AS APPROPRITE FOR INTENDED VOYAGE, A VESSEL MUST CARRY ADEQUATE AND UP-TO-DATE COAST GUARD LIGHT LIST AND ACCESS LOCAL NOTICE TO MARINERS. VESSEL HAS OUT DATED LIGHT LIST AND NOT MONITORING LOCAL NOTICE TO MARINERS.

Starboard Generator Raw Water intake hose is not of the reinforced type and is collapsed while in operation. Install proper reinforced hose. Vessel restricted to dockside operation only using the port generator until repairs are completed to the satisfaction of the Coast Guard.

Electrical equipment on a vessel must be installed and maintained to: (b) protect passengers, crew, other persons, and the vessel from electrical hazards. Top deck forward liquor storage area has two junction boxes that are not mounted.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Aft starboard life jacket container on third deck is missing jacket count labeling.

Each vessel must be equipped with at least two operable portable battery lights, one of which is at the operating station and the other at the entrance of the machinery space. Engine room portable light was missing.

Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected. Starboard main deck has a cracked window.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the person being transported. Small refrigerator in lower forward space is not secure.

Electrical equipment on a vessel must be installed and maintained to: (b) protect passengers, crew, other persons, and the vessel from electrical hazards. Refer panel 208V extension cord is damaged with exposed conductors.

Electrical equipment on a vessel must be installed and maintained to: (b) protect passengers, crew, other persons, and the vessel from electrical hazards. Both potable water pumps need to be mounted at the bases and are missing the pressure switch covers.

Portlight covers below the main deck must be fitted with hinged dead covers. Port side crew quarter closet is missing a deadlight cover.

Electrical equipment on a vessel must be installed and maintained to: (b) protect passengers, crew, other persons, and the vessel from electrical hazards. The outboard dryer receptacle has a broken outlet and an incorrectly sized faceplate.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] main piping in engine room is leaking. Two valves in fire main system are leaking (overboard discharge and top riser).

The Fire screen door leading to crew offices does not close completely. Adjust door to close properly.

Repaint the vessel draft marks with contrasting colors.

Conduct Fire, MOB, and MTSA drills in presence of attending Marine Inspectors.

An adult life jacket must be provided for each person carried on board a vessel. Vessel only had 411 out of the 420 adult life jackets required; provide 09 adult life jackets.

A fire hose with a nozzle must be attached to each fire hydrant at all times. The fire hose in fire station #3 was not connected to the fire hydrant; reconnect hose.

Spanner wrenches must be provided for each fire hydrant required by this regulation. Fire hydrant #2 did not have a spanner wrench; provide missing wrench.

Portable fire extinguisher must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the cognizant OCMI. The bracket for fire extinguisher #4 was not secured to anything; secure bracket in an appropriate location.

Except as otherwise provided in this section, rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks, that provide strength and support equivalent to fixed rails. Deck rails must include a top rail with the minimum height required by this section, and lower courses or equivalent protection as required by this section. The railing in the crew space lowest deck aft where the shore tie plugs are was missing an equivalent level of protection for the gap in the rail; provide equivalent protection for this space.

A crew accommodation space and a work space must be of sufficient size, adequate construction, and with suitable equipment to provide for the safe operation of the vessel and the protection and accommodation of the crew in a manner practicable for the size, facilities, service, route, speed, and modes of operation of the vessel. The deck hatch in the most forward lower deck crew accommodation space is loose; secure deck hatch.

Each connection to a conductor of terminal part of a conductor must be made within an enclosure. Remove unauthorized wiring located under the bridge console for the A/C system and install connection points inside a J-box/enclosure.

A vessel must carry either a first aid kit approved under approval series 160.041 or a kit equivalent contents and instructions. The First aid kit has expired ointment.

Electrical equipment on a vessel must be installed and maintained to protect passengers , crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Relay for emergency lighting located under the bridge console is chattering and need to be replaced. 46 CFR 120.310

The Main deck fire station located in the PORT side salon located inside cabinet needs to be labeled.

Electrical equipment must protect crew and persons from hazards. Properly terminate dead end with that originates from a junction box for the shore tie, midship on the transom in the lazarette. Additionally, Junction box has several openings on the top and is not protected from wet environments.

Electrical box located on the portside of ladder leading to Lazarette space is missing a knockout plug. Electrical equipment must be protected from wet environments.

The main deck hatch leading to the cooler space is missing the gasket and must be watertight. Install proper gasket.

Receptacles throughout the vessel are incorrectly wired with open grounds, polarity reversed and several missing covers. Electrical systems must be installed and maintained to prevent hazards to crew and passengers.

The following hoses are missing double clamps. Port SSDG R/W for wet exhaust, Port and Starboard SSDG R/W suction hoses, Engine Driven Fire/Bilge pump suction and discharge on Port MDE.

The following hoses were found dry rotted, split and in disrepair: Starboard SSDG R/W Flex hose to exhaust, Starboard MDE R/W Flex hose to MDE Exhaust and the Starboard MDE Flex hose to the tube/bundle cooler.

