In General.-A vessel engaged in commercial service that does not carry passengers, shall maintain a video surveillance system.(b) Applicability.-The requirements in this section shall apply to-(1) documented vessels with overnight accommodations for at least 10 individuals on board that are-(A) on a voyage of at least 600 miles and crosses seaward of the Boundary Line; or (B) at least 24 meters (79 feet) in overall length and required to have a load line under chapter 51; Vessel self-reported 01 SASH cameras were inoperable. Provide proof of interim mitigation until repaired. 4 6USC 4901

The construction of ballast systems shall be in accordance with the requirements of sub-chapter F (Marine Engineering). A survey, either general or partial shall be made every time a defect is discovered which affects the safety of the vessel. Vessel notified Sector Detroit OCMI of pinhole leak in port #5 ballast tank. Water leakage enters the forward end of #4 cargo hold. Rectification to be made prior to carriage of any cargo that should not come into contact with water in #4 hold, or 14 days, whichever is sooner. 46 CFR 96.03-1 46 CFR 91.30-1

Bulkheads in galleys shall be of class A construction. Galley service window closures were not operable. Closures must close to make for a complete bulkhead in the event of a fire. Closure mechanism needs to be serviced.

Electrical installations on vessels must ensure the protection of passengers, crew, other persons, and the vessel from electrical hazards. Tape was discovered on a wire for the exciter for the #3 generator, presenting a fire hazard. Vessel is prohibited to use the #3 generator and it must be tagged out if not in operation. Prior to operations, the material condition of the affected wire must be to the satisfaction of the attending Marine Inspector.

Each boiler must be fitted with a steam pressure gauge. Both gauges on the auxiliary boilers are missing the cover for the gauges. Gauges must be repaired/replaced with a calibrated gauge.

Electrical installations must be inspected at the inspection for certification and periodic inspection to determine mechanical and electrical condition and performance. OCMI requires the company to provide objective evidence of the reliability of the vessel's electrical installations including but not limited to all 3 SSDGs, switchboards, transformers, and the bow thruster motor. The following tests shall be conducted: megger testing, vibration analysis of the generators, excitation circuit inspection, and load bank testing under operational conditions. All testing shall be conducted by a qualified 3rd party technician. Technician shall provide a report detailing each test and noting any repairs that may be required.

Vessels operator installed Hydraulic Hoses (2) of approx 30 feet in length to replace tubing from the bow thruster to the tank on the deck above. Prior approval from the OCMI was not sought or obtained. Prior to departing Ludington, Michigan remove hoses greater than 30 inches in length and plumb IAW 46 CFR 56.

Vessel reported to the NRC that they discovered a loss of Hydraulic Fluid and observed a sheen around the vessel. Upon furhter investigation a failed seal was found on the Bow Thruster Hub. Repair and prove proper operation pror to departure from the Verplank Cobb Dock, Muskegon, Mi.

A survey, either general or partial, according to the circumstances shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or efficacy or completeness of its equipment. The survey shall be such as to ensure that the necessary repairs have been effectively made, that the material and the workmanship are in all respects satisfactory. Vessel reported failure of bow thruster seal that caused hydraulic fluid to leak into the Rouge River/Lake. Provided that all those responsible for the safe navigation of the vessel, on all routes sailed, are comfortable to operate vessel without use of the bow thruster, ensure proper repair and efficacy of bow thruster seal prior to its use. 46 CFR 91.30-1(a)

The Chief Engineer shall immediately report if any machinery is found to be in "bad condition" to the Master, owner, and the [name withheld] shavings were found in the crankcase upon inspection. Prove proper operation of the Port MDE within 14 days. Due to the design of the propulsion system, no restrictions are placed on the ship at this time.

Emergency diesel generators must have an audible alarm that sounds when the engine is experiencing high cooling water temperature. The E-gen has a visual but no audible alarm for high water temperature. An audible alarm must be installed to the satisfaction of the attending marine inspector.

