Vessel draft marks have been painted over. Each vessel must—(1) Have permanent draft marks at each end of the vessel. Paint draft marks on port and starboard on bow and stern to the satisfaction of the attending Coast Guard marine inspector.

Owner reported vessel has a leak in the KY side rake. No major repairs affecting the safety of the vessel with regard to hull, machinery or equipment shall be made without the knowledge and approval of the Officer in Charge Marine Inspections. Vessel shall proceed to dry dock and make repairs, prior to carrying passengers, to the satisfaction of the attending Coast Guard Marine Inspector.

On a vessel authorized to carry one or more vehicles, suitable chains, cables, or other barriers must be installed at the end of each vehicle runway. In addition, temporary rails or equivalent protection must be installed in way of each vehicle ramp, in compliance with the following: Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers and crew and must be be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. Please consult with this office before installation to ensure that it meets the intent of this regulation.

Fire pump must be fitted with an operable pressure gauge.

Each stowage container for life jackets must not be capable of being locked. Remove locking mechanisms or provide written procedures for crew that ensures the locks are always removed before the beginning of daily operation.

Placards containing instructions for the donning and use of the EVERY TYPE of life-jacket aboard must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers.

Wiring joints and splices shall be mechanicaly secure and made in a junction boxes IAW 46 CFR 183.05-50 (Old "T")

Replace hose on portable bilge pump IAW 46 CFR 182.25-5

Provide and properly install captive chains in way of all deck hatches for the void spaces to the satisfaction of an attending Coast Guard Marine Inspector.

Provide a means to accurately count all passengers on board the vessel prior to each voyage. The passenger count must be communicated verbally or in writing and available on shore in the event of an emergency

Make repairs and demonstrate proper operation of the stern navigational light on the Ohio end of the vehicle ferry to the satisfaction of an attending Coast Guard Marine Inspector. Vessel is not permitted to operate outside of daylight hours and in periods of restricted visibility until corrected.

Provide and install a special flashing light on the forward end of the vehicle ferry barge to meet the requirements of the Inland Navigation Rules, Rule 24, paragraph F(ii). The light must flash between 50-70 times per minute and have an unbroken arc of not less than 180 degrees and not more than 225 degrees.

Provide retro reflective tape in way of all three life ring buoys to the satisfaction of an attending Coast Guard Marine Inspector.

Remove to good metal and replace vertical stiffeners on collision bulkhead (Ohio end) void #1.

Remove and replace two (2) floor frames in void #2 closest to Kentucky end.

Identify and repair cracks in overhead lap weld in all voids. Suggest making a complete weld instead of skip weld.

Back gouge or drill stop and weld crack in deck top of #4 void.

Remove and replace wasted floor frames as marked in void #4 closest to Kentucky end.

Remove to good metal and replace over head deck stiffener in void #4 as marked.

Remove and replace wasted over head deck stiffeners as marked in void #5.

Back gouge or drill stop hole in Kentucky end collision bulkhead and weld.

Remove and replace wasted frame near pulley mount in void #6.

Adjust or modify cable penetration in void #6.

Alternate all clips in way of the wire rope that is used to lower and raise the vehicle ramps.

Provide retro reflective tape in way of all three life ring buoys to the satisfaction of an attending Coast Guard Marine Inspector.

Provide and post a copy of the stability letter under glass in the wheelhouse of the LITTLE BOONE and ensure all pages of the stability letter are visible.

Provide retro reflective tape for a total of 16 adult life jackets and 2 children's life jackets. In addition make proper repairs to the torn seam on one adult life jacket.

Make suitable repairs (plug holes) in all three life ring buoys to prevent penetration of water that would compromise the buoyant material.

Replace the wasted lanyard attached to the floating water light on one of the life ring buoys. Lanyard should be a minimum of 3 feet and no more than 6 feet long, made of a dark color if synthetic or be resistant to Ultraviolet light.

Remove the plastic wire tie securing the electrical wire that supplies electrical power to the navigation lights from the LITTLE BOONE. The wiring is secured to the pad eye on the swivel of the barge.

Provide captive chains for all watertight hatches of sufficient strength to prevent its loss.

Bulkhead veriticals and framing around port and starboard areas of ramp cable ways are in need of attention in the collision bulkhead in the forward void (Kentucky End). Owner will provide a repair plan, work can be accomplished dockside.

