A vessel must be provided with a piping system that meets 56.50-50 in subchapter F of this chapter. Crew unable to demonstrate satisfactory suction from engine room bilge pipe. 46 CFR 119.510

All microprocessor or computer based systems must meet the requirements of part 62 in subchapter F of this chapter. Technician unable to complete engine overspeed DVTP based on as built limitations. Provide documentation from OEM or demonstrate test in compliance with DVTPs. 46 CFR 121.620

Each vessel must be provided with a visual and audible alarm at the operating station to indicate high water level in each of the following spaces: A space with a through-hull fitting below the deepest load waterline. Aft battery void missing high water alarm. 46 CFR 119.530

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Engine room door that opens to main deck accommodation space has a gap at the bottom that is not vapor tight. 46 CFR 116.405

Each individual bilge suction shall be fitted with a suitable bilge stainer. Lazarette bilge strainer found broken and no longer capable of blocking debris. 46 CFR 56.50-50

The minimum capacity f the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 psi at the pump outlet. The pump outlet shall be fitted with a pressure gauge. Local fire pump pressure gauge maxes out at 60 psi and spikes all the way around to zero when energized. Replace gauge with higher readout to give accurate indication of pressure. 46 CFR 181.10-1 (Old T)

A distribution line must extend at least 2 inches beyond the last orifice and be closed with a cap or plug. Fire suppression piping in port aft engine room ends with discharge horn and does not have 2 inch extension. 46 CFR 118.410

At each inspection for certification of a vessel, the operator shall be prepared to conduct tests and have the vessel ready for inspection of machinery…including the following: inspection of all machinery essential to the routine operation of the vessel including generators and cooling systems. Number one chill water pump in auxiliary machinery space missing on/off switch. 46 CFR 115.804

At each inspection for certification of a vessel, the operator shall be prepared to conduct tests of electrical equipment including the following: inspection of all cable as far as practicable. Multiple dead end wires present in engine room at the base of the stairs on the bulkhead. 46 CFR 115.806

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents. First-aid kit found with expired medicines. 46 CFR 121.710

At each inspection for certification of a vessel, the operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull, including the following: inspection of all accessible parts of the exterior and interior of the hull. Fractured gusset found in starboard side of steering void, located two frames back from the forward bulkhead. 46 CFR 115.802

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel experienced a loss of propulsion on both main engines. Provide root cause and repair issue to the satisfaction of the Marine Inspector. 46 CFR 119.200(a)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel experienced a loss of propulsion. Provide root cause and repair issue to the satisfaction of the Marine Inspector. 46CFR119.200(a)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Modifications have been made to propulsion control and alarm systems since original casualty/installation. OCMI is not confident that the equipment on board matches what is approved and the validity of the currently approved DVTP. Submit all modifications and updates to DVTP to MSC for approval and testing by the OCMI.

At each inspection for certification and at every other vessel inspection all observed unsafe practices must be corrected. The vessel's forward fire station door on the second deck was unable to be opened in a suitable manner. 46 CFR 115.830(a)

Each life jacket kept in a stowage container must be readily available. The vessel's 2nd deck lifejacket storage is missing a handle. 46 CFR 117.78(a)(2)

Vessels to which this subchapter applies must meet the applicable provisions in subchapter S (Subdivision and Stability) of this chapter. Vessel found to have a leak in the watertight bulkhead between engine room and sewage tank void space. Submit repair proposal and make repairs after approval.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel experienced a loss of propulsion. Cause of LOP must be investigated and repaired.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Vessel reported back flow of seawater into void space due to faulty check valve on the bilge manifold. Repair to the satisfaction of the attending Marine Inspector.

Each survival craft must be secured with by a painter with a float-free link permanently attached to the vessel…stowed so that the when the vessel sinks the survival craft floats free and, if inflatable inflates automatically. IBAs were found with painter/inflation lanyard unattached. 46 CFR 117.130(a)(1) & (2)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. Temporary battery charger found charging house batteries under bridge console. 46 CFR 120.200(b)

Electrical equipment exposed to the weather must be watertight. Electrical outlet on port quarter of main deck is missing cover. 46 CFR.210(b)

Each opening in an exposed weather deck must have a means for closing it weather tight. Coaming on main deck aft doors is bent and not allowing doors to fully close. 46 CFR 171.122(f)(1) "Old S"

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened from either side. Forepeak/Galley escape hatch has T handle stored inside hatch. Therefore, this hatch is only able to be opened from one side. 46 CFR 118.425(b)

A grease extraction hood must be equipped with a dry or wet chemical fire extinguishing system meeting the applicable sections of NFPA 17. Galley hood does not have an inspected and certified system in place. No cooking in galley until system is installed and certified. 46 CFR 118.425(b)

Except as otherwise provided in this section every vessel must be fitted with a suitable magnetic compass designed for marine use, to be mounted at the primary operating station. Vessel does not have a magnetic compass. Must install magnetic compass. 46CFR121.402(a)

Doors in A-Class fire control boundaries must meet the following additional requirements: A door must not be undercut more than 12.7 millimeters (0.5 inches) above the door sill or deck covering. Rugs and carpets must not pass through doorways, but linoleum and similar deck coverings may. Carpet passes under doors leading to forward stairway on maindeck. Must remove carpet passing under door. 46CFR116.(c)(5)

A-Class bulkheads or decks must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads, and decks. They must be so constructed that, if subjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. In addition, they must be so insulated with approved structural insulation, bulkhead panels, or deck covering so that, if subjected to the standard fire test for the applicable time period listed below, the average temperature on the unexposed side does not rise more than 139 °C (250 °F) above the original temperature, nor does the temperature at any one point, including any joint, rise more than 181 °C (325 °F) above the original temperature. Insulation was found falling off the overhead over the stairs leading into the auxiliary machinery space. Ensure insulation is installed properly. 46CFR116.415(a)(2)(i)

Each portable fire extinguisher on a vessel must be of a type approved by the Commandant. Galley fire extinguisher is not Coast Guard approved. Replace with approved fire extinguisher. 46CFR118.500(a)

The vessel removed 12.88LT of batteries and battery equipment from the third void (battery compartment). The vessel will replace the batteries at latter date. The following restrictions will be imposed on the vessel until a new stability test is complete: 1. The vessel is restricted to the carriage of 500 passengers and a max persons count of 550. 2. The potable water tanks are required to be pressed up daily to counteract the loss of the forward weight.

Support CO2 manifold piping.

Prove galley hood extinguishing system is installed IAW NFPA 17A.

Fire detection Panel is unapproved. Vessel will have till next Dry Dock 31Jan2019 to replace with a Coast Guard approved fire panel.

Submit company roving fire watch policy to Coast Guard.

Obtain and display emergency escape sign near emergency escape ladder in auxiliary room.
