Captain reported vessel's port main engine turbo had an oil leak and caught on fire while underway. The ITV BTU SPECIAL towed the VIVIAN MCGINNIS back to vessel's home port at OHR MM 480. All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquids. Conduct root cause analysis and identify source and cause of fire. Make repairs prior to moment to the satisfaction of the attending RO.

Master failed to implement TSMS pre-underway checklist whereby the Automatic Identification System (AIS) was not transmitting so that the vessel location could be tracked by other commercial traffic in the area. The nonconformity of not implementing TSMS procedures and not using pre-underway checklist, resulted in a collision between VIVIAN MCGINNIS and JAMES GARRETT that let to an estimated $96,000 of damage. The management must demonstrate that they implemented the policies and procedures as contained in the TSMS and the entire organization is adhering to the safety management program. Provide root cause analysis of the nonconformity and correction report to the satisfaction of the attending Coast Guard marine inspector.
