CAROLINIAN

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. The primary overcurrent protection for the DC circuit was found with blown 30 amp fuses that were modified with copper "jumpers" to bypass the fuse. The conductor is not protected by overcurrent protection.

Emergency instructions must include at a minimum the information found in 46 CFR 185.25-5 which require closing machinery space openings in the event of a fire. The machinery space opening for the natural air duct on the stbd side was frozen open and cannot be closed in the event of a fire.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Operational test of all overboard discharge and intake valves and watertight bulkhead pipe penetration valves. The sea water intake valve for machinery cooling on the port main engine was found leaking at the valve thread coupling.

Piping used in a vital system must: (1) Be composed of ferrous materials except when: (i) Nonmetallic piping materials are permitted by § 182.720; or (ii) Nonferrous metallic piping materials are permitted by § 182.730; Flexible rubber hose that does not comply with § 182.720 was found installed in the vital piping for the fwd void bilge pickup. The rubber hose is installed in between original metallic piping and does not comply with 182.40-5 or 182.710.

Piping used in a vital system must: (1) Be composed of ferrous materials except when: (i) Nonmetallic piping materials are permitted by § 182.720; or (ii) Nonferrous metallic piping materials are permitted by § 182.730; The bilge suction piping for the fwd void space was found corroded in the AC room space.

When necessary for safe operation, the cognizant OCMI may place specific stability restrictions in a stability letter or on the Certificate of Inspection of a vessel not more than 65 feet (19.8 meters) in length, which, under § 178.310 of this part, complies with the requirements of § 178.320 of this part. The stability letter for the vessel requires that the bilge remain clean and dry at all times. The marine inspector identified approximately 1 foot of water in the AC room bilge and approximately 1-2 gallons of oil in the machinery space bilge.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The owner reported that the vessel lost hydraulic steering with eight passengers on board while underway. The vessel is equipped with dual propulsion and the master navigated safely back to the dock without further incident. The crew identified a fractured flexible hose which led to the hydraulic leak. The vessel is restricted from passenger operations until the steering and repairs are verified by a marine inspector. 46 CFR 176.700

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection (OCMI) on or after March 11, 1996, must comply with New Subchapter T regulations. Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment. Overcurrent protection must be provided for each ungrounded conductor. Conductors must be protected in accordance with their current carrying capacities. Cable and wire for power and lighting circuits must: Meet Section 310-13 of NFPA 70; Be listed by Underwriters Laboratories (UL), as UL Boat or UL Marine cable; or Meet § 111.60-1 in subchapter J of this chapter for cable, and § 111.60-11 in subchapter J of this chapter for wire. The company installed new engine control modules along with new power conductors sometime after 1996. Crew members installed a 30-amp double pole AC breaker to protect the 10 AWG conductor in the DC circuit from the battery switch to the main engine control module panel. The double pole AC breaker is not rated or approved as overcurrent protection for a DC circuit. This exact installation was found on one of the company's sister vessels which resulted in the conductor overheating at the battery switch and causing a fire in the engine room. The installation is not approved and is not to the satisfaction of the OCMI. 46 CFR 183.115

An electrical installation that has any current carrying conductor intentionally connected directly or indirectly to the metal hull shall be subject to certain additional requirements as determined by the [name withheld] additional requirements are contained in applicable regulations in Subchapter J. The neutral of a dual-voltage AC system shall be solidly grounded at the generator switchboard with an ammeter in the ground connection as per 46 CFR 111.55-10(b). The marine inspector identified that the dual voltage AC generator neutral is not grounded at the switchboard or anywhere onboard. If a neutral wire is not connected to ground, the primary electrical hazard is the potential for a significant shock hazard due to the possibility of the neutral wire becoming energized, causing exposed metal parts of appliances or equipment to reach a dangerous voltage if a fault occurs, potentially leading to severe electric shock if touched by a person. The installation is not to the satisfaction of the OCMI. 46 CFR 183.10-30

There is no ground fault detection installed at the switchboard. The installation is not to the satisfaction of the OCMI. 46 CFR 183.10-30

A vessel of not more than 19.8 meters (65 feet) in length is not required to carry a rescue boat unless: The vessel carries passengers on an open or partially enclosed deck; and the OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. The CAROLINIAN was deemed to be adequate as its own rescue craft by the OCMI so long as a sufficient rescue platform is used in conjunction with the vessel due to the free board of the vessel. The rescue ladder on board does not have a sufficient platform at the bottom that can be used by the crew member to retrieve a person. The crew member must hold the ladder with one hand and retrieve the person in the water with the other. The rescue platform was also found to be unstable unless lashed to the CAROLINIAN and no lashings were used during the observed drill. Provide an updated rescue platform with a sufficient platform to retrieve personnel at the bottom and lash the ladder to the side of the vessel when performing drills. 46 CFR 180.210(b) and 46 CFR 176.30-1

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18). Letters and numbers must be at least 50 millimeters (2 inches) high. Provide new 2" high letters for CHILD life jacket storage in each compartment used for storage. CHILD life jackets cannot be stored on the bridge due to the door that can be closed while underway separating the bridge from the passenger space. 46 CFR 185.604(f)

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters "Emergency Exit, Keep Clear." The fwd emergency exit in the centerline void space where the AC system is located is not marked IAW regulations. 46 CFR 185.30-15

