#3 ship service generator was inoperable and unable to prove overspeed test completion as required. Make repairs and completed overspeed test in accordance with 46 CFR 111.12-1(b)

While the vessel was under way it discovered a small crack on the fwd deck, in way of the forward bulkhead. The BOSN noticed that a small amount of water was leaking through the crack while conducting a wash down. The vessel shall make repairs to the satisfaction of ABS or the Coast Guard.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this sub chapter. Vessel reported pin hole leak between stbd ballast tank #5 and stbd cargo hold #4. Vessel is required submit repair proposal and to make repairs prior to departure.

The installation of all systems of a marine engineering nature, shall be in accordance with the requirements of Subchapter F. While underway from Ashtabula, OH, the vessel's port clutch began smoking. Port MDE was secured and vessel authorized to anchor at Point Peelee Park (MSU Toledo's AOR) to conduct diagnosis and repair. Crew disassembled the clutch assembly and replaced pressure plates and friction pads. Port MDE and clutch were satisfactorily tested while underway from Point Peelee Park, Canada to the entrance of the Detroit River with no issues noted. Accepted objective evidence in the form of statements from master and Chief Engineer and satisfactory operation of Port MDE.

MEANS SHALL BE PROVIDED FOR CLOSING ALL VENTILATORS SERVING MACHINERY SPACES. THROUGHOUT THE SHIP VARIOUS ACCESS ROUTES ARE MISSING OR HAVE COVERS THAT DO NOT EFFECTIVELY COVER THE VENTELATION ACCESS SPACES. MAKE REPAIRS TO THE SATISFACTION OF THE COAST GUARD.

AFTER LOADING AND PRIOR TO DEPARTURE THE MASTER SHALL ENSURE THE SHIP IS IN COMPLIANCE WITH ITS LOAD LINE CERTIFICATE. tHE 3D GUILLOTINE DOOR FROM THE BOW, IN THE SHIP'S TUNNEL, DOES NOT FULLY CLOSE AS REQUIRED AND MUST BE CORRECTED PRIOR TO DEPARTURE, TO THE SATISFACTION OF THE COAST GUARD.

VESSELS SHALL BE PROVIDED WITH SATISFACTORY BILGE PUMPING CAPABILITY TO FILL AND DRAIN BILGE TANKS. THE PIPE SERVICING THE #1 PORT BALLAST TANK AND THE FOREPEAK HAS ABOUT A 1/4"-1/2" HOLE THAT MUST BE SEALED TO THE SATISFACTION OF THE COAST GUARD.

WATERTIGHT DOORS MUST INDICATE WHETHER THEY ARE OPEN OR CLOSED AT THE OPERATING STATION, WHICH IT DOES NOT. PROVE PROPER OPERATION TO THE SATISFACTION OF THE COAST GUARD.

The vessel's Master notified MSU Cleveland that a qualified 1st Mate is unavailable at this time and the ship is sailing to Stoneport, MI from Cleveland, OH on or about 0700, 24DEC2024. The Master shall ensure a 3-watch system is maintained, work/rest requirements are met. Relief is due to meet the ship on Thursday 26DEC2024 and the Master shall notify the OCMI at USCG Sector Northern upon meeting their manning requirement or if the situation changes to require an extension.

The construction of ballast systems shall be in accordance with the requirements of sub-chapter F (Marine Engineering). A survey, either general or partial shall be made every time a defect is discovered which affects the safety of the vessel. Vessel notified Sector Detroit OCMI of pinhole leak in port #1 ballast tank. Location is about 1 foot away from two previous cracks written up on deficiency issued 25 NOV 24.Temp repair to be made prior to departure from next port of call at Fairport Harbor, OH. Permanent repairs to be made during 2025 Winter Layup. 46 CFR 96.03-1 46 CFR 91.30-1

Vessel reported a pin hole leak in the fire main piping. All piping, valves, and fittings shall meet the applicable requirements of subchapter F. Make repairs to damaged fire main piping to the satisfaction of the local OCMI.

An inspection shall be made whenever any important repairs or alterations are undertaken. Temporary repairs have been completed on the two cracks to the satisfaction of ABS and [name withheld] repairs shall be made during 2025 winter layup. 46 CFR 91.45-5

Repairs shall be made to the affected frames and shell plating to the satisfaction of the OCMI upon arrival to next port of call and prior to any future movement.

