SUPER VOYAGER III

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The vessel center shaft sheared off and the vessel lost the center propeller. Replace center shaft and propeller.

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. All the inflatable rafts were found with the painters attached to the shackle directly to the vessel and not to the weak-link.

A vessel which, complies with requirements in subchapter S of this chapter, must have stability details on the vessel's Certificate of Inspection, a stability letter issued by the cognizant Officer in Charge, Marine Inspection (OCMI) or the Commanding Officer, Marine Safety Center An Inclining Experiment was performed on 02MAY24 after the vessel was re-powered at the last shipyard period in April 2024 - MISLE - 7848036. The vessel's current Stability Letter dated 01DEC11 allowed for the carriage of 114 Total Persons. Vessel's passenger count is restricted to 53 Total Persons (50 Passengers and 3 Crew) with zero (0) passengers allowed on the upper deck, until fixed ballast is installed and the new Stability Letter is issued by the Marine Safety Center.

A vessel which, under 46 CFR 178.310 complies with requirements in Subchapter S of 46 CFR, must have stability details on the vessel COI or stability letter issued by the Marine Safety Center. During a recent dry dock period the vessel has undergone numerous modifications resulting in a significant weight change and therefore must conduct a new stability test, prior to the carriage of passengers.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs; Vessel's center strut tube is cracked on forward edge. Blast strut and shaft log for inspection of all welds.

The marine inspector may conduct a visual examination and may require nondestructive testing of the propeller shafting whenever the condition of shafting is in question; shaft is damaged and must be renewed along w/ cutlass bearing.

If the annual inspection reveals deficiencies in your vessel's maintenance, you must make any or all repairs or improvements within the time period specified by the OCMI. Vessel operating with 03 deficiencies open past due date

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). 2 fixed power pumps of 50 GPM Bilge pumping arrangement was not operational or of sufficient capacity.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. Bilge high water alarm worked intermittently.

A hatch exposed to the weather must be watertight. Fore peak and generator space failed hose test.

Waste receptacles. Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom.

Except as otherwise provided in this section, each space accessible to passengers or used by the crew on a regular basis, must have at least two means of escape, one of which must not be a watertight door. Marked escape hatch from rod storage room was blocked by a rug. hatch was freed up and rug removed.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter. 14 life jacket lights were extinguished. Batteries were replaced and made operational.

Vessel required to be equipped with an operable fire pump. Existing fire pump control box located in the engine room requires repair/replacement due to worn (internal) components (contacts)

Vessel's EPIRB required to be registered with NOAA on an Annual basis. Existing EPIRB regisration has expired.

Vessel required to be equipped with serviceable life saving equipment. Existing life floats require maintenance/repair to its main body (paint/markings/retro-reflective markings), netting, painter lines, and peripheral pendants.

Vessels required to be equipped with two paddles for each life float. Existing paddles require replacement due to UV deterioration/degradation.

Vessels portable fire extinguishers require annual servicing. Existing portable fire extinguishers are expired/require annual servicing.

Vessels lower deck hatch/access covers are required to be made watertight. Existing hatch/access covers IWO fwd storeroom & (old) crew quarters/rod room are not secured/watertight.

Vessel required to be equipped with a fixed bilge system capable of dewatering any compartment. Existing fixed bilge system is not capable of drawing suction from the fwd voids of the vessel.

Vessel required to be equipped with emergency fuel shut-off valves. Existing fuel shut off valve reach rod IWO port engine room requires repair/(re)connecting.

Vessel's main deck (exterior) hatches are required to be made watertight. Existing hatch for lazarette is damaged/not allowing hatch to seal effectively.

Flexible hoses in vital systems are required to be secured with at least two clamps. Existing shaft log is missing securing clamps.

Type I PFD's are required to be equipped with PFD lights. Five (05) child sized PFD lights were extinguished/inoperable.

Vessels pressure vessels are required to be fitted with relief valves. Starboard air receiver's relief valve was seized shut and was not capable of being hand lifted.

Vessels required to be fitted with emergency/remote fuel shut off valves. Existing universal joints for port reach rod assembly were found worn.

Vessel required to be equipped with a suitable steering system. Port tiller arm IWO lazarette requires repair/machining to ensure proper fit for rudder post.

Vessels DSC equipped radio-telephones are required to be programmed with vessel particulars. Existing VHF-FM radiotelephone is not properly programmed with (current) MMSI #.

Vessel required to be equipped with Class A AIS. Existing Class A AIS unit is not properly programmed with MMSI #.

