The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies. Vessel has new crew that have not conducted drills.46 CFR 185.52

Each survival craft must be: (1) Secured to the vessel by a painter with a float-free link permanently attached to the vessel. Painter weak link has no service or documentation attesting to breaking strength.46 CFR 180.130

Each survival craft required on a vessel by this part must meet one of the following: (2) For a life float—Approved under approval series 160.027 or other standard specified by the Commandant. -01 Life float doesn’t meet 160.027 with punctures in exterior. -Additionally, other life floats required to be cleaned to facilitate further inspection. -Hi Visibility tape is required for all life float

No marine employer may engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Vessel rep stated they had two new hires but could not provide documents attesting to drug test or randomized drug testing program. Vessel has adequate crew without two new members. Must provide documentation prior to them working aboard vessel in a commercial setting.46 CFR 16.201

A vessel must be provided with bilge pumps. Not more than 49 passengers, not more than 65 ft bilge pumps required 1 fixed power pump (10 GPM) and 1 portable hand pump (10 GPM). Fixed bilge pump did not remove water from bilge at a rate of 10 GPM.

A suitable metal marine type strainer meeting the requirements of the engine manufacturer must be fitted in the fuel supply line in the engine compartment. Excessive buildup of residue on racor fuel filters

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Engine room bilges held excessive oil and debris.

Bilge high level alarms. On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each normally unmanned space. One of the six visual bilge alarms at the operating station was not operating.

- Remove all dead end wires in wheelhouse, engine room and forepeak. - Fuse panel in wheelhouse was found with multiple areas of corrosion creating an electrical hazard. Remove corrosion. - Fuse was missing from cable to bus bar creating an electrical hazard. Remove cable if not in use. - Marine grade wires must be used in electrical installations. Non marine grade wiring was found in wheelhouse installations. - Battery in wheelhouse was found with a build up of corrosion. Clean battery. -Amazon power strip was found powering two Inverters to the radio installations. Hard wire in inverters if approved in electrical plan. Remove power strip. - Breaker panels were mostly found with illegible labels if any were present. Label all breaker panels. 2 breakers were found missing in the forward lounge. Either replace or put blanks in as approved in electrical plans that will be submitted. - Receptacle was found broken in wheelhouse. Replace receptacle labeled "side". - Wiring was found hanging from the deck into the bilges with broken. Reattach above the bilges. - Generator control box was found open with wiring exposed. Remove zip tie and demonstrate proper functionality

Electrical Installation does not appear to have been reviewed or approved by the cognizant OCMI as required. Submit electrical plans in accordance with PEG E2-23 to the cognizant OCMI for review and approval. Temporary repair of foam was found where the deck meets the stem. Submit repair proposal.

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly. EPIRB requires monthly tests and to be logged

Each floating water light installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion[1]resistant clip.The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the water light to be quickly disconnected from the ring life buoy. Life ring buoy 3ft waterlight line requires replace along with a installation of a corrosion resistant clip.

Temporary repair of foam was found where the deck meets the stem. Submit repair proposal.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places. Lifejacket placard covered with television. Also need a secondary placard for donning instructions for second type of lifejacket.

Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation. Provide copy of Certificate of documentation.

Electrical equipment exposed to the weather must be watertight. PA system on cabin overhead has wiring spliced together with electrical tape. Connection needs to occur in an enclosure.

Permission for repairs and alterations. Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to cognizant [name withheld] alterations must be approved by the OCMI before work is started. Hydraulic lines to mechanical anchor windlass were replaced without notification to OCMI.

Guards for exposed hazards. An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Guard on port main diesel engine incorrectly installed leaving rotating machinery exposed.

Watertight integrity. A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight: (1) A hatch on a watertight trunk that extends at least 12 inches above the weather deck. Hatch used to connect shore power cable to vessel shore power junction box located in engine room is not weathertight.

Penetrations and openings in watertight bulkheads in a vessel less than a 100 gross tons. Penetrations and openings in watertight bulkheads must – (2) Have means to make them watertight. Several through hull fittings in watertight bulkheads located in lounge and forward peak were not properly sealed failing to make them watertight.

FCC Safety radiotelephony was expired, FCC station authorization was not on board, and operator did not have a valid Marine Radio Operator Permit. Vessel did not have light list or tight tables available.

Employer should provide EAP to all crew members and have an information display. Vessel did not have an EAP display on board nor did they provide documents attesting to training.

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessel has not completed the annual exam in specified time period.

Aft engine room STBD side top corner frame had rotted from deck clamp down 8 in. deck is soft in this area. Material will need to be removed for further inspection.

Discovered wasted butt block port side behind fuel tank. Approved temp repair. Pull and examine fasteners in area and replace butt block next dry dock. Next drydock is dated 31Jan2026.

Discovered worm damage in fuel tank compartment, second frame from forward bulkhead STBD side near keel. Approved temp repair. Pull and examine fasteners in area. Replace frame next drydock dated 31Jan26.

Discovered 1/2 deep hole/bore in floor plank in tank compartment. Between 4th and 5th frame from forward bulkhead STBD side near keel. Approved temp repair. Pull and examine fasteners in area. Replace Plank next drydock dated 31Jan26.

