A survey shall be made every time a defect is discovered or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs have been effectively made. The vessel discovered multiple weepers from the ballast tanks into the cargo unloading tunnel. One leak originated from #1 Port WBT, approximately at Frame 15. The other three weepers originated from #2 Starboard WBT, approximately at Frames 19, 23 and 24. Affect repairs to satisfaction of attending Marine Inspector.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel reported a pin hole leak on on the top of their #7 ballast tank. Vessel is authorized to proceed to Calcite, MI for permanent repair.

The emergency fire pump is inoperable due to a blown bearing and bent shaft, vessel is authorized to proceed to Calcite, MI for permanent repairs.

When alteration or repair of boilers, pressure vessels, machinery, safety valves or piping systems becomes necessary, the work must be done under the cognizance of the Officer in Charge Marine Inspection. It must be done in accordance with the regulations in effect at the time such vessel or installation was contracted for or built (whichever is latest), or in accordance with the regulations in effect for new construction. (1) Following an unverifiable event, the vessel identified that two propeller blades were found broken. Vessel shall provide repair proposal to repair or replace any broken or damaged propeller blades and conduct a survey of associated main propulsion system to the satisfaction of Class and U.S. Coast Guard. (2) Associated damage may have been caused by strike or grounding and verification of structural integrity of vessel's structure adjacent to the main propulsion system and rudder shall be verified. Vessel shall provide plan to verify structural integrity of these adjacent spaces to the satisfaction of Class and Coast Guard.

Steering pump was not operating as design. Make repairs to ensure proper operation of steering pump

Exhaust must be insulated. Lagging was found removed from manifold on starboard main. Replace lagging.

Fire main valves on the starboard deck line were found to be leaking. Repair or replace the forward 3 fire main valves on the starboard deck.

Rescue boat test must be conducted during re-inspections to ensure engine is properly operational. Battery failed to sufficiently provide enough power to start the battery during test. Ensure supplied battery holds sufficient charge to start the rescue boat engine.

Crew discovered a leaking flange/gasket and adjacent pipe section in the fwd-port deck fire-main. These components were recently damaged by ice expansion. Crew is authorized to affect temporary repairs to restore immediate system functionality. Permanent repairs are authorized to be completed underway after departure from the General Mills Grain Dock in Buffalo, NY and prior to arrival in Sandusky, OH (as agreed to with the company Marine Superintendent [name withheld]). Provide before/after photos to MSU Duluth of temporary, and permanent, repairs; and statement(s) from Chief Engineer attesting to satisfactory operational pressure tests for both temporary and permanent repairs. The Captain of the vessel reported that while getting underway post annual inspection the vessel ballasted and noticed a minor leak coming from the #1 port ballast tank. The leak is reported to be small but requires a permanent repair. The ballast level was lowered and the leak stopped. Vessel has departed Sturgeon Bay and is in route to its next port, prior to departure from the next port permanent repairs are to be made and tested to ensure leak tightness. To be cleared by a CG Inspector or to the satisfaction of Class.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. New system partially installed to service shaft seal, provide MSC approval and MI validation of installation prior to system coming into service.

During annual inspection the vessel was found to be actively taking on water. There were thousands of gallons in the bilges. CG inspector found cause, crew was unaware. The emergency bilge suction valve was pouring water out of it due to a valve failure. Vessel not ready for inspection. The emergency bilge suction valve is to be replaced or proved to work as designed to eliminate engine room flooding.

In the inspection of hulls, boilers, and machinery of vessels, the standards established by the American Bureau of Shipping, see part 90, subpart 90.35 of this chapter, respecting material and inspection of hulls, boilers, and machinery, and the certificate of classification referring thereto. All Class certificates on board are expired at this time. All Class certificates are to renewed and or extended prior to departure.

Each nameplate for a circuit breaker must show the electrical load served and the setting of the circuit breaker. Multiple circuit breakers through the vessel are not labeled. Label all circuit breaker distribution panels as required.

Each general emergency alarm signal must be marked “GENERAL ALARM—WHEN EMERGENCY ALARM SIGNAL RINGS GO TO YOUR STATION” in red letters at least 1/2 inch high. All contact makers through out the tunnel on the vessel are missing placards. Replace all placards in tunnel.

General Alarm system must be located in under-deck passageways in cargo areas. Contact makers at gate 30-31 and gate 42-43 were fount to be non operational. Repair or replace contact makers.

