A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. The vessel reported water ingress from an unknown source in STB inner-bottom. Water was leaking through vent of inner-bottom tank into the engine room bilge. Divers discovered no significant hull damage/ moderate temp repairs were made. Vessel is to proceed to National Maintenance Shipyard, UMR MM 196 to determine cause/source of water ingress and make permanent repairs as needed. During transit vessel must adhere to submitted transit plan. If conditions change during transit notification must be made to the nearest USCG unit. ***These deficiency must be cleared to the satisfaction of a Coast Guard marine inspector prior to returning vessel to service. In lieu of Coast Guard attendance, the company may use their Third Party Organization, ABS, to accept/approve the repairs and provide a report to the Coast Guard inspector listed below.****

The ITV MARK DULEY was operating at 828.1 MM OHR when a loaded hopper barge broke free from the fleet and collided with the ITV MARK DULEY on the port bow leaving a inset and fracture in the forward transverse ballast tank opening the hull for water ingress. A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Vessel shall proceed to Paducah KY for permanent repairs by Saturday December 2, 2018 to the satisfaction of the attending Coast Guard marine inspector.