Pits along starboard side shell water line need cleaned and welded.

4 bottom longitudinal frames are wasted and need replaced in the forward of the collision bulkhead.

Make permanent repairs to broken welds on the first, second and third transverse deck frames from forward bulkhead in third void. LEGACY COMPONENT DESC: Transverse Frame

Make permanent repairs to holes in deck around starboard hatch in number six void. LEGACY COMPONENT DESC: Hull Plating

ON M/V LITTLE BOONE, INSTALL SAFETY (COTTER) PINS TO PROPERLY SECURE CROSS-OVER STEERING BAR JOINT PINS, NOT LATER THAN THE COMPLIANCE DATE.

PROVIDE ONE 50' X 1 1/2" LENGTH OF APPROVED FIRE HOSE WITH A NOZZLE ATTACHED, NOT LATER THAN THE COMPLIANCE DATE. THE NOZZLE CAN BE EITHER A 1/2" SOLID STREAM TYPE, OR A COAST GUARD APPROVED COMBINATION NOZZLE.

REWELD ALL FRACTURED WELDS WHERE UNDER-DECK FRAMES ATTACH TO DECK IN ALL VOID COMPARTMENTS, NOT LATER THAN THE COMPLIANCE DATE.

ON M/V LITTLE BOONE, INSTALL "MARINE TYPE" TOWING LIGHT FIXTURES, NOT LATERTHAN THE COMPLIANCE DATE.

INSTALL FIXED BILGE PUMPS AND PIPING SYSTEM IN ACCORDANCE WITH 46 CFR 182.25 NOT LATER THAN THE COMPLIANCE DATE.

INSTALL SIDELIGHTS AND STERNLIGHT TO COMPLY WITH INLAND NAVIGATION RULE NO.24: "LIGHTS FOR VESSELS BEING TOWED ALONG SIDE", NOT LATER THAN THE COMPLIANCES DATE.

PROVIDE FIRE PUMP WHICH HAS MINIMUM CAPACITY OF 60 PSI AT THE PUMP OUTLET NOT LATER THAN THE COMPLIANCE DATE.

REPLACE AFT NAVIGATION LIGHT WIRING WITH UL LISTED STRANDED CONDUCTOR TYPE,AND ENSURE CONNECTIONS ARE WATERTIGHT NOT LATER THAN THE COMPLIANCE DATE.

REPAIR/REPLACE THE HAND FIRE/BILGE PUMP AS NECESSARY.

INSTALL ADDITIONAL GUARDS IN THE RAIL OPENINGS ADJACENT TO THE ACCOMPANING TOWBOAT.

PROVIDE ON BOARD THE VESSEL TWO MORE 24" APPROVED, PROPERLY-MARKED LIFE RINGS.

THE VESSEL IS RESTRICTED TO THE CARRIAGE OF 28 ADULTS UNTIL TWO ADDITIONAL LIFE JACKETS ARE PLACED ON BOARD, INSPECTED, AND STAMPED.

RUN CONTINUOUS WELD ALONG TOP OF COLLISION BULKHEAD AT KENTUCKY END OF VES-SEL.

MAKE PERMANENT REPAIRS TO TWO SMALL HOLES THROUGH HEADLOG STRENGTH MEMBER AT OHIO END APPROXIMATELY TWELVE INCHES (12") FROM WEST CORNER.

MAKE PERMANENT REPAIR TO SPLIT SEAM IN DECK KNUCKLE ON EAST SIDE OF OHIO CENTER COMPARTMENT BETWEEN NO. 2 AND NO. 3 FRAMES FROM CENTERLINE BULKHEAD.

REPLACE ALL THE PIVOT PINS ON BOTH LOADING RAMPS. THE VESSEL IS RESTRICTEDTO THE LOADING OF VEHICLES WITH A GROSS VEHICLE WEIGHT OF LESS THAN 12,000POUNDS UNTIL THESE PINS HAVE BEEN REPLACED.

INSTALL AN ACCEPTABLE SAFETY SYSTEM TO PREVENT THE BARGE FROM BREAKING AWAY FROM THE TOWBOAT.

PRESENT PLANS TO THIS OFFICE FOR THE DESIGN OF A SAFETY SYSTEM TO PREVENT THE BARGE BREAKING AWAY FROM THE TOWBOAT.
