An interconnected detection and alarm system must: be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d), vsl had detectors in crew accommodation, audible in hall/galley was heard, but no visual w/in the crew accommodation. Make system meet visual regulation. 46 CFR 181.450(a)(2)

Exhaust piping shall be properly supported by non-combustible hangers or blocks. Multiple exhaust hangers were missing securing bolts. 46 CFR 182. 20-20 (Old T) 46 CFR 182. 15-20 (Old T)

46 CFR 176.830 (a) At each inspection all observed unsafe practices shall be corrected. Missing deck plate and unsecured deck plates in e/r.

A vessel must comply with the structure design requirements of ABS alumuium vessel rules if more than 100ft in length. Vessel has several punctures, fractures, and broken welds in the AFT transam area marked "CG"

Crew berthing and washing areas must now prominently display details concerning company policies how to report issues, and steps to respond to sexual assault and sexual harassment.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. An interconnected detection and alarm system must: (1) Consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory accepted by the Commandant according to 46 CFR subpart 159.010, as meeting UL 217 (incorporated by reference, see 46 CFR 175.600); (2) Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system; (3) Contain an independent power source; and (4) Alarm on low power. Fire pump suction piping at elbow to the pump had a 3/8 inch hole that would not allow pump to self prime.

Main engine RACOR filters did not have required plugs on sediment valves for main engines.

STBD Generator RACOR filters were leaking fuel.

Switch board was missing interlock bar.

#1 MPDE control panel had evidence of water leaking from soft patch in overhead.

Holes were discovered above #1MPDE exhaust and #4 MPDE air boxes and a cracked frame in the lazerette.

Crack was discovered on the Main deck mid body.

Frequency gauge was inoperable.
