Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel experienced loss of flanking rudder steering. Troubleshoot, repair, and conduct operational test as required to the satisfaction of the TPO (TVIB).TPO shall provide notification of cleared deficiency within 72 hours IAW CG-CVC Policy Letter 17-10.

The officer in charge of a navigational watch must keep the navigation assessment up-to-date to reflect changes in conditions and circumstances. This includes updates during the voyage or trip as necessary. At each change of the navigational watch, the oncoming officer in charge of the navigational watch must review the current navigation assessment for necessary changes. Company policy requires a updated navigation assessment be logged at each change of watch. The 1800 navigation assessment was not completed until 21:41 (post casualty). Document navigation assessments as required.

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. Vessel policy requires all towing gear be inspected and results documented at crew change outs. Crew did not complete towing gear inspection as required. Complete towing gear inspections and complete record keeping as required.

Conduct an internal SMS audit in accordance with TSMS and implement corrective actions as required.
