Fire doors in main vertical zone bulkheads and stairway enclosures double-leaf doors equipped with a latch necessary to their fire integrity shall have a latch that is automatically activated by the operation of the doors when released by the system. Double-leaf fire doors are missing the sequencing bars in doors 8.15 and 4.18. Sequencing devices on fire doors 8.27, 9.14, 9.26, 10.15, and 11.09 are not operating as designed, permitting doors to open in an improper sequence. Additionally, all fire doors identified did not properly indicate their open position from the bridge.

Procedure 2.05.01 Lint Filters, Laundry Services, & Linen Procedures requires that lin filters in drying machines located in laundry be cleaned at least four times a day and documented. PSCOs examined the laundry at 130 and discovered that the 0700 and 1000 cleanings were not conducted and logged on the Laundry Dryer Filter Cleaning Log for 29 Apr 23.
