The emergency check off list shall include procedures to cut off air supply to a fire-close hatches, ports, doors, and ventilators. The ventilation closures for the fwd ventilation ducts to the engine space do not have proper gasket material to cut off the air supply to the space. 46 CFR 185.25-1

Batteries shall be secured against shifting with motion of the boat. The port and stbd main engine starting batteries are not secured for movement. 46 CFR 183.05-20(b)

All electrical installations shall be to the satisfaction of the OCMI. The postitive conductor attached to the stbd main engine starter was found with a bend creating resistance which could lead to a fire. Straigten the connection so that there is no bend in the connection. 46 CFR 183.01-15

Diesel engines shall be water cooled. The stbd main engine rubber hose found in the engine cooling unit is corroded and shall be replaced. 46 CFR 182.20-10

A power driven fire pump shall be installed on each mechanincally propelled "S" vessel. The fire pump installation was found missing mounting bolts and the fire pump can be moved by hand. Permanently mount the fire pump. 46 CFR 181.10-1

The equipment required for lifefloats and buoyant apparatus must meet the following specifications:Each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant. Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Four life float paddles were found fractured and are not ready for service. The paddles shall be replaced like and in kind. 46 CFR 180.175

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel suffered a loss of propulsion marine casualty due to the engine unexpectedly shutting down. It is assumed that a crew member misaligned the fuel system to the main engine causing fuel starvation. Operator must ensure crew is properly trained for fueling evolutions and that the loss of propulsion was not caused by faulty machinery.

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. Lagging covering exhaust for port side engine was damaged and must be replaced in kind.

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, completed to the satisfaction of the OCMI, must comply with 46 CFR 183 (New T) as per 46 CFR 183.115(c). MI identified "new" conductors installed in the DC circuit inside of the engine room that are not marked as per regulations. Cable and wire for power and lighting circuits must comply with 46 CFR 183.340(d) which states that the conductor shall be marked with its AWG size and suitability. Furthermore, the conductor was found installed and only supported by the generator exhaust piping, creating a fire hazard. 46 CFR 183.115(c)

Spaces containing machinery shall be fitted with at least two ducts to furnish natural or mechanical supply and exhaust ventilation. One duct shall extend to a point near the bottom of the compartment. Where forced ventilation is installed, the duct extending near the bottom shall be the exhaust. The machinery space forced exhaust mechanical ventilation was found inoperable. The exhaust duct piping was found disconnected in the middle of the compartment and is not installed in a manner that would provide exhaust ventilation from the space. 46 CFR 182.20-45(c)

An electrical installation that has any current carrying conductor intentionally connected, directly or indirectly to the metal hull, shall be subject to additional requirements as determined by the [name withheld] additional requirements are contained in 46 CFR subchapter J. As per 46 CFR 111.30-10(c)(12)(ii), for dual voltage, grounded-neutral, AC distribution systems, an ammeter shall be provided to indicate current flowing in the ground connection. The ammeter shall have a full scale range of 10 amperes. An ammeter switch of the spring return to "on" shall be provided. The dual voltage generator was found without ground fault detection installed. 46 CFR 183.10-30(a)

Each hatch exposed to weather must be watertight except weathertight hatches identified in 46 CFR 171.124(a)1-3. The flush deck hatches to the engine room compartment on the main deck were found with wasted gaskets and the hatches cannot be made watertight. 46 CFR 177.13-1

A vessel of not more than 19.8 meters (65 feet) in length is not required to carry a rescue boat unless: The vessel carries passengers on an open or partially enclosed deck; and the OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. The CELTIC was deemed to be adequate as its own rescue craft by the OCMI so long as a sufficient rescue platform is used in conjunction with the vessel due to the free board of the vessel. The rescue ladder on board does not have a sufficient platform at the bottom that can be used by the crew member to retrieve a person. The crew member must hold the ladder with one hand and retrieve the person in the water with the other. The rescue platform was also found to be unstable unless lashed to the CELTIC and no lashings were used during the observed drill. Provide an updated rescue platform with a sufficient platform to retrieve personnel at the bottom and lash the ladder to the side of the vessel when performing drills. 46 CFR 180.210(b) and 46 CFR 176.30-1

After July 1, 1968, all life floats and buoyant apparatus shall be international orange in color. The two life floats stored on top of the vessel are discolored due to mold. The life floats do not meet the approved standard or color and shall be painted with the approved [name withheld] paint. 46 CFR 180.10-5(b) and 46 CFR 180.200

Conductors shall be protected from overcurrent in accordance with their current carrying capacity. MI identified a blown fuse in the DC circuit that was soldered with wire in order to bypass the fuse. The installation does not protect the conductor. 46 CFR 180.05-50(c)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). The VHF on board does not have the GPS linked to the DSC function and the long/lat will not transmit if the Distress DSC button is used. 46 CFR 184.502

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18). Letters and numbers must be at least 50 millimeters (2 inches) high. Provide new 2" high letters for life jacket storage in each compartment used for storage. 46 CFR 185.604(f)

All wiring shall be run as high as practical above the bilges. The two main engine starting conductors and generator starting conductor were found installed in the bilge and without supports. 46 CFR 183.05-50(a)

Wiring joints and splices shall be mechanically secure and made in junction boxes. MI identified conductors in the DC circuit improperly spliced and not made secure in junction boxes. 46 CFR 183.05-50(b)

A ring life buoy must be of an approved type and constructed IAW subpart 160.050 of 46 CFR. The ring life buoy was found faded in color and does not meet the standard found in 46 CFR 160.050. 46 CFR 180.30-1

Repair inoperable temperature gauge for port propulsion engine.

Life float must remain in good and serviceable conditions. Two life floats on board have severe outer layer damage due to sun exposure. VSL is restricted to operate within (03) miles of land per 46CFR180.205 (e)until such time when VSL meets 46CFR180.205(d).

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel Must be equipped with an operational and valid CAT II EPIRB prior to going beyond 3NM from shore.
