All machinery and equipment that is not in proper working order (including missing or malfunctioning guards or safety devices) must be removed; made safe through marking, tagging, or covering; or otherwise made unusable. Due to severe nature of deficiencies, vessel must submit transit plan satisfactory to OCMI prior to movement.

Towing vessels with a Towing Safety Management System (TSMS) must be operated in accordance with the TSMS applicable to the vessel. The technical and/or operational of the deficiencies documented below provide objective evidence of a serious failure, and lack of implementation of the vessel’s TSMS. An inspection satisfactory to the OCMI shall be carried out prior to clearance of this deficiency and release from detention.

The vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be questioned by the [name withheld] an in-service inspection, the forwardmost void, port side was opened and 26” of water was discovered in the space. In addition, there was a 4-inch hole discovered between this port void and the adjacent starboard void. The source of the water could not be identified. Evaluate the damage and make repair proposal.

The vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be questioned by the [name withheld] an in-service inspection, the aft void, port side was found to be sealed with silicone. The silicone was removed, the hatch removed, and the void was found to have 6 inches of water and waste petroleum product in the space. In addition, there were multiple deck frames that were wasted. The source of the water and petroleum product could not be identified. Evaluate the damage and make repair proposal.

In addition, there were multiple deck frames that were found to be wasted. The source of the water could not be identified. Evaluate the damage and make repair proposal.

In addition, there were multiple deck frames that were found to be wasted. The source of the water and diesel could not be identified. Evaluate the damage and make repair proposal.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Discovered approximately 6 inches of thick oil throughout the engine room bilge. Dispose of oil and clean engine room bilge.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel, and which may allow water to enter the vessel. These devices must be suitable for the expected route. Multiple void scuttles were found with wasted deck rings. The scuttle covers were not supported properly (wasted seating rings) and could allow the covers to fall through into the voids. Repair/replace the seating rings.

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Vessel had multiple pieces of lifesaving equipment arranged with carabiners, preventing said equipment from being readily deployed.

Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. Discovered multiple holes due to excessive corrosion through the steering space bulkhead. Found one freeing port was caulked closed and the other at the opposite quarter was left open. Crop and renew wasted areas.

The vessel’s pilot house jacking ram is leaking oil on the main deck creating a safety and potential environmental hazard.

Prior to getting underway for the first time on a particular towing vessel, each crewmember must receive a safety orientation. The pilot on watch at the time of the inspection had been onboard for two days. He stated that he had received the safety orientation, but it was not logged. Ensure training is properly conducted and logged.

The owner or managing operator must implement procedures to identify and mitigate health and safety hazards, including but not limited to; electrical. A junction box on the stbd side of the pilothouse under the console was missing its cover and had improper connections. Make proper connections and install cover.

A senor for the STBD main engine, inboard, had exposed wires hanging from the fitting with loose electrical tape. Install correct fitting for the sensor.

The port generator drive belt was missing, and wires were tangled in the pulleys. Remove wires, install belt, satisfactorily operate generator.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. A deck penetration in the forwardmost starboard void was found to have an unused electrical conductor still installed, and the opening was not sealed to prevent rain from entering the void. Remove unused wire and properly seal deck penetration.

a)The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. The navigation assessment must incorporate the requirements of pilothouse resource management of § 140.640, assess operational risks, and anticipate and manage workload demands. (b) The officer in charge of a navigational watch must keep the navigation assessment up-to-date to reflect changes in conditions and circumstances. This includes updates during the voyage or trip as necessary. At each change of the navigational watch, the oncoming officer in charge of the navigational watch must review the current navigation assessment for necessary changes. (c) The officer in charge of a navigational watch must ensure that the navigation assessment and any updates are communicated to other members of the navigational watch. (d) A navigation assessment entry must be recorded in the TVR, official log, or in accordance with the TSMS applicable to the vessel. The entry must include the date and time of the assessment, the name of the individual making the assessment, and the starting and and ending points of the voyage or trip that the assessment covers. Navigation assessments are not being documented to show all requirements to consider. Crew is not making updates when there are changes in conditions. Document navigation assessments in TVR or IAW TSMS.

Essential systems or equipment must be regularly tested and examined. Tests and examinations must verify that the system or equipment functions as designed. If a component is found unsatisfactory, it must be repaired or replaced. Test and examination procedures must be in accordance with manufacturer's instructions or the Towing Safety Management System (TSMS) applicable to the vessel, if the vessel has a TSMS. Crew is unfamiliar with operating parameters of equipment and is unaware of any manufacturer's or TSMS instructions they can refer to. Provide crew with required references for the testing and examination of essential systems and conduct training with crew on operating parameters.

The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness for the route or service of a vessel. In the course of the damage survey, additional wastage was noted in vessel's hull on the wear plate directly above vessels port wheel, on vessel's port strut, and around vessel's rudders. Vessel must ensure seaworthiness of vessel to the satisfaction of the attending Marine Inspector. TPO may be used for verification purposes.
