The general emergency alarm systm shall be audible throughout all the accommodation and normal crew working spaces and open decks. The general alarm could not be heard on the aft portions of the 02, 03, and 04 open decks. 74SOLAS(97)III/50

The general emergency alarm system shall be supplemented by either a public address system or other suitable means of communication. The public address system enable the broadcast of messages into all spaces where crew members are normally present. Several speakers on the aft 02, 03, and 04 decks were inoperable. 74SOLAS(97)III/6.4.2

Doors in fire-resisting divisions are required to be self-closing. The fire door leading to passage 1-87-2 from the galley becomes jammed on the deck when opened completely and will not self close. 74SOLAS(97)II-2/47.2

The vessel must have a planned maintenance program to ensure continued safe operations of all vital systems. In review of the vessel's SAMM status multiple L4 (Highest criticality) was found deferred. In addition, new PMs based on a calendar interval are activated with a 11/10/1998 last date of completion distinguishing the PMs past due at activation. Provide a corrective action plan to address the deferment of critical items and the erroneous reporting of newly activated items to the Coast Guard. 46 CFR 62.50-20(h) Extended till nect ACP Annual exam.

Machinery spaces of a category A shall be adequately ventilated. Multiple supply ventilation ducts where found fouled when deteriorated insulation material. Remove all debris and provide a corrective action plan to address the continued blockage from the deteriorated insulation. SOLAS(97) II-1/35

After the survey of a ship no changes shall be made to the arrangement of the machinery and equipment. The ventilation ducting located in the upper E/R inside the storage locker has deteriorated potential diverting air flow. SOLAS (97) I/11 SOLAS (97) II-1/35

The Incinerator Sludge Tank denoted on the vessel's IOPP 3.1 is missing from the ORB weekly sludge soundings. MARPOL Annex I/17

The hull shall not be used for for a return system, the 120V system indicated a ground. Isolate and repair ground. SOLAS(97) II-1/45.3.1

Vessels utilizing low sulfur F/O for compliance with ECA must establish written change over procedures. On June 17 2021, the vessel took on F/O wit a .8% sulfur content and consumed it without established procedures. Provide procedures prior using F/O above the permissible sulfur content. MARPOL ANNEX VI/14.6

Every ship shall provide documented evidence of the fitness of the alarm system. The crew exclaimed that they routinely modify alarm set points based on operation conditions to limit nuisance alarms. Provide an operating procedure to ensure the continued fitness of the alarm system. SOLAS(97) II-1/46.1 SOLAS(97) II-1/51

Repair the damaged FSD 5-90-2. SOLAS(97) II-2/47

The fire control plan was updated 2015 to include symbols to IMO Resolution A.952(23). When updated the corresponding symbols should have been used to identify fire control equipment IAW ISO 17631.The symbols for the fire control equipment installed in the engineering spaces have not been updated. SOLAS(04) II-2/15

Vessels #1 deck hatch not water tight: Leaking found under the deck hatch on A deck, near the port side aft part of hatch. Make hatch closure water tight. 46 CFR 42.09-25

Doors provided to ensure the watertight opening which are used while at sea shall be capable of being remotely closed from the bridge. Vehicle Door (4-50-1) is inoperable by remote means. 74 SOLAS (97) II-1/25-9.2

Stairways which penetrate more than a single deck shall be protected by self closing doors at all levels. The following fire doors did not properly latch: (05-90-2)(01-97-3)(02-91-4). 74 SOLAS (1997) II-I/46.1

Before the ship leaves port and at all times during voyage all life-saving appliances shall be in working order and ready for immediate use. Life buoy is missing next to #4 lifeboat. 74 SOLAS (2014)III/20.1

Electrical installations shall ensure safety of crew and ship from electrical hazards. Davit Motor Control power available indicators were inoperable. 74 SOLAS (04) II-1/40.1.3

Below the bulkhead deck two means of escape shall be provided. Ensure that all emergency escape hatches are operable. Escape hatch (5-71-2) does not open. 74 SOLAS (97)II-2/28.1.1

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. Loadline marks are worn and need to be repainted. ICLL Reg. 8

Prior to activation from ROS, the following items are required: -Fire and abandon ship drills including testing of the fire main and lifeboats. 74 SOLAS III/18.3

