BOONE NO. 9

During annual inspection 15 life jackets were found in poor condition. An adult size life jacket must be provided for each person carried on board a vessel. In addition a number of child size life jackets equal to 10% of the number of person permitted on board. Replace old life jackets with ones that are in satisfactory condition for service to the satisfaction of the attending Coast Guard marine inspector. Vessel shall reduce passenger count from 140 passengers to 120 passengers

Four life jakets were sun bleached, dry rot with hole. Replace to the satisfaction of the attending Coast Guard Marine Inspector.

6 life jackets were sun bleached or torn. Failure to comply with life jacket requirements Replace life jacket to the satisfaction of the attending Coast Guard Marine Inspector.

On a vessel authorized to carry one or more vehicles, suitable chains, cables, or other barriers must be installed at the end of each vehicle runway. In addition, temporary rails or equivalent protection must be installed in way of each vehicle ramp, in compliance with the following: Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers and crew and must be be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. Please consult with this office before installation to ensure that it meets the intent of this regulation.

Vessel was unable to get underway to preform man overboard drill to verify crew competency.

BOONE NO. 9 is restricted to daylight operations only as a result of inoperable navigational lights on the barge.

The BOONE NO. 9/DEBORAH A suffered a subsequent loss of propulsion in as many days with passengers on board. Prior to placing the vessel back in service and carrying passengers, provide a written technicians report detailing the cause of the engine failure. Upon completion of repairs and prior to carrying passengers, all repairs must be evaluated by an attending Coast Guard Marine Inspector.

Evaluate the cause of the loss of propulsion that occurred on August 22, 2014 while the vessel was pushed up to the Ohio Landing. Prior to carrying passengers, an attending Coast Guard Marine Inspector must inspect the completed repairs and conduct an underway evaluation to determine the suitability of the vessel's propulsion system.

Provide a "NO SMOKING" sign on the Ohio end of the ferry. There is currently only one "NO SMOKING" sign on the vehicle ferry on the Kentucky end but is not visible to all passengers.

Provide appropriate signs to inform all vehicle operators to turn off their motors and set their parking breaks at all times when the ferry is underway.

Replace all bilge suction pick-up valves with a suitable valve and strainer that is at least three times the nominal pipe size in accordance with old "T" 46 CFR 182-25(b).

Replace the worn and illegible emergency instructions with new ones and ensure the emergency instructions meet the content of 46 CFR 185.512.

Provide a new first aid kit or replace all expired medication s ensuring the first aid kit has all the required contents and directions for use in accordance with 33 CFR 160.041.

Properly mark all three (03) life ring boys with the vessels name. All markings are faded and illegible at the time of inspection.

Provide two (02) additional approved adult PFD's. Total count on board was 138 and the required equipment on the COI is for 140 for the total persons onboard.

Demonstrate the operational readiness of the fire main system including the ability of the crew to remove the cover for the fire pump while the vessel is underway, the suitability of the connection of the fire hose to the hydrant in its current arrangement and training of all crew members in the operation and use of the fire pump to the satisfaction of an attending Coast Guard Marine Inspector.

Remove scale from bow and stern voids.

Properly install bulb assembly in Kentucky side starboard running light.

Replace life jacket with torn webbing.
