A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. During ballast water discharge from the #1 starboard ballast tank a sheen was observed from the discharge. Vessel is allowed a one time transit to Zug Island to discharge cargo with out conducting ballast operations. After discharging cargo vessel can transit to Nicholson Ecorse Terminal to conduct assessment and identify cause of oil in ballast water. After assessment vessel must conduct permanent repairs to the satisfaction of the local OCMI prior to departure. ***Amended CG835*** Allows ballast operations except for the #1 starboard tank.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this subchapter. Vessel reported an issue with use of the port MDE related to the control bus communication module. Vessel is authorized to transit to Zug Island, MI and shall contact Sector Detroit OCMI to the status of the port MDE prior to departure. 46 CFR 46 CFR 58.05-1

The vessel must comply with the Sulfur Oxide (SOx) emission limits specified in Regulation 14.4.3 of MARPOL Annex VI, as required under 33 USC 1903 (b)(2) by the required date (MARCH 2028). All conversion or modification work necessary to achieve compliance shall be conducted under the oversight of the Officer in Charge, Marine Inspection (OCMI), with plan review performed as appropriate.

Outboard side of the rescue boat is extremely oxidized and no longer international orange. Onboard flares are not stowed properly and are wet inside bag.

Company created Oil Record Book that is not in compliance with CFR. Bottom of page should be completed and signed by Master and not Chief Engineer.

Forward pilot house exhaust and supply motors have been removed. Ships crew is currently leaving the watertight doors and fire screen doors open to create natural ventilation which is not affective.

During the testing of the vessels engine room CO2 fire suppression system the systems main valve failed and remained stuck in the open position. The inoperable valve in the CO2 system allowed agent to bypass the time delay and alarms. Repair or replace the valve and have the system re-tested in the presence of the attending USCG Marine Inspector.

Durning the inspeciotn/testing of aux boiler, the outboard safety did not open at or prior to MAWP. Inboard safety was operating properly. Repair or replace to the satisfaction of attending CG Marine inspector. 46CFR54.15-10

Fire and Smoke detection panel funtions but only displays locations on the main panel in the passageway. Prior to departing Detroit, MI prove prover operation of location functions at all stations.

Vessel reported fracture between #2 and #3 starboard water ballast tank at the 16'level. Vessel to have repairs carried out to the satisfaction of class and USCG at earliest opportunity but not to exceed 2 weeks. Please keep USCG informed of repairs and class attendance. Cite: 46 CFR 92.01

The fire main has a pin-hole leak that does not diminish the pressure required for the system. Repair faulty section within 14 days to the satisfaction of the Coast Guard or Class.

Hairline crack discovered in watertight door frame at frame 117 on starboard walk tunnel.

An approximate 5" crack between the #6 & #7 ballast tank was discovered. Vsl plans to affect repairs on its next voyage into Detroit or Toledo. CG 835 issued for 14 days requiring a report from class attesting to repairs.

the #2 cylinder on the stbd main was reading faulty temps resulting in the pitch on the propeller reducing. While at acnhorage the rack on the system was replaced and temps are reading normal.

during routine inspection of ballast tank, the crew found an 8" crack between the port side #6 & 7 ballast tanks.

Make repairs to 3 inch crack in bulkhead dividing ballast tanks #2 and #3 on the starboard side.

5JUL19 the vessel lost partial power due to a shaft generator breaker opening unexpectedly. The suspected cause is bad contactors in the unloading switch board for the stern thruster. The contactors appear excessively worn which may have resulted in phase issues causing the shaft generator breaker to open. These bad contactors are to be replaced and proper operation witnessed by an attending CG Marine Inspector before the stern thruster may be placed back into service. The stern thruster is to remain locked out and tagged out until this 835 is cleared. The vessel may continue service during this time. However, in instances where a stern thruster may be deemed necessary by the Master for the safe navigation of the vessel appropriate tug assistance must be provided. 835 due date is 4SEP19.