Periodic testing and inspection of low pressure heating boiler (Serial # S-3824) requires the following inspections/tests, which are past due: hydrostatic pressure test. This inspection and test must be completed to the satisfaction of the attending Marine Inspector.

Before the vessel leaves port each life saving appliance must be in working order and ready for immediate use. Lifeboat was taking on water near the boat plug. Must be repaired prior to departure.

All marine engineering and their appurtenances shall be installed IAW 46 CFR Subchpater F. Main engine exhaust had substantial exhaust leak. Repair prior to departure.

Each life buoy must be marked with the vessel name and the hailing port. Multiple life buoys stenciling was faded and need to be re-stenciled.

All piping and fittings must meet applicable requirements in subchapter F. Fire main coupling on deck needs to be repaired or replaced.

All crew spaces are to be constructed and arranged in a manner so that they can be kept in a clean, workable, and sanitary condition. The exit from accommodation spaces on the main deck, port side exit has a threshold that is missing an presents a tripping hazard. Vessel must make temporary repairs to remove hazard and make permanent repairs during winter layup 2026.

Cargo vessels must meet the requirements in this subpart. Per Coast Guard letter dated May 1, 2024, the vessel was approved an alternative lifesaving arrangement with the caveat that an approved rescue boat and davit under 46 CFR 160.156 also be installed in place of the open top lifeboat. Plans for installation of a Coast Guard approved rescue boat shall be submitted to the OCMI within 30 days, and installation shall be completed prior to completion of winter layup 2026.

Each life jacket and immersion suit must have a life jacket light under approval series 161.112. Multiple life jacket lights were not operable.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this sub chapter. The vessel reported a pinhole leak in the #1 starboard ballast tank approximately 7 feet up that is leaking into the #1 cargo hold. Vessel is to make repairs prior to departure and provide repair survey report to MSU Chicago.

Vessel made notification to MSU Toledo of crew shortage due to unavailable replacement 1st Mate. Master shall ensure work/rest requirements are met, and all maintenance performed in accordance with SMS. Master is authorized to continue operation while short one 1st Mate until next port of call in Stoneport, MI and required to find a replacement with the same or greater credential. The Chief Mate shall conduct a risk assessment prior to each cargo operation, underway transit, and special evolution. If no replacement is found, the master/operator must provide a new request to the cognizant OCMI with: 1) work/rest logs for previous period of crew shortage; 2) documentation that a substantial effort was made to find a replacement; and 3) statement from the vessel Master and Chief Mate attesting that the vessel is sufficiently manned. 46 CFR 15.515(a)

Vessel is permitted to transit from Muskegon, MI to Port Inland, MI in order to have repair team meet vessel and determine necessary repairs for holes in ballast tank adjacent to #1 Cargo Hold at approximately 19 feet. Vessel is to submit permanent repair proposal and make permanent repairs to satisfactory of USCG and ABS prior to departure from Port Inland, MI.

Vessel/company made notification to Sector Lake Michigan OCMI of crew shortage due to departure of 1st asst. engineer on 19 Aug 2024 in Manistee, [name withheld] shall ensure a 3-watch system is maintained, work/rest requirements are met, and all maintenance performed in accordance with SMS. Master and Chief Engineer shall conduct a risk assessment prior to each cargo operation, underway transit, and special evolution for the duration of absence. Vessel stated an additional 2nd asst engineer is on board to maintain watch rotations. If no replacement is on board prior to Sept 11,2024, the master/operator must provide notification to the cognizant OCMI at the port of call with the following documentation: 1) work/rest logs for the previous period of sailing with crew shortage; 2) documentation demonstrating substantial effort was made to find a replacement; and 3) statement from the vessel Master and Chief Engineer attesting that the vessel is sufficiently manned.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to ensure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel reported a pinhole leak in the STBD fire main IVO FR 180. A soft patch has been applied and is holding following operational testing. Vessel shall effect permanent repairs to the fire main IVO FR 180 to the satisfaction of USCG MI and/or Class prior to departing Drummond Island, MI. 46 CFR 91.30-1 - 705: Prior to departing Drummond Island, MI - ac

