Windows and airports must be fitted with frames of steel or equivalent material. Glazing beads or angles of steel or equivalent material must be installed to hold glass in place in windows or airports in a fire control boundary in event of a fire. Several windows were missing metal tabs to hold windows in place in the event of a fire. Replace tabs as needed to the satisfaction of the Marine Inspector.

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 lbs) applied at any point. Deck railing on the stbd side aft towards the paddle wheel is broke. Make repairs to the satisfaction of a Marine Inspector.

A door must be capable of operation from either side by one person. Doors in the dining room and fwd deck were inoperable. Provide evidence that the doors are operating as normal to a Marine Inspector.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. May 22, 2026 vessel reported that their coupling failed on the STBD engine. Vessel is allowed to transit to Richland, Washington to conduct repairs. Repairs to the coupling failure must be made to the satisfaction of the Marine Inspector prior to carriage of passengers.

April 23, 2026 Vessel reported inoperable starboard side z drive due to damage caused by fouling string of buoys. Vessel is allowed to transit to Umatilla, OR to conduct repairs, repairs must be made to the satisfaction of the attending Marine Inspector prior to carriage of passengers.

Whenever there is a hazardous condition, as defined by § 114.400 of this chapter, on board the vessel, the owner, master, agent, or person in charge shall immediately notify the Captain of the Port of the port or place of destination and the Captain of the Port of the port or place in which the vessel is located of the hazardous condition. Reports and photos from the vessel show a diesel fuel leak on the starboard tank. Vessel is authorized to perform temporary repair according to the submitted repair proposal. Provide proof of repairs to the satisfaction of the MI, ensuring no hazardous conditions exist to the vessel or environment. Furthermore, provide a permanent repair plan for the fuel tank.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. The company reported an allision with the McNary Lock and Dam while transiting upriver on the Columbia River. Pictures from the allision show evidence of damage to the rub rails and possible hull integrity on the starboard amidship main deck. Vessel shall make temporary repairs to railings to prevent man overboard incidents. Additionally vessel engineers shall conduct increased rounds. Vessel shall be made available for a damage survey with a Coast Guard Marine Inspector at the earliest oppotunity. *Permenent repairs must be affected prior to carrying passengers in the 2026 operating season.*

Vessel reported overheating of the Port Azimuth Thruster coupling Vessel moored at Port of Umatilla and conducted repairs to the coupling, tested operation satisfactory. Vessel is to provide a report and evidence of permanent repairs to the satisfaction of Marine Inspector.

Sector Columbia River received a report from the AMERICAN WEST that they experienced a loss propulsion due to a failure of the pump that services their day tank. You are to make repairs to the affected fuel pump and prove proper operation to a Marine Inspector. Complete prior to getting underway with passengers.

High risk service spaces (9) include - (5) Cleaning gear locker or small storeroom in an accommodation area; (a)(2)Bulkheads and decks must be classed as A-60, A-30, A-15, A-0, B-15, B-0, C, or C' based on the following: (i) A-Class bulkheads or decks must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads and decks. They must be so constructed that, if subjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. Table 116.415 (b) shows that high risk service spaces leading to high risk accommodation must be an A-60 boundary and to low risk accommodation A-15 boundary. In the crew accommodation spaces, attending marine inspector observed two accommodation spaces converted to storage space with linens. The converted spaces did not have doors or means of preventing spread of fire. Marine inspector observed this space conversion may potentially affect vessel tonnage. Reattach doors to the two storage spaces to ensure it meets boundary requirements. 46 CFR 114.400 46 CFR 11.415 (a)(2) 46 CFR 116.415 (b)

Suitable means, such as a manual damper, automatic damper, or vent cover, must be provided in an accessible location outside the space served by the ventilation duct for shutting off the passage of air through the ventilation duct in the event of fire. Fire damper in the aft engine room bulkhead was blocked open with a pry bar. This damper was found wired open during the MAR 30 inspection this year.

Each battery installation must be protected from falling objects. Install a cover for the top of the battery bank located on the port side of the auxiliary machinery space and repair the connection to the battery from the charger.

Significant rust scaling and rust of the hull plate found forward of the auxiliary machinery space off frame 10. It was also noted that 5 of the 6 intercostal longitudinal stringers were wasted beyond repair. Temporary repairs have been installed by welding temporary stiffeners and installing reinforced concrete patches in the areas of corrosion on the hull plate. Make permanent repairs to the shell plate and supporting structure around the sewage treatment tank in the axillary machinery space. Until this is completed. limit the filling of the chlorine tank to be done only by qualified personnel. A wide mouth funnel will be used to mitigate chlorine from getting into the bilge. The crew must make hourly rounds of the axillary machinery space. As proposed have a third party vendor conduct monthly UT soundings of the affected area while the vessel is in operation this season. Provide the results to MSU Portland.

