Ship's crew discovered a 30 inch fracture in the transverse bulkhead between 6 Stbd and 7 Stbd ballast, adjacent to the 2nd transverse stiffener above the bottom shell, starting at the mid bulkhead girder and extending inboard. Temp repair was conducted by Hansen's and NDT witnessed by attending ABS Surveyor. Local inspector's were not given the opportunity to attend because proper notification was not made by the company to the local OCMI. This repair is to be made permanent during upcoming winter layup period to the satisfaction of local OCMI.

No person may use any equipment listed in paragraph (c) of this section for transfer operations unless the vessel or facility operator, as appropriate, tests and inspects the equipment in accordance with paragraphs (b), (c) and (f) of this section and the equipment is in the condition specified in paragraph (c) of this section. Hoses must be tested at a static liquid test pressured of 1.5 MAWP and tests must be conducted at least annually. Vessel has no records of hoses testing for their oil transfer hoses. Vessel must not conduct over the water oil transfer using ships hoses until they are properly tested, or replaced with new certified hoses. 33 CFR 156.170(a)

The installation of all systems of a marine engineering nature, together with the details of design,construction, and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering) of this chapter. Vessel reported fire on port MDE turbo, suspected cause being cracked exhaust pipe. Affect repairs at current location prior to continuing to NPOC. 46 CFR 96.03-1(a)

The installation of all systems of a marine engineering nature, together with the details of design,construction, and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering) of this chapter. Valve for the emergency fuel shutoff on the supply side of the port MDE is leaking fuel, and the valve for the emergency fuel shutoff on the return side of the STB MDE is leaking fuel. Until valves are repaired / replaced, Vessel must have a "small discharge containment" set up beneath each leak and complete rounds at least every 30 minuets to monitor the containment to reduce fire risk as much as possible. 46 CFR 96.03-1(a)

Tests must verify that automated vital systems are designed, constructed, and operate in accordance with all applicable requirements of part 62 of this subchapter. The tests must be based upon the failure analysis, if required by § 62.20-3(b) of this subchapter, functional performance requirements, and the Periodic Safety tests of § 61.40-6. (b) Tests must be performed immediately after the installation of the automated equipment. Vessel has installed a new CCP control system. DVTP’s and PTSP's have been tested with corrections noted under revision 1 dated 24OCT24. Vessel is to submit approved revision to the USCG Marine Safety Center. 46 CFR 61.40-3

Each valve must be opened and examined by the marine inspector at the interval specified in table 1 to § 61.05-10. Valve inspections could not be completed on both boilers during winter layup 2025. Vessel shall open valves on both boilers and make ready for inspection at the next convenient and available port within 30 days. 46 CFR 61.05-15

Periodic tests and inspection must be made of the main and auxiliary machinery, boilers, and other equipment as prescribed in this part. STBD boiler(CHI-74-70) could not be made available for fireside, waterside or safety relief valve tests and inspections. Vessel shall open fireside, waterside and make ready for inspection at the next convenient and available port within 30 days. 46 CFR 61.01(a)

No repairs, replacements, or alterations, except emergency repairs, must be made to boilers, pressure vessels, their mountings or internal fittings, safety valves, piping systems, or pressure appliances without prior approval by the Officer in Charge, Marine Inspection. During Fire side inspection of the Port Auxiliary Boiler(CHI-74-69), marine inspectors discovered 03 fractured repair welds inside the burner housing. Vessel has completed an approved temporary repair. Vessel must crop and renew the area prone to fracturing under the direction of a marine inspector prior to fit out 2026. 46 CFR 59.01-5

The design and arrangement of electric apparatus must afford accessibility to each part as needed to facilitate proper inspection, adjustment, maintenance, or replacement. Main switchboard in engine control room had excessive debris behind it. 46 CFR 111.01-7(a)

Where fitted, it shall be the duty of the master to see that the emergency lighting and power systems are operated and inspected at least once in each week that the vessel is navigated to be assured that the system is in proper operating condition. Emergency lights extinguished in the engine control room. 46 CFR 97.15-30(a)

Electrical equipment in locations requiring exceptional degrees of protection as defined in § 110.15-1 must be enclosed to meet at least the minimum degrees of protection in ABS Marine Vessel Rules, section 4-8-3, Table 2, or appropriate NEMA 250 type for the service intended. Lighting fixtures throughout the engine room were found broken, oil soaked, or missing protective covers; thus, no longer satisfying the degree of protection required for the space. 46 CFR 111.01-9(b)