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Vessel experienced multiple losses of power events due to undetermined electrical issues. Electrical issues may have resulted in a small fire/smoke to break out in the port ventilation room space. Vessel intends to replace switchboard breaker for the #2 Generator in the ECR to resolve issue with inability to parallel generators. Provide Technician assessment/report of electrical system(s) to include operation of new digital governor installations, and any renewed parts while on load. 46 CFR 111.01-1(a)(1); 17

The means for securing the doors weathertight must be permanently attached to the doors or bulkheads and arranged so that the doors can be secured weathertight from both sides of the bulkhead. During the inspection, marine inspectors found multiple doors in the superstructure that could not be secured weathertight. 46 CFR 45.113(b)

Hatchways in position 1 and 2 must have weathertight hatch covers with gaskets and clamping devices. Hatchway #6 was found to have a temporary repair at the port aft corner which appears to have caused the corner to lift approximately ¾ of an inch rendering it not weathertight. 46 CFR 45.145(a)

Operational readiness. Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Forward liferaft arrangement on the starboard bow has been altered with additional stanchions and ice block tarp rendering the original arrangement unusable. 46 CFR 199.190

Each vessel must have an illuminated magnetic steering compass, mounted in a binnacle, that can be read at the vessel's main steering stand. Light to illuminate the magnetic compass was found disconnected and missing light bulb. 33 CFR 164.35(b)

Electrical installations and electrical equipment must be inspected to determine mechanical condition and performance. Particular note must be made of circuits added of modified after the original issuance of the COI Vessel has numerous unsupported wires, dead end wires, improper wire penetrations, and unapproved wire runs in bulkheads throughout the entire vessel. Submit a repair proposal for identifying these deficiencies, removal of dead end wiring, and properly routing and supporting wire/cable runs IAW subchapter J and IEEE. 46 CFR 111.01-1

Each vessel must carry not less than 12 rocket parachute flares approved under approval series 160.136. Vessel did not have the required amount of flares on the bridge. 46 CFR 199.60(a)

Each hydraulic system must be provided with arrangements to maintain the cleanliness of the hydraulic fluid, appropriate to the type and design of the hydraulic system. During steering tests the marine inspector found leaks on pumps and port and stbd fwd steering ram hoses. 46 CFR 58.25-20(a)(1)

The auxiliary steering gear must be capable of moving the rudder from 15° on either side to 15° on the other in not more than 60 seconds with the vessel at its deepest loadline draft and running at one-half maximum ahead service speed or 7 knots, whichever is greater. Vessel was unfamiliar with testing of auxiliary steering gear system during inspection. 46 CFR 58.25-10(c)(2)

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. Vessel appears to be using an updated test procedure during engineroom inspection tests that has not been approved by the Coast Guard. Submit updated test procedures to the Coast Guard for approval. 46 CFR 61.40-1(a)

High J/W temp alarm should sound at 190 and shut down at 205. Emergency generator High J/W shut down temp reached 225 and did not shut down. Make repairs and test to the satisfaction of attending CG Marine Inspector. 46 CFR 112.50-1

Each superstructure or deckhouse used for accommodations of the crew must be approved by the Commandant or the approved assigning authority with regard to general strength and weathertightness. Bridge wing hatch coaming found deteriorated and patched with doubler. Submit repair proposal to OCMI for approval. 46 CFR 45.109 Extended deficiency until 2025 winter layup

Piping, including valves, pipe fittings and flanges, conveying vapors, gases or liquids whose temperature exceeds 150 °F., shall be suitably insulated where necessary to preclude injury to personnel. Radiator piping on bridge is missing insulation. 46 CFR 56.50-1(k)

The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex I to these Rules In a vessel of 50 meters or more in length: a towing light, 3 miles. Vessels Navigation light panel on the bridge was showing an alarm for the towing light. 33 CFR 83.22