The placarding on the main deck entrance to the elevator reflects six (6) persons as the maximum capacity allowed. The certification provided to the CG after the elevator was tested in April indicates the passenger elevator was tested to a maximum of four (4) persons or 1000lbs. Immediately properly Placard the elevator as indication on the test certification.

Starboard main propulsion drive is inoperable. Make repairs and conduct satisfactory underway test of the vessel's starboard main propulsion drive in the presence of a CG marine inspector.

Conduct a satisfactory man overboard drill in the presence of attending CG marine inspector.

Remove the excess flammable liquids from 04 deck areas to include paint thinner on top of port bridge wing battery bank.

Elevator is inoperable. Repairs must be made by a certified technician and operational test conducted in the presence of a CG marine inspector prior to using the elevator for passengers.

Ring buoy grab lines & float light lines require replacement on both port & starboard wheelhouse ring buoys.

Appropriately fill cable transit in lower machinery space overhead to space aft of scullery with USCG approved material.

Make proper repairs/splice to fluorescent light wiring under aft 04 deck covered area.

Make proper repairs/splices to dead end wire and capped wires under aft deck piano.

Replace broken cover on A/C electrical outlet located in 04 deck starboard storage closet.

Forward lounge door on main deck missing uppermost securing device.

Forward lounge windows, starboard side, are missing exterior securing clips.

Provide a valid FCC Communications Act Safety Radiotelephony Certificate and an FCC Vessel Bridge-To-Bridge Radiotelephony Certificate.

Prove proper operation of rescue boat's outboard engine.

Prove proper operation of both Z drives L/O flow and elvel alarms, provide tech report from ZF.

Replace missing lighting fixture cover in lower engine room under stbd main engine.

Replace plugs for port and stbd side fuel vent combing.

Remove box fans from galley ventilation

Make permanent repairs to leaks in center vertical keel cooler prior to 2016 passenger operations.

Ensure all main deck receptacle covers are watertight.

Repair as original and prove proper operation of emergency electrical system. System includes, but is not limited to electrical transformer, batteries, and inverter, supplying power for emergency lighting and navigation lights. Until this item is completed, vessel is restricted to operating underway only during daylight hours and must be moored to a shore side dock during hours of darkness.

Conduct repairs and prove proper operation of failed generator. To clear deficiency provide signed statement by master and chief engineer describing failure, repairs, and tests conducted to ensure proper operation prior to getting underway.

Vessel was transiting into a lock when the current turned the vessel causing the aft starboard side to come into contact with the wall. Aft deck had buckled and one handrail stanchion was broken close to the deck just forward of the paddlewheel cantilever.

Relocate all cable splices associated with the lower crew city head vent fans so that thay are all in proper enclosures (connector boxes) per 46 CFR 120.340(g) to be completed no later than 07Jul08.

Remount all connector boxes for the gray and black water lift pumps controllers (located in the lower crew city center bilge void) to there mounting brackets. Replace all broken boxes and reseal their covers. 46 CFR 120.340. To be completed no later than 07jul08

Provide proper mount supports for each crew head vent fan located behind the head FRP modules, so that the fans are no longer supported by the vent tubing and in several cases, by ropes attached to the overhead. 46 CFR 116. to be completed no later than 07Aug08

Provide CG Marine Safety Center approved plans to modify the ventilation system for the lower crew city space so that all crew head exhaust fans vent directly to the vessels exterior. Prior to departure from Astoria with passengers, provide a transit plan to OCMI/COTP for review and approval.

Provide written proposal for repairs to the stbd hydraulic unit piping. Welded repairs to piping assembly shall be inspected both internally and externally. A Hydrostatic test of 1.5 times the maximum allowable working pressure shall be conducted IAW 46 CFR 56.97-30 and to the satisfaction of the attending CG Marine Inspector.

Provide drawings for review and approval for all new fuel transfer piping, coolers, day tank, etc... in the generator room.

Remove or end in junction box dead ended improper wiring starboard side of main hydraulic tank behind starboard main engine.

Satisfy structural issues outlined in Marine Safety Center letter H2-0700936 of 27MAR07.

Received call from Master stating that the starboard main diesel engine experienced a large oil leak and was shut down. The Cummins Tech rep later found a seized piston and connecting rod. Prove proper operation of the engine prior to carrying passengers.

Vsl is to proceed on its current voyage from Snake River, WA to port of Longview, WA under its own power w/ tug escort, w/ 138 passengers & 46 crew members on board, per plan submitted to this office on 23AUG06. It is understood that vsl will be escorted by Shaver tug UMATILLA. Vsl will contact USCG Sector Portland IAW the comm plan outlined in submitted Req For Alt Vessel Itinerary plan.

