Vessel must undergo an annual inspection 3 months before or after each anniversary date. Anniversary date January 20. Annual inspection was not conducted within the required time frame. COI issued 20JAN2021 is not valid.

There must be a sufficient number of Able seafarers or certified persons on board for launching survival craft for abandonment of total number of persons on board. No one on board had a valid US merchant mariner document or credential with a lifeboat endorsement.

Vessels with overnight accommodations for at least 10 individuals of at least 79 ft and required to have a loadline or documented vessel over 500 GRT are required to have surveillance systems & response training. Vessel is 211 ft, required to have load line and 1454 GRT. Cameras not installed. Cite: 46 USC 4901

There must be at least two lifebuoys, each with attached self-activating smoke signals. The port smoke signal was not attached to the lifebuoy.

Before vessel leaves port and at all time of voyage each lifesaving appliance must be in working order and ready for immediate use. Vessel’s lifebuoy’s reflective tape does not meet the requirements in 46 CFR 160.150. and (2) ring buoy light brackets cracked.

Each vessel must have an amplifier-type announcing system that will supplement the general emergency alarm. Vessel does not have an operable public address system.

Boundary bulkheads shall be of "A" class construction. The cable penetrations through the bulkhead between machinery spaces are not sealed.

An inspection required whenever any important repairs are undertaken. New hatch installed in the tension machine room and jet pump hatch not sealing. USCG MI required to witness NDT.

Plans required for alterations of existing vessels. Dive control shack was removed. No plans available to confirm that it was built to any standard or to confirm it was an existing Portable Accommodation Module. Provide or submit plans for review prior to reinstalling dive control shack.

Vessels over 1000GT are required to have two independently operating fire pumps. Emergency fire pump did not work on emergency power.

Fire detection panel systems must meet all applicable requirements of sub-chapter H. Fire detection panel had an active trouble alarm.

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Dive Control Room had a missing "blank" cover on distribution panel.

If a vessel is to be constructed, altered, or repaired, the plans, information, and calculations required by this part must be submitted to the OCMI in the zone where the vessel is to be altered or the Marine Safety Center. The vessel has a portable accommodations module (PAM) installed on the back deck of the vessel as defined in CG-ENG Policy Letter No. 01-16 paragraph 5.a.i. The PAM on board has not been certified in accordance with this policy. Correct deficiency prior to use of this portable accommodation.” 46 CFR 127.120

All lifesaving appliances and arrangements must be in accordance with subchapter W. Each liferaft or group of liferafts must be arranged for float-free launching. The hydrostatic release units used for float-free arrangements for all liferafts on board were hooked up incorrectly. - 46 CFR Part 90.27-1 / 46 CFR Part 199.130(c)(7) - (17c)

Vessels assigned a load line, see subchapter E (Load Lines) for closure of openings, etc. Watertight penetration from deck leading to portside of engine space was found leaking water into the space. Make watertight. - 46 CFR Part 92.01-5 – 17c

Each vessel must meet the applicable requirements in subchapter S of this chapter. For each watertight door which is in a required subdivision bulkhead an indicator must show whether the door is open or closed. Two watertight emergency escape hatch indicators to the machinery space were inoperable. - 46 CFR Part 93.01-1 / 46 CFR Part 170.270 (e) – 17c

Electrical junction box for tension machine control box has openings and needs to be properly capped off 46CFR111.81-1 (a) The requirements of this subpart apply to each outlet box used with a lighting fixture, wiring device, or similar item, including each separately installed connection and junction box. (b) An outlet box must be at each outlet, switch, receptacle, or junction point. (c) Each outlet or junction box must have a cover unless a fixture canopy, switch cover, receptacle cover, or other cover is used.

Walkway above anchor handling wires needs to be rewelded. 46CFR92.25-15 Guards in dangerous places- Suitable hand covers,guards, or rails shall be installed in way of all exposed and dangerous places such as gears, machinery, etc.