The respective piping systems installed on ships shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings,pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. Water was leaking from the port, starboard and auxiliary ballast pumps. 46 CFR 56.01-1 (b)

Each arrangement for the storage, distribution and use of oil in a pressure-lubrication system must be as such to ensure the safety of the vessel and all persons aboard. Oil was found to be leaking from a crankcase cover on stbd side of stbd main engine. 46 CFR 56.50-60(m)

Vessel reported 4" long crack located at the inner bottom tank top near the aft end of the tunnel. Unknown cause. Vessel will proceed to Port Inland, MI. to conduct repairs. Make permanent repairs to the satisfaction of the attending class surveyor and/or local USCG OCMI prior to departure from Port Inland, MI.

A survey, either general or partial, according to the circumstances, shall be made every time a defect is discovered which affects the efficacy of its equipment. Master notified [name withheld], of a worn clutch plate on the starboard main diesel engine requiring replacement. Vessel anchored and transferred propulsion to the port main diesel engine to complete replacement of clutch plate. Chief engineer provided attestation letter of the replacement and operational condition of the new clutch plate.

Submit repair proposal to cognizant OCMI for permanent repairs to fuel tank in way of shell damage.

Make permanent repairs to the satisfaction of the attending marine inspector and/or class surveyor.

Vessel reported a loss of thier port engine due to a dropped valve on a cylinder head. Proceed to Toledo, OH to repair port main engine to the satisfaction of a Coast Guard Marine Inspector. 46 CFR 97.30-5

EACH VESSEL MUST HAVE A GENERAL ALARM SYSTEM THAT MEETS THE REQUIREMENTS OF THIS PART. 04 GENERAL ALARM BELLS IN THE STARBOARD CARGO TUNNEL WERE NOT FUNCTIONING PROPERLY. 46CFR90.25-1 46CFR113.25-3 17APR2023: extended due to 03 of the faulty bells being operational, with a relacement for the fourth being brought on board. The general alarm was audible IVO the fourth, with that bell still being inoperable.

EACH EMERGENCY GENERATOR SHALL BE CAPABLE OF CARRYING ITS FULL RATED LOAD WITHIN 45-SECONDS AFTER CRANKING IS STARTED. THE VESSEL'S EMERGENCY GENERATOR STARTS AND THEN SHUTSDOWN DUE TO THE OVERSPEED SHUTDOWN ACTIVATING. 46CFR90.25-1 46CFR112.50-1(d)

EACH SELF-PROPELLED VESSEL SHALL MUST BE PROVIDED WITH A POWER DRIVEN PUMP OR PUMPS CONNECTED TO THE BILGE MAIN AS REQUIRED IN TABLE 56.50-55(a). THHE BILGE/BALLAST PUMP ON THE STARBOARD SIDE OF THE ENGINE ROOM HAS SIGNIFICANT LEAKAGE, CAUSING THE BILGE HIGH LEVEL ALARM TO REMAIN ACTIVATED. PUMP ALSO EXPERIENCING SEVERE VIBRATION. 46CFR90.20-1 46CFR5650.55(a)(1)

EACH LIFEBOAT SHALL BE LAUNCHED EVERY 3-MONTHS, CONSISTING OF LOWERING THE LIFEBOAT TO THE WATER AND MANEUVRING IT WITH THE OPERATING CREW ONBOARD VESSEL HAS NOT HAD AN ABANDON SHIP DRILL OR MANUEVERED THE LIFEBOAT IN THE WATER SINCE AT LEAST JANUARY. 46CFR90.27-1 46CFR199.180(d)(4) 17APR2023: Due date extended due to weather circumstances experienced while attempting to perform the drill.

