Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation.
The primary overcurrent protection for the DC circuit was found with blown 30 amp fuses that were modified with copper "jumpers" to bypass the fuse. The conductor is not protected by overcurrent protection.

The company installed new engine control modules along with new power conductors sometime after 1996. Crew members installed a 30-amp double pole AC breaker to protect the 10 AWG conductor in the DC circuit from the battery switch to the main engine control module panel. The double pole AC breaker is not rated or approved as overcurrent protection for a DC circuit. This exact installation was found on one of the company's sister vessels which resulted in the conductor overheating at the battery switch and causing a fire in the engine room. The installation is not approved and is not to the satisfaction of the OCMI.

The neutral of a dual-voltage AC system shall be solidly grounded at the generator switchboard with an ammeter in the ground connection as per 46 CFR 111.55-10(b).
The marine inspector identified that the dual voltage AC generator neutral is not grounded at the switchboard or anywhere onboard. If a neutral wire is not connected to ground, the primary electrical hazard is the potential for a significant shock hazard due to the possibility of the neutral wire becoming energized, causing exposed metal parts of appliances or equipment to reach a dangerous voltage if a fault occurs, potentially leading to severe electric shock if touched by a person. The installation is not to the satisfaction of the OCMI.

A vessel of not more than 19.8 meters (65 feet) in length is not required to carry a rescue boat unless: The vessel carries passengers on an open or partially enclosed deck; and the OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. The CAROLINIAN was deemed to be adequate as its own rescue craft by the OCMI so long as a sufficient rescue platform is used in conjunction with the vessel due to the free board of the vessel. The rescue ladder on board does not have a sufficient platform at the bottom that can be used by the crew member to retrieve a person. The crew member must hold the ladder with one hand and retrieve the person in the water with the other. The rescue platform was also found to be unstable unless lashed to the CAROLINIAN and no lashings were used during the observed drill. Provide an updated rescue platform with a sufficient platform to retrieve personnel at the bottom and lash the ladder to the side of the vessel when performing drills.
