The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this subchapter. Vessel reported loss of propulsion on 07APR2026 leaving TORCO docks for the second time in 2 weeks due to the oil mist system. The company is required to submit documentation from an authorized professional, such as a certified technician, specialist, or manufacturer's representative, verifying the successful repair of the vessel's propulsion system.

All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of subchapter F (marine engineering). Vessel reported a loss of propulsion upon departure from the CN Ore Dock in Duluth, MN. Vessel is required to make satisfactory repairs to the main propulsion engine to the satisfaction of the Coast Guard and Class Society prior to departure. If the vessel needs to shift births prior to departure to address repairs vessel management must request to do so through MSU Duluth. 46 CFR 90.20-1(a)

Abandon-ship drills must include lowering of at least one lifeboat after any necessary preparation for launching. Vessel was unable to lower open lifeboat due to the mooring arrangement of the vessel. Vessel is to perform a lowering of the lifeboat in the presence of a marine inspector at the next available port of call. 46 CFR 199.180(c)

Vessel is authorized to transit from Gary, IN to shipyard in Toledo, OH to make repairs to #9 Port Ballast Tank. Repairs/deficiency to be cleared by MSU Toledo Marine Inspectors.

Vessel reported damage to port ballast tank #9 when grounding temporarily on approach to the Port of Gary, IN. Conduct repairs to the satisfaction of ABS and MSU Chicago. Provide class survey report to MSU Chicago Inspections Division at email listed below. A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs which affects the safety of a vessel. The survey shall be such as to insure that the necessary repairs have been effectively made, that the material and workmanship of such repairs are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter.

Master made notification to MSU Chicago of crew shortage due to consent based discharge of the one QMED who departed the vessel on 10/04/2023 in Gary, IN, for a family emergency. Master expects mariner to reattach to the vessel or be replaced on or around 10/08/2023. Vessel is authorized to operate without QMED until 10/08/2023. The vessel/company is required to continue to make reasonable efforts to attain a replacement at each port.

Master made notification to [name withheld], of crew shortage due to consent based discharge of AB [name withheld], MMC ref# [withheld], who departed the vessel on 7/13/2023 due to a family emergency. Vessel is authorized to operate with one less AB until 7/27/2023. The vessel/company is required to continue to make reasonable efforts to attain a replacement at each port.

On 16Dec2021 Vessel reported running aground IVO the port number one ballast tank while while transiting outbound Conneaut, OH near Conneaut West Breakwater Lighthouse break wall. Vessel shall complete a hull damage survey to the satisfaction of an attending Coast Guard Marine Inspector or Class Society Surveyor prior to the carriage of cargo.

Depth sounder observed was not operational. Vessel reps stated cable was cut, FBS electricians making repairs. Repairs required, with supporting evidence to be provided to CG MI before vessels departure. IAW 46 CFR 96.27-1

Observed the pilot house rudder angle indicator was off up to three degrees. Rudder angle indicator shall be recalibrated, with supporting evidence provided to CG MI within 30 days, as required by 46 CFR 113.40

Unable to witness lowering, release, operation, and retrieval of the vessels rescue Boat, due to vessel being moored starboard side. In the presence of an attending USCG Marine Inspector and within 30 days lower, release, operate, and raise the Rescue Boat as required by 46 CFR 199.45

Fuel hose servicing MDE failed. Replace affected hose. Provide pictures and statement from Chief Engineer as to the satisfaction of repairs and engine tests.

Liferafts were not stowed sufficiently ready for use so that 2 crew members could complete preparations for embarkation and launching in less than 5 mins. The life rafts were renewed this year and are wider than the previous life rafts. Stanchion arrangement or stanchions shall be modified to accommodate the larger life rafts.

Charts have not been updated. Update charts as required or procure new charts.

Great Lakes Pilot and Coast Guard Light List are outdated (2017). Procure updated navigation publications.

Make permanent repairs to the holed fire main in cargo hold #2. Once complete, a hydro test of 1.5 x the relief valve setting on the largest fire pump of the repairs shall be witnessed by a CG MI.

Crew member assigned AB duties on the Watch Quarter Station Bill has AB OSV endorsement. Remove crewmember from AB duties.

First Mate's stateroom has a plugged shower drain which backs up into the sink in the head below and spill into the Third Mates room. Prove proper operation of the shower drain or move crew members to another stateroom.

Port fwd mooring winch motor is grounded. Renew or repair winch motor.

CPP shows wrong direction alarm when pitched ahead. Prove proper operation of the CPP control panel.

Vessel sustained Starboard side damage, ballast tanks 2-5. during a grounding.

Effect permanent repairs to hull plating fractures in ballast tanks 2 and 3. Repairs shall be made to the satisfaction of the cognizant Officer in Charge, Marine Inspection.

Vessel is to make permanent repairs to previously approved temporary cement box repairs in stbd ballast tanks #2, #3, and #4 at next dry-dock. Vessel is scheduled for dry-dock in January of 2019 in Sturgeon Bay, WI. Repairs shall be made prior to next operating season to the satisfaction of the attending marine inspector. 46 CFR 91.30-1 (a)

Vessel is to make temporary repairs to leaking rivets found in #2, #3, and #4 stbd ballast tanks (6 total) through the use of cement boxes. Repairs must be made to the satisfaction of the attending marine inspector prior to departure. 46 CFR 91.30-1(a)

Completed a full visual & UT inspection of all butt weld joints in the LO systems. ABS approved Temporary Emergency Pipe Couplings, one covering all pipe sizes must be on-board the vessel pending permanent repairs.

