Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. Vessel reported loss of generator power and port generator failure. Rectify within 14 days. 46CFR120.200 (a).

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck". MIs observed no emergency lighting in the aft stalls in the men and womens bathroom. Provide additional emergency lighting in the aft head. 46 CFR 120.432

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant OCMI". MIs observed a worn exit markings on the facade covering the watertight bulkhead in the forward male and female bathroom. Additionally, provide exit signage on both water tight doors to clearly articulate the direction and means of escape from restroom though Fixed water sprinkler system compartment to Main Deck.

"Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed." Vessel reported excessive wear on both stern drive units installed on the vessel. Prior to the carriage of passengers, the vessel must submit a tow plan for approval. Once approved, the vessel must only operate IAW approved tug assist plan during memorial day weekend and vessel must cease all operations when wind speeds exceed 25 knots. After 27 May 2024, the vessel must receive a full tech report and perform permanent, necessary repairs to both stern drives prior to carrying passengers. Vessel must perform satisfactory sea trial in the presence of CG MI prior to carrying passengers.

Each nozzle must be of a type approved in accordance with the approval series 162.027. Nozzle fitted on forward firehose was found leaking at the time of inspection

Nonconducting mats or grating must be provided on the deck in front of each switchboard. Vessel was found to be missing a section of the nonconducting mat in front of the switchboard above the fuel crossover.

Repair insulation in forward elevator shaft.

Install insulation missing from bow truster.

Repair damaged insulation in port engine room inboard of generator.

Secure hole at the deck on port and STBD side into engine room from vestibule with fire rated material to meet fire rating of bulkhead.

Replace damaged fire hose in lower galley.

Replace/ secure battery cover in forward STBD engine room.

Replace missing light cover in STBD engine room over stern drive.

Install blank in electrical panel in engine room vestibule in space #9.

Conduct satisfactory PSTP in the presence of Coast Guard Inspectors.

Electrical equipment on a vessel must be installed and maintained to protect passengers and the vessel from electrical hazards including fire caused by or originating in electrical equipment and electrical shock. -Myriad extension cords and daisy chaining of power strips found throughout vessel. -Broken ground prong found still in the galley outlet receptacle. Was found taped over. -Electrical box in stbd aft engine room next to the watertight door needs to be mounted securely to bulkhead -Replace all missing and broken light covers in port and stbd engine room -DP9 electrical panel cover does not close

120.340(g) Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Myriad wires splices made outside of an enclosure throughout the vessel.

120.370(b) Receptacle outlets and attachment plugs for portable lamps, tools, and similar apparatus operating at 100 volts or more, must have a grounding pole and a grounding conductor in the portable cord. -Multiple plugs for equipment throughout vessel with the grounding prong removed or broken. -Multiple portable heaters installed onboard vessel without grounding prong.

120.324(c) Ground detection must be provided that: For an alternating current system, meets §111.05-27 in subchapter J of this chapter; and For a direct current system, meets §111.05-29 in subchapter J of this chapter.Ground detection system for 480v, 120v, 24v DC, 12v DC circuits not operational

115.810(a) At each initial inspection for certification the owner shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment. Sprinkler water tank sight glass was not connected to the tank properly.

122.612 (i)(2) Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: “CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. CO2 label required on entrance into engine room hatch.

46 CFR4.06-1(e) The marine employer shall ensure that all individuals engaged or employed on board a vessel are fully indoctrinated in the requirements of this subpart. Crew not trained and unable to identify reporting requirements following a serious marine incident

115.802 Inspection and operation of all watertight closures in the hull, decks, and bulkheads including through hull fittings and sea valves; Inspection and operation of all weathertight closures above the weather deck Forward scuttle requires servicing and gasket replacement. Water intrusion is apparent in most forward scuttle space. Stbd hatch on stern requires servicing and gasket replacement.

116.700 Crew work space must be of adequate construction to provide for the safe operation of the vessel and the protection and accommodation of the crew in a manner practicable for service. Large deck plate in galley evidenced severe wastage to supports eliminating installation of the deck plate. Deckplate was found in the vertical position against the bulkhead and a temporary small piece of steel installed to enable transiting through the space.