Fire fighting detection and alarm systems shall be kept in good working order. The fire alarm control panel on the bridge is displaying a trouble error. Correct to the satisfaction of the RO. 74 SOLAS (14) II-2/14.2.1.2

Stairways which penetrate more than a single deck shall be protected by self closing doors at all levels. Discovered 9 fire doors throughout the vessel that did not latch properly. Four of the fire doors were corrected on the spot. Correct to the satisfaction of the RO. 74 SOLAS (97) II-1/46.1

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. 1 life buoy needs to be relabeled and 1 life buoy needs the black grab line replaced. Both are located on the stbd weather deck. Correct to the satisfaction of the RO. 74 SOLAS (14) III/20.1

Doors required to be self-closing shall not be fitted with hold-back hooks. However, hold-back arrangements fitted with remote release devices of the fail-safe type may be utilized. Correct to the satisfcation of the RO. 74 SOLAS (97) II-2/47.2

Electrical installations shall ensure safety of crew and ship from electrical hazards. Observed dead end wire on O3 deck. Remove or secure ends of wire in an enclosure. Correct to the satisfaction of the RO. 74 SOLAS (04) II-1/40.1.3

A means of communication shall be provided between the navigation bridge and the steering gear compartment. The sound powered phone X1JV did not work on the bridge and the panel of another sound powered phone on the bridge needs to be tightened. Correct to the satisfaction of the RO. 74 SOLAS (97) II-1/29.10

Ensure that the emergency escape hatches on the E and F cargo decks can easily and swiftly be opened. Correct to the satisfaction of the RO. 74 SOLAS (97) II-2/28.1

Observed a leak on D deck, hold 2 near frame 50. There was a trash can placed underneath to contain the constant drip of water. Determine the cause of the leak and repair. Correct to the satisfaction of the RO. 74 SOLAS (14) II-1/3-1

Provide annual fire fighting servicing report for portable extinguishers and fixed systems. Correct to the satisfaction of the RO. 74 SOLAS (14) II-2/14.2.2

Steering gear compartments shall be provided with handrails and grating or other nonslip surfaces to ensure suitable working conditions in the event of a hydraulic fluid leakage. Non-skid is inadequate to prevent slipping in steering gear compartment.74 SOLAS (14) II-1/29.13.2

Equipment shall be maintained to conform with the provisions of the regulations. Wye hydrant valves at fire station 17 & 46 leaking. 74 SOLAS (14) I/11(a)

Electrical installations shall be such that the safety of crew from electrical hazards will be ensured. Install protective devices in openings of electrical panels. 74 SOLAS (14) II-1/40.1.3

Closed ro-ro cargo spaces carrying motor vehicles with fuel in their tanks for their own propulsion shall comply…electrical equipment and wiring shall be of a type suitable for use in explosive petrol and air mixtures. Multiple light fixtures identified in 6Hold CDeck were found broken with the bulbs exposed. 74 SOLAS (97) II-2/53.2.4.1

Equipment shall be maintained to conform with the provisions of the regulations. Multiple broken light fixtures were identified in the purifier room and machine shop. 74 SOLAS (14) I/11(a)

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The two life jackets in the engine control room were not serviceable.

All electric cable and wiring external to equipment shall be at least of a flame retardant type and shall be so installed as not to impair their original flame-retarding properties. The jacking gear controller cable for engines 1B and 2B had exposed wire due to deterioration of the insulation.74 SOLAS (97) II-1/45.5.2

Containers, brackets, racks, and other similar stowage locations for life-saving equipment shall be marked with symbols in accordance with the recommendations of the Organization. All EEBDs observed on board the vessel were not properly marked. 74 SOLAS (2014) III/20.10 (97) II-2

Where A class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc, arrangements shall be made to ensure that the fire resistance is not impaired. A garden hose was routed through multiple ports in stairway.