Noted in a 24 hour period while sitting at the dock, water level in tank compartment raised from .25" to almost 4". locate water ingress and repair.

It was discovered bulkhead between Tank Compartment and Lazarette was not water tight. After reviewing subdivision calculations bulk head is required to be water tight. Make bulkhead water tight.

While underway discovered vessel making water in the aft STBD corner of lazarette. Water ingress was noted from chine up to 7th-8th plank. Pull and Examine fasteners in area. make repairs to make watertight.

Vessel must schedule in-the-water internal structural exam with Sector San Diego as part of concentrated wooden vessel inspection campaign to verify condition of vessel's hull. email [email withheld] to schedule. Must be completed prior to carriage of passengers.

Pre-engineered fixed gas fire extinguishing systems must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Upon testing, port main engine was not automatically shut down by the system.

All machinery must be installed and maintained in such a manner as to afford adequate protection from personnel injury. Belt guard was exposed on port main engine and must have suitable cover.

The materials used and the minimum thickness allowed in the construction of independent fuel tanks shall be as indicated in Table 182.20-25(a)(1); fuel tanks constructed of steel with a capacity over 150 gallons shall have a minimum thickness of 0.1793 inches (MSG7). Heavy scaling rust on both port and STBD steel fuel tanks was observed on all accessible sides of the tanks, remaining steel material is unknown without further testing/inspection. Provide plan to local OCMI with one of the following options: NDT/gauge thickness of tanks, remove tanks for further inspection and hydrostatic testing, or replace tanks. Replacement of tanks require USCG construction oversight to include plan approval. Must be addressed prior to carriage of passengers.

On 27SEP2023 Vessel reported the fuel line rupturing on the generator resulting in the generator failing on 24SEP2023. Vessel made repairs to the generator but the repairs must be verified by a marine inspector prior to the vessel carrying passengers.

Each vessel that operates on the high seas must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. There was no hydrostatic release to EPIRB mount at the time of the inspection. Must install prior to sailing beyond high seas marker.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired/replaced. Top life float in fwd stack is no longer in serviceable condition. Life float must be removed and destroyed.

All non metallic material in vital piping systems shall possess characteristics adequate for its intended service and environment. Two (02) sections of the overboard discharge non metallic piping for bilge system is questionable in material. Must show to the CG Marine Inspector that material is adequate in approved specifications or replace material with approved non metallic sections.

Exhaust piping on both main engines must be suitably insulated to prevent injuries. Need to install outer cover to insulation of exhaust piping to both main engines to the satisfaction of the USCG Marine Inspector.

On a vessel of at least 26 feet in length, visual and audible alarm must be provided at the operating station to indicate a high water level. 04 of 06 bilge alarms were found inoperable, the 02 that were found operational did not have an acceptable volume of an audible alarm. Water ingress observed in lazarette. Vessel operator must make necessary repairs to the satisfaction of the Coast Guard prior to operating.

Fire detection and extinguishing systems. Fire detection and extinguishing systems must be installed in compliance with 181.400 through 181.420 of this chapter. 02 of 02 independent modular smoke detectors were found inoperable with the batteries removed. Owner to replace batteries and test smoke detectors to the satisfaction of the Coast Guard prior to operating.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Marine Inspector identified water ingress through STBD rudder packing, Repair to the satisfaction of the Coast Guard prior to operating.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Upon arrival for Annual Inspection, Owner /Operator informed Marine Inspector that STBD gear box was unable to engage the STBD Maine Diesel Engine in ahead or astern. Owner/Operator stated this casualty occurred on 14OCT2022, however, it was never reported to the Coast Guard. Make proper repairs to the satisfaction of the Coast Guard prior to operating.

An internal structural examination conducted in compliance with § 176.600 may be conducted while the vessel is afloat or out of the water and consists of a complete examination of the vessel's main strength members. Vessel upon entering the water from Dry Docking, experienced water ingress from the STBD aft engine room bilge from a seam, and STBD between frames three and four six feet outboard of keel.

An internal structural examination conducted in compliance with § 176.600 may be conducted while the vessel is afloat or out of the water and consists of a complete examination of the vessel's main strength members. Vessel upon entering the water from Dry Docking, experienced water ingress from the STBD aft engine room bilge from a seam, and STBD between frames three and four six feet outboard of keel.

The bilge alarms for the forepeak and tank space are inopperable. Repair/replace the identified bilge alarms so that they produce an audible and visual alarm at the operating station. Rounds of these spaces must be conducted at least once every hour while the vessel is underway or at anchor until this deficiency is rectified.

Repair all patches on bilge piping.

Provide a valid FCC Radio Safety Telephony Certificate. Complete within ninety (90) days.

Replace all expired items within the first aid kit.

Renew both port and starboard side nav lights.

Renew failed valve in bilge manifold system

Renew flexible hose bilge overboard piping.

Provide up-to-date FCC Station License.

Conduct acceptable safety drills in pressence of CG MI.

Provide 03 adult PFDs. Passenger count reduced to 49 until corrected.

Provide 3 adult lifejackets.

Provide new battery for floating waterlight

Provide new lifering line

Provide float free arrangement for EPIRB. Restricted to no more than 3nm.
