Prior to Departure: The owner/operator must rectify the loss of electrical power redundancy. Repair the No. [1 or 2] Ship Service Generator and verify its proper operation, including load sharing and safety shutdowns, to the satisfaction of the Officer in Charge, Marine Inspection (OCMI).

A general arranagement plan shall be provided for all accommodation spaces. Accommodation spaces for the galley/incinerator room was altered when door was reomved. Company must ensure incinerator remains tagged out, and seal off this space to prevent any contamination of galley. This must be done to the satisfaction of the attending marine inspector. 23APR26: Doorway was sealed, vessel authorized to depart and seal must be inspected and looged weekly. Company must provide permanent repair proposal and conduct repairs prior to fit out 2027.

An inspection must be conducted whenever important repairs are made. Any pending steel work must be inspected by CG or ABS prior to departure.

Each vessel must have a navigation light panel on the bridge that visually and audibly signals the failure of the navigation lights. Panel does not alarm the failure of LED lights. Vessel crew is to conduct daily rounds to verify operation of lights until panel is replaced. Company must provide repair proposal within 30 days.

At each inspection for certification, the marine inspector will determine that all fire-extinguishing equipment is in suitable condition. The paint locker was damaged and the fixed co2 system was removed during the repair. Co2 system must be re-installed and inspected by certified 3rd party prior to departure.

Fire pumps must be fitted with a pressure relief set to relieve at 125 psi or 25 psi over the pump pressure. Piping for the pressure relief side has a small pin hole leak. Authorized a temporary repair via ABS approved coupling. Permanent repair to be conducted via crop/renew during winter layup 2027.

No repairs or alterations shall be made without the knowledge of the [name withheld] docking in Duluth, MN, vessel allided with Hallett Dock #5, incurring damage to port forepeak around frame #4 above the waterline. Buckling occurred above uppermost stringer; 2 tripped brackets discovered. Vessel is granted 90 days to affect repairs in conjunction with interval granted by class (ABS). 46 CFR 91.45-1(a)

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel self reported wastage discovered during chipping and painting of cargo hatchcovers. 46 CFR 91.30(a)

Vessel discovered crack at weld repair / frame seam near frame 78 on the port side of the vessel, low in the aft engine room. Vessel must submit a repair proposal to this office, be accepted by this office, and complete the agreed upon repair prior to departing the port of Duluth/Superior. 46 CFR 91.45-1(a)

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. However, in special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. Vessel reported leak in the #2 cargo hold from #2 POrt Ballast tank at frame 23 and #2 STBD ballast tank at frame 2 at the welds where the frame meets the slope plate. Vessel removed previous cement box patch and re-installed a new cement box patch. Vessel is required to contact Sector Sault Ste. [name withheld] upon arrival to confirm box patch is holding. Vessel is to contact nearest OCMI if box patch is not holding. Vessel must conduct permanent repairs during winter lay-up to the satisfaction of the Coast Guard to clear deficiency as long as box patch is holding. 47 CFR 92.01-10

Vessel discovered water leaking through the repair made on 6APR25 near frame 78 on the STB side of the vessel, low in the aft engine room. Vessel must submit a repair proposal to be agreed upon by the USCG, and complete the agreed upon repair prior to departing the port of Marquette, MI. 46 CFR 91.45-1(a)

Vessel discovered water leaking through the side shell, from a weld seam / frame seam near frame 78 on the STB side of the vessel, low in the aft engine room. Vessel must submit a repair proposal to this office, be accepted by this office, and complete the agreed upon repair prior to departing the port of Duluth. 46 CFR 91.45-1(a)

A fixed CO2 fire fighting system must be installed with a time delay (TD) that provides enough time for one person to walk from the farthest area of the protected space to the primary exit. The vessel has a 30 second TD installed but when the Marine Inspectors walked from the lowest point of the engine room, at the 30 second mark, they were still two decks away from the primary exit. The 30 second time delay must be replaced with a 60 second time delay, IAW §95.16-45.

Vessel captain reported a small leak from ballast tank 6 port. Vessel will proceed to Conneaut, OH to offload and conduct repairs. Make permanent repairs to the satisfaction of the attending class surveyor and/or local USCG OCMI prior to departure from Conneaut, OH.

