Except as provided by paragraph (f) of this section, each vessel 79 feet (24 meters) or more in length must be equipped with a fixed, self-priming, powered, bilge pump connected to a bilge manifold. Galley bilge and E/R suction operated intermittently. 46 CFR 28.255(c)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. Noted wire chaffing gear worn and disconnected from sewage pump station control box exposing conductors. 46 CFR 120.220(a)

A number of child-size life jackets equal to at least 10% of the number of persons permitted on board must be provided. 03 child-size life jackets had tears in them. Crew replaced with spares and marked satisfactorily. 46 CFR 117.71(b)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 in subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Alcohol swabs are expired on board. Crew provided valid ones to have on board. 46 CFR 122.212

Chalk test or hose test all water tight doors and hatches. Doors chalk tested 27JAN26- HZ Hatches to be replaced.

All electrical instantiations shall be to the satisfaction of the Officer in Charge, Marine Inspections. VSL reported unexpected loss of the port generator. Provide servicing report prior to carrying passengers. 46 CFR 183.01-15 (a)

Immediately after the addressing of resultant safety concerns, the owner, agent, master, operator, or person in charge, shall notify the nearest Sector Office, Marine Inspection Office or Coast Guard Group Office whenever a vessel is involved in a marine casualty consisting in- (4) An occurrence materially and adversely affecting the vessel's seaworthiness or fitness for service or route, including but not limited to fire, flooding, or failure of or damage to fixed fire-extinguishing systems, lifesaving equipment, auxiliary power-generating equipment, or bilge-pumping systems. 46 CFR 4.05-1 (a)(4)

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: "CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION." The entrances to the space in which the CO2 cylinders are stored do not have the required signage. 46 CFR 122.612(i)(2)

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. Marine employers must establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Vessel was unable to show enrollment in a Drug and Alcohol Testing Program. 46 CFR 16.201, 46 CFR 16.230

A vessel owner or operator required to have a response plan shall conduct exercise as necessary to ensure that the plan will function in an emergency. The following are the minimum exercise requirements for vessels covered by this subpart: shore-based spill management team tabletop exercises, which must be conducted annually. In a triennial period, at least one of these exercises must include a worst case discharge scenario; oil spill removal organization equipment deployment exercises, which must be conducted annually; and an exercise of the entire response plan, which must be conducted every 3 years. Vessel was unable to provide documentation of the required exercises listed above. 33 CFR 155.1060

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Each first-aid kit must meet the requirements in ISO 18813:2006(E) paragraph 4.12 (incorporated by reference, see § 199.05).(i) A first-aid kit may be considered acceptable if it meets all of the requirements of ISO 18813:2006(E) paragraph 4.12, except that it does not contain the burn preparations. It must be clearly marked on the first-aid kit that it does not include the burn preparations.(ii) The active ingredients in medicinal products must conform to over-the-counter (OTC) drug regulations set out in 21 CFR part 330. The first aid kit's inventory did not meet the requirements set out in ISO 18813:2006(E) paragraph 4.12. 46 CFR 121.710 46 CFR 199.175(b)(10)

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Vessel’s Bridge to Bridge Radio Certificate has not been updated to reflect vessel’s new name. 46 CFR 121.502 47 CFR 80.1005

Internal combustion engine exhausts, boiler and galley uptakes and similar sources of ignition shall be kept clear of and suitably insulated from any woodwork or other combustible matter. There is a two-foot section of missing insulation outboard of both the port and starboard generators exhaust system. 46 CFR 177.10-5 (b)

On a vessel of more than 65 feet in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. First and Second deck aft outdoor speakers are currently inoperable. 46 CFR 121.610 (b)

Vessels contracted for on or after July 1, 1961, which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H. Doors in stairway enclosures, other than those which are normally locked, shall be of the self-closing type. 03 doors in stairway enclosures failed to latch when released. 46 CFR 177.10-5(a), 46 CFR 72.05-25(b)(9)(i). 14 days.