Vessel required to be equipped with a fire main system. Existing ball valve handles for port & starboard fire stations along main deck are wasted.

No major repairs or alterations affecting the safety of the vessel with regards to hull, machinery, or equipment shall be made without the knowledge and approval of the cognizant [name withheld] conducting an interal inspection of the port main diesel engine bay, attending Inspector called out fractured weldments IWO bottom longitudinal framing at aft transverse framing. Temporary repairs are authorized until next scheduled hull examination scheduled for 28FEB2022.

Vessels lifesaving equipment is required to be marked with retro-reflective markings. Vessels retro-reflective markings IWO life floats were worn/deteriorated.

Floating water light batteries are required to be renewed annually. Vessels life float batteries were expired.

Each vessel that opeates on the high seas must have on board an FCC Type Accepted Cat. 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessels hydro-static release unit (HRU) was found expired (beyond service life).

Each life float must be equipped with paddles (min. 4') and be lashed to life float to which it belongs to and must be buoyant. Vessels paddles IWO life floats were found cracked/checked/deteriorated.

Vessel must be equipped with an operable MF/HF radio-telephone if operated beyond between 20nm & 100 nm. Vessels microphone for radio-telephone was deteriorated/required repair/replacement.

Vessel is required to be fitted with a suitable magnetic compass and must be illuminated. Vessels magnetic compass was not illuminated.

Vessel is required to be equipped with navigational lights. Vessel's not under command (NUC) lights are not properly wired/configured.

Vessels watertight hatches are required to be marked "WATERTIGHT HATCH KEEP CLOSED". The watertight hatch IWO lazarette is missing required markings.

Vessels emergency escape hatches/scuttles are required to be marked "EMERGENCY ESCAPE KEEP CLEAR". Vessels emergency escape hatch/scuttle IWO crew quarters/accomodations is not marked on the exterior/top.

Vessel is required to be fitted with emergency fuel shut-off valves, properly installed/marked to indicate operation. Vessels emergency fuel shut-off valves were not marked with direction of operation.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Vessels lighting fixture IWO serving area (above refrigerator) was found fractured/broken.

Each fire nozzle must be of a corrosion resistant material and be capable of being changed between a solid stream and spray pattern. Vessels fire nozzle(s) were of the proper type/style, however, the nozzle were seized in the closed position.

Vessel must be equipped with batteries of adequate capacity to supply the loads for three hours. Vessels emergency communications battery IWO fwd storeroom has excessive ring terminals installed.

Each distribution panel must be totally enclosed and be of the dead front type. Vessels panelboard IWO fwd storeroom is not installed properly (missing labels/markings to indicate service).

Vessel required to be equipped with watertight bulkheads. Vessels watertight bulkhead IWO fwd. crew accomodations has a bulkhead penetration that is not considered watertight.

Vessel required to be equipped with watertight hatches/covers. Vessels hatch IWO fwd crew quarters/accomodations space is not equipped with securing hardware/gasketing.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Vessels lighting fixture IWO crew quarters/accomodations space was found fractured/broken.

Vessel required to be fitted with watertight hatches/scuttles. During operational test of watertight hatch IWO lazarette, attending Inspector(s) noted that the hatch would not fully seal.

Vessels auxiliary machinery is required to be equipped with an operable temperature gauge. Vessels port SSDG temperature gauges was found faulty/inoperable.

Vessel is required to be capable of dewatering any compartment. Vessels electric submersible bilge pump IWO lazarette failed to operated when tested.

Vessel must be equipped with a main steering gear that is of adequate strength. Excessive play was noted IWO port rudder post/tiller arm (keyway), port & stbd tiller arm connections.

Vessel's electrical conductors are required to be secured every 24" via metal supports. Electrical conductors IWO crew quarters/accomodations (overhead) are not properly secured.

Vessel required to be equipped with watertight hatches/covers. Vessels hatch IWO fwd storeroom space is not equipped with securing hardware/gasketing.

A bilge pipe piercing the collision bulkhead must be fitted with a screw down valve on the fwd side of the collision bulkhead and operable from the weather deck. Vessels screw down valve IWO forepeak was seized in the open position.

Vessels fire pump is required to be equipped with a pressure gauge. Vessels fire pump pressure gauge was found inoperable during testing.

Vessels emergency escape hatches are required to be marked IAW 46 CFR 185.606. The emergency escape hatch for the overnight accomodations space was not marked (main deck side).