Internal combustion engines of 2250 kw and above or having cylinders of more than 300 mm bore shall be provided with crankcase oil mist detectors. The oil mist detection system alarm on engine 2A was disconnected. 74 SOLAS (1997) II-1/47.2

Boilers shall be provided with means to supervise and control the quality of the feedwater. The boiler’s feedwater treatment system was decommissioned and a new system was installed and was found energized by an extension cord. 74 SOLAS (1997)II-1/32.5

Every boiler essential for the safety of the ship and designed to contain water at a specified level shall be provided with at least two means for indicating its water level, at least one of which shall be a direct reading gauge glass. #2 boiler was missing a guard for the sight glass. 74 SOLAS (1997)II-1/32.6

Equipment shall be maintained to conform with the provisions of the regulations. The "high" indicator light cover for the Oily Waste Gravity Tank was missing; multiple gauges on the generators were compromised multiple gauges on MDEs were found missing glass covers, two flow meters for the lube oil purifiers were broken, gauges throughout the engine room were past calibration dates. 74 SOLAS (97) I/11(a)

Equipment shall be maintained to conform with the provisions of the regulations. An emergency light in the Engine Control Room was extinguished. 74 SOLAS (97) I/11(a)

The company should establish and maintain procedures to control all documents and data which are relevant to the safety management system. The company shall ensure that valid documents are available at all relevant locations. 02 lock out/tag out tags were identified in the engineroom without corresponding JSAs. There were multiple yellow caution tags identified in the engineroom that were not identified in the SMS and the crew had conflicting responses regarding their use. 74 SOLAS (2014) IX-1/3.1 ISM Code 11.1, 11.2

Prior to activation from ROS, the following items are required: -Manning review including crew licenses, credentials, and endorsements -Fire and abandon ship drills -Testing of emergency generator SOLAS III/18.3, STCW

74 SOLAS (14) II-2/14.2.1 Structural fire protection shall be kept in good order as to ensure its required performance. Structural fire protection was found missing in the steering compartment in the fwd-upper corners. Repair to the satisfaction of RO.

74 SOLAS (14) II-2/14.2.1 Firefighting systems shall be kept in good working order and readily available. Fire hose was missing at fire station #69. Correct to the satisfaction of RO.

74 SOLAS (04) II-1/25-9 Indicators are to be provided at the control position. The watertight door position indicator at the remote manual station for the aft class 3 WTD is inoperable. Repair to the satisfaction of RO.

74 SOLAS (14) I/11 After any survey of the ship, no change shall be made in the structural arrangements, machinery, equipment, and other items covered by the survey. Observed the electric panel for the stern ramp open with ongoing maintenance occurring without the ship’s crew aware of work. Correct the satisfaction of RO.

74 SOLAS (04) II-1/25-9.2 Indicators are to be provided at the control position. Starboard remote WTD open/close indicator is labeled backwards for the stbd AC Room-ER WTD. Correct to the satisfaction of RO.

74 SOLAS (04) II-1/25-9.2 Indicators are to be provided at the control position. The manual control operation handle for the port and stbd AC space WTDs are not labeled to show open or close operation or the ship’s crew. Correct to the satisfaction of RO.

74 SOLAS (14) II-2/14.2.1.2 Fire -fighting appliances shall be kept readily available. Portable fire extinguisher outside EOS is missing. Correct to the satisfaction of RO.

Electrical installations shall be such that the safety of crew and ship from electrical hazards will be ensured. Non-permanent electrical cord installed on the air dyer in upper engine room powering solenoid drain valve. Correct to the satisfaction of RO.

74 SOLAS (14) II-2/14.2.1.2 Fire -fighting systems and appliances shall be kept readily available. Fire hose at station #17 is stacked on another hose and not stowed on its proper dedicated rack. Correct to the satisfaction of RO.

74 SOLAS (04) II-1/40.1.3 Electrical installations shall ensure safety of crew & ship from electrical hazards. Observed numerous tripped & trouble alarms on actively running & secured equipment on the following power control panels: GCC-P432, GCC P402, and GCC P433. Correct to the satisfaction of RO.

Electrical installations shall ensure safety of crew & ship from electrical hazards. A 100 Amp breaker is installed for distiller No. 1 and the breaker panel index has it labeled calling for a 15 Amp breaker. Correct to the satisfaction of RO.

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use; Found leak at fire station #147 in valve port side bridge wing. 74 Solas (2014) II-2/14.2.1.2

74 SOLAS (04) II-1/26.1 Machinery and fittings shall be of a design and construction adequate for the service for which they are intended and shall be to reduce to a minimum any danger to persons. Ventilation screen in the lower engine room is broken. Repair to the satisfaction of RO.