Each Generator arranged for parallel operation must have reverse-power relay tested annually. While conducting vessels COI renewal inspection, exhaust bellow was taken off #2 SSDG to repair #3 SSDG since #2 SSDG had high operational hours. Once exhaust on #2 SSDG is repaired, verify no exhaust leaks and test reverse power relay prior to using for ships power to the satisfaction of attending Coast Guard Marine Inspector. 46 CFR 111.12-11

Each device must have a nameplate showing the device’s function and electrical load served. While conducting vessels COI renewal inspection, found circuit breaker lower left corner of switchboard in engine control room missing nameplate. Replace missing name plate for that breaker to the satisfaction of attending Coast Guard Marine Inspector. 46 CFR 111.30-15

Electrical installations on vessels must ensure protection from passengers, crew, other persons, and vessel from electrical hazards. While conducting vessels COI renewal inspection, found circuit breaker lower left corner of switchboard in engine control room missing the cover that is attached to the breaker handle providing cover from inside the switchboard as breaker is turned on and off. Replace missing cover to the satisfaction of attending Coast Guard Marine Inspector.

All general alarm bells shall be identified by red lettering at least 1/2 inch high: “GENERAL ALARM—WHEN BELL RINGS GO TO YOUR STATION.” Repair or replace general alarm signage next to the sewage tank in the engine room. 46 CFR 97.37-7(a)

In a space described in § 113.25-9(a), where the general emergency alarm signal cannot be heard over the background noise, there must be a red-flashing light or rotating beacon, in addition to the general emergency alarm signal, that:(1) Has sufficient intensity above the background lighting that would alert personnel in the space;(2) Is activated whenever the general emergency alarm signal in the space are activated; Numerous general alarm lights in the engine room are not working. 113.25-10(a)

Each power ventilation system stop station must be marked, “In Case of Fire Break Glass and Operate Switch to Stop Ventilation;” and have a nameplate that identifies the system(s) controlled. Break glass station near the galley was not labeled correctly or clearly. 46 CFR 111.103-7

Each device must be free of design defects such as rough or sharp edges that may cause bodily injuries or that would allow toxic substances to escape to the interior of the vessel. Vessel sewage tank appeared to be leaking from the bottom. Vessel shall have tank gauged or additional means the marine inspector finds adequate. Submit gauging report to USCG MSU Toledo. 33 CFR 159.95(a)

n general, compliance with the standards established by the American Bureau of Shipping will be considered as satisfactory evidence of the structural efficiency of the vessel. Vessel's deck plating around the sewage tank is 36% corroded from its original thickness. Vessel shall enact permanent repairs to the satisfaction of the local OCMI. 46 CFR 92.01-10

Piping systems used where the fluid temperature exceeds 150 °F must be suitably insulated as necessary to preclude injury to personnel. Marine inspectors found steam insulation and oil piping lagging missing throughout the engine room. Complete a full survey of all missing or damaged insulation and lagging and replace. 46 CFR 56.50-1(k)

All vessels shall have efficient guard rails or bulwarks on decks and bridges. Found broken handrail in the port side of the engine room. 46 CFR 92.25-5

Each outlet or junction box must have a cover. Marine inspectors found numerous wire connections exposed and without covers throughout the engine room. 46 CFR 111.81-1

Exhaust pipe installation must conform to the requirements of ABYC P-1 and Chapter 6 of NFPA. Vessels STB M/E had an exhaust leak due to a broken baffle. 46CFR58.10-10(a) 46CFR58.10-5(d)(1)

An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs or alterations are undertaken. Vessel has reported and corrected nine separate fuel line issues for their port and starboard main engines within the last 90 days. The most recent report specifically being for the port main engine in way of the low pressure fuel return line for the #15 cylinder on 16 Dec 2024. Vessel shall ensure assessment of both main engines is completed and all repairs verified and tested by attending third party technician attesting to the satisfactory working condition of the engines, to the satisfaction of Class Surveyor and CG Marine Inspector.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. During examination of previous repairs made to both main diesel engines, excess oils were found in the bilge and vicinity of lower engine compartments on the port and starboard engine rooms. Vessel shall conduct general cleanup of these areas to the satisfaction of CG Marine Inspectors within seven calendar days.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel is required to complete a classification society survey prior to departing Duluth, MN attesting to the appropriateness of repairs to the port main engine fuel leak that occurred on 16DEC2024. Vessel's crew shall also make extra rounds of the engine room to ensure any fuel leaks are discovered and reported.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. The lower level engine room around PMDE and recent fuel oil line repair has excessive oil/residue on engine, deck grating, overhead fixtures, bulkheads, and various piping. Remove/clean all oil/residue fire hazards and submit evidence of completion to local OCMI prior to departing US Steel, Gary, IN.