Install adequate emergency lights within the forward and aft accommodations.

Pilot [name withheld] lights for the #4 & #5 Starboard & #5 Port Unloading Tunnel Gilly Gates not illuminating, make proper & effective repairs.

General Alarms bells within conveyor mans workshop and unloading tunnel gate 75 (port side) non-operational, make proper & effective repairs.

Observed wasted rails-stations supports on aft exterior stack and boat decks, where wasted make proper & effective repairs.

Sump grating wasted in unloading tunnel in way of the forward starboard WTD. Remove & renew grating.

Renew where remaining all worn, cracked, or painted port light gaskets per survey dated 2/8/2018.

Renew where remaining all missing port light-dead light covers & dog's per survey dated 2/8/2018.

Install missing light fixture covers in the starboard and port passengers quarters.

Paint flaking off the Port & Starboard navigational-running lights backing. Repaint with flat black paint.

Renew all old or worn emergency escape exit signs/stickers within the unloading tunnel.

Remove or remount loose control switch in forward unloading tunnel in way of the conveyor room.

Repair leaking steam condensate pipe in the unloading tunnel in the vicinity of Gate #81.

Renew fractured and leaking wash down valve, unloading tunnel, forward, port side in sump room.

Rescue Boat launching instructions faded. Renew rescue Boat Launching Instructions/placard.

#1 Fire plug (Aft of Pilot House) make permanent repairs to fractured pipe and demonstrated repairs to CG MI.

Observed plastic wire ties supporting communication cables on the overhead throughout the aft accommodation passageways. Install metallic supports around cables every 24 inches.

Rescue Boat, unable to launch due to vessels moored stbd side to dock wall. In presence of a CG Marine Inspector; Lower, release, operate, retrieve, and stow the vessels Rescue Boat.

Internal inspection of Engine Room Control Air & Aft Unloading Tunnel Air Receivers reflect excesses rust in tank bottoms. Remove or renew both Air Receivers on or before 31JAN2019

The #2 Aux Boiler's HFO low temp alarm and shutdown were tested and the boiler shutdown would not occur. Demonstrate proper operation of the boiler shutdown upon low HFO temp as per approved PSTPs.

The Class III water tight door at Bulkhead 183 (fwd machinery space lower level) would not close under power. Demonstrate consistent proper operation of the Class III door. To be verified from all control stations.

The bridge's steering gear control power failure alarm could not be verified. The PSTPs did not list a proper procedure that allowed for testing. Demonstrate proper operation of steering gear control power failure alarm on bridge. Re-write PTSPs to allow for proper testing of the alarm.

The #2 Aux Boiler's HFO High temp alarm and shutdown were tested and the boiler shutdown would not occur. Demonstrate proper operation of the boiler shutdown upon high HFO temp as per approved PSTPs.

46 CFR 62.20 Periodic Safety Test Procedures. On board testing was completed. However, PSTPs are not yet approved for the vessel. Provide approval letter for PSTPs by 4JUN17. If any comments are returned from MSCs review they are to be discussed with MSD Sturgeon Bay for resolution and shipboard testing as required.

46 CFR 56.25-20 Bolting used on piping flanges on all new piping installations have unapproved bolting materials. SAE J429 type bolts are installed and must be replaced with the bolting materials called out for in the approved CG & ABS drawings. This includes all new piping systems. All substandard bolts must be replaced or a formal equivalency approval must be obtained from MSC within 30 days.

New piping systems are to be evaluated via approved testing method to determine adequacy of welds in girth butt pipe joints for all new piping installations due to improper weld procedure. All piping system butt welds are to be sufficiently evaluated for adequacy; testing results as well as each joint location are to be tracked on drawings with findings & provided to the CG. Repair all by 1MAR18.

Resolve the outstanding comment ID'd for the CO2 shutdowns in MSC Letter E2-1701895. Install & test resisters for main engine E-Stop and CO2 pressure switches for the main engine & 1-4 service generators. Prove alarm is received at ECR when a break in a line is simulated. Demonstrate proper function of main engine e-stop as well as main engine and 1-4 service generator pressure switches.

46 CFR 91.55-10 Attending CG Marine Inspector questions if the Generator and HFO compartments ventilation system installed comply with the applicable 46 CFR regulations therefore operator shall submit drawings for the Generator and HFO compartments ventilation systems to USCG Marine Safety Center for review and approval.

46 CFR 91.55-10 Provide approved drawings to USCG MSD Sturgeon Bay reflecting modifications to Steam System P&ID GESAB drawing. Drawing was redlined and requires a new approval letter.

In accordance w/ the approved drawings the economizer installation is to have a second pressure differential switch installed on the circulation pumps which is to be tested for proper operation after installation. To be verified by a CG Inspector. The econmizer may be put into service and utilized.
