The Vessel Master made notication to [name withheld], that they vessel will be sailing short 01 AB. The Master has indicated that a qualified replacement AB is not expected to be available until the vessel's port of call in Gary, IN, on or about 09 AUG 2026. Accordingly, the vessel is permitted to proceed with one less AB for a period not to exceed seven (07) days, expiring on 09 AUG 2026. A three person watch system and work/rest hours must be maintained in accordance with applicable laws. The vessel/company shall make reasonable efforts at each ports to find a replacement. Vessel shall noptify the OCMIU, SECTOR DETROIT, of any changes. 705c (7 days)

All inspected vessels are issued a COI indicating minimum crew complement; crew was missing 3rd mate needed to meet minimum manning. 46 CFR 15.105(c)

Each rescue boat must carry as appropriate equipment specified in this part; fire extinguisher had not been serviced annually. 46 CFR 199.725(b)(9)

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket; (07) lifejackets were missing lights. 46 CFR 177.70(b)(4)(i)

Panelboard switch units shall be provided with a circuit directory card marked with the description of load served; rated load was not listed on any distribution panel directory. Cross reference available wiring references and add rated loads to each distribution panel directory throughout the vessel prior to next topside inspection. 46 CFR 111.40-1(h) (Old "J")

In addition to prescribing watch requirements, 46 U.S.C. 8104 sets limitations on the working hours of credentialed officers and crew members, prescribes certain rest periods, and prohibits unnecessary work on Sundays and certain holidays when the vessel is in a safe harbor. It is the responsibility of the Master or Person in Charge to ensure that these limitations are met. The work-rest logs being maintained currently are not satisfactory to prove this to be the case as they do not record time worked during drills (e.g see rough log with work-rest hours on 29Dec2025). Begin logging drills as work in work-rest system vessel has opted to use; make available for inspector at next topside. 46 CFR 15.710

Each auxiliary boiler must be available for examination at intervals specified by 46CFR61.05-10. Aux boiler SerNo CHI-7825 shall be tagged out until the following inspections and tests have been completed: Mountings Open/Removed, Fire and water side inspections, boiler safety valve SerNo BT08067 shall be adjusted to relieve at not more than 45 psi. Due prior to boiler operation.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Each vessel is to be provided with two bower anchors(stocklass. The starboard anchor windlass is not operational and the starboard anchor and chain were lost. Provide a repair proposal and make repairs to starboard windlass and install starboard anchor prior to departure from Superior, WI.

Vessel is required to have the manning as specified on the COI. Vessel does not currently meet the minimum manning specified on COI and is short 2 ABs. Update WQSB to document emergency resposibilities. Vessel shall return to required minimum manning prior to departure from Superior, WI.

Vessel reported issue with port inboard main engine that prevents the engine from providing thrust to the prop. Vessel is equipped with 2 mains per prop and has not lost ability to maneuver. Vessel authorized to transit to Conneaut, OH for cargo offload and then to Ashtabula, OH for winter layup. Vessel master must inform CG of any subsequent issues during transit that may reduce vessel's ability to maneuver. CG/ABS will verify full operation of port inboard engine prior to vessel breaking out for spring shipping season.

Master made notification to [name withheld], of crew shortage due to consent based discharge of 3rd Mat Johnaton Deering, ref# 3860923, and 1st Assistant Engineer [name withheld], ref#173470. This consent based discharge is needed to return these crewmembers to their previously assigned vessel that was temporarily out of service for repairs. The vessel is authorized to sail with one less 3rd Mate and one less 1st Assistant Engineer fro 10 days until 8 July 2024. Three person watch system and work/rest hours must maintained in accordance with applicable laws. The vessel/company shall make reasonable efforts at each port to find a replacement. Prior to the expiration of the deficiency, the vessel/company is required to provide proof of adequate effort to find crew replacements.

The port SSDG was removed for a major overhaul and a temporary generator was installed on the port aft main deck. Make permanent repairs and install the port SSDG to its original location.

In addition to the emergency power source required under part 112 of this chapter, each self-propelled vessel must have at least two electric generating sources. Vessels reported the port generator is not currently operational.

Vessel cannot transit during storm or gale warnings are in effect by National Weather Service. Vessel must contact each Captain of the Port Zone prior to entering to inform them of entry and report if any new failures occur. Assist Tugs shall be used in accordance with voyage plan in addition to having an assist tug for the Straits of Mackinac, and informing Sector Northern Great Lakes VTS and Command Center the specific assist tugs being used for transit in the Straits and [name withheld] System. Notification must be made prior to entering Gros Cap. For Sector Northern Great Lakes area transit, tugs can depart once the vessel is safely west of the Mackinac bridge. Vessel must submit temporary repair plan for review and acceptance by local OCMI in Sturgeon Bay, WI. The port generator shall be repaired in 30 days from this 835V issuance date.

