All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. #1 MDE had a severe oil leak on the outboard aft of the engine causing a constant flow of hot oil onto the engine electrical components and directly into the bilge.

Resolution: Rebuilt engine was installed. Operation tests performed including remote shut downs. All guages were functional from all operating stations.
he construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel sustained multiple gashes in the port side hull area after striking a Single well structure. Vessel is to return back to Venice to make repairs.

Repairs include, but are not limited to replacement of propulsion equipment or systems. Vessel propeller servicing the main diesel propulsion engine number 1 (portside-outboard engine) lost a blade while operating, resulting in excess vibration. Properly replace propeller as in kind and also must submit via email to [email withheld] a written statement from vessel designer/manufacturer that vessel can be safely operated on 3 engines without compromising the safety operational envelope of the vessel while carrying cargo and persons.