Electrical equipment must be wired IAW 46 CFR 120.340 and old t 46 CFR 183.10-20. Portable cords such as SO, STO or similar cannot be used for permanent installations. Remove cord that supplies power to the AC control panel located on the starboard side of the pumproom and properly wire. 46 CFR 120.340

Electrical equipment exposed to the weather must be watertight. An electrical outlet on the middle deck portside did not have a watertight cover; provide missing cover.

Battery connections for the starboard ship service generator are loose. Connections must be tight to prevent arching.

The VGP applies to discharges incidental to the normal operation of all non-recreational vessels of 79 feet or greater in length. The VGP requires vessel owners/ operators to perform training, inspections, monitoring, and reporting and take assessments and corrective actions as necessary. Ensure vessel is keeping retentions of Permit Authorization and Record of Inspection (PARI) forms and submission of annual reports to the satisfaction of attending Marine Inspector. Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port and Starboard main engine airsep systems have oil leaks present and require maintenance as per instruction. Perform airsep system maintenance as per instruction to the satisfaction of attending Marine Inspector. 46CFR182.05-1 Old "T"

All marine employers must submit drug testing program data required by 49CFR40.26 and appendix H to 49CFR40. Latest MIS Data Collection Form is not completely filled out and is missing signature. Provide evidence of current due MIS report submitted properly to DOT agency regulating vessel operations to the satisfaction of attending Marine Inspector and provide all records for drug testing that employers are required to keep as per 49CFR40.333. 46CFR16.500(a) 49CFR40.26 49CFR40.333

Provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment and reasonably safe from electrical shock of personnel. Vessel center food warming tray/counter system has exposed wiring, missing knobs, and missing indicator light covers. Restore food warming tray/counter system to original condition or make electrically safe, secure power, tag out, wire removal to the satisfaction of attending Marine Inspector. 46CFR183.01-5 Old "T"

Cooking and heating equipment shall be suitable for marine use. Stand up food warmer in galley corner has a broken temperature gauge and is pushed out of its mountings at the bottom. Repair/replace food warmer temperature gauge to proper working condition to the satisfaction of attending Marine Inspector. 46CFR184.05-1(a) Old "T"

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. The starboard bridge wing controlling has a missing stop push button and can allow water entry into the circuit enclosure. Provide weather-tight integrity to the enclosure box to the satisfaction of attending Marine Inspector. 46CFR183.01-15(a) Old "T"

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Aft engine room bulkhead has a pipe/hose penetration that is currently filled with a flammable polystyrene compound based foam. Remove polystyrene material and replace with suitable non-combustible type to the satisfaction of attending Marine Inspector. 46CFR177.10-5(a) Old "T"

All bulkheads and decks with A-60 rating shall be so insulated with approved structural insulation that the average temperatures on the unexposed side would not rise more than 250F above the original temperature,nor would the temperature at any one point, including any joint, rise more than 325F above the original temperature, within the time of 60 minutes. The overhead in the "old catering room" is missing sections of its installed insulation. Replace insulation to the satisfaction of attending Marine Inspector. 46CFR177.10-5(a) Old "T" 46CFR72.05-10(c)(1)

All doors, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. The release mechanism shall be so designed that the door will automatically close in the event of disruption to the control system. Doors throughout vessel that are designated separating structural fire boundaries, are missing their magnetic automatic closing abilities. Restore door operating systems to original as per MSC approval letter H2-00267 (16710/ALUM 331) dated 22MAR1990 or submit proposal to have doors normally kept closed with all hold backs removed to the satisfaction of attending Marine Inspector. 46CFR177.10-5(a) Old "T" 46CFR72.05-25(b)(9)(ii)

Due consideration will be given to hazards caused by the vessel's type of fuel, construction material, equipment, cargo, etc. The fire loads currently present in spaces below the main deck are out of compliance with what MSC has approved for Fire Load Calculations for the vessel examined 22MAR1990. Ensure current fire loads on-board vessel are in compliance with approved calculations from examined 22MAR1990 document. 46CFR181.01-5 Old "T"

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. Openings for cables/hoses at both bar areas and a closet area up forward on vessel has an opening in overhead for cables to pass through. Openings must be sealed to prevent quick spreading of heat/smoke/fire hazards to satisfaction of attending Marine Inspector. 46CFR177.10-5(a) Old "T"

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank shall be operable from outside the tank compartment. Starboard shutoff gets caught on deck plate when activated and port shutoff requires lubrication. Restore remote shutoffs to proper operation to satisfaction of attending Marine Inspector. 46CFR182.20-40(b)(3) Old "T"

Retroflection material required on life jackets must be Type I material that is approved under 46CFR164.018. Fifteen (15) life jackets were found with wrong retroflection material type or worn patches. Replace/repair retroflection material to satisfaction of attending Marine Inspectors. 46CFR180.25-25 Old "T"

Doors in bulkheads required to be Class A-60, A-30, or A-15 shall be of hollow steel or equivalent metal construction solidly filled with approved structural insulation capable of meeting the requirements for a Class A-15 bulkhead. Prove that glass doors at aft end of vessel are rated for A-15 or restore doors to originals as per approved plans. 46CFR177.10-5(a) Old "T" 46CFR72.05-25(b)(1)
