Fire-extinguishing appliances shall be kept in good order and be available for immediate use at all times. LPCO2 Instruction Manual and posted instructions state that the stop valve should normally be open. Inspectors discovered the valve in the closed position. Vessel's crew stated they normally keep this valve closed and there is a small placard at the fire control station instructing the crew to open the valve. 74 SOLAS (97) II-2/21 17ac

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should ensure that each ship is: manned with qualified, certificated, and medically-fit seafarers in accordance with national and international requirements. The vessel is currently short 3 of the required crew , 2 ABs and 1 Oiler, as specified on the Certificate of Inspection. Additionally, 1 of the 2 ABs currently missing from the crew, has been missing since 25 April. The company is to conduct an internal investigation into their processes regarding their obligations under US law and the ISM Code, to ensure the safe manning of the vessel. Complete internal investigation and develop corrective actions within 90-days, have ready for RO review at the next annual verification of the DOC.

Machinery spaces of category A shall be adequately ventilated so as to ensure that when machinery or boilers therein are operating at full power in all weather conditions including heavy weather, an adequate supply of air is maintained to the spaces for the safety and comfort of personnel and the operation of the machinery. Any other machinery space shall be adequately ventilated appropriate for the purpose of that machinery space. The PLC cabinet, inboard side, within the reefer container transformer room, overheats in hot ambient conditions and needs the cabinet doors open with a fan blowing in to keep the equipment operating. Crew placed welded/cardboard covers over original ventilation ports and installed a temporary flexible ventilation system to directly discharge on PLC cabinet. 74 SOLAS (04) II-1/35
