MAN O WAR

Vessels shall be provided a dependable source of emergency power and lighting necessary for the safety of the passengers and/or crew in an emergency. Both Port and Starboard ER did not have sufficient emergency lighting at the top near the door to see handrails or door handle to exit.

Distribution panels shall be protected against accidental contact. Vessel had an electrical blank missing in the Port ER panel.

In each crew berthing area and wash area, the owner or operator must display information in a visible location regarding sexual assault and sexual harassment, retaliation, and drug and alcohol usage and the procedures and resources to report crimes, including sexual assault and sexual harassment IAW 46 USC 11101 and CVC Policy Letter 23-04. Vessel had not yet equipped each berthing areas and wash area.

Vessels shall maintain a master key control system, control access and use of master keys, and observe specific information handling protocols as required IAW 46 USC 3301 and CVC Policy Letter 23-06. The company did not have a policy for the Master Key for the vessel or authorized persons.

Vessel required to have a stern light visible from at least 2 miles. Stern light was extinguished and inoperable.

Electric installations on vessel's must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Breaker panel on the bridge and O2 deck level had a missing blank and missing nameplates. 46 CFR 111.01 (a)(2)

The Officer in Charge, Marine Inspection, (OCMI) may issue a permit to proceed to another port for repair to a vessel, if in his or her judgment it can be done with safety, even if the COI has expired or is about to expire. Vessel is authorized a one way transit from Port of New Iberia, LA to Port Fourchon, LA to make permanent repairs to the vessel's pads. Vessel is to follow approved Sail Plan during this voyage. If vessel must deviate from approved Sail Plan they must notify the nearest OCMI as soon as practicable. Upon arrival to Port Fourchon, LA vessel shall notify MSU Houma Command Duty Officer at [phone withheld]. 46 CFR 91.05-1(a)

OCMI may issue a permit to proceed to another port for repair if the vessel can complete the trip safely. MSU Morgan City OCMI has issued a permit to proceed to the lift boat MAN O WAR, ON 1045921, granting a one time transit from Bollinger Amelia Repair in Amelia, LA to Marine Industrial Fabricators (MIF) in New Iberia, LA. This transit shall be conducted IAW the transit/vessel relocation plan submitted by All Coast to MSU Morgan City. The vessel must make notifications to MSD Lafayette upon arrival. This Permit to Proceed will only be valid until the vessels arrival at MIF in New Iberia.

Holes were found in the hull of the vessel in 5 locations. Significant hull metal wastage was observed on over 50% of the hull plating. Require hull to be blasted to bare metal for a better assessment.

During an exam if deterioration to the hull plating or structural members is discovered, the OCMI may require the vessel to be drydocked or otherwise taken out of service to assess the extent of the damage to affect permanent repairs. ISE found many frames, longitudinals, and angels in every tank/void wasted or broken. These were called out and marked with green spray paint. These must be cropped and renewed in kind.

No repairs affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without knowledge of the [name withheld] noted in excess of 10 doubler plates used on the hull interior and exterior. Remove doublers. Crop and renew hull plating in these areas.

Liftboat jacking systems, liftboat legs, liftboat leg pads, and arrangements for supply of water to fire mains, as well as the items listed by §126.430 of this subchapter, will normally be inspected during an inspection for certification and periodic inspection to determine whether the liftboat is in satisfactory condition and fit for the service intended. Vessel has not completed "leg-to-can" and the "can-to-pad" connection welds IAW CG Policy Letter 07-02. 46 CFR 134.120 Visual inspection of the leg rack on the STBD leg showed a crack running down the rack to leg weld about 4', then extending across the rack, and back down the other side of the rack to leg weld 4'. Vessel must complete the 10 year inspection to prove the legs structural integrity.

There must be ground detection for each ship's service power system and power distribution system. The vsl starboard ground detection was not functioning for the either switchboard. 46 CFR 111.05-21

Fuel systems must have non metallic hoses installed meeting SAE J1942. Port engine generator fuel hose was not in serviceable condition. 46 CFR 56.60-25

Each switchboard must be installed meeting requirements of IEEE 45-2002. Vsl switchboard door was falling off. 46 CFR 111.30-1

Non-conductive mats must be installed in front of switchboards for personnel protection and must extend the entire length and width. No switchboards in either engine room had appropriate non-conductive mats installed.46 CFR 111.30-11

Cables should be supported to prevent undue sagging and the distance between supports should not exceed 610mm. Cables in both engine rooms were no longer supported by cable supports. IEEC 45-2002 25.5

Visual inspection identified a crack @ rack butt port leg at approx 45ft. NDT (wet mag)conducted prior to completion of inspection. [name withheld] confirmed that the rack Butt had a crack. Submit repair proposal for permanent repairs.

Door on STBD E/R Shaft tach box for STBD engine will not close.

Reset button on controller for #1 Air Compressor missing leaving an opening in front of controller.

Bulkhead sealant @ penetration going into STBD E/R and A-60 insulation missing in fiddley between machinery/Accommodation space.

Failed fire drill. Drill started of bad because the crewmember that reported the fire gave incorrect location. The drill continued to get worst when the vent closures to isolate the space could not be closed. Perform satisfactory fire drill.

Prove proper operation of vent closures prior to operation. During Fire drill an attempt to close most failed, including a broken dogs and seized hinges.

Replace and prove proper operation of port scuttle from Engineroom (will not open and center bushing cracked).

Deck Plating above STBD E/R at forward super structure is wasted leaking water in engine room. C/R to good metal.

Provide valid load line certificate issued by class.

Complete dry dock / ISE call out items to attain dry dock credit.

Replace nonoperational fire main pressure gauge on stbd fire pump and gauge located on bridge.

Show proper operation of port shaft tachometer.

Replace synchroscope and test reverse power relay in presence of USCG Marine Inspector.

Provide proper operating KW gauge on #2 generator.

Replace leaking fuel oil line on #1 Generator.

Replace wasted fuel oil line on crane prime mover prior to crane operation.

Replace wasted workboat cradle prior to 2018 annual inspection.

Clean generator windings on both generators prior to 2018 annual inspection.

Overspeed device was inoperable.

Small paint locker on upper deck was wasted in several areas. Crew to repair locker.

Fiddley ventilation heater enclosure was severely wasted exposing electrical components. Crew to repair/renew.

Prove proper operation of STBD SSDG overspeed device.

Provide self closing devices for both engine room fire doors.

Provide operational emergency lighting for the life rafts.

ROX Blox fitting from engine room vesitbule to AC space was improperly installed.

Proceed to SEAPORT 1 at Port Fuchon, LA to conduct damage survey, effect repairs as needed, and schedule re-inspection with MSU Houma prior to sailing.

Document all drills in detail including persons involved in the drill, lessons learned

Document all tets prior to leaving port and log it in the ships log

Provide a Lifesaving Equipment Maintenance Record for all equipment as per manufacture specs

Hyd jacking system has threaded connections vs split flange connections in the engine room. Replace all fittings with appropiate/rated split flange connections

Radar inoperable

Buoyant apparatus life line wasted

Provide updated fire/safety plan for approval.

Demonstrate that all components in hydraulic jacking system meet system operating pressures and substantially comply with 46 CFR 58.30 as required by NVIC 8-91.

Provide automatic shut downs for internal combustion engines drawing air from inside engine rooms (protected space) or provide calculations that CO2 capacity of system accounts for volume of CO2 ingested by engines prior to stopping them, there remains sufficient CO2 reserve to extinguish fire.

Provide central propulsion control station in engine room per NVIC 1-78.

Obtain ABS approval of stern leg repair plans.
