Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. Marine inspector observed missing handrails on the top deck. Installation of handrails to be made to the satisfaction of the TPO (ABS), submit TPO report to MSU Texas City.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. The alcohol testing device was last calibrated over one year ago. Vessel to provide TPO evidence of newly calibrated alcohol testing device with report forwarded to MSU Texas City.

The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck to prevent it from rolling under heavy sea conditions. MIs observed the semi-portable extinguisher was not properly secured. Deficiency was corrected on the spot.

Propulsion machinery including main engines, reduction gears, shafting and systems, must; be maintained to ensure proper operation. Inspectors observed port main engine in an inoperable state due to fractured camshaft mounting bracket.