Vessel Master notified Sector Detroit OCMI of vessel losing each of its two independent marine radars, separately. Each vessel must have a marine radar system for surface navigation. To be rectified prior to departure from next port, Chicago, IL. Operating conditions limited to what stated in issued Letter of Deviation: Daylight hours only and only while visibility is not less than 1 NM. 33 CFR 164.35(a)

In general, compliance with the standards established by ABS, will be considered as satisfactory evidence of structural satisfaction of the structural efficiency of the vessel. Notified by vessel master that two areas of wastage were found: 1. 3/4" by 1/2" hole on hatch #11. 2. Deck wastage aft of pilothouse leading to transfer room below. Effect proper repairs to wasted areas on cargo hatch #11 and aft of pilothouse to the satisfaction of the attending ABS surveyor.

Vessel was transiting in ballast, north bound in the Detroit River, when the breaker on the #2 ship service generator tripped and caused a momentary loss of power. Vessel shall diagnose, repair, and test the generator/breaker and notify the Coast Guard of proper operation and provide tech report or attestation from the Chief engineer that all associated systems are fully operational. 46CFR 90.25-1 46CFR 110.30

A survey, either general or partial, according to the cirumstances, shall be made every time an accident or a defcrt is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to that the vessel complies in all respects with the regulations in this subchapter. Provide Class report attesting to safe passage prior to departure.

An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs are undertaken. Vessel reported failure of the number one steering pump while at anchor. Vessel will offload cargo in St. Joseph, MI and transit to Burns Harbor, Indiana to make repairs to the satisfaction of the USCG or attending Class surveyor.

Each propeller on a rescue boat must be fitted with a propeller guard with a maximum opening of 76 mm (3 in) on all sides on which a person is likely to be exposed. Propeller guard on the rescue boat is missing a section on the top port quarter.

The general structural strength of the hull must be sufficient for the draught corresponding to the freeboard assigned and must be approved by the Commandant. Ships built and maintained in conformity with the requirements of a classification society may be recognized by the Commandant as possessing adequate strength. Vessel contacted pier while mooring resulting in an approx. 8" crack in sideshell. Make permanent repairs to the satisfaction of USCG MIs and ABS surveyors prior to departure from Monroe, MI.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Temporary doubler repair made to cargo hatch cover #7 starboard side without notice to or approval from local unit.

Quick-acting Class 1 doors are permitted in any location on a vessel that operates on the Great Lakes and is required to meet the damage stability standards of subpart H of part 172 of this chapter. Watertight doors in the vessel tunnel located between Cargo holds 1 and 2, and Cargo holds 4 and 5 not operational and could not be closed to meet watertight integrity.

Each hatch exposed to the weather must be watertight. Forward anchor winch space emergency escape hatch starboard side to weather deck could not be sealed completely with daylight showing through.

Satisfactory means, permanently attached, shall be provided for closing the openings of all cargo vents on deck of the vessel. All screw down vent openings on the main deck for ballast and void tanks were found inoperable and could not be closed to meet requirements.

Means shall be provided for stopping all fans in ventilation systems serving machinery and cargo spaces and for closing all doorways, ventilators and annular spaces around funnels and other openings to such spaces, from outside these spaces, in case of fire. Large holes discovered through Emergency generator space intake and exhaust ventilation covers not effectively shutting off the passage of air into machinery space. Submit permanent repair proposal to the OCMI for approval.

Bulkheads of galleys, paint and lamp lockers, and emergency generator rooms shall be of “A” Class construction. “A” Class bulkhead between galley and crew mess discovered to have holes throughout.

A ship of 100 gross tons or more constructed before July 1, 1974, must equip each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe during oil transfer operations with a portable container of at least a 5 U.S. gallon capacity. Portable containment under port side fuel vent had holes in the bottom due to wastage.

