A door must be capable of operation from either side by one person. Doors in the dining room and fwd deck were inoperable. Provide evidence that the doors are operating as normal to a Marine Inspector.

Whenever there is a hazardous condition, as defined by § 114.400 of this chapter, on board the vessel, the owner, master, agent, or person in charge shall immediately notify the Captain of the Port of the port or place of destination and the Captain of the Port of the port or place in which the vessel is located of the hazardous condition. The company reported an allision with the McNary Lock and Dam while transiting upriver on the Columbia River. Pictures from the allision show evidence of damage to the rub rails and possible hull integrity on the starboard amidship main deck. Vessel shall make temporary repairs to railings to prevent man overboard incidents. Additionally vessel engineers shall conduct increased rounds. Vessel shall be made available for a damage survey with a Coast Guard Marine Inspector at the earliest oppotunity. *Permenent repairs must be affected prior to carrying passengers in the 2026 operating season.*

Vessel reported overheating of the Port Azimuth Thruster coupling Vessel moored at Port of Umatilla and conducted repairs to the coupling, tested operation satisfactory. Vessel is to provide a report and evidence of permanent repairs to the satisfaction of Marine Inspector.

Sector Columbia River received a report from the AMERICAN WEST that they experienced a loss propulsion due to a failure of the pump that services their day tank. You are to make repairs to the affected fuel pump and prove proper operation to a Marine Inspector. Complete prior to getting underway with passengers.

High risk service spaces (9) include - (5) Cleaning gear locker or small storeroom in an accommodation area; (a)(2)Bulkheads and decks must be classed as A-60, A-30, A-15, A-0, B-15, B-0, C, or C' based on the following: (i) A-Class bulkheads or decks must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads and decks. Table 116.415 (b) shows that high risk service spaces leading to high risk accommodation must be an A-60 boundary and to low risk accommodation A-15 boundary. In the crew accommodation spaces, attending marine inspector observed two accommodation spaces converted to storage space with linens. The converted spaces did not have doors or means of preventing spread of fire. Marine inspector observed this space conversion may potentially affect vessel tonnage. Suitable means, such as a manual damper, automatic damper, or vent cover, must be provided in an accessible location outside the space served by the ventilation duct for shutting off the passage of air through the ventilation duct in the event of fire. Fire damper in the aft engine room bulkhead was blocked open with a pry bar. This damper was found wired open during the MAR 30 inspection this year.

Each battery installation must be protected from falling objects. Install a cover for the top of the battery bank located on the port side of the auxiliary machinery space and repair the connection to the battery from the charger.

Significant rust scaling and rust of the hull plate found forward of the auxiliary machinery space off frame 10. It was also noted that 5 of the 6 intercostal longitudinal stringers were wasted beyond repair. Temporary repairs have been installed by welding temporary stiffeners and installing reinforced concrete patches in the areas of corrosion on the hull plate. Make permanent repairs to the shell plate and supporting structure around the sewage treatment tank in the axillary machinery space. Until this is completed. limit the filling of the chlorine tank to be done only by qualified personnel. A wide mouth funnel will be used to mitigate chlorine from getting into the bilge. The crew must make hourly rounds of the axillary machinery space. As proposed have a third party vendor conduct monthly UT soundings of the affected area while the vessel is in operation this season. Provide the results to MSU Portland.

The placarding on the main deck entrance to the elevator reflects six (6) persons as the maximum capacity allowed. The certification provided to the CG after the elevator was tested in April indicates the passenger elevator was tested to a maximum of four (4) persons or 1000lbs. Immediately properly Placard the elevator as indication on the test certification.

Starboard main propulsion drive is inoperable. Make repairs and conduct satisfactory underway test of the vessel's starboard main propulsion drive in the presence of a CG marine inspector.

Conduct a satisfactory man overboard drill in the presence of attending CG marine inspector.

Remove the excess flammable liquids from 04 deck areas to include paint thinner on top of port bridge wing battery bank.

Elevator is inoperable. Repairs must be made by a certified technician and operational test conducted in the presence of a CG marine inspector prior to using the elevator for passengers.

Ring buoy grab lines & float light lines require replacement on both port & starboard wheelhouse ring buoys.

Appropriately fill cable transit in lower machinery space overhead to space aft of scullery with USCG approved material.

Make proper repairs/splice to fluorescent light wiring under aft 04 deck covered area.

Make proper repairs/splices to dead end wire and capped wires under aft deck piano.

Replace broken cover on A/C electrical outlet located in 04 deck starboard storage closet.

Forward lounge door on main deck missing uppermost securing device.

Forward lounge windows, starboard side, are missing exterior securing clips.

Provide a valid FCC Communications Act Safety Radiotelephony Certificate and an FCC Vessel Bridge-To-Bridge Radiotelephony Certificate.

Prove proper operation of rescue boat's outboard engine.