Each muster station and embarkation station must be adequately illuminated by lighting with power supplied from the vessel's emergency source of electrical power. Spotlights at embarkation station to allow for illumination of water in to which liferaft is to be launched are not on emergency circuit. Reconfigure electrical to provide emergency power to spotlights within 90 days.

Each lifeboat, rigid liferaft, and rescue boat, unless otherwise stated in this paragraph, must carry the equipment listed in this paragraph. First-aid kit - The active ingredients in medicinal products must conform to over-the-counter (OTC) drug regulations set out in 21 CFR part 330. All items in the first aid kit on the rescue boat expired between 2017 to 2019. Replace all expired items in the first aid kit.

Illustrations and instructions in English, and must be conspicuously displayed at each muster station. The illustrations and instructions must include information on the method of donning lifejackets. No lifejacket donning instructions are posted.

At each inspection for certification and periodic inspection the marine inspector shall conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as he feels necessary to check safe operation.Port main engine lube oil shutdown found to be inoperable Port engine failed to shutdown during a simulated loss of oil pressure. Return system to an operational condition and conduct testing with CG presence.

Vessels in ocean, coastwise, or Great Lakes service, except unmanned barges, must be fitted with anchors. chains, and hawsers in general agreement with the Standards established by the American Bureau of Shipping Both forward anchor windlass' are inoperable.

(a) Where fire extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation must meet the requirements of this part. (b) Use of non-approved fire detection systems may be acceptable as excess equipment provided that— (3) Coast Guard plan review is completed for wiring plans.Fire detection system installed, but not fully verified IAW plan approval. Complete plan verification in the presence of CG MI.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Engine room bilge found to have excessive amounts of oily water present. Transfer oily water to an appropriate waste facility and make space available for follow on inspection.

Fire detection, manual alarm, and supervised patrol systems are not required except in special cases; but if installed, the systems must meet the applicable requirements of 46 CFR, part 76 of subchapter H (Passenger Vessels) of this chapter. CO2 system in incinerator space is not properly locked out.

A cement box temporary hull repair was affected in the vessel's 6 port ballast tank. This temporary repair is considered satisfactory for continued vessel operations, but must be checked annually and permanently repaired by no later than the next scheduled drydocking to the satisfaction of USCG/ABS.

There are a significant amount of improper or damaged cable penetrations in class A structural fire boundaries. Areas include stair towers, machinery space to accommodation boundaries, decks, overheads, etc. Cable penetrations in class A boundaries shall be of an approved type and meet class A standards. Many of the multi-cable transits have had their blocks drilled out and improperly modified. This vessel is to be fully surveyed for these failures. ALL cable penetrations are to be corrected with approved materials IAW their manufacturers instructions. To be cleared by a CG Marine Inspector. Penetration seals must meet testing in FTP Code under approval series 164.138, or ASTM E-814.

"A" Class door from engine room to accommodation space on main deck would not close properly. Repair and adjust door to be self closing prior to departure.

Multiple junction boxes from newly installed fire detection system were left uncovered in cargo tunnel (gate 3, 13,17); need cover. Open cable connector on junction box IVO roller 245.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. The self-igniting light may not be attached to the lifebuoys required by this section to be fitted with lifelines. Multiple waterlight brackets were found broken and or missing. Replace a broken or missing waterlight brackets.

Each survival craft must be stowed so that neither the survival craft nor its stowage arrangements will interfere with the embarkation and operation of any other survival craft or rescue boat at any other launching station. Access to port life rafts blocked by a cart and several 4” pipes stored on the deck. Remove all equipment locking access.

Multiple instances of ring life buoys with faded stenciling or otherwise inservicable. Complete maintenance to return all ring life buoys and required attachments to servicable condition.

Vessels that operate in fresh water at least six months in every 12 month period since the last drydock examination must be examined in accordance with the intervals set forth in Table 91.40-3(b). Vessel is past due on Internal Structural Examination (ISE) due date; make vessel ready for ISE within 30 days.

Vessel replaced jacket water pump and 8 cylinder packs on the inboard port engine. Run port main engine to operating speed for adequate time, per manufacturer recommendation, and provide a statement from the Chief Engineer attesting to proper operation of engine, prior to exiting MSU Duluth OCMI zone/US waters.

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Master reported vessel experienced a loss of propulsion in Marine City, MI while transiting from Calcite, MI. Vessel crew dropped anchors and were DIW for five minutes when vessel regained power and made way to its berth in Marine City, [name withheld] and ABS to attend to verify cause of the loss of power. Vessel may transit when USCG Sector Detroit receives Class Survey and Technician Report attesting to any repairs made and/or finding vessel in satisfactory condition.