Several breakers on the vessel have opened without a known cause which has caused unexpected losses of power to various systems. There is an apparent issue that must be resolved to ensure continued safe operation. Appropriate servicing technicians are to evaluate the ships electrical system and identify the cause(s) of these failures. Proper repairs are to be made and proper operation witnessed by an attending CG Marine Inspector. This vessel has a periodically unmanned engine room (ACCU). Until the root cause of these failures is found and repaired the engine-room must be manned with at least one licensed engineer when the vessel is in restricted waters or in close quarters navigation situations. This is in addition to times the Master or Chief Engineer feel it is necessary for safe operations. 835 due date is 4SEP19.

ire extinguishing equipment may remain in service so long as it is maintained in good condition. During monthly SAP inspection, vessel had an accidental release of CO2 in the bow thruster room emptying the cylinder. Provide fully charged CO2 cylinder in bow thruster room prior to departure from Toledo, OH.

Vessel failed to submit ANOA within 24hrs of ETA.

Upon discovery of a failed nitrogen accumulator unit for the rescue boat davit hydraulic system, the davit was temporarily fitted with a manual crank actuator (from sister vessel) for the purpose of enabling the vessel to make underway at completion of fit-out. Make permanent repairs to the davit arm, restoring it to original condition, and demonstrate satisfactory operation. Tests must be witnessed by USCG prior to departure from port where repairs are made.(46 CFR 199.45 (c)(2

Contact MSU Toledo with repair plan to fix crack in number 1 ballast tank between ballast tank #1 and #2, and conduct proper repairs.

Make temp repairs to weeping rivets (coupling - as discussed with attending Fleet Engineer [name withheld]) identified in #7P ballast tank. (5 rivets total; located in bottom rivet seam in way of inset floors)

Clean mud out of #8P ballast tank and make ready for inspection of possible damage.

Crop and insert damaged sections of floors at frames #170, 171, 172, and 173 where inset in #9P ballast tank.

The flexible coupling (Vulkan Torsional Vibration Damper) on the stbd ME/red-gear has failed. Make satisfactory repairs. Vessel authorized to operate with one main engine only in accordance with the USCG Ninth District Instruction 16710.1 (Guidance for Vsl's Operating with Reduced Propulsion). (If vsl continues on 1 engine only for more than 1 week, addn'l cond's will be applied via CG-835).

Crop and insert identified sections of remaining damaged floors (from frames #125-143) of #7 P ballast tank, that were not addressed for repair by 15JUL14.

There is a gradual inset in way of the bottom bilge turn, riveted seam, of the the #8P ballast tank (entire length). All bilge floors are buckled in the same vicinity. Crop and insert identified damage in deep web frame bilge floors (4 total).

Crop and insert damaged bilge floor frames identified in the #8P ballast tank in way of the gradual inset of the bilge turn. These floor frames lie between FRs 143 and 163 (Deep web frame bilge floor damage previously addressed in item #9).

Complete permanent repairs to firemain leak near frame 33 in port walk tunnel, to be completed prior to departing Toledo, OH and before the compliance date, to the satisfaction of the cognizant OCMI.

Provide tech rep report concerning failure of the starboard shaft generator and load sharing program failure.

Stbd shaft generator is limited to open lake and inport operation until further testing is completed.

Provide local start/stop controls for steering gear pumps in steering gear room.

Repair leaking steering gear automatic actuating hydraulic valve.

Issued requirement to: Renew 1 ½" Tee fitting in port boiler bottom blow piping.

Issued requirement to: 2. Renew ½" weldlet in steam supply piping to #1 SSTG prime mover just before steam stop. Due date: 2011 sailing season or when main steam plant is taken off line, whichever comes first.

Ensure emergency escape hatch can be opened from the ladder (Counter spring beyond adjustment to offset weight of hatch).