Electric installations on vessels must ensure protection of crew, other persons, and the vessel from electrical hazards. Belt bell fixture installed starboard side tunnel within Cargo hold 4 space wasted and cracked open presenting exposed wiring and fire hazard. 46 CFR 111.01-1(a)(2)

If the pipe discharges from a space that is not manned or does not have continuous bilge water monitoring, a valve described in paragraph (a)(1) of this section must be operable above the freeboard deck. All reach rods on deck that control overboard valves below the main deck were seized and inoperable. 46 CFR 45.155(c)

Pipes piercing the collision bulkhead shall be fitted with screwdown valves operable from above the bulkhead deck and the valve shall be fitted inside the forepeak tank adjacent to the collision bulkhead. The forepeak reach rod was seized and inoperable. 46 CFR 56.50-1(b)(1)

Each side scuttle to a space below the freeboard deck, or to a space within an enclosed superstructure, must have a hinged inside deadlight which is designed so that it can be secured watertight over the side scuttle. The inside port hole and deadlight cover securing devices were seized and inoperable, causing the side scuttle to not be able to be closed watertight. 46 CFR 45.139(b)

Hatchways in position 1 and 2 must have weathertight hatch covers with gaskets and clamping devices. Numerous hatchcovers found to have gaps in corners between the cover and the coaming. Hatchcovers appear to be severely deteriorated. Provide maintenance plan and procedures for repairing/replacing affected hatchcovers to the OCMI. 46 CFR 45.145(a)

Electrical instalations and electrical equipment must be inspected to determine mechanical condition and performance. Particular note must be made of circuits added of modified after the original issuance of the COI Vessel has numerous unsupported wires, dead end wires, improper wire penetrations, and unapproved wire runs in bulkheads throughout the entire vessel. Submit a repair proposal for identifying these deficiencies, romoval of dead end wiring, and properly routing and supporting wire/cable runs IAW subchapter J and IEEE. 46 CFR 110.30-5

In general, connections and terminations to all conductors must retain the original electrical, mechanical properties of the cable. Emergency generator battery cables are starting to separate from connection terminals. Vessel shall repair cables to the satisfaction of the attending marine inspector. 46 CFR 111.60-17 (a)

MI observed that emergency generator space ventilation closure was corroded and had holes in it. Vessel shall submit repair plan with estimated completion date, and temporary plan to secure ventilation in case of fire to OCMI for approval. 46 CFR 92.15-10 (b)

All safety and relief valves for use on pressure vessels shall be set to relieve at a pressure which does not exceed the “maximum allowable working pressure” of the pressure vessel. Control air pressure vessel has a relief valve set 5 psi higher than MAWP of pressure vessel. Vessel shall replace relief valve to the satisfaction of USCG Marine Inspector. 46 CFR 54.15-10 (a)

The respective piping systems installed on ships shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. MI observed hydraulic leaks on STBD fwd steering ram seal, STBD aft steering ram seal, PORT aft steering ram seal, PORT steering directional control valve, and deck equipment hydraulic pump seal. Vessel shall submit repair plan with estimated completion date to OCMI. 46 CFR 56.01-1 (b)

The respective piping systems installed on ships shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. MI observed #3 SSDG raw water piping had a temporary patch and a pin hole leak in section located near pump, and #1 SSDG raw water piping had a temporary patch in in section located near pump. Vessel shall submit repair plan with estimated completion date to OCMI for approval. 46 CFR 56.01-1 (b)

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. Vessel did not have a complete set of USCG approved PSTP’s onboard. Vessel shall review PSTP’s, submit to MSC for review if changes are made, or request copy of most recent approved PSTP’s to retain onboard. 46 CFR 61.40-1 (a)

All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. Numerous vents frozen. 46 CFR 92.15-10(a)

The hull, superstructure, structural bulkheads, decks, and deckhouses shall be constructed of steel. Numerous bulkheads on superstructure decks in way of ventilation and door openings are holed, severely wasted and patched with wood. Provide repair proposal to OCMI for approval. 46 CFR 92.07-10(a)

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries and storerooms, other than small service lockers, shall be of “A” Class construction. Discovered two makeshift patches in the lower portion of the bulkhead between upper belt motor room and bottom landing of stairway to the bridge. Provide repair proposal to OCMI for approval. 46 CFR 92.07-10(c)

If the annual inspection reveals deficiencies in your vessel's maintenance, you must make any or all repairs or improvements within the time period specified by the Officer in Charge, Marine Inspection. Submit current Operations and Safety Management Systems Manual and most recent internal vessel/management audits to the OCMI for review and approval. 46 CFR 91.27-1 (a)(3)

23NOV23: MSD Sturgeon Bay received notification from Captain of the M/V CALUMET of a sailing short request/notification. Vessel currently moored in Green Bay and set to depart for Sector Sault St. [name withheld] zone 24NOV23. Vessel must continue to make required notifications when transiting between COTP zones. Replacement anticipated to arrive on or before 06DEC2023.

