P/V valves must comply with the requirements of 46 CFR 162.017. #2 cargo tank P/V was seizing up when testing the spring mechanism.

No repairs or alterations affecting the safety of the vessel or its machinery shall be made unless applicable requirements in the chapter are met. Barge owner/operator reported the stern void is full to the waterline of water with extent of damage unknown. 46 CFR 2.01-15(a) CG attended barge on 4/19 and identified a fracture that is approx 2-3" long x 1/8" wide. Crack is located in the stern void corner wrap knuckle and water ingress has been slowed due to temp cedar plug. Barge is permitted to transfer to Cooper Fleeting area during daylight hours only while it awaits location for permanent repair. While in fleeting area barge must be monitored and notification must be made to local CG if condition changes/worsens. Authorization from local CG required prior to transit to yard for permanent repair. No repair work is permitted until authorized by local CG, final clearance of 835 required by attending CG marine inspector. Update 03MAY22: No repairs or alterations affecting the safety of the vessel or its machinery shall be made unless applicable requirements are met. Vessel has a fracture in the stern void, approx. 2-3" by 1/8". Temporary repair has been enacted and inspected by Marine Inspectors. Vessel is permitted to transit to Plaquemine Point Shipyard in Sunshine, LA to enact permanent repairs and arrive no later than 03 June 2022 in accordance with Coast Guard D8 Policy Letter 09-01. No cargo may be carried and the temporary repair must be regularly monitored. If the temporary repair fails or is compromised, local Coast Guard unit must be notified. Local Coast Guard unit must be contacted for repair process.

Repair railing where indicated.

Install chafing gear at fuel hoses for prime mover where indicated.

Sidelights of vessels of 20 meters or more in length shall be fitted with matt black inboard screens and meet the requirements of 84.15. Install missing navigation light screens to the satisfaction of the attending Coast Guard Marine Inspector.

Make permanent repairsto all marked areas on the internal framing - See Narrative summary for detailed list of repairs

Prove proper operation of the cargo pump emergency shutdown.

Replace #1 stbd and #4 port void covers, they asre wasted.

Hydro test cargo piping system and stencil piping with test date.

Pin hole in weld. Temporary repairs in-place. Isolate effected piping, make permanent repairs prior to utilizing above deck loading pipe.

MAKE PROPER REPAIR TO WELD SEAMS AND BUTTS AT HULL PLATE AND CARGO TANK TOP, INSIDE AND OUT. NO CARGO OPERATIONS UNTIL THIS REQUIREMENT IS COMPLETED.

PROVIDE HOMEPORT AND DRAFT MARKINGS.

RENEW GASKETS ON ALL CARGO TANK ULLAGE AND CARGO HATCH OPENINGS.

PROVIDE VESSEL RESPONSE PLAN, COFR, OIL TRANSFER PROCEDURES AND CARGO PIPE DIAGRAM.

DEMONSTRATE PROPER OPERATION OF EMERGENCY SHUTDOWN FOR PRODUCT TRANSFER PUMP.

PROVIDE CALIBRATED PRESSURE GAUGE FOR PUMP DISCHARGE.

RENEW/REPAIR #3 CARGO TANK ULLAGE CLOSURE.

PRIOR TO USE OF CARGO TRANSFER SYSTEM, PERFORM EMERGENCY SHUT DOWN TESTING OF CARGO PUMP ENGINE WHILE WITNESSED BY COAST GUARD MARINE INSPECTOR.

PRIOR TO USE OF CARGO TRANSFER SYSTEM, PROVIDE EVIDENCE OF CARGO PIPING HYDROSTATIC TESTING.

PRIOR TO PLACING THE VESSEL BACK IN SERVICE, MAKE THE VESSEL AVAILABLE FOR A CARGO TANK INTERNAL EXAM.

UPON OFFLOADING THE VESSEL AND PRIOR TO RELOADING, MAKE PERMANENT REPAIRS TO THE BILGE KNUCKLE FRACTURES IWO THE #2 PORT WING VOID. EFFECTIVE TEMPORARY REPAIRS ARE TO BE MAINTAINED.

UPON OFFLOADING THE VESSEL AND PRIOR TO RELOADING, MAKE PERMANENT REPAIRS TO THE 6" FRACTURE IWO THE #1/#2 PORT WING VOID TRANSVERSE BULKHEAD. EFFECTIVE TEMPORARY REPAIRS ARE TO BE MAINTAINED.

GOUGE AND REWELD APPROX 5" CRACK IN STBD. BOW 2' DOWN FROM THE MAIN DECK 4' AFT OF THE CORNER WRAPPER.

REPAIR THREE DOGS MISSING FROM BOW VOID MANHOLE COVER.

REPLACE IN KIND 50"X52" SECTION OF THE PORT RAKE KNUCKLE FROM THE BOTTOM OFTHE PORT CORNER WRAPPER DOWN 50"X52".

REPAIR FWDMOST DOG ON HATCH LEADING TO PUMPROOM AT NEXT GAS-FREE AVAILABIL-ITY BUT NOT LATER THAN 28FEB87.

REPAIR INSULATION ON CARGO PUMP DIESEL ENGINE EXHAUST NOT LATER THAN 05APR 86.
