Fire-extinguishing appliances shall be kept in good order and be available for immediate use at all times. LPCO2 Instruction Manual and posted instructions state that the stop valve should normally be open. Inspectors discovered the valve in the closed position. Vessel's crew stated they normally keep this valve closed and there is a small placard at the fire control station instructing the crew to open the valve. 74 SOLAS (97) II-2/21 17ac

Any required fixed fire detection and fire alarm system with manually. Inspectors discovered that fire panel for loop eight was in fault. In addition numerous smoke detectors were missing their covers in the cargo spaces and above the #3 generator. 74 SOLAS (97) II-2/13.1.1 50a

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum reduce to a minimum any danger to persons on board. Inspectors noticed #5 generator oil leaks on governor and main rear seal. 74 SOLAS (04) II-1/26.1 50a

Each cargo vessel must carry at least one rescue boat. Vessel self reported their fast rescue boat main shaft bearing failed, class notified, sourcing parts.

MI found that the stern door was secured and watertight when closed but vessel's crew was unable to properly secure the door with locking pins on the port side.

The condition of the ship and its equipment shall be maintained to conform with provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The main engine's electronic control from bridge and engine control room is inoperative. Only local control of engine remains. Provide report from recognized organization providing analysis of the cause of failure and statement regarding fitness for service.

Vessel will sail short 1 x Oiler as required by COI. Master states crew member has quit for personal reasons and company will be unable to fill position prior to scheduled departure of 17JUL.

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should ensure that each ship is: manned with qualified, certificated, and medically-fit seafarers in accordance with national and international requirements. The vessel is currently short 3 of the required crew , 2 ABs and 1 Oiler, as specified on the Certificate of Inspection. Additionally, 1 of the 2 ABs currently missing from the crew, has been missing since 25 April. The company is to conduct an internal investigation into their processes regarding their obligations under US law and the ISM Code, to ensure the safe manning of the vessel. Complete internal investigation and develop corrective actions within 90-days, have ready for RO review at the next annual verification of the DOC.

Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. STBD side embarkation ladder showing signs of dry rot and/or deterioration at the eye splice.

Each vessel of 10,000 gross tons or more must have, in addition to the radar system under §164.35(a), a second marine radar system that operates independently of the first. Vessel's 9 GHz (S-band) radar inoperable. 33 CFR 167.37(a)

Machinery spaces of category A shall be adequately ventilated so as to ensure that when machinery or boilers therein are operating at full power in all weather conditions including heavy weather, an adequate supply of air is maintained to the spaces for the safety and comfort of personnel and the operation of the machinery. Any other machinery space shall be adequately ventilated appropriate for the purpose of that machinery space. The PLC cabinet, inboard side, within the reefer container transformer room, overheats in hot ambient conditions and needs the cabinet doors open with a fan blowing in to keep the equipment operating. Crew placed welded/cardboard covers over original ventilation ports and installed a temporary flexible ventilation system to directly discharge on PLC cabinet. 74 SOLAS (04) II-1/35

Except as specified in paragraph 3 of this regulation, any ships of 10,000 gross tonnage nd above shall be fitted with oil filtering equipment complying with paragraph 7 of this regulation. OWS circulation pump/motor unit failed while conducting OWS testing, thus rendering the unit unusable. MARPOL I/14.2

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Emergency switchboard 220V system has a ground on the L1 phase. 74 SOLAS (04) II-1/45.1.3

All waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. Notated multiple waste storage bins were constructed of combustible materials on upper weather deck between superstructure and open deck. 74 SOLAS (97) II-2/18.5

In addition to complying with the specific provisions for fire integrity of bulkheads and decks mentioned elsewhere in this part, the minimum fire integrity of bulkheads and decks shall be prescribed in tables 44.1 and 44.2. Structural fire protection in battery room and paint locker severely deteriorated, not in compliance with fire ratings required by table 44.1. 74 SOLAS (97) II-2/44.1

Fixed fire detection system continuously fails. Vessel to submit corrective action report to initiate permanent solution. Submit CAR ot OCMI by 31MAR2016.

Vessel had port state control deficiencies that were not documented in SMS. Vessel could not provide any written guidance regarding how to document externally identified non-conformities. Vessel must document the non-conformities and provide written documentation of policy to OCMI by 31MAR2016.

After failed attempts at repair in Fremantle, WA, 835 cleared/reissued to allow vessel to sail to Singapore to replace the system. S-VDR to be replaced prior to sail from Singapore.

Vessel SVDR is not operating properly. Make repair and provide technician report attesting that the SVDR is fully operation. Complete requirement to the satisfaction of FEACT.

Hydraulic rams on the free fall lifeboat davit are not functional leaving vessel without an adequate secondary means of launching the lifeboat. Repair and demonstrate proper operation prior to departure from Singapore, prior to 24MAR2016.

Free fall lifeboat retaining bridle padeye's are wasted and enlongated. Replace or repair padeye's to origional condition

2nd Deck port fwd ballast tank vent piping is wasted at tank top intersection. Repair as origional.