The general strength of the hull must be sufficient for the draught corresponding to the freeboard assigned and must be approved by the Commandant. Ships build and maintained in conformity with the requirements of a classification society may be recognized by the commandant as possessing adequate strength. Vessel reported a crack in the bulkhead of the #6 STBD Ballast tank. Ballast water is leaking into unloading tunnel. Vessel is to make proper repairs to good metal in Sandusky, OH. Repairs must be to the satisfaction of the Coast Guard and Class.

Vessels Hydraulic pump on the offloading pump had been found with a leaky seal. Vessels crew shall repair or replace to ensure proper operation and ensure pollution prevention.

01 General Alarm bell was found inoperable in the vessels tunnel.

Vessel informed Sector Detroit of crew shortage of 1st Assistant Engineer. Master shall maintain a 3-watch system and ensure work/rest requirements are met. All maintenance shall continue to be performed in accordance with prescribed safety management system. The Master and Chief Engineer shall perform risk assessments prior to cargo operations and transits. Vessel is authorized to continue operations while short 1st Assistant Engineer until 18OCT23, fulfilling replacement with crew member holding equal to or greater credential. Master must report to cognizant OCMI if unable to procure replacement and submit the following: 1) work/rest logs for previous period of crew shortage; 2) documentation that a substantial effort was made to find a replacement; and 3) statement from the vessel Master and Chief Engineer attesting that the vessel is sufficiently manned.

Each rescue boat must carry an approved and inspected fire extinguisher. During the inspection, Inspectors identified the fire extinguisher in the rescue boat was not labeled with an inspection tag. Vessel owner/operator shall replace the extinguisher with an extinguisher that has been serviced and has the correct approval number.

Flooding discovered in engine room void space below deck. Determine cause of flooding and make plans for mitigation to the satisfaction of ABS/CG.

Monitor engine room void tank top manholes to ensure watertight integrity for the duration of transit from General Mills, Buffalo, NY to Donjon Shipbuilding and Repair Erie, PA for dry dock.

While needle gunning 2nd deck of house aft bulkhead IWO Cooks stateroom, an approx 1 inch hole was discovered. Crop and renew to good metal to the satisfaction of a Coast Guard Inspector or ABS Class Surveyor no later than September 17, 2022.

Forward most Liferaft should be marked and labeled in accordance with 46CFR199.178 (d) and embarkation ladder should be stowed in a ready position. embarkation ladder for forward most liferaft stowed in Bos'n store without labeling or marking stowage location. Properly label and correctly stow embarkation ladder in accordance with 199.110(f)1.

Multiple Lifebuoys found with deteriorating Grab Lines. Replace Lifebuoys as needed and replace with Lifebuoys in accordance with 46CFR160.050.

All Bilge pipes used in...machinery space,...shall be made of steel or other acceptable material. Rubber Hose leading from Aft Peak Tank into Bilge piping arrangement should be removed and properly secured.

AT EACH REINSPECTION, RESCUE BOATS WILL BE INSPECTED TO ENSURE THE PROPER CONDITION AND OPERATION OF THE RESCUE BOAT. THE VESSELS RESCUE BOAT HAD A BENT GRAB RAIL, THAT WAS ALSO PULLED FROM THE FIBERGLASS ON THE BOTTOM OF THE VESSEL. ENSURE REPAIR IS MADE IN ACCORDANCE WITH VIKING SPECIFICATIONS. 46CFR199.45(b)(2)

Following piping systems shall be hydrostatically leak tested in the presence of a marine inspector at a pressure of 1 1/2 times the maximum allowable working pressure of the system: Firemains, but not less than 150 psi. Vessel found three pin hole leaks on the fire main, starboard side by hatch #3. At this time temporary repairs were carried out. Pressure test and permanent repairs are made to the satisfaction of the attending Marine Inspector prior to departure of winter lay up.

