Note: Damage associated with a Marine Casualty; therefore 'Self Reported' or 'Worklist Item' options are not applicable. More frequent inspections may be required, if necessary, by the Officer in Charge, Marine Inspection, to see that the hull, equipment and appliances of the vessel comply with the marine inspection laws, and the regulations of this subchapter and other subchapters where applicable. Damage was discovered below the waterline in the number one port void consisting of an approximate half inch in diameter puncture with two hairline fractures extending approximately one inch from the puncture.

Scantlings, material, and workmanship shall be at least equivalent to the requirements of ABS or other recognized classification society. The vessel sustained damage with a crack in the starboard bow rake. Temporary repairs were made and vessel is authorized transit to discharge and conduct permanent repairs. No other cargo operations are permitted until permanent repairs are made and cleared by a Coast Guard Marine Inspector.

RCT-5 was reported to have a pinhole in the # 4 port stern void. Make permanent repair to the above pinhole after offload of cargo to the satisfaction of an attending Coast Guard Marine Inspector. RCT-5 may not load cargo until repair is completed to the satisfaction of an attending Coast Guard Marine Inspector. Vessel may proceed to Bobby's yard at 3 mile after discharging cargo to be pulled out and affect repairs.

An offshore tank barge with a cargo capacity of 250 or more barrels that is carrying oil or a hazardous material as cargo and an inland tank barge with the capacity of 250 barrels that is carrying oil or a hazardous material as cargo must meet paragraph (a) of this section or be equipped with; (1) A coaming, at least 4 inches high but not more than 8 inches high, enclosing the immediate area of the cargo hatches, loading manifolds, and transfer connections, that has a capacity, in all conditions of vessel list and trim to be encountered during the loading operation, of at least one-half barrel per hatch, manifold, and connection within the enclosed area. The operator of a vessel with a capacity of 250 or more barrels of oil, or hazardous material shall provide transfer procedures that meet the requirements of this part. MI's unable to verify barge has transfer procedures. 33 CFR 155.720

Every tank vessel subject to the regulations in this subchapter shall be inspected every 5 years or more often, if necessary, by the Coast Guard to see that the hull, boilers, machinery, equipment, apparatus for storage, and appliances of the vessel comply with marine inspection laws, and the regulations. Vessel has lapsed on its required inspections. After receiving an application for inspection, the OCMI will inspect a tank vessel in his or her jurisdiction once every 5 years. The OCMI will ensure that every tank vessel is of a structure suitable for the carriage of flammable and/or combustible liquids in bulk and for the proper grade or grades of cargo the vessel carries while in service. Vessel past due on inspections therefore vessel is prohibited from operating and conducting cargo operations. 46 CFR 31.10-15

Vessel must have approved Vessel Security Plan. Vessel must provide approved VSP prior to cargo operations

Vessel currently has a 1/16" x 1/16" pinhole fracture in the bilge knuckle of the #4 port void, 7' aft of the #5 void watertight transverse bulkhead, vsl will make temporary repairs, offload cargo, and return to make permanent repairs once cargo transfer has been completed.

Vessel did not have means to test high/overfill alarms.

Open/close indicator on center cargo tank valve is inoperable.

Stud for vapor collection line missing.

Electrical cables to high level alarms not installed correctly (not intrinsically safe), shore tie alarm receptacle caps port and starboard missing.

6" fracture in stbd stern corner, 10" below gunwale.

Barge is not allowed to conduct cargo operations because CTI, ISE and Drydock is expired.

Provide required cargo piping diagram for the oil transfer procedures.

PROVE PROPER OPERATION OF THE CARGO TANK HIGH/OVERFLOW ALARM SYSTEM.

Vessel Response Plan did cover the COTP zone that the vessel is operating in. Issued CG-835 to either limit the barge to only those cargo that don't require a VRP or update the VRP.

Vessel operaters are to amend vessel cargo transfer procdures to comply with 33 cfr 155.750(d)

Make proper repairs and prove proper operation of vessel installed high level alarm/shutdown (overfill protection)

PROVIDE CARGO INFORMATION CARDS FOR ALL PERMISSIBLE CARGOES.

REPLACE CARGO HATCH SEALS ON #1 AND #2 CARGO TANKS.

OPEN THE VAPOR RECOVERY AND CARGO TANK VENT PIPING FOR INSPECTION OF THE ENTIRE PIPING SYSTEM IAW MARINE SAFETY MANUAL VOLUME II CHAPTER 27 SECTION 27.I.3.

VAPOR RECOVERY OPERATIONS SHALL NOT BE CONDUCTED UNTIL THE VAPOR RECOVERY SYSTEM HAS BEEN APPROVED BY USCG (G-MSC) IAW 46CFR39.10.13 AND THE COI HAS BEEN AMENDED TO AUTHORIZE VAPOR RECOVERY OPERATIONS.
