Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. It was reported that the stop solenoid valve on the main engine was faulty and had caused the main engine to not start. Make appropriate repairs to the satisfaction of the cognizant marine inspector.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The disposable hydrostatic release that allows the EPIRB to float free, is expired and the EPIRB is not mounted to the vessel to allow float free operation. Install a new un-expired hydrostatic release, and mount the EPIRB to the vessel in a manner that allows it to float free if the vessel sinks.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The FCC Safety Radio Certificate was expired. A new FCC Safety Radio Certificate must be obtained and placed onboard the vessel.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The EPIRB registration is expired. Register the EPIRB with NOAA.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: (1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection; (2) Perform the duties and functions of patrolmen or watchmen required by this chapter; or, (3) Are specifically assigned the duties of warning, mustering, assembling, assisting, or controlling the movement of passengers during emergencies. There was no drug testing program in place for 2026. Establish a drug testing program in compliance with 46 CFR 16.230,and ensure all crewmembers are enrolled.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. There is no detector installed in the main machinery space. Install a working fire detection device in the main machinery space that is interconnected to the other fire detection devices already installed.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A space with a through-hull fitting below the deepest load waterline, such as a lazarette; The required high water visual and audible alarm in the lazarette was not functioning. Repair the high water alarm so that it functions with a visual an audible indicator at the operating station.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. Three of the life jacket lights were not operating. Ensure all life jacket lights operate properly.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. The wooden deck joist supporting the main deck at the starboard side of the engine room access hatch is rotted and unsupported causing a safety hazard to passengers and crew transiting this area. It is also degrading the structural integrity of the main deck. Repair the wooden deck joist to the satisfaction of the attending Coast Guard Marine Inspector.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. The starboard side of the hull amidships sustained major damage from the bulkhead deck to the top of the bulwark including the railings while the vessel was being hauled out. Repair the damage to the hull, bulwark, and railings to the satisfaction of the attending Coast Guard Marine Inspector.

Except for engine exhausts, each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 150 millimeters (6 inches) above the deepest load waterline must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails. The 1.5” ball valve for the starboard main engine cooling water inlet was damaged and would not close completely against the flow of water. Install a new valve to the satisfaction of the attending Coast Guard Marine Inspector

Except for engine exhausts, each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 150 millimeters (6 inches) above the deepest load waterline must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails. The 1.5” ball valve for the port main engine cooling water inlet was damaged and would not close completely against the flow of water. Install a new valve to the satisfaction of the attending Coast Guard Marine Inspector

Except for engine exhausts, each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 150 millimeters (6 inches) above the deepest load waterline must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails. The 2” gate valve for the sewage tank overboard discharge was damaged and would not close completely against the flow of water. Install a new valve to the satisfaction of the attending Coast Guard Marine Inspector.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. The detector in the forward crew bunk room is missing, and the galley detector is not interconnected with the engine room detector. Install a properly functioning interconnected fire detection system in all required spaces aboard the vessel.

Stbd running light inoperable. All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Repair or replace.

No designated closure devices on board for engine room vents. provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Provide closure devices designated for engine room vents.

Cable penetrations on the fwd bulkhead of the engine room allow for the passage of vapors into the fwd storage space. Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. Make fwd engine room bulkhead vapor-tight.

Port Engine starts inconsistently. Port engine should start consistently in accordance with manufacture design. Repair to make port engine start consistently per original design.

Drug testing program employee enrollment letter only has vessel captain listed and is missing the rest of crew members. Enroll all crew members in vessel drug testing program.

Vessel unable to provide FCC Marine Radio Operator Permit. Provide FCC Marine Radio Operator Permit.