Vessel found a pinhole leak in their fire main on the main deck while conducting a round. Vessel applied temporary patch. Apply permanent repair to the fire main by next port to the satisfaction of the Class and Coast Guard. 46CFR95.10-15

Fire main piping on deck has multiple leaks. Repair to the satisfaction of attending Coast Guard marine inspector, prior to departure.

Prove proper operation and alarm function of the thermal fluid water heater prior to use.

Replace ruptured jacket water hose on the STBD main diesel engine. All marine installations shall be in accordance with requirements in subchapter "J". 46CFR96.03-1

Starboard side fire main piping on the spar deck had two leaks. Replace piping and conduct hydrostatic testing in the presence of a Coast Guard Marine Inspector.

Due tot he vessels mooring arrangement, unable to verify proper operation of the davit, winch, brakes and release hook at light load condiction. Demonstrate the above in the presence of a Coast Guard Marine Inspector.

Vessels generator room and paint locker fixed fire extinguishing systems require plan approval. The current approved drawings and flow calculations are from 1973 and were for an ANSUL system. Both spaces are now primarily KIDDE systems. Fixed fire fighting extinguishing systems shall have approved Coast Guard pland and the installation of the system is required to meet a Coast Guard type approved system with the components being from the same manufacturer. Submit plans per the provided Marine Safety Center guidelines E1-06, Guidelines for Carbon Dioxide Fire Suppression Systems.

Cable penetrations were found to not meet the structural fire proctection requirments. Decks and bulkheads separating accommodations, stairways, and machinery spaces shall be of "A" class construction. Install properly listed and approved systems to ensure class " A" boundaries are properly maintained in the locations provided in the proposal provided.

Due to a collision, a 12-24" crack is present in the side shell of the stbd #2 ballast tank, in the #2 and #3 bays. Effect temp/permanent repairs to the hull to the satisfaction of class, prior to departure.

Vessel shall conduct an operational test of the rescue boat and associated equipment, to include lowering to the water and exercising the davit, to the satisfaction of CG Marine Inspector.

Vessel shall have current Fire Control Plan posted throughout vessel. Make vessel available for Marine Inspectors to physically verify updated fire control plan.

Repair/renew cracked port light in WTD of forward bow compartment.

Repair/renew broken fire main valves on station 1 and 2 port side forward.

Drain all exterior deck-light globes of standing water.

Renew/repair Fuel shutoff placard on starboard aft station.

MI's noted all inflatable life rafts were improperly secured to mounting stanchion. Vessel made immediate correction in accordance with manufacturer's recommendations.

Rescue boat outboard engine pull start cord wasted and inoperable. Renew as original and demonstrate proper operation to MI.

Repair or renew the draft sensor for the port E/R furnace. Prove proper operation of sensor.

Incinerator shall remain secured. Fuel system and electrical system shall be secured to burner. Incinerator shall be renewed or repaired. Repairs or new installation shall be to the satisfaction of USCG and subject to plan review.

Panelboards P-130-10, P-409-5, P-103-L3, P-409-6, P-409-7, L-14 require updated circuit directories.

Each entrance to a space storing CO2 cylinders must be marked as follows: "CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS SPACE AND CAUSE SUFFOCATION."

Spaces protected by CO2 must be marked as follows: "CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES SO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING."

Spaces into which carbon dioxide may migrate must be marked as follows: "CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. DISCHARGE INTO NEAR-BY SPACES CAN COLLECT HERE. WHEN ALARM OPERATES VACATE IMMEDIATELY."

The On/Off cycling controller for the thermal fluid header is not functioning correctly. Renew or repair controller and prove proper operation in accordance with PSTPs.

A bearing in the port M/E jacket water pump failed while underway. Repair or replace jacket water pump and make available for inspection within 7 days IAW 46 CFR 50.05-10 and D9 Instruction "Guidance for Vessels Operating with Reduced Propulsion"

VSL port ships service genset was non operational.

Stern tube circulating pump and stern tube head tank shall remain locked out & secured until permanent repairs are completed to the stern tube seal.

Make permanent repairs to starboard main engine turbor charger.

Periodic safety test procedures need to be updated for the new SSDG's. Procedures for the EDG and reverse power relay need to be created and submitted for approval.