Noted (5) General Alarm bells removed from the portside working tunnel, (3) operational General Alarms remain on portside. Prove adequate audible coverage during unloading operations to the satisfaction of the attending Marine Inspector IAW 46 CFR 113.25-9.

Incident of Certification Inspection after an extended layup period and dry-dock, completed full COI inspection with review of ICRs. Revise the following ICRs for the appropriate references and inspection procedures: ICR #s: G-04, E-01, B-05, E-02, and B-09.

Amend, update, or tailor the following ICRs as discussed: L05 C, L08 C, L08 F, M01 F2, M11 D&E, M16 A2, M16 B2, M17, P01 F, P10 14-16, 02B A3 & A7, 02B B2-6, Q3 A2-6, Q3 B5, and Q6.

Amend/update Periodic Safety Test Procedure (PSTP) to include new equipment/required equipment as discussed. NVIC 1-69, 46 CFR 61.40-1 (e.g. include new monitoring equipment, add to power failure that when shifting back to normal power burners shutdown, previous approved procedures includes low F/O temp (required in NVIC 1-69) yet no such sustem is currently installed, etc.

Incinerator control box is wasted through. Renew with enclosure of appropriate NEMA Type for weather deck location. 46 CFR 111.70-3

Make proper repairs to three electrical cords, outer sheathing found separated from male plug (exposing wire), located in aft laundry space IAW 46 CFR 111.60-11.

Ensure emergency escape hatch is watertight, located on main deck between cargo hatches #09 and #10 IAW 46 CFR 45.135.

Install proper warning placard outside the lamp locker, a CO2 protected space adjacent to the forward paint locker IAW 46 CFR 97.37-9.

Low water alarm and low-low water limit did not activate while water level was still visible in the sight glass. Adjust/modify system as needed to have water level alarms and limits activate with water in the glass. NVIC 1-69, 46 CFR 62.10-1(a).

Numerous weather deck flood lights installed with long lenghts of flexible cord permanently installed in wire runs and attached hand rails (e.g. stack flood light for viewing smoke.) Provide proper electrical service to lights or remove. 46 CFR 111.60(b)

Provide and install USCG approval plate on stbd lifeboat IAW 46 CFR 160.35.13(c).

Interlake voluntarily decided to "deposit" their COI due to lack of economic demand. The COI will remain active and will be returned to the company upon request and completion of all their inspection dates and outstanding requirements to the satisfication of USCG and ABS.

Provide copy of ABS form "Sum A" concerning Stbd main propulsion boiler repairs and machinery automation tests and boiler safety valve settings to [name withheld] prior to 05 May 07.

Set Port and Stbd main boiler safety valves IAW 46 CFR 52.01-120 (a) (6) and IAW manufactures "boiler design data" and in no case, higher than the MAWP. Safeties to be tested to the satisfaction of the attending C.G. Marine Inspector.

As per phone conversation with Skip Woods, the vessel is not to operate their aux. boiler until the next SIP inspection.

Bilge alarm response time needs to be reduced, was excessive (3-5 minutes).

Replace gasket on #4 +5 hatch STBD side.

Reduce emergency fire pump pressure from 175 psi to 125 psi.

After Discharge of Cargo in Green Bay, Vessel to proceed to Bay Ship Yard in Sturgeon Bay WI for Inspection and make repairs in way of the Leaky Rivet in the number two Port Ballast Tank. All repairs to be completed to the satisfaction of the attending USCG Marine Inspector prior to on loading cargo.

Examine cement box patch in #4 Port Ballast tank after de-ballasting. Examination shall be conducted for a minimum of three ballast/de-ballast evolutions.

Remove cement box patch and make permanent repairs to bottom plate, #4 Port Ballast Tank, between frames 98 and 99, approximately 15 feet inboard of the turn of the bilge at the vessels next dry docking.

Vsl is directed to proceed in ballast to currently unknown port for repairs to bottom plate IWO fr. 98 approx 15' inboard from port side.