THE PROPER CONDITION AND OPRATION OF THE SURVIVAL CRAFT LAUNCHING APPLIANCES SHALL BE TESTED FROM LIGHT TO FULL LOAD AT PERIODIC/RENWEAL INSPECTIONS. A LIGHT/FULL LOAD TEST HAS NOT BEEN CONDUCTED SINCE 2019. 46CFR90.27-1 46CFR199.45(b)(1)

EACH GENERATOR ARRANGED FOR PARALLEL OPERATION MUST HAVE REVERSE-POWER OR REVERSE-CURRENT TRIPS. VESSEL UNABLE TO DEMONSTRATE OPERATION OR THE REVERSE-POWER TRIP FOR GENERATOR #3. 46CFR90.25-1 46CFR111.12-11(f)

EACH CABLE INSTALLATION SHALL MEET THE SPECIFICATIONS OF IEEE SECTION 25, AS NOTED IN THIS PART. MULTIPLE UNSUPPORTED CABLES/WIRES WERE FOUND THROUGHOUT THE BRIDGE AND IN THE OVERHEAD THROUGHOUT THE HOUSE. DEAD END WIRES WERE LOCATED THROUGHOUT THE VESSEL. 46 CFR 90.25-1 46 CFR 111.60-5

BOILER SAFETY VALVES SHALL BE TESTED IN ACCORDANCE WITH 46CFR61.05-10. VESSEL'S SAFETY VALVES HAVE NOT BEEN TESTED. TEST SAFETY VALVES IN THE PRESENCE OF A CG MARINE INSPECTOR PRIOR THE OPERATION OF THE BOILERS. 46CFR90.20-1 46CFR61.05-10

CLOSURES FOR HULL OPENINGS BELOW THE FREEBOARD DECK SHALL BE AS STRONG AS THE STRUCTURE IN WHICH THEY ARE ATTACHED AND WATERTIGHT. GANGWAY DOOR GLASS BROKEN. 46CFR45.135 17APR2023: extended due date for 07 days to allow for arrival /installation of part. Deadlight cover required to remain in place.

VESSELS OF 1500GT OR MORE SHALL HAVE TWO FIRE PUMPS LOCATED IN SEPARATE SPACES WITH SEPARATE SOURCES OF POWER. THE VESSEL'S EMERGENCY FIRE PUMP IS INOPERABLE. 46CFR95.10-5(b)

EACH LIFERAFT MUST BE ARRANGED IN A FLOAT-FREE ARRANGEMENT. ALL THREE OF THE VESSELS LIFERAFTS WERE INSTALLED WITH THEIR CONTAINER UPSIDE DOWN AND WITH THE WEAK LINKS IMPROPERLY ATTACHED. 46CFR90.27-1 46CFR199.130(c)

PORTABLE FIRE EXTINGUISHERS SHALL BE INSTALLED IN ACCORDANCE WITH TABLE 95.50-10(a) AND THE FIRE CONTROL PLAN. SEEVRAL FIRE EXTINGUISHERS WERE STREWN ABOUT THE DECK, NOT IN THEIR DESIGNATED MOUNTING BRACKET. ENSURE PORTABLE ECTINGUISHERS ARE IN THE DESIGNATED, NUMBERED LOCATION WITHIN SEVEN(07) DAYS. 46CFR95.50-10

VESSELS OPERATING IN FRESH WATER FOR AT LEAST 6 MONTHS IN A 12 MONTH PERIOD SINCE THE LAST DRYDOCK ARE SUBJECT TO DRYDOCK, INTERNAL STRUCTURE, AND CARGO TANK INTERNAL EXAMS AS PER THE TABLE IN 46CFR91.40-3(b). THE VESSEL'S BALLAST TANKS AND VOIDS WERE NOT READY FOR INSPECTION AT THE SCHEDULED TIME FOR THE INTERNAL STRUCTURE EXAM, DUE TO ACCUMULATION OF WATER, MUD, AND SCALING. MAKE BALLAST TANKS READY FOR INSPECTION. 46CFR91.40-3(b)

Following piping systems shall be hydrostatically leak tested in the presence of a marine inspector at a pressure of 1 1/2 times the maximum allowable working pressure of the system. Vessel crew found pin hole leak in the fire main line within the engine room on a weld seam along its butt joint leading to a flange. Pressure test and repairs are to be made to the satisfaction of the attending marine inspector or to the attending Class Surveyor prior to departure from Cleveland, OH. 46CFR95.10-15(a)/ 46CFR56.97-40(a) 17ac

Throttle controls failed to answer immediate changes in direction and speed. Prove proper operation of MDE throttle controls, to the satisfaction of CG Marine Inspector, prior to departing the Cleveland area.