Lifesaving appliances and arrangements are subject to tests and inspections described in paragraph (b) of this section during vessel boardings to ensure that the appliances and arrangements comply with applicable requirements, are in satisfactory condition, and remain fit for the service. Due to ice conditions at the pier, vessel unable to demonstrate launching, operating, and recovering rescue boat. Conduct operation in presence of Coast Guard marine inspector at soonest availability, within 30 days.

During the 2023 drydock-extension examination extensive wastage/surface-pitting was discovered on the upper-inside gasket-mating surfaces of multiple hatch-coamings (see photos uploaded to documents of MISLE Act: 7626156). Make permanent repairs to wasted hatch-coaming gasket-mating surfaces to original condition. Repairs to be completed and approved by USCG and/or ABS prior to departure from next drydock period scheduled for 2024.

Within the fire main system, all piping, valves, and fittings shall meet the applicable requirements of subchapter F (Marine Engineering) of this chapter. Five fire main valves on the exterior port side main deck, were found leaking during operational test. Repair leaks to satisfaction of attending marine inspector.

Acceptance of lifesaving appliances will remain in effect unless the OCMI deems their condition to be unsatisfactory. Observed the outer casing of the forward #1 inflatable liferaft to be cracked. Conduct appropriate repair or replacement through approved liferaft servicing facility to satisfaction of attending marine inspector within 30 days.

On U.S. inspected ships, oily water separating (OWS) equipment and bilge alarms must be approved under 46 CFR 162.050. Vessel's OWS is not operable due to cracked pump. Repair OWS in accordance with manufacturer's instructions, to satisfaction of attending marine inspector, prior to utilizing OWS. Vessel is restricted to discharging oily mixtures to reception facility.

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work except as modified by § 91.45-1. The general scope of the plans shall be as noted in § 91.55-5. Vessel changed loading program to Cargomax system, approved by ABS in 2017. Submit plans for new loading program system to MSC for approval, within 60 days.

Each fire hydrant must have a spanner wrench. Multiple hydrant locations on the port and starboard side cargo deck were not properly equipped with spanner wrenches.

Electrical wire enclosures must be secured to prevent exposure to electrical hazard. The wire connection box in the B Deck Fan Room was uncovered.

Hull structural standards must be satisfactory to ABS. Make satisfactory temporary repairs to the windlass room port side aft bulkhead and frame brackets prior to departure. Make permanent repairs within 30 days.

Periodic safety tests (PSTP) must be a step-by-step/check off list format and approved by the Coast Guard. Vessel has new equipment installed and it is not reflected in the PSTP. Review PSTP, make changes, and re-submit to the Marine Safety Center for approval.

The emergency firepump valves were not correctly lined up resulting in no firemain pressure when tested. Crew found the alignment problem and proved proper operation proved proper operation. Corrected on spot.

Provide documentation of launching, release, operation and retrieval of rescue boat. 46 CFR 199.160

Vessel's Incinerator Room does not have Coast Guard approved plans for the Fixed Fire Extinguishing System. This system also has mixed and matched components from either KIDDE or ANSUL. Fixed Fire Extinguishing systems shall have approved Coast Guard plans and the install- ation of the fixed fire extinguishing system is required to meet a Coast Guard type approved system with the components being from the same manufacturer. Replace mixed/matched components as per a Coast Guard approved plan. Submit plans as per the provided Marine Safety Center guidelines E1-O6, Guidelines for Carbon Dioxide Fire Suppression Systems. Incinerator shall not be in- service until fixed fire fighting system plans are approved and the system is tested by a Fire Servicing Company, in the presence of a Coast Guard Marine Inspector. 46 CFR 95.05-10 46 CFR 162.038 46 CFR 91.55-10

Vessel's electrical installations must ensure protection of the crew from electrical hazards. 120AC panel in ECR and the main emergency generator switchboard have grounds that need to be traced and rectified. 46 CFR 111.01-1

Periodic safety tests (PSTP) must be a step-by-step/check off list format and approved by the Coast Guard. Vessel has new equipment installed and it is not reflected in the PSTP. Review entire PSTP, make changes, and re-submit to the Marine Safety Center for approval. 46 CFR 61.40-10 46 CFR 50.20-5

In general, connections and terminations to all conductors must retain the original electrical and fire resisting properties of the cable. There are dead end wires above the #3 generator in the wire transit. Remove wire or terminate properly. 46 CFR 111.60-17