74 SOLAS (04) II-1/40.1.3 Electrical installations shall ensure safety of crew & ship from electrical hazards. Sensor in the MDE 2A L/O piping is missing a cover and has exposed wiring. Repair to the satisfaction of RO.

74 SOLAS (14) II-2/13.4.2.1.1 The means of escape shall have emergency lighting provisions. Light in the escape trunk in the lower port E/R wasn’t illuminated and escape trunk wasn’t properly lit. Correct to the satisfaction of RO.

Fire fighting detection and alarm systems shall be kept in good working order. The fire alarm control panel in ECR is displaying an error alarm. Correct to the satisfaction of the RO. 74 SOLAS (2014) II-2/14.2.1.2

DC Wifcom system penetrates A class boundaries in superstructure and stair towers using non approved penetration systems. Repair in the presence of ABS Surveyor. 74 Solas (2014) II-2/14.2.1.1

The fire resistance of doors shall, as far as practicable, be equivalent to that of the division in which they are fitted. Provide ABS survey assessment on fire doors and fire boundaries of the galley space. 74 SOLAS (1997) II-2/47.1

After any survey of the ship under regulations 7,8,9, or 10 has been completed, no change shall be made in the structural arrangements without the sanction of the Administration. Vessel fan supply 6-52-2 in cargo space found inoperable, repair to satisfaction of class. 74 SOLAS (2014)I/11

All power operated sliding watertight doors shall be provided with means of indication which will show at all remote operating positions whether the doors are open or closed. Light indicator bulbs on bridge were burnt out. 74 SOLAS (1997) II-I/25-9.2

74 SOLAS (2014) II-2/14.2.1.2 Fire -fighting appliances shall be kept readily available. Portable fire extinguisher No. 184 was found on deck with no servicing tag or safety pin.

33 CFR 159.7(c) When operating a vessel on a body of water where the discharge of untreated sewage is prohibited, the operator must secure each Type III device in a manner which prevents discharge of sewage. Untreated MSD system overboard pipe wasn’t properly secured. Ship’s crew removed the handle to the overboard valve.

74 SOLAS (1997) II-2/15.2.5 Every oil fuel pipe which would allow oil to escape from a service or storage tank, shall be fitted with a valve directly on the tank capable of being closed from outside the space. EDG remote fuel oil shut-off valve did not close.

Non-metallic fuel hose must be in accordance with SAE J1942. Vessel found to have worn fuel hose bent/kinked and unable to confirm J1942 spec. Directed to replace fuel hose on emergency diesel generator. SOLAS II-2 Regulation 4-2.2.5.1

Vessel's Periodic Testing of Vital Sytem Automation is lacking the procedures to adequately perform the periodic safety tests required by 46 CFR 61.40-10. The current revision of the vessel's PSTP was approved by the USCG in 2002, but it fails to address several of the systems required by the regulations.

Prior to activiation: -Conduct satisfactory abandon ship and fire drill. -Review manning and MMDs in accordance with the vessel's COI. -Provide updated and corrected chart and navigation publications.

Closed ro-ro cargo spaces carrying motor vehicles with fuel in their tanks for their own propulsion...above a height of 450mmfrom the deck and from each platform for vehicles...electrical equipment of a type so enclosed and protected as to prevent the escape of sparks...Several lamp coverswere missing or broken in the overhead lighting in C and D decks.

There shall be provided a fixed fire detection and fire alarm system complying with the requirements of reg 13. The fixed fire detection system shall be capable of rapidly detecting the onset of fire. On port side C-deck Hold 1, a smoke detector was missing cover. Ensure detector is fully functional.

Two means of escape shall be provided from each machinery space of Category A. Removal all gear stowed in the machinery space emergency escape.

The minimum fire integrity of bulkheads and decks shall be as prescribed in tables 44.1 and 44.2. Stairways and ro-ro cargo spaces shall meet A-30 boundaries. -On F deck several areas of insulation was falling away from bulkheads or the steel was heavily rusted. -Insulation was pulled off on B deck, hold 2 emergency escape.