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. MI identified a fracture on the #1 transverse beam in the fwd void space and on the backing strip for the vertical weld on the port hull in the fwd void space. 46 CFR § 176.700

Each distribution panel and switchboard must be in as dry a location as practicable, adequately ventilated, and protected from falling debris and dripping or splashing water. Each switchboard must be fitted with a dripshield. The ventilation distribuition panel located in the engine room on the starboard side is missing a drip shield and installed directly under the fire main piping. 46 CFR § 183.330

All vessels except non self-propelled vessels shall be provided with suitable steering apparatus. The operator reported a loss of steering while underway. 46 CFR 182.30-1

All electrical installations shall be to the satisfaction of the OCMI. The air conditioner pump conductors were found burned due to resistance and the air conditioner is out of service. Replace the conductors in accordance with 46 CFR 183 New "T". 46 CFR 183.01-15(a)

Repair damaged light fixture in steering compartment.

Cut the excess electrical wire for bile alarm in steering compartment and install support brackets to secure the wire.

Propulsion machinery shall be capable of operating at constant marine load without exceeding its design limitations (46CFR182.05-1). Port propulsion engine sustained alarm conditions for high engine air inlet temperature, high temperature reduction gear and high pressure fuel.

Replace all identified worn out & damaged gaskets for doors & hatches.

Repair fuel leak on fuel hose fitting on port propulsion engine racor fuel filter.

Install missing front cover on electrical panel in aft engine room.

Replace damaged engine room ventilation ducts.

Replace wasted electrical junction box for AFT deck flood light.

Replace damaged engine RPM gauges at AFT control station.

Troubleshoot and repair low fuel pressure alarm for port propulsion engine.

Vessel reported a loss of propulsion while transiting back to port. Vessel returned on her port ME. Vessel is to conduct permanent repairs prior to carrying passengers.

Repair wasted remote reach rod for suction valve on bilge pipe in space forward of collision bulkhead.

VSL's SSB radio is not programed to operate in frequency. Program proper frequency into SSB and show proper operation.

Escape hatch must be mark with 1 inch letter "EMERGENCY EXIT, KEEP CLEAR" as required by 46 CFR 185.30-15. Re-stencil faded mark on emergency hatch in wheel house.

Watertight door must be marked on both sides 1-inches "Watertight Door - Close in Emergency" as required by 46 CFR 185.30-25. Stencil watertight door forward of engine room.

Vessel on ocean route required to have operational Sing Side Band radio as required by 47 CFR 80. Repair or replace inoperable SSB. Restricted vessel route to no more than 100 NM from shore until SSB radio is properly install and attest by attending MI.

Make repair IAW with approved design and operational test to be completed to the satisfaction of the attending MI.

Found hydraulic oil leaking from steering gear piping. Bilge must be free of oil and fire hazards as required by 46 CFR 176.830. Repair hydraulic oil leaks and remove oil in the engine room bilge.

Found engine oil leaking through engine valve cover. Engine room bilge must be freed of oil engine oil create fire hazard as required by 46 CFR 176.830. Replace valve cover gasket.

Found lagging on generator exhaust pipe saturated with oil. Exhaust lagging must be free of oil to reduce fire hazard as required by 46 CFR 176.830. Replace oil saturated exhaust lagging.

Found automobile light bulb installed on anchor light. Anchor light must be approved under UL 1104. Replace light bulb with UL 1104 approved light bulb.

Flexible hose designed for use with hose clamps may have two clamps at both ends of the hose. Missing one hose clamp on fuel supply line to fuel filters. Install hose clamp.

Air condition shall be fit for service intended. Water leaking from salt water pump. Repair as original.

Flexible hose must be protected from mechanical damage. Found fuel supply hose to STBD engine chafing against engine foundation. Install hose protection.

Exhaust pipe not expose to cooling water must be insulated. Found exhaust pipe for SSDG & both propulsion engines missing lagging insulations. Install missing lagging.

Piping must comply with Subpart G, part 182. Found fire main pipe leaking. Repair damaged pipe.

Vessel sustained hull damage to port side shell due to typhoon. Submit repair plan to CG, conduct repair IAW approved plan to be attest by attending MI.

Horn not functioning. Repair/replace horn.

Provide proof that 50 % of the deckhands have First-Aid/ CPR training by the American Red Cross, American Heart Association or a course approved by the local OCMI in accordance with NVIC 1-91.

Label emergency exits and escape hatches "EMERGENCY EXIT KEEP CLEAR"

Label all watertight doors and hatches "WATERTIGHT DOOR KEEP CLOSED" or "WATERTIGHT HATCH KEEP CLOSED" as appropriate.

Found EPIRB teather line not attached to the vessel. Connect EPIRB teather line to vessel.

Found all batteries attached to knife style quick disconnects. Replace knife style with approved quick disconnect throughout vessel.

Charts and publications not corrected to date in non-adherence with 46 CFR 184.420.

Fore-peak deck board fractured. Replacement required not later than 28 Oct 2002

Two pin holes observed at hull - aft deck centerline interface. Temporary repair required to restore watertightness required nlt 28 Oct 02, with permanent repair required at next drydock.

IN ACCORDANCE WITH 46 CFR 182.25-5(A) PROVIDE INDIVIDUAL BILGE LINES AND BILGE SUCTIONS FOR THE FOREPEAK TANK & THE NEXT AFT WATERTIGHT COMPARTMENT.