Valves must be designed for the maximum pressure for which they may be subjected to. During winter layup the forward ballast valve in the tunnel, starboard side, was not properly drained which caused the valve body to crack open. 8 inch valve needs to be replaced as it is providing constant water flow into the tunnel. 46 CFR 56.20-9

All pressure vessels must be provided with protective devices. #1 start air receiver's pressure relief valve did not properly seat after being tested. Valve shall be replaced and re-tested. 46 CFR 54.15-5

Each embarkation ladder must approved under 46 CFR 160.017-13. Vessel's embarkation ladder on the port side, aft did not have a rung until about 12 feet down from the place it was deployed over the side. Readjust arrangement to provide safe embarkation means. 46 CFR 199.110

At each inspection for certification and periodic inspection the quarters, toilets, and washing spaces shall be examined by the inspector to be assured they are in sanitary conditions. Showers throughout accommodations were missing paint, rusted and had mold. There was a leaking valve within the 1st Mate's stateroom that caused pooling water. Rectify to sanitary condition. 46 CFR 91.25-40

During house inspection further issues noted: broken toilets, light fixtures, bulbs, etc. Vessel delivered 20 page document showing needs of each accommodation space. These items have been documented in MISLE and will be required to be fixed and verified by a marine inspector. 46 CFR 91.25-40

Clutch hub on stbd MDE is inop. Prove proper operation to the satisfaction of CG within 7 days.

Vessel reported a failure of the raw water pump on the port main engine. Repair or replace pump to the satisfaction of the attending marine inspector and demonstrate proper operation of port main engine.

Suspected leak from seal on propeller. Make permanent repairs to the equipment that led to the pollution incident on 23 JUL 2020 so that the equipment is no longer leaking oil.

The number 3 generator tripped offline causing the vessel to go dark for approximately 4 mins.

Due to the vessels mooring arrangement, unable to verify proper operation of the rescue boat davit, winch, brakes and release hook at light load condition. Demonstrate the above in the presence of a Coast Guard Marine Inspector.

Emergency lights located throughout the house were not marked with a letter "E" at least 1/2" high. Label all emergency lights with a letter "E" at least 1/2" high.

Vessel had to shut down Port side engine due to a leak coming from a weld in the jacket water system. Effect repairs and provide proof of proper operation.

Cable penetrations were found to not meet the structural fire protection requirements. Decks and bulkheads separating accommodations, stairways, and machinery spaces shall be of "A" class construction. Install properly listed and approved systems to ensure class "A" boundaries are properly maintained in the locations provided in the proposal provided.

CO2 system in Incinerator space was released due to failure to follow operating instructions and leaving incinerator door open. Prior to returning Incinerator to service refill and service Fixed CO2 system IAW the KIDDE type approved design, maintenence and service manual, including all tests required to prove HAD proper operation, and prove proper operation of all controls on the Incinerator.

During loss of power the CPP pump seal failed. effect repairs to system and prove proper operation prior to departure from Cleveland, Ohio.

Vessel was unable to lower the open Life Boat, as well as get the life boat underway during the Periodic COI inspection due to location at the dock. Vessel is required to lower the life boat into the water and have crew maneuver the life boat in the water in the next 30 days.

Use of the onboard incinerator is prohibited until the co2 system associated with the incinerator is serviced and recharged by a fire service professional.

Provide a certificate from a professional fire service company stating the incinerator co2 system is in proper working condition.

Prove proper working condition of the incinerator door, cable, pulley, and counterweight to an attending CG Marine Inspector.

Make Permanent repairs to the starboard main engine which has sustained two occurrences of de-clutching. Provide technical service report after repairs.

The fixed CO2 fire extinguishing system for the incinerator space was unintentionally discharged as a result of activation of the heat-activated-device caused by overloading/overheating the incinerator. Recharge and conduct functional test of incinerator space fixed CO2 system. The incinerator is to remain secured until the CO2 system has been recharged and fully restored to functional order.

Provide service/tech report on low lube oil shutdown within the MAST automation.

Make permanent repairs to port side hatch coaming on #8 hatch prior to departing Port Erie. Vessel permitted to transit, must fill gap with boom or foam and in the case of adverse weather use tarping. Repairs to be completed to the satisfaction of cognizant OCMI.

Rescue Boat placard A and B weights only account for a differenc of 820 pounds and does not match the approval certificate found in the Viking manaul. Ensure proper A and B weights are listed in both the Viking manual certificate and on the Rescue Boat placard.