Vessel shall effect permanent repairs to port main reduction gear lube oil cooling system to the satisfaction of the attending Marine Inspector or to the attending Class Surveyor prior to departure from Detriot, MI.

EACH LAUNCHING APPLIANCE MUST BE APPROVED UNDER 46 CFR PART 160, SUBPART 160.132 FOR USE WITH THE INTENDED CRAFT, WITH A WINCH APPROVED UNDER 46 CFR PART 160, SUBPART 160.115 FOR USE WITH THE INTENDED CRAFT. VESSEL TEMPORARILY IS UTILIZING A LIFEBOAT ACQUIRED FROM THE M/V VALO (FORMERLY M/V AMERICAN VALOR), IN ACCORDANCE WITH SECTOR DETROIT LETTER 048-23. ALL TERMS OF LETTER 048-23 ARE TO BE MET AT ALL TIMES WHILE THE LETTER IS IN EFFECT. 46CFR90.27-1 46CFR199.150(a)(1)

BEFORE THE VESSEL LEAVES PORT AND AT ALL TIMES DURING THE VOYAGE, EACH LIFESAVING APPLIANCE MUST BE IN WORKING ORDER AND READY FOR IMMEDIATE USE. LIFEBOAT HAD A FULL HULL DETERIORATION/PENETRATION ON STARBOARD BOW. 46CFR90.27-1 46CFR199.190(a)

MULTIPLE SELF-IGNITING WATERLIGHTS HAD MISSING TENSIONING GASKETS (O-RINGS), WITH ONE (STARBOARD LIFEBOAT DECK) HAVING A DAMAGED LIGHT FIXTURE. NO HULL REPAIRS/ALTERATIONS SHALL BE MADE WITHOUT THE KNOWLEDGE OF THE [name withheld] EXAM IN THE CARGO TUNNEL, MI's DISCOVERED UNAUTHORIZED REPAIRS TO THE BALLAST TANK/TUNNEL BULKHEAD. THE VESSEL'S BALLAST TANKS AND VOIDS ARE NOT READY FOR INSPECTION, DUE TO ACCUMULATION OF WATER, MUD, AND SCALING. MAKE BALLAST TANKS/VOIDS READY FOR INSPECTION. PROVIDE A PLAN FOR THE REMOVAL OF MUD, WATER, AND SCALE FROM THE BALLAST TANKS AND DOUBLE BOTTOMS/VOIDS TO CG MI. 46CFR91.40-3(b)

IF DURING AN INTERNAL STRUCTURAL EXAM, DAMAGE OR DETERIORATION IS DISCOVERED, THE OFFICER IN CHARGE, MARINE INSPECTION, MAY REQUIRE THE VESSEL ASSESS THE DAMAGE AND TO EFFECT PERMANENT REPAIRS.DURING THE INTERNAL STRUCTURE EXAM OF THE PORT SIDE BALLAST TANKS, DAMAGE WAS DISCOVERED IN 1P AND 5P TANKS. THIS DAMAGE WAS IN ADDITION TO THAT NOTED BY THE COMPANY AND ABS DURING THEIR INSPECTIONS. THE DAMAGE NOTED WAS: - 5P – 2ND SIDESHELL LONGITUDINAL (SSL) FROM THE TOP DEFLECTED, CROP/RENEW - TRIPPING BRACKET AT 2ND SSL DEFLECTED, CROP/RENEW - 1P – FR28 – DEFLECTED BETWEEN 2ND AND 4TH SSL FROM THE TOP (ABOVE INSERTS ALREADY COMPLETED), CROP/RENEW 46CRR91.40-3(c) 46CFR91.45-5

Bow thruster tunnel has a leak at the location of a prior repair allowing water intrusion into machinery space. The vessel is required to conduct repairs to the satisfaction of ABS and/or the cognizant OCMI.

