Vessel failed to demonstrate proper activation of the emergency stop for the generator serving the Thermal Fluid Heater. Vessel shall rectify prior to conducting cargo operations. 46 CFR 58.01-25

Manufacturers of pressure vessels and other components, which require specific fabrication inspection in accordance with the requirements of this sub-chapter, shall submit and obtain approval of the applicable construction plans prior to the commencement of such fabrication. During COI inspection , marine inspector found a lengthy (over 30 feet) flexible metallic hosing coiled around welded pipe with multiple threaded fitting connections. Vessel's representative stated such hose installation was installed for heat tracing purposes. Vessel's drawing attached to mid-body gauging report or piping diagram does not provide evidence that such installation was previously approved by the Coast Guard. Vessel shall submit plans to the Marine Safety Center for approval prior to the carriage on any elevated temperature cargoes. 46 CFR 50.20-1(b)

Vessel's cargo tranfer procedures and/or signs on board the vessel do not address the proper operational requirement for energizing or hooking up the shore tie cable to be completely de-energized (live) before connecting or disconnecting the shore tie from the receptacle as well as the operational procedure for barge to barge power transfer operation. Vessel shall have an adequate system in place, whether mechanical or procedural, to prevent possible electrical shock hazard when handling the shore tie plug on board. This shall be completed to the satisfaction of a Coast Guard Marine Inspector. 46 CFR 111.01-1(a)(2)

The CBC 328 barge was in tow headed from Pascagoula, MS to New Orleans, LA when the forward timberhead bitt sheared off the deck. Upon re-arrival back to Pascagoula, a damage survey was intended to be done. However, T/V [name withheld] and tow of two barges including the CBC 328 proceeded underway for Port Arthur, TX, Upon arrival to next port for discharge, the CBC 328 shall schedule a damage survey with the cognizant OCMI.

Barge authorized to conduct cargo offload then lay to TT-237 for permanent repair.

Permanent repairs are to witnessed to the satisfaction of resident Coast Guard.