Installation of all systems of a marine engineering nature shall be in accordance with the requirements of 46 CFR subchapter F; the port MDE had a fuel line failure and a temporary repair using mild steel pipe was completed. The fuel line is no longer in compliance with the requirements of 46 CFR 56.60-75. Make permanent repairs to the port MDE fuel line prior to departure from Gary, Indiana. Until corrected, engineering crew is to maintain a regular roving fire watch throughout the main space. Tug assist is required at the Chicago lake front starting at the [name withheld] Light (LL 19610). Coordinates 41.37-49.146N, 087-19-13.098W.

At approximately 1708 local time RCVD report from vessel that while underway on Lake Huron a fitting failed on cylinder four outboard low pressure fuel line. C/E reported failure due to vibration and loosening. SME was powered down and line and connection were replaced with on board spares at 1630 local time. SME was tested and is fully operational. 835V issued corrected on spot (10c).

Vessel Marine Superintendent reports delays in obtaining necessary parts, and technical support, for repairs to the vessel's bow-thruster. Vessel is authorized to continue to operate with assist-tug support, required during transit of all restricted waterways (and all other waters where deemed necessary by the vessel's Captain). Vessel to contact respective COTP zones if unsure of which waters qualify as restricted. Upon completion of repairs, submit technician's report, certifying satisfactory repairs/testing, to MSU Duluth, MN.

Vessel reports loss of bow thruster function due to a failed rectifier in the motor controller cabinet. Part is on-order with anticipated delivery, and subsequent installation in Gary, IN, upon ship's arrival in that port. Vessel is authorized to transit from Duluth, MN to Gary, IN with assist-tug required in all restricted waters (to include: departure from Duluth, MN, St. Mary's River system, and approaches to Gary, IN; and anywhere else that the Captain deems necessary). Vessel Captain to provide technician's report to MSU Duluth upon satisfactory bow thruster repairs/tests (to be completed prior to departing from the Port of Gary, IN).

Received notification from vessel that they will being sailing short a first mate.

Portions of the stbd MDE exhaust lagging are contaminated; Both MDE's exhaust lagging is laid in place but not properly secured/wrapped tight. Clean or replace contaminated lagging and properly install & secure all MDE exhaust lagging. Until corrected, engineering crew is to maintain a regular roving fire watch throughout the main space.

Fire hazards shall be minimized insofar as is reasonable and practicable; insulation around the stbd main engine caught fire. Vessel is authorized to continue transit from Duluth to Cleveland. Upon arrival in Cleveland, vessel is to have attending Marine Inspector clear the deficiency prior to departure. Alternatively, vessel may contact Sector Northern Great Lakes when transiting through the locks to request a MI to attend and clear this deficiency. Until an attending MI has cleared this deficiency, vessel is to maintain a roving watch with rounds at intervals not to exceed 30 minutes. Fire equipment is to be readily available during these rounds.

Vessel reported only the master and 1st mate have the first class pilotage endorsement. The vessel’s COI requires 1 master first class pilot and 3 mate first class pilots but allows for the holder of a master or mate license of appropriate tonnage to substitute for one mate first class pilot while navigating the open waters of the Great Lakes, as defined in the route permitted and conditions of operation section. The 2nd mate and 3rd mate do not have first class pilotage. Vessel is authorized a one time transit to Gary, Indiana, in accordance with the submitted watch schedule plan. If work/rest limitations and the 3 watch system cannot be maintained due to vessel traffic, adverse weather, or other reasons, the vessel shall anchor or moor, in accordance with their submitted plan, until the vessel is in compliance with applicable regulations including 46 USC 8104. The vessel shall meet the first class pilotage requirements, as stated on the COI, prior to departing next port of call of Gary, Indiana.

The rudder stock must be of adquate strength and capable of steering the vessel at maximum ahead speed, to the satisfaction of the Officer in Charge, Marine Inspection. The ship's port engineer reported that they could physically see a bolt missing and another one is loose on the rudder stock. The fasteners must be reattached and the rudder must function as designed to the satisfaction of the CG and Class.