Exhaust piping on generator exhaust system is missing lagging 46CFR58.10-15(c)(2) Where considered necessary, gas turbines and associated exhaust systems shall be suitably insulated or cooled, by means of lagging, water spray, or a combination thereof.

The light guard on the reefer light needs to be replaced. 46CFR111.75-20(b) (b) Nonemergency and inside-type decorative luminaires in environmentally protected, nonhazardous locations must meet the applicable luminaire-type requirements of ANSI/UL 1598 or IEC 60092–306:2009 (both incorporated by reference; see § 110.10–1 of this subchapter). These luminaires must also meet Clauses 7.4, 8.1, 8.3, 11.2, 13.4, and 17.2 of ANSI/UL 1598A (incorporated by reference; see § 110.10–1 of this subchapter), except in an accommodation space, navigating bridge, gyro room, radio room, galley, or similar space where it is not subject to damage.

SOPEP plan has to be approved by the Coast Guard. Vessel's SOPEP plan needs new approval letter.

46cfr58.10-10 All installations shall be of marine type engines suitable for intended service; #2 and #3 Generators had cracked fuel hoses; replace hoses to make suitable for intended service.

An indicator light must be installed in the pilothouse and show whether the door is open or closed; The generator room escape hatch door indicator switch was removed and not operational; make repairs IAW 170.270(e) to indicate if door is open or closed

At each inspection the marine inspector will determine if fire extinguishing equipment is in suitable condition. CO2 supply piping found severely corroded. 46 CFR 91.25-20(a)

Operating generators must provide a continuous and uninterrupted source of powre for the ship's service load under normal operational conditions. All three of the vessel's generators had some mechanical issue and ship's power is occasionally interrupted. 46 CFR 111.10-4

Piping whose temperature exceeds 150 deg. F shall be suitably insulated where necessary to preclude injury to personnel. #3 SSDG had exposed surfaces/piping over 150 deg. F. 46 CFR 56.50-1(k)

Flywheels shall be provided with adequate covers or guards. Aft jet pump fly wheel is exposed. 46 CFR 58.01-20

All crew spaces are to be arranged in a manner suitable to the purpose for which they are intended. Crew room #3 was being used as a storage space. 46 CFR 92.20-15 & 20

Oil record book is not being properly maintained. Provide 1 month of oil record book logs showing proper entries and receipts for waste oil disposal IAW 33 CFR 151.25.

Vessel crew unable to demonstrate proper operation of OWS and OWS display screen inoperable. Make repairs to system and demonstrate satisfactory operation to attending Coast Guard Marine Inspector. Discontinue use of system until repairs are made.

Oil transfer procedures not on board. Provide oil transfer procedures and maintain on board vessel IAW 33 CFR 155.720.

Identified clad welds on fire main piping IVO of the #1 fire main pump. Also identified that the discharge pressure gauge and check valve for the same is inoperable. Make permanent repairs.

Identified that the pump housing for the #1 and #2 industrial jet pumps were holed through and shaft packing glands were improperly adjusted resulting in uncontrollable leakage during operation. Complete temporary repairs by indicated date and final repairs to be completed by next dry-dock.

Class "A" door into Galley did not close properly. Service to ensure proper operation.

Class "A" door at base of stairtower improperly modified. Replace with authorized class "A" door.

No documentation available for annual test of pressure vessel relief valves.

Remove all obstructions to the liferafts on both Port and Starboard sides, prior to departing.

Provide a satisfactory Stability Letter that correctly identifies Deck Cargo vice Saturation Dive system, prior to departing.

Provide Stability information that accounts for cargo on all decks, to allow Master to determine if the vessel is operating Safely.

Submit all information to MSC for equivalent determination of the Hyperbaric Rescue Chamber (Singapore "Lexmar") prior to its use. This vessel must be made inoperable until final approval from MSC and if found satisfactory the Coast Guard approval stamp must be applied on the data plate.

Investigate air intake contamination for Surface Supplied Diving compressors, cleanout intake piping and conduct air quality tests prior to use.