The prime mover is able to meet all requirements to take the load and power required equipment. The under voltage trip mechanism, which shuts down the generator if the voltage drops below a set threshold, does not work and a replacement is not available. This safety, does not affect the designed function of the prime mover to provide emergency power. The under voltage trip must be repaired within 30 days.

Vessel reported hydraulic leak on the port steering system. Temporary repairs were carried out to the system to allow for operation of both pumps. Vessel may transit from Port Huron anchorage to Sarnia, Canada to carry out repairs to the satisfaction of Class and Coast Guard. Cite: 46 CFR 96.03-1 46 CFR 58.25 ***UPDATE*** As reported by the owner's representative, the pipe fitting (Nipple) for the pressure gauge on the hydraulic pipe from the steering gear pump unit number 1 to the steering gear was fractured causing hydraulic pressure drop. At this time the pressure gauge was removed and the pipe fitting (Nipple) was blanked off by welded tube and blank socket. The steering gear was tested functionally and no leakage was noted. It is recommended that the pressure gauge and the pipe fittings on the hydraulic pipe from the steering pump unit number 1 is to fixed and dealt with to the satisfaction of the attending surveyor. Deficiency to be corrected during winter lay-up period

Each alarm annunciator, fire detector, test station, manual station, and vibrating bell must be approved under Subpart 161.002 of this chapter and meet the requirements of this subpart. Vessel reported that the fire detection system covering the House [accommodation spaces, galley, etc.] was not alarming due to a failed circuit board. Service technician had attended and confirmed issue and that it does not affect detection system in Engine Room. Vessel to maintain 24 hour manned roving watch until repairs are made to the satisfaction of the attending Marine Inspector or to the attending Class Surveyor. CITE 46 CFR 96.05-1(a) 46 CFR 113.10-1

STBD Steering reported to be INOP.

Illumination bulb is inoperative on the magnetic compass.

The indicator switch on the WTD @ Frame 36, is inoperative.

The WTD @ cargo hold #2 and #3 is too tight, and in need of maintenance.

Operational test must be conducted.

Renew belt guard on the dredge pump in the tunnel. The guard is damaged, which exposes passing personnel to moving parts.

Failure of port main engine clutch. Cracked carbon fiber disc wedged in not allowing vessel to de clutch. See narrative for more information.

#3 gen overspeeding when started. Provide tech report, and conduct PSTP prior to use.

Radar not rotating.

Light list and Coast Pilot not up to date.

Vessel Bridge to Bridge radio cert expired.

Stern light and Out of Command light not functioning.

Vessels Great Lakes Agreement expired. Vessel has plans to update upon returning to US waters.

Athwartship stiffeners for cargo hold hatch cover #13 are wasted and fractured. ABS accepted temporary repair proposal to install stiffeners to exterior/weather side of hatch cover and to complete permanent repairs within 30 days. Complete permanent repairs to hatch cover #13 to the satisfaction of attending class surveyor and/or USCG Marine Inspector.

Ships aux cooling line, located in the engine room has two soft patches. Replace section of line with temp repairs.

Engine room vents, located on aft deck, do not have suffcient covers. Provide means to secure ventilation.

One immersion suit was taken out of service, due to small hole in seem. Vessel had extra on board.

ABS accepted temporary repair proposal to install stiffeners to exterior/weather side of hatch cover and to complete permanent repairs during winter lay-up. Complete permanent repairs to hatch cover #14 to the satisfaction of attending Class surveyor and USCG Marine Inspector.