The minimum fire integrity in ro-ro cargo spaces in decks shall meet foot note h and *. Decks separating ro-ro cargo spaces shall be capable of being closed reasonably gas tight and such divisions shall have "A class" integrity in so far as is reasonable and practicable. Deck penetration in starboard B deck was damaged and no longer gas tight. Insualtion removed on wiring in overhead.

Electrical equipment and wiring, if fitted, shall be of a type suitable for use in explosive petrol and air mixtures. On port side A/B deck, found improper electrical cable for residential/automotive pipe heater.

A ship of 300 GT or more constructed after June 30, 1974 must have a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe, for a ship of 1600 or more gross tons and have a capacity of one barrel. The port side aft diesel refueling station has no containment around the hose/fill station.

Each portable container of flammable or combustible liquid used for portable auxiliary equipment must be stowed in a pain locker or an open location designated by the master.

A main electri lighting system which shall provide illumination throughout those parts of the ship normally accessible to and used by the crew shall be supplied by the main source of electrical power. -Main lighting circuit in forward B deck did not illuminate. -Lights were extinguished or intermittently operated throughout the cargo holds.

In the event of a power failure to any one of the steering gear power units, an audible and visual alarm shall be given on the navigation bridge. The steering failure lamp on the port side did not illuminate.

Where air pipes to ballast and other tanks extend above the greeboard or superstructure decks, the exposed parts of the pipes shall be of substantial contruction. The following gooseneck vents were found wasted: -Outboard of fire station 107 -Void vent, stbd side A deck, across from fire station 122 -IBFO Aft (on port side A deck) -A deck port side ballast tank at collar/closing device.

Muster and embarkation stations shall be adequately illuminated. The lifeboat and life raft embarkation station lights had heavy corrosion on the light mounting flanges, articulating poles, and stand posts.

Ever lifebuoy shall be fitted with a grabline...The grabline shall be secured at four equidistant oints around the circumference of the buoy to form four equal loops. The lifebuoy on aft portside A deck was found completely wasted.

Each lifebuoy shall be marked in block capitals of the Roman alphabet with the name and port of registry of the ship it is carried. Each lifebuoy on board was missing the ship's port of registry.

Fire extinguishing appliances shall be kept in good order and be available for immediate use at all times. -Foam piping wasted at fire station 6E -Fire station 26 hydrant leaking at stem.

In all ro-ro cargo spaces where the crew is normally employed the number and locations of escape routes to the open deck shall be to the satisfation of the Administration. When the ship blacked out, approximately 30 secsonds went by without emergency power. The decks were completely dark and the emergency escapes must be marked with photo-illuminescent signage so crew can proceed to escapes if needed.

Containers, brackets, racks, and other similar stowage locations for life-saving equipment shall be marked with symbols in accordance with the recommendations of the Organization (MSC/Circ.955), indicating the devices stowed in that location. The ship's EPIRB was not marked.

The condition of the ship and its equipment shall be maintained to confrom with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. -Standing water identified in AB deck stbd side near ramp and ramp to C/D deck -Hydraulic oil leak from B deck hold 2 side shell door -AB deck hydraulic tnak for ramp winch motor leaking into containment and onto deck. -Hydraulic oil leaks on deck and around hydraulic motors for water tight doors. -C deck Hold 1 Vent fire damper missing pin; may be the same on other fire dampers -Corrosion on flying bridge. Several areas identified; railins, VHF antenna stand, around mast head lights. -Substantial corrosion was identified throughout F deck including coaming, deck ventilators, and fire dampers -Substantial corrosion was found on the port side lifeboat embarkation deck. The steel was especially thin in one are that a dime sized hole was made in the deck. Corrosion was identified surrounding the decks of the davits and winches on both lifeboat embarkation stations.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. On aft port side A deck, under the ensign, an electrical rat's nest was identified.

Engine-room Telegraph shall alarm when orders are given from the bridge. An audible alarm did not function when orders were given. In addtion, the mismatch light for the STBD propulsion control did not illuminate.

Piping shall maintain the integrity of the system. #2 fuel oil booster pump was leaking into a full catch tray.

Control of main propulsion must be in duplicate. #3 PLC which serves as control for half of the main propulsion was not functioning.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The #1 lifeboat's rudder would not turn.