Following piping systems shall be hydrostatically leak tested in the presence of a marine inspector at a pressure of 1 1/2 times the maximum allowable working pressure of the system: Firemains, but not less than 150 psi. Vessel found a pin hole leak on a the fire main pipe on deck at the #6 fire station, port side. Repairs were carried out but system was not pressure tested in accordance with CFR's. Pressure tests are to be carried out prior to vessels departure from winter lay up berth to the satisfaction of the attending Marine Inspector. Cite: 46 CFR 95.10-15(a) 46 CFR 56.97 40(a)(7)

Temporary repairs were accepted to realign the rudder angle indicator. Permanent repairs are due to be completed at the next dry dock period.

Fist sized inset, IVO Frame #18, in #1 Port ballast tank. Install a box patch, to the satisfaction of CG and Class, prior to departure. This will be considered a temp. repair until more extensive shell plate is replaced before March 31, 2022.

Prove proper operation of two independently functioning radar units.

Effect permanent repairs to 8" vertical fracture located within the #1 Stbd WBT approximately 8' above the turn of the bilge.

5-6" crack in port side of hull. Allowing 1-2 Gallons a minute into ballast tank #01.

Vessel discovered a crack in the #1 ballast tank, in the FWD bulkhead. ABS attended for temp repair, and gave 60 days to make a perm repair.

Make permanent repairs to emergency fire pump. Temporary repairs will be sufficient for 30 days or until fixed, whichever is sooner.

Conduct lifeboat drill in the presence of the Coast Guard, at next port of opportunity.

Hull plating set in, holed and internal structures distorted to match, in way of 2 secondary frames directly abaft starboard frame number 175, within starboard generator room. Satisfactory temporary repairs completed this date. Permanent repairs to same shall be completed prior to operating in 2019 operating season and no later than April 30, 2019, to the satisfaction of the cognizant OCMI.

Make permanent repairs to the hull plating 10" vertical fracture in way of the stem bar on the port side at the loaded waterline.

Make permanent repairs to sideshell hull damage in way of the #1 starboard side ballast tank to the satisfaction of ABS and Coast Guard prior to loading cargo.

Not able to lower the vessel due to mooring arrangement.

Prime mover's water pump was leaking.

Start batteries for the emergency generator need to be secured.

Oil pump discharge pipe was weaping/leaking.

Condemned under SB18/08 v2 and/or 19/08 v2 - Floor peel test for degraded glue for the Crewsaver brand.

Fire detection/alarm system was disabled/unplugged due to nuisance alarms, this condition had been reported to the [name withheld] proper installation and operation of fire detection/alarm system to satisfaction of the cognizant OCMI.

Electrical power and lighting systems in conveyor tunnel and coal handling areas shall be made suitable for hazardous locations, as per section 33 of the IEEE Standard 45, to be completed to the satisfaction of the cognizant OCMI, prior to carrying coal.

Several areas of wastage observed on the aft incinerator stack, required crop & renewal.

Majority of crewmembers room, thermostat covers were missing, renew where missing.

Main steam stop and isolation valve were leaking by at valve inspection. Boiler is to remain off line until valves are made serviceable and annual testing is completed per the approved PSTP.

At completion of stbd m/e overhaul, the lagging has not been re-installed on the exhaust manifold and main exhaust stack metallic coupling. The vessel's C/E has requested to leave lagging off for 2 days duration to monitor for potential leaks. Replace lagging as originally configured after authorized period of time to identify and resolve any potential leaks. (46CFR58.10-10)

Make permanent repairs to starboard M/E crankshaft and assembly. Perform operational tests in presence of CG Marine Inspector.

Worn crank bearing causing damage to crank case, STBD Eng

Damage sustained to the aft bulkhead of the port side fuel service day tank, located in the E/R.

Aft auxiliary boiler is to remain "offline" until repairs are made to the main steam stop and isolation valve, refractor work is completed on the aft end, and PSTP's are demonstrated. Both valves were leaking by after mounts removed, and reinstall.

Ch. Engineer has updated PSTP's and several of the procedures have been found to be incorrect and reorganization is not included as a revision. Submit via OCMI to MSC for current revision.