Vessels overnight accomodations space(s) accessible to crew on a regular basis must have at least two means of escape. The secondary means of escape (hatch) was found siliconed down to prevent water intrusion from fresh water washdowns.

Each door, hatch or scuttle used as a means of escape but be capable of being operated by one person, from either side, in both light and dark conditions. Handles and securing devices must be permanently installed and not capable of being easily removed. The emergency escape hatch for the overnight accomodations space was not equipped with a dogging handle inside the space.

The operation of all weathertight closures above the weather deck shall be inspected a t each initial and subsequent inspections for certification. Attending Inspector noted that the secondary means of escape (watertight hatch) from the overnight passenger accomodations was not seated/adjusted properly.

All portable/semi-portable fire extinguishing equipment shall be in a serviceable condition at all times. Attending Inspector noted that the B-II extinguisher located in the overnight accomodations space was damaged (upper handle) and required servicing/repair/replacement.

Vessels overnight accomodations shall be fitted with an independent modular smoke detector IAW 46 CFR 181.450. Vessels overnight accomodations space was missing the required smoke type/style smoke detector.

Starboard main diesel engine experienced a catastrophic failure of the connecting rod IWO cylinder #6.

Dead ended wiring noted IWO helm/operating station.

Fractured weldments for weighted ring on life float (starboard side, top float)

Peripheral grab line around life floats were found deteriorated (by UV light).

Paddles for life floats were found deteriorated/damaged beyond repair and not tethered properly.

Lifefloat installation was missing spacers between each float.

Life float outer coverings damaged/holed/torn.

Weak links for life float installations were not properly marked/identified with manufacturers tags.

Ship's whistle was inoperable at time of inspection (require repair/repalacement).

Fire nozzles were found damaged/required repair. Rubber cone around nozzle was fractured which prevented the user to adjust the pattern of the nozzles.

Watertight door for machinery space was fractured/wasted @ upper/lower hinges.

Watertight door gasket IWO machinery space exterior door was cracked/imroperly installed.

Type I PFD water lights were inoperable at time of inpsection (dead batteries).

Upper deck PFD stowage was missing two child sized Type I PFD's

Ring life buoys markings were faded/required repalcement (retro-reflective markings).

Open junction box cover IWO port upper pilothouse (former lighting connection).

Fractured junction box IWO upper deck (overhang)aft of pilothouse, port side.

Upper deck PFD stowage locations are missing donning instructions/placards.

Upper deck lighting fixture cover was fractured/broken, requires replacement.

Flexible hose assemblies for primary fuel fill(s) were not properly secured (double hose clamps).

Starboard ships service diesel generator missing guard for rotating machinery IWO lazarette.

Owner/operator to provide operating instructions for fixed bilge/fire systems (in writing). Once vetted, instructions to be posted in machinery space for crews use.

Fractured weldments noted IWO starboard frame (lower) of POTW tank (lazarette).

Crew quarters missing approved smoke detector (overnight accomodations).

Missing two portable lights on board.

Dye pen welds(by qualified party) port, center, and stbd shaft logs to prove welds.

Temporary repairs were made to aft watertight bulkhead of crew accomodations space (@ bottom of ladder) without USCG oversight.

Vessel experiences a unusual vibration while operating in the 1400 RPM (+/-) range as a result of bump/go grounding.

Vessels portable fire extinguishers were out of service dates for annual inspection.

Vessel was not equipped with proper quantity of visual distress signals.

Vessel not equipped with markings for upper deck capacity IAW COI (max. 40 passengers).

Reach rod handle for anchor locker/forepeak bilge suction was fractured/inoperable.

Attending Inspectors noted a previous sounding tube penetration that was left open between the lower decks of the forward store room and the bilge.

The starboard reach rod assembly for the emergency fuel shut off has a fractured swivel fitting.

Attending Inspectors noted minor fuel leaks IWO MDE F/O returns to tank.

Primary air receivers were beyond their inspection dates as prescribed by vessels COI.

O-rings in fire hose connections were dry-rotted/deteriorated & required replacement.

Paddles for life floats were in need of repair/replacement.

Batteries in floating waterlights required renewal.

Lifefloat exterior coating was in need of repair/re-coating due to UV light (fading).

V/W longitudinal main frame to transverse frame (4 frames from the aft bulkhead) in the void space below the crews compartment as discussed. Welds were cracked on each side of the transverse frame IWO the longitudinal main frame.

V/W towing bit at the base in the forpeak space on the port side as discussed.
