Machinery and equipment installed on each vessel must be suitable for the vessel and its operation for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. #1 MDE had a severe oil leak on the outboard aft of the engine causing a constant flow of hot oil onto the engine electrical components and directly into the bilge. *** Vessel is authorized to proceed directly to New Iberia for repairs to #1 MDE. Vessel shall not operate #1 MDE unless an emergency occurs. Vessel shall inform MSD Lafayette of location of repairs*** 46 CFR 182.100

At each initial and subsequent inspection of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems. Port generator had an oil leak from the rear of the prime mover that splashed onto the electrical wires and components. 46 CFR 176.804(c)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Switchboard panel had an active ground. 46 CFR 183.200

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Distribution panel in generator space has a blank missing for one of the breakers. 46 CFR 183.200(c)

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead. Watertight door entering lazarette had 1/2"-1" gap on 02 dogs. 46 42.15-10(a)

Each Vessel operating on a Coastwise route must be provided with life floats of an aggregate capacity that will accommodate at least 100 percent of the total number of persons on board. Life rafts are currently removed for servicing and to accommodate work on engine. Coast Guard will verify in person that life rafts are properly installed. Cite 45CFR180.204(c)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating neccesary requirements. Number two engine currently removed for work. Cite: 46CFR182.200(b)

he construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel sustained multiple gashes in the port side hull area after striking a Single well structure. Vessel is to return back to Venice to make repairs. 46 CFR 177.100 60/C

A vessel must have a valid Certificate of Documentation. Vessel does not have a valid COD on board. IAW 46 CFR 67.7

A vessel must carry accurate and up-to-date navigation publications. Vessel does not have updated Nav rules and currents publications. IAW 46 CFR 184.420(a).

Wire connections must be made in an enclosure. The water column wires are cut. IAW 46 CFR 183.340(g).

Code 1301: Repairs and alterations that affects the safety of the vessel must not be made without the approval of the OCMI, except during emergency. Repairs include, but are not limited to replacement of propulsion equipment or systems. Vessel propeller servicing the main diesel propulsion engine number 1 (portside-outboard engine) lost a blade while operating, resulting in excess vibration. Properly replace propeller as in kind and also must submit via email to [email withheld] a written statement from vessel designer/manufacturer that vessel can be safely operated on 3 engines without compromising the safety operational envelope of the vessel while carrying cargo and persons.

Vessel is reporting both Port propellers caught up on the rocks during grounding. Vessel must prove safe operation prior to departing EPS dock Venice, LA to Superior shipyard, Golden Meadow, LA for dry dock.

Vessel is to proceed to emergency dry dock and conduct damage survey in dry dock at Superior Shipyard, Golden meadow, LA and make all neccessary repairs prior to departure to the satisfaction of the attending Marine Inspector.

Replace exhaust lagging and missing bolt on #1 MDE. Vessel representative had it removed to for replacement, not completed at time of inspection.

Clean excessive residue on port generator exhaust fan.

Repair watertight bulkhead penetration from removed wire spaces between generator & rudder room with same material as existing.

Replace seal on treated sewage lift pump.

Tire bracket caused wear on hull plating. Crop and renew

Provide approved steering system plans.

Provide approved Oily Water Separator plans.