115.818 At each inspection for certification and at every other vessel inspection, quarters, toilet and washing spaces, galleys, serving pantries, lockers, and similar spaces may be examined to determine that they are serviceable and in a sanitary condition. Cleanliness of bathrooms

PROPULSION MACHINERY MUST BE SUITABLE IN TYPE AND DESIGN FOR PROPULSION REQUIREMENTS OF THE HULL IN WHICH IT IS INSTALLED AND CAPABLE OF OPERATING AT CONSTANT MARINE LOAD. VESSEL REPORTED CASUALTY TO STARBOARD ENGINE. 46CFR119.200 REQUIREMENTS TO CARRY PASSENGERS AS FOLLOWS: 1. VESSEL MUST BE ESCORTED BY TUG WITH HORSEPOWER EQUIVALENT TO 10% OF CELESTIALS DEAD WEIGHT TONNAGE AT ALL TIMES WHEN PASSENGERS ARE ABOARD AND VESSEL IS UNDERWAY. 2. VESSEL MUST CONDUCT BRIEF WITH TUG PRIOR TO GETTING UNDERWAY ON PROCEDURES TO TAKE IN CASE TUG IS NEEDED. 3. VESSEL MUST CHECK OUT WITH VTS AND CHECK IN WITH VTS FOR ALL TRIPS.

Auxiliary generator for AC system suffered internal component failure during routine maintenance checks with no passengers onboard. Vessel self-reported failure. Vessel must submit a written and signed technical report addressing the following in order for this deficiency to be rectified: 1. Provide causal analysis of failure. 2. Provide identification of parts replaced/installed and statement that repair is complete. 3. Conduct satisfactory operational load test. 4. Provide statement that generator is fully repaired, fully functional and that a satisfactory load test has been completed.

Vessel experienced a hydraulic leak on port z-drive. Vessel rep to submit a tech report to USCG SEC NY attesting to sat repairs or have a stdby tug available 1900-2200 on 03AUG19 prior to carrying pax.

Demonstrate proper operation of vessels sprinkler system. 46 CFR 115.810

Provide current Safety Radio Telephone Certificate. Vessel is limited to 1000 ft from shore. 47 CFR 80.59(a)(2)

Observed numerous areas on the hull with pitting. Blast the following areas for further inspection: Port side outboard running along the chine up 3 feet toward deck line from midship to stern quarter, Starboard outboard midship section above the chine appx. 3'x4', and 3 sections along the keel running from bow to midship.

Vessel not ready for ISE. Clean out bilge in the fore peak tank for further inspection.

Not ready ISE. Clean out bilge in the galley space for further inspection. Bilge was obstructed with deck plates and oily water residue.

Remove excess hydraulic oil from aft machinery space (steering gear room).

Repair hydraulic oil leak from the starboard Z-drive system.

Crop and renew to good metal deck plate support bracket located on the starboard aft main deck.

Make chill cabin door A-0 and provide escape device on inside of door.

Demonstrate proper operation of 24V ground detection panel.

Place insulation in overhead of forepeak where missing.

Replace voltmeter for fire detection system batteries.

Replace gasket on inspection hole and sight glass on sprinkler system water tank.

Replace gauge on air compressor for sprinkler tank.

Prove proper ability of fire doors to close that were inoperable at time of inspection.

Replace emergency light in forward section of galley.

Replace exit sign in STBD female head.

Prove proper operation of galley hood fire extinguishing system. Until galley hood suppression system is fully operational, provide 01 fire watch for each deck in galley when equipment that has cooking surfaces with temperatures >250F are in use.

Vell;s port Z drive will not go into neutral, must not use this engine during operation unless needed for an emergency until fixed to normal operations, tech report received and proper operations verified by Coast Guard Marine inspector.

Vessel must have a plan/other vessels in stby while operating in this condition and will not be allowed to operate with passengers on Saturday until cleared by CGMI.

Smoke detector in the stbd aft female head is inoperable. Replace batteries in smoke detector.

The watertight door indicator at frames 41 and 42 for the stbd engineroom door is inoperable.

Port amidships fire safety door does not close upon activation. Ensure door properly closes.

Stbd outboard ovens in galley are not labeled. Label accordingly.

Grease hood is leaking grease; clean hood properly.

Write testing procedures for all alarms installed on z-drive.

Remove aresol paint cans from port engine room.

Replace both escape hatch gaskets for port and stbd engine room.

Fix ground on port side transformer and provide a report from a licensed electrician.

Repair forward bilge alarm in the galley.

Place tape on arrows of exit sign in aft head space.

Replace grab line on all ring buoys.

Passenger and crew limited to 356 passengers until damaged life jackets are replaced.

Replace battery and light on ring life buoy float light.

Make repair in kind to H/O return line iwo of Port Z-Drive service tank. Submit technician report to local [name withheld] All repairs must be made to the satisfaction of the attending Marine Inspector.

Centerline fire screen doors do not latch. Prove proper operation of FSD.

Aft fire hose holed. Replace aft fire hose.

First aid kit missing numerous items. Provide first aid kit supplied IAW 160.041

Labels on emergency escap hatche FWD secondary means of escape FWD storage, 2 in galley labels not clearly legible. Label IAW 122.606

Provide employee enrollment list for DAPI

Ground indicated on 75KVAammeter and 112 1/2 kva and 170 kva TXFR lights. Identify source of grounds and eliminate.
