A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. During ballast water discharge from the #1 starboard ballast tank a sheen was observed from the discharge. Vessel is allowed a one time transit to Zug Island to discharge cargo with out conducting ballast operations. After discharging cargo vessel can transit to Nicholson Ecorse Terminal to conduct assessment and identify cause of oil in ballast water. After assessment vessel must conduct permanent repairs to the satisfaction of the local OCMI prior to departure. ***Amended CG835*** Allows ballast operations except for the #1 starboard tank.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this subchapter. Vessel reported an issue with use of the port MDE related to the control bus communication module. Vessel is authorized to transit to Zug Island, MI and shall contact Sector Detroit OCMI to the status of the port MDE prior to departure. 46 CFR 46 CFR 58.05-1

The vessel must comply with the Sulfur Oxide (SOx) emission limits specified in Regulation 14.4.3 of MARPOL Annex VI, as required under 33 USC 1903 (b)(2) by the required date (MARCH 2028). All conversion or modification work necessary to achieve compliance shall be conducted under the oversight of the Officer in Charge, Marine Inspection (OCMI), with plan review performed as appropriate.

Outboard side of the rescue boat is extremely oxidized and no longer international orange. Onboard flares are not stowed properly and are wet inside bag.

Company created Oil Record Book that is not in compliance with CFR. Bottom of page should be completed and signed by Master and not Chief Engineer.

Forward pilot house exhaust and supply motors have been removed. Ships crew is currently leaving the watertight doors and fire screen doors open to create natural ventilation which is not affective.

During the testing of the vessels engine room CO2 fire suppression system the systems main valve failed and remained stuck in the open position. The inoperable valve in the CO2 system allowed agent to bypass the time delay and alarms. Repair or replace the valve and have the system re-tested in the presence of the attending USCG Marine Inspector.

Durning the inspeciotn/testing of aux boiler, the outboard safety did not open at or prior to MAWP. Inboard safety was operating properly. Repair or replace to the satisfaction of attending CG Marine inspector. 46CFR54.15-10

Fire and Smoke detection panel funtions but only displays locations on the main panel in the passageway. Prior to departing Detroit, MI prove prover operation of location functions at all stations.

Vessel reported fracture between #2 and #3 starboard water ballast tank at the 16'level. Vessel to have repairs carried out to the satisfaction of class and USCG at earliest opportunity but not to exceed 2 weeks. Please keep USCG informed of repairs and class attendance. Cite: 46 CFR 92.01

The fire main has a pin-hole leak that does not diminish the pressure required for the system. Repair faulty section within 14 days to the satisfaction of the Coast Guard or Class.

Hairline crack discovered in watertight door frame at frame 117 on starboard walk tunnel.

An approximate 5" crack between the #6 & #7 ballast tank was discovered. Vsl plans to affect repairs on its next voyage into Detroit or Toledo. CG 835 issued for 14 days requiring a report from class attesting to repairs.

the #2 cylinder on the stbd main was reading faulty temps resulting in the pitch on the propeller reducing. While at acnhorage the rack on the system was replaced and temps are reading normal.

during routine inspection of ballast tank, the crew found an 8" crack between the port side #6 & 7 ballast tanks.

Make repairs to 3 inch crack in bulkhead dividing ballast tanks #2 and #3 on the starboard side.

5JUL19 the vessel lost partial power due to a shaft generator breaker opening unexpectedly. The suspected cause is bad contactors in the unloading switch board for the stern thruster. The contactors appear excessively worn which may have resulted in phase issues causing the shaft generator breaker to open. These bad contactors are to be replaced and proper operation witnessed by an attending CG Marine Inspector before the stern thruster may be placed back into service. The stern thruster is to remain locked out and tagged out until this 835 is cleared. The vessel may continue service during this time. However, in instances where a stern thruster may be deemed necessary by the Master for the safe navigation of the vessel appropriate tug assistance must be provided. 835 due date is 4SEP19.

Several breakers on the vessel have opened without a known cause which has caused unexpected losses of power to various systems. There is an apparent issue that must be resolved to ensure continued safe operation. Appropriate servicing technicians are to evaluate the ships electrical system and identify the cause(s) of these failures. Proper repairs are to be made and proper operation witnessed by an attending CG Marine Inspector. This vessel has a periodically unmanned engine room (ACCU). Until the root cause of these failures is found and repaired the engine-room must be manned with at least one licensed engineer when the vessel is in restricted waters or in close quarters navigation situations. This is in addition to times the Master or Chief Engineer feel it is necessary for safe operations. 835 due date is 4SEP19.

ire extinguishing equipment may remain in service so long as it is maintained in good condition. During monthly SAP inspection, vessel had an accidental release of CO2 in the bow thruster room emptying the cylinder. Provide fully charged CO2 cylinder in bow thruster room prior to departure from Toledo, OH.

Vessel failed to submit ANOA within 24hrs of ETA.

Make temporary repairs to sideshell hull damage in way of the starboard forepeak, between frames #2 and #3 to the satisfaction of ABS and the U.S. Coast Guard Officer in Charge, Marine Inspection.

Upon discovery of a failed nitrogen accumulator unit for the rescue boat davit hydraulic system, the davit was temporarily fitted with a manual crank actuator (from sister vessel) for the purpose of enabling the vessel to make underway at completion of fit-out. Make permanent repairs to the davit arm, restoring it to original condition, and demonstrate satisfactory operation. Tests must be witnessed by USCG prior to departure from port where repairs are made.(46 CFR 199.45 (c)(2

Contact MSU Toledo with repair plan to fix crack in number 1 ballast tank between ballast tank #1 and #2, and conduct proper repairs.

Contact MSU Toledo with repair plan to fix 3" crack in number 5 ballast tank leaking into the unloading tunnel and conduct proper repairs.
