 generator is missing. 74 SOLAS (97 Cons.) II-1/26.1

The construction of all doors and door frames in “A” class divisions, shall provide resistance to fire as well as to the passage of smoke and flame. The door from the galley into the corridor and the door from the galley to the mess room are equipped with holdback hooks rendering them non “A” class divisions. 74 SOLAS (97 Cons.) II-2/30.2

1551 All ships 150 gross tonnage and upwards shall be fitted with a daylight signaling lamp using an energy source of electrical power not solely dependent upon the ship’s power supply. Ship was not supplied with a daylight signaling lamp complying with the regulations. 74 SOLAS (14 Cons.) V/19.2.2.2

1581 The angular position of the rudder shall be indicated on the navigation bridge. The readings for the rudder angle on the bridge differs between 5-8 degrees from the reading in the steering room. 74 SOLAS (97 Cons.) II-1/29.11.1

The required transfer procedures must be permanently posted or available at a place where the procedures can be easily seen and used by members of the crew when engaged in transfer operations. The vessel’s transfer procedures are not posted or in a location which can easily seen during operations. 33 CFR 155.740(c)

Oily water separator was found to have a temporary repair using flex hose. 74 SOLAS (2014 Cons) CH. I/Reg.11

Cargo ports and other similar openings in the sides of ships below the freeboard deck shall be fitted with doors so designed as to ensure watertightness. Rags are being used in place of gasket material on the port side shell door below the deck line.

Fire extinguishing medium, CO2, shall be stored in a room which shall be situated in a safe and readily accessible position and shall be effectively ventilated. Ventilation for the CO2 room is inoperable due to missing electrical supply fuses, rendering the space hazardous for entry.

All electrical cables and wiring external to equipment shall be at least of flame retardant type and shall be so installed as not to impair their original flame retardant properties. Two cables running through the engine room overhead have severe deterioration of the external jacket, exposing the internal shielding.

The liferaft container shall be marked with all of the required markings. The port and starboard liferaft container labels were found faded, torn, and illegible.

Self-igniting lights shall be capable of burning continuously or flashing at a rate of 50 flashes per minute. Port side self-igniting light does not operate.

The international shore connection shall be kept aboard the ship together with a gasket, four 16mm bolts, 50 mm in length, and eight washers. No washers are located with the international shore connection.

The condition of the ship & its equipment shall be maintained to conform w/ the provisions of the present regulations to ensure the ship in all respects will remain fit to proceed to sea w/o danger to ship or pers on board. A temp repair was made to H2O cooling lines #1 Gen using a soft patch w/ rubber & hose clamp.Flex joint cargo vent line broken ,fire detect DK2 cargo hold disconnected.

Port side forward deck 2 has a transformer temporarily wired and set on a metal shelf. Installation was not approved by the Administration and poses a safety hazard.

0750 - Fire fighting systems and appliances shall be kept in good working order and readily available. Fire hose on port quarter contains several holes and gashes - 74 SOLAS (2014) II-2/14.2.1.2 - 10C

1220 - The lines which indicate the load line assigned in accordance with these regulations shall be forward of the center of the ring. Vessels load line marking on the port side is not in accordance with certificate - ILLC 66 Annex I/Reg 6 - 16bc

1230 - Efficient guard rails or bulwarks shall be fitted to all exposed parts of the freeboard and superstructure decks. Port and starboard guard rails are missing in multiple locations. Bulwark for guard rail located in the bow thruster room is broken -ILLC 66 Annex I/Reg 25.2 - 16bc

0750 - Maintence testing and inspections shall be carried out based on the guidlines developed by the organization. High pressure CO2 system protecting the paint locker has not been serviced since 10JUN15. CO2 bottle no labeling and identification - 74 SOLAS(2014) II-2/14.2.2.1 - 16bc

Crew cut anchor leaving vessel with only one operable anchor. 74SOLAS (Cons. Ed. 14) CH. I/Reg 11 (a) Action taken: 16bc

The angular position of the rudder shall be recognizable in the steering gear compartment. During steering gear test the rudder angle indicator in the steering gear room stayed in idle position. 74 SOLAS (97 Cons Ed) CH. II-1/Reg. 29.2.3.11.2 Action taken: 16bc

FSS CH 5.2.1.2.1 The piping distribution of fire extinguishing medium shall be arranged so that a uniform distribution is obtained. CO2 is delivered to paint locker via a hose that cannot sustain high temperatures or high pressure. Provide permanent installation of piping that is steel or equivalent material.

ILLC Annex1 Reg8. Load line marking shall be permanently marked on the sides of ships to the satisfaction of the Administration . Load line is neither raised nor punched, just painted.

74 SOLAS CH II- 1/Reg 26(b): Cargo ships of less than 5000GT shall have a self contained emergency source of power which may consist of accumulator batteries. The emergency batteries are bubbling. Provide safe working emergency batteries that have adequate power to the appropriate equipment for at least 3 hours.

System unable to be activated due to low amperage in electrical system.

In respect of the regulations for cargo ships of less than 1,600 tons gross tonnage(as measured under national systems), the keels of which are laid after 31 Dec 1985, the administration may continue to apply the interim cheme until 18 Uuly 1994. Have class society review all SOLAS certificates or submit response as to why National Tonnage is being used on certificates instead of ITC.