Prove proper operation of the port side propulsion system upon completing all necessary repairs to the port paddlewheel gear box. Upon arrival in Longview, WA on or before 24AUG06, the vessel may not get underway with passengers until this item has been satisfied. This will require that the vessel proceed underway with marine inspector(s) on board for a trial run to test the repairs.

Make permanent repairs to leaks reported in the closed jacket water cooling system on the stbd main diesel engine. Upon arrival in Longview, WA on or before 24AUG06, the vessel may not get underway with passengers until this item is satisfied.

Make permanent repairs to the port generator. Until such repairs are completed, the vessel will operate with tug assistance with pasengers allowed as detailed in the plan submitted.

Provide proof of current FCC marine radio operator licenses for both masters. 47 CFR 80

Provide 7 additional Type I CG approved PFD's (to achieve a total of 217 on board). Vessel limited to carrying NMT 210 adults until cleared.

Prove proper operation of the port SSDG, to the satisfaction of the attending marine inspector. 46 CFR 120.310/120.312 No passengers may be carried u/w until requirement is corrected. As a condition of satisfying this requirement, the vsl should provide substantial findings to explain how casualty occurred.

Make permanent repairs to hydraulic line where leaking from a fractured weld, and make repairs available for CG inspection. This item must be completed prior to operation with passengers.

Lay to Clarkston, Washington under tug assist. Provide copy of technician's report indicating cause of failure and repairs made to correct steering casualty. Provide copy of statement from the Master indicating that all testing of the steering system (post repair) has been completed and found satisfactory. Until this item is cleared, vessel is to remain moored in the port of Clarkston, WA.

Provide fire servicing certificate.

Repair port generator belt guard IAW 46CFR177.960.

Install vapor barrier over insulation in all machinery spaces having a common boundary with accomodation spaces IAW 46CFR177.405(e).

HAVE NAVAL ARCHITECT SUBMIT PLANS TO THIS INSPECTION OFF DETAILING PROPOSEDPERMANENT REPAIRS TO CORRECT REOCCURRING CRACKS IN PROPULSION WHEEL HUB. UPON APPROVAL OF THESE PLANS, COMPLETE PERMANENT REPAIRS TO THE SATISFACTION OF ATTENDING MARINE INSPECTOR.

MAKE PEMANENT REPAIRS TO SHAFT PLATE WHERE ATTACHED TO PADDLEWHEEL ON THE STARBOARD SIDE. CONDUCT NON-DESTRUCTIVE TESTING TO WELDS ON PLATE ATTACHMENT ON THE PORT SIDE. CONDUCT SEA TRIALS AFTER ALL REPAIRS COMPLETE AND SUBMIT WRITTEN CERTIFICATION AS TO CONDITIONS OF REPAIRS AND SEA TRIAL.

Replace bolts in split flange couplings with a grade 8 bolt of the correct lengths and sizes, as indicated in SAE J518. Until bolts are replaced the master is to insure that a bi-weekly check of the torque on these bolts is conducted and logged.

Steam clean engine room to remove all oil residue created from blown seals from split flange couplings.

Install chaffing gear on flexhose and piping in the hydraulic system where making contact throughout the entire system.

Make adjustments to the Sinclear Logic Panel to eliminate loss of pilot house control on reset of alarm. A caution sign is to be installed to prevent reset of alarm until repairs are made.

Make adjustments and fine tune the controls for the main steering gear in the pilot house.

Make adjustments to the pilot house control system to allow control of engine RPM when adjusting paddlewheel speed. Until adjustments are made theengine control station is to be manned during manuvering evolutions. ie: entry into locks and in and out of port.

Install audible alarm to indicate loss of prt and stbd gear oil pumps or toindicate a non-start of these pumps upon main engine start.

Prove adequacy of ventilation for CO2 storage space to insure space temp does not exceed 130 degrees. Relocate natural vent for space outside of thegalley.

Install the locking mechanism on the port fire screen door from the galley into the dining hall.

Repair/replace and demonstrate operation of the general alarm bell in the Paddlewheel Lounge.

Repair/replace fitting to case drain on the stbd fwd hydraulic pump where leaking.

Prove proper operation of port and starboard overspeeds for the ship service generators.

Replace with proper grade material or test to the satisfaction of CG MSC, the 4 Cross Port Relief Valves.

The Paddle Wheel Lounge and the Dining Hall are restricted to passengers during high noise levels. These areas may be utilized with adaquate hearing protection and at reduced vessel speeds when noise level is lower.

Provide operational data to show that the main power generating system is capable of handling the ship service loads. Either by properly load shedding or automatic parrallel of generators during high load demands.

Conduct a noise survey of the following areas and install insulation as determined by the survey: 1) Paddle Wheel Lounge, 2) Dinning Area 3) Galleyand 4) Crews Quarters.

Ensure door to galley from the dining hall on the starboard side close properly.

Demonstrate that electronics for the starboard propulsion engine operates properly after installation of new electronics control unit.