Each cargo vessel must carry a lifeboat with aggregate capacity to accommodate the the total number of persons on board. The current lifeboat has insets at the bow and stern, along the port side. While the lifeboat sits with an even keel, and is not taking on water while deployed, the sharpness of the insets bring the integrity of the hull into question. Vessel is to replace the lifeboat in accordance with 46 CFR 160.156 and installed in accordance with 46 CFR Subchapter W, by next Winter Lay Up to the satisfaction of the Coast Guard.

EACH VESSEL MUST HAVE AN ILLUMINATED MAGNETIC STEERING COMPASS, MOUNTED IN A BINNACLE, THAT CAN BE READ AT THE STEERING STAND.

The main steering gear and rudder stock must be of adequate strength for and capable of steering the vessel at maximum ahead service speed, which must be demonstrated to the satisfaction of the cognizant Officer in Charge, Marine Inspection. During transit in the St. Marys River the rudder was stuck at Starboard 25 degrees for 50 seconds rendering the primary steerage of the ship inoperable. Conduct testing to diagnose this anomaly at anchorage and before transiting through the St. Marys river. Provide attestation to [name withheld] identifying Master's confidence in a safe voyage with mitigating steps if there is a re-occurrence. Also provide technician report prior to departing next port of call.

Main propulsion machinery must be provided with automatic shut-off controls. Vessel has a faulty fuel oil pressure switch that caused the engine to de-clutch when operating near the Sault locks on 2SEP23. Vessel must make permanent repairs to the faulty switch prior to departing Two Harbors MN. 46CFR90.20-1 46CFR58.05-10

Vessel master reported an issue with their outboard port steering pump, which caused it to not be operational. Vessel crew shall trouble-shoot the system to identify the causal factor(s) for the steering pump to become not operational and conduct repairs and test steering using the outboard port steering pump to the satisfaction of the attending Coast Guard Marine Inspector or Class Surveyor prior to departing Duluth, MN. Vessel master shall make arrangements to have an appropriate tug escort from Mud Lake to Gros Cap (St. Marys River system) and Duluth/Superior Harbor Entrance. 46 CFR 91.25-35(a) 46 CFR 61.20-1

Vessel experienced vibration from propeller associated equipment and has determined that it is most likely caused by ice damage. Vessel is permitted to proceed to Duluth, MN at reduced speed and appropriate weather conditions to effect immediate repairs to the satisfaction of Class or attending Coast Guard Marine Inspector. 46CFR58.05-1(a)

Due to mooring arrangements the required 1.1 overload weight test could not be completed. The life boat is to be tested IAW 46 CFR 199.190(i) due to the renewal of the COI. This test is to be followed by satisfactory demonstration of a person overboard drill. All to the satisfaction of an attending CG Marine Inspector. 46CFR199.190

On B deck above the breaker panels at the door that leads to A deck there is an approved Muli-cable transit system to ensure structural fire adequacy of the overhead. Within this MCT there are four cables that enter a single MCT block. This MCT is to be corrected with the installation of an approved MCT system rated for the fire rating of the overhead and installed IAW the manufacturer's manual. (multiple cables may not penetrate a single block). To be inspected and air tested to the satisfaction of an attending CG Marine Inspector.

Each vessel of 10,000 gross tons or more must have, in addition to the radar system under § 164.35(a), a second marine radar system that operates independently of the first. Vessel's secondary radar system is inoperable.

The installed light bulb on the magnetic compass is inoperable and needs replacement.

Aux. Boiler #-S4317, has a chunk of insulation missing on the fire door, that must be repaired with approved material, prior to being used.

Wooden blocks on the davit, that hold the lifeboat in place while stowed, must be replaced due to rotted condition. Must be completed no later than 01JUN2021.

The steering is operating at reduced capability. The steering system is equipped with two hydraulic pumps per rudder. Only one steering pump per rudder can be energized due to an undiagnosed fault. The system fault shall be diagnosed, repaired and satisfactorily tested prior to departing next U.S. Port.

Did not conduct drills to maintain social distancing practices (COVID-19) during inspection. Drills (Fire and abandon ship) to be conducted in the presence of Coast Guard inspectors at a future date.

