MI identified "new" conductors installed in the DC circuit inside of the engine room that are not marked as per regulations. Cable and wire for power and lighting circuits must comply with 46 CFR 183.340(d) which states that the conductor shall be marked with its AWG size and suitability. Furthermore, the conductor was found installed and only supported by the generator exhaust piping, creating a fire hazard.

The machinery space forced exhaust mechanical ventilation was found inoperable. The exhaust duct piping was found disconnected in the middle of the compartment and is not installed in a manner that would provide exhaust ventilation from the space.

A vessel of not more than 19.8 meters (65 feet) in length is not required to carry a rescue boat unless: The vessel carries passengers on an open or partially enclosed deck; and the OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. The CELTIC was deemed to be adequate as its own rescue craft by the OCMI so long as a sufficient rescue platform is used in conjunction with the vessel due to the free board of the vessel. The rescue ladder on board does not have a sufficient platform at the bottom that can be used by the crew member to retrieve a person. The crew member must hold the ladder with one hand and retrieve the person in the water with the other. The rescue platform was also found to be unstable unless lashed to the CELTIC and no lashings were used during the observed drill. Provide an updated rescue platform with a sufficient platform to retrieve personnel at the bottom and lash the ladder to the side of the vessel when performing drills.