Machinery Control Room emergency escape hatch does not fully open to allow use of locking device. Make alteration/repairs to hatch to ensure easy escape by one crewmember operation from control room

Affect repairs to correct jacket water leak in port MDE air box prior to departing next port. Temporary and permenent repairs must be made to the satisfaction of class and the Coast Guard. Vessel may operate on one engine, provided it complies with D9INST 16701.1 GUIDANCE FOR VESSELS OPERATING WITH REDUCED PROPULSION. Vessel must notify cognizant COTP prior to operating in their zone.

Seal jacket water leak noted in the air box of the Port MDE.

Approx. 6-7inch fracture found in airbox of port engine block causing jacket water leak. Currently both engines and bow thruster are fully functional. Vessel is allowed to operate on single engine (stbd) in order to effect proper repairs upon departure from Chicago. Repairs are to be completed to the satisfaction of the cognizant OCMI. Previous weld repair of crack in air box developed a pinhole leak resulting in leak of jacket water into air box while engine was running. Repairs to be made to satisfaction of attending USCG MI prior to further operations.

Bottom longitudinal plate seam weld of bottom slope plate in cargo hold 5 found leaking ballast water into cargo hold, requires vee and welding to satisfaction of attending USCG Inspector prior to loading cargo.

Vessel is allowed to transit to Rogers City MI to conduct permanent repairs to is port main engine to the satisfaction of an attending USCG Marine Inspector and an attending Class Society surveyor.

Vessel must provide a written class survey report attesting that permanent and satisfactory repairs have been performed to the port main engine cracked air box no later than arrival in [name withheld].

Crop and renew steel in the damaged area, identified by ABS surveyor, in the port anchor pocket. Earlier in the day the CALUMET suffered an allision with the Marathon Fuel Dock along the Cuyahoga River. As a result, the ship tore a vertical crack (Approx. 20") along the forward seam where the port anchor pocket meets the side plate.

Repair leak in the #5 Cargo Hold, coming from the #7 Ballast Tank, no later than 29JUL2011.

Ships crew was unable to demonstrate lifeboat releasing gear due to heavy ice in harbor. Issued 835 requirement for crew to lower lifeboat and demonstrate function of releasing gear.

Automatic closure device for fire door to galley in A Class bulkhead was missing/had been removed. Issued requirement to install closing device on galley door.

Crew was unable to obtain satisfactory chalk test on all watertight doors in tunnel. Knife edges on WTDs were painted and in need of maintenance/cleaning/adjusting/gaskets. Issued requirement to clean knide edges and provide proper chalk test on WTDs in tunnel.

Ladder rails into #3 cargo hold deteriorated from corrosion and metal rods enclosing ladder were broken and pulled away from frame. Subsequent opening could allow someone to fall backwards into cargo hold. Issued 835 requirement to: Renew wasted ladder in #3 cargo hold.

Make permanent repairs to frames 195, 196 and 197 IWO portside laundry room. Crop and renew hull plating IWO frame 195 where temporary repair exists.

Noted a 4" hole in the side shell, port side aft. Hole is approx. 8 ft above water line on the frame between [name withheld] office and Laundry. Issued 835 to submit a repair proposal, prior to departure from Fairport, that is acceptable to Class (ABS) and [name withheld] Vessel can move around the port of Fairport to accommodate other vessel traffic.

Starboard main diesel engine suffered major casualty with damage to #5 port & stbd cylinders, pistons, connecting rods & crankshaft. Issued 835 to 1. Affect permanent repairs to stbd main engine by 22 Oct 2010. 2. Provide ABS report of Condition of Class and 3. Provide Alco Tech's report of engine casualty (upon receipt)..

Repair or renew safety relief valve on after auxiliary boiler. Test operation of safety valve and provide Sector Detroit Prevention Dept. with lifting and seating pressures. Above items to be completed prior to placing boiler in service.

Provide repair proposal for stbd boiler, cracked shell & resurface refactory. Unit not to be operated prior to repairs completed.

After replacing the #7 piston of the stb. main engine, it was discovered that there was still a problem with the engine. Upon further inspection, it was dertermined there was a possible problem with the crankshaft. Engine tech reps will evaluate further and effect necessary repairs in Cleveland. The vessel will be allowed to sail on one engine in the meantime.

Prior to placing Aux Boiler units into service, prove proper operations to an attending CGMI.

Replace hose due to cracking discovered during inspection.

Prior to departure place required safety plaqarding on all entrances to e/r and at sirens.