Vessels contracted for on or after July 1, 1961, which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H. Doors in bulkheads required to be Class A-60, A-30, or A-15 shall be of hollow steel or equivalent metal construction solidly filled with approved structural insulation capable of meeting the requirements for a Class A-15 bulkhead. Door in the stairway leading up to the bridge had a hole penetrating the steel on both sides of the door. 46 CFR 177.10-5(a), 46 CFR 72.05-25(b)(1). 14 days.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Lifejacket stowage locations did not clearly separate child and adult lifejackets. 46 CFR 117.78(a)(5). 30 days

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Life buoy stowed aft of the bridge is deteriorated. 46 CFR 115.808. 30 days

Each vessel must meet the applicable requirements in Subchapter S of this chapter. Each hatch cover must be attached to the hatch frame or coaming by hinges, captive chains, or to other devices to prevent its loss. Hatch leading to steering gear room and to bow thruster space did not have a device to prevent loss. 46 CFR 177.13-1, 46 CFR 171.124(b)(2). 30 days

The exercise procedures section of the VRP must address the exercise program to be carried out by the vessel owner or operator to meet the requirements in 33 CFR 155.5060. The following are the minimum exercise requirements: (1) Qualified individual notification exercises, which must be conducted quarterly; (2) Emergency procedures exercises, which must be conducted quarterly; (3) Shore-based spill management team tabletop exercises, which must be conducted annually. In a triennial period, at least one of these exercises must include a worst case discharge scenario; (4) Oil spill removal organization equipment deployment exercises, which must be conducted annually; and (5) An exercise of the entire response plan, which must be conducted every 3 years. Vessel had no records of required exercises per vessel’s NTVRP. 33 CFR 155.5035(g), 33 CFR 155.5060(a), 33 CFR 155.1060. 30 days.

At each inspection for certification and at every other inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Can of fuel and open bucket full of oil were found in aft Engine Room compartment. 46CFR115.830(a)

Penetrations and openings in water-tight bulkheads must have means to make them water tight. Penetration in aft engine room bulkhead was open when dead end wires were removed. 46CFR171.114(a)(2)

Batteries shall be secured from shifting with motion of the boat. Small battery box in Bow thruster space unsecured.

Doors in A-Class fire control boundaries in stairways must not have hold backs. Fire door leading to the bridge was held open with a hold back floor stop. 46CFR116.435

Vessels of more than 64 feet must have at least one fire ax located in or adjacent to the primary operating station. Fire ax not properly mounted.46CFR118.600

The respective piping systems installed on ships and barges shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. Vessels vent pipe for STBD sea chest found leaking water. 46CFR56.01-1

Gears, couplings, flywheels and all machinery capable of injuring personnel shall be provided with adequate covers or guards. Vessels STBD MDE missing guards. 46CFR58.01

Non-metallic flexible hose must be in accordance with SAE J1942. Vessel 03 raw water hoses for the bow thruster engine have multiple hose lengths clamped together. Replace hoses.

Bail to the #3 and #4 station fire nozzle were broken. 46 CFR 181.15-10(d)(2) (OLD T)

Install fixed CO2 system in accordance with approved plans. Submit plans to the USCG Marine Safety Center for approval. Complete installation inspection with local OCMI. 46 CFR 118.410

Structural fire protection was observed missing from multiple locations throughout vessel's first and second decks. All electrical installations shall be to the satisfaction of the [name withheld] electrical wiring was observed behind first deck aft icemaker in contact with deck. "Old T" 183.01-15(a)

The construction and arrangement of a vessel must allow for the safe operation of the vessel, giving consideration to provisions for a seaworthy hull. Vessel's watertight door between engine room and galley did not properly seal when closed. 46 CFR 116.100

The master shall conduct sufficient fire drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. MOB, abandon ship, and fire drills were not observed during inspection. 46 CFR 122.520(a) 46 CFR 122.524(a)

Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Dead-end wires observed connected to to old grey-water tank level float. 46 CFR 120.200(b)

The installation of portable fire extinguishers must be to the satisfaction of the cognizant [name withheld] brackets for portable fire extinguishers located in the galley and engine room were of incompatible design to the portable fire extinguishers stored within them, resulting in damaged horns on both engine room CO2 portable extinguishers. 46 CFR 118.500

Vessels contracted after July 1, 1961, which carry more than 150n passengers shall meet the requirements of subpart 72.05 of Subchapter H of this chapter. Vessel was missing fire protection required by Table 72.05-10(d) in the bow thruster engine room as well as the bow thruster compartment. 46CFR177.10-5 (Old T)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld]/stiffeners on the aft water tank wall had cracked welds from the bulging of the water tank. Submit repair proposal to this office.