Vessel reports discovering a loss of 3-4" of water in jacket- water head tank, and intrusion of water into the oil of the PIME. As a result, the PIME has been shutdown and taken out of service. Vessel authorized to load cargo in Superior, WI and subsequently transit to Conneaut, OH for offloading, and PIME repairs. Make complete repairs to PIME prior to departure from Conneaut, OH and provide service technician's report, and a statement from the C/E, certifying restoration of full engine functionality to MSU Cleveland or MSU Duluth.

Port rudder failed to respond while on port pumps. Master switched to stbd pumps and regained full functionality. Provide tech service report regarding intermittent mode failure and cause and make permanent repairs to original condiiont prior to operation on port pumps.

Repair/replace bearing on stbd inline generator

Vessel was unable to lower the open Life Boat, as well as get the life boat underway during the Annual COI inspection due to location at the dock. Vessel is required to lower the life boat into the water and have crew maneuver the life boat in the water in the next 30 days.

One of the port propeller flukes was found to have significant inter-granular corrosion during inspection. The fluke number is HT79GA. A replacement fluke is not immediately available and must be ordered. Fluke # HT79GA may remain in service until a proper replacement in kind is available. In no case may the damaged fluke remain in service past 31JAN19. The fluke must be permanently repaired IAW ABS rules or replaced in kind by this date. Any adverse change in condition to the port side propulsion system or failures are to be immediately reported to the local [name withheld] or permanent repair to be verified by an attending CG Inspector.

Due to loss of port rudder, vessel is required to discharge current cargo and make permanent repairs to rudder and damaged port propeller. Vessel is allowed to transit to Indiana Harbor to off load cargo and then proceed to Sturgeon Bay for repairs. During transit vessel is required to make safety rounds to ensure that there is no water ingress in spaces that could be affected by the loss of the port rudder at intervals of not more than two (2) hours During transit to Indiana Harbor, vessel is required to notify each USCG Sector as the vessel enters each zone and ensure that there are no further requirements for safe transit (ie. tug assist). When transiting the Sector Sault Ste. [name withheld] zone a tug assist will be required and the vessel will only be allowed to transit the locks during daylight hours. 46 CFR 58.25-10

Helmsman must remain on watch at all times until autopilot failure(loss of CDU) alarm is installed and tested to the satisfaction of the attending CG MI.

Repair/replace affected areas damaged after grounding on the Conneaut Harbor breakwall. Areas to be identified after dry-docking in Erie, PA.

Submit revisions to the Design Verification Test Procedure (DVTP’s) for the Steering Stand to address all comments in the USCG Marine Safety Center letter E2-1701283 dated 18 April, 2017.

After the USCG Marine Safety Center considers the Steering Stand Design Verification Test Procedure (DVTP’s) are satisfactory for shipboard testing, conduct shipboard testing in the presence of the Cognizant OCMI Marine Inspector.

Determine source of hydraulic fluid leak on stern thruster/stern tube. Provide report to cognizant OCMI attesting to repair and satisfactory operation of stern thruster. Stern thruster is to remain secured from operation for remainder of voyage to Duluth/Superior.

A helmsman shall remain on watch at the main steering console AT ALL TIMES while underway until shipboard testing of the Steering Stand Design Verification Test Procedures (DVTP’s) are verified to the satisfaction of attending CG Marine Inspector.

When maneuvering or while navigating the river systems the Engine Control Room shall be manned with one licensed engineer. Another crew member that is knowledgeable on the operation of the steering system shall be immediately available on request of the licensed engineer on watch until shipboard testing of the Steering Stand Design Verification Test Procedures (DVTP's) are verified by a CG MI

Vessels Bow Thruster is not operational. Until corrected, to the satisfaction of the cognizant OCMI, when operating in restrictive water (Sturgeon Bay, in & out of Soo Locks, & Duluth Harbor) vessel must have tug assistance.

Vessel has uncompleted repairs to watertight bulkheads as follows: Watertight bulkhead at fr 90 between cargo holds 5&6, watertight bulkhead at fr 36 between cargo holds 2 & 3, and watertight bulkhead at fr 21 between cargo holds 1 & 2. Details of required repairs were issued via hard copy 835 which is scanned into activity and listed in narrative portion dated 20MAR.

Make permanent repairs to lifeboat: Replace deteriorated internal wooden components to include: port fore plywood walkway/seating; port fore outboard 2" X 10" bottom plank; and starboard aft outboard 2" X 10" bottom plank, to the satisfaction of the cognizant OCMI.

Repair fracture in the #7 Starboard Ballast tank that is leaking into the conveyor tunnel prior to departure from port.

Conduct Man Overboard drill to satisfaction of the attending Marine Inspector and conduct a satisfactory test of the lifeboat hook release mechanism.
