Note: Damage associated with a Marine Casualty; therefore 'Self Reported' or 'Worklist Item' options are not applicable. More frequent inspections may be required, if necessary, by the Officer in Charge, Marine Inspection, to see that the hull, equipment and appliances of the vessel comply with the marine inspection laws, and the regulations of this subchapter and other subchapters where applicable. Damage was discovered below the waterline in the number one port void consisting of an approximate half inch in diameter puncture with two hairline fractures extending approximately one inch from the puncture.

Scantlings, material, and workmanship shall be at least equivalent to the requirements of ABS or other recognized classification society. The vessel sustained damage with a crack in the starboard bow rake. Temporary repairs were made and vessel is authorized transit to discharge and conduct permanent repairs. No other cargo operations are permitted until permanent repairs are made and cleared by a Coast Guard Marine Inspector.

RCT-5 may not load cargo until repair is completed to the satisfaction of an attending Coast Guard Marine Inspector. Vessel may proceed to Bobby's yard at 3 mile after discharging cargo to be pulled out and affect repairs.