M/V AMERICAN SPIRIT reported an issue with its Starboard Main Engine where crew shut it down due to observing high temperature readings on its exhaust system. Vsl to burn only light diesel fuel until appropriate repairs are made. A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. M/V AMERICAN SPIRIT reported an issue with its Starboard Main Engine where crew shut it down due to observing high temperature readings on its exhaust system. Vsl to burn only light diesel fuel until appropriate repairs are made. 46 CFR91.30-1(a)

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. A 6-8" crack is reported in the port sideshell in the #1 ballast tank. Conduct temporary repairs, as proposed-drill stop the crack in the shell, back gauge the crack, weld both sides of the crack. Submit permanent repair proposal and complete subsequent repairs, after discharging cargo at next port, to local OCMI.

All engine installations shall be suitable for the intended service, and designed and constructed in conformance with subchapter F. Vessel Master made notification of a turbocharger failure on the port main engine. Vessel allowed to proceed directly to next port of call in Conneaut, OH and shall rectify prior to departure from Conneaut, OH. 46 CFR 58.10-5(a) 46 CFR 58.10-10(a)

The survey shall be such as to ensure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel's port main engine was shutdown due to a leak in the fuel line. Master and Chief Engineer shall submit an attestation prior to arriving at St. Mary's River system stating the vessel is safe to sail to Two Harbors, MN. 46CFR91.30-1 10C

The starboard-aft bilge high-level alarm annunciator (at the engine control room panel) is not functioning. Repair annunciator and prove proper functioning.

The primary fuel line heat lagging on both main engines is oil-saturated (outer covering). Replace lagging on both main engine fuel lines. Provide invoice and photos of installed lagging to MSU Duluth.

The port main engine lube-oil cooler is leaking on the inboard side from flange connection. Correct condition to prevent lube-oil system leak. Chief Engineer to provide signed statement to MSU Duluth certifying that repairs have been satisfactorily completed.

The stability letter does not have the most appropriate stability conditions that should be followed by the vessel. Marine Inspector noted that the stability letter does not have the appropriate loading manual cited. There is a more up to date loading manual "Revision F" approved by ABS in 2019 that supersedes "Revision D" loading manual dated March 1992 noted on the stability letter. Vessel shall resubmit their msot up to date manual to MSC for a new stability letter. Send proof of new stability letter to USCG.

Lifeboat release gear must be oeprationally tested under a load of 1.1 times the total mass of the lifeboat when loaded with its full complement of persons and equipment whenever overhauled or at least once every 5 year. Vessel could not deomstrate a dunamic test of the release gear during inspection due to lifeboat being shore side over the gangway. Vessel shall conduct a dynamic test of the release gear in accordance with the regulations cited prior to or at next port of call and send evidence to USCG.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to ensure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. During transit to Superior, WI for winter lay-up #4 SSDG suffered mechanical failure rendering inoperable. Vessel shall conduct repairs during winter layup in Superior, WI and will provide class survey report or satisfactory inspection by USCG.

The Installation of all systems of an electrical engineering nature shall be in accordance with the requirements of sub chapter J. The #1 generator overloaded and tripped it's circuit breaker while transiting across Lake Superior. Vessel restricted fro using the #1 generator. Submit Tech report attesting to properly functioning #1 generator prior to use. 46CFR96.05-1

Cargo hatch cover and combing of #23 hold no longer provides watertightness after a loading boom fell onto cargo hatch combing. Vessel has completed temporary repairs and will sail to Toledo, OH to discharge cargo and conduct permanent repairs.

This 835 amends the original 835 issued on 5/23/2022 for submission of PSTPs to MSC. PSTPs are to be revised as per any MSC return for revision letters if issued. Once approved the approval letter must accompany the PSTPs on board for future inspections. The newly approved PSTPs are to be tested in the presence of an attending Coast Guard marine inspector prior to the vessel getting underway during spring break out in 2024, NLT 15MAR24.

This 835 amends the original 835 issued on 5/23/22 for a plan to be submitted to the CG to have the openings below the main distribution panel in the engine control room sealed to maintain the required “A” class boundary between the control room and the machinery space as required by 46 CFR 92.07-10(c). Fire stop has been proposed as the approved method of structural fire protection. The installation for the fire stop shall follow the approved installation procedures listed for the product used. The installation shall be proved to be properly completed to the satisfaction of an attending CG marine inspector followed by an air test with soapy water to ensure a proper seal and installation. Final installation and verification by the CG shall be completed NLT 30SEP23.

Master made notification to [name withheld], of crew shortage due to consent based discharge of 1st Asst. Eng. [name withheld], MMD [withheld], who departed the vessel on 09AUG23 in Marquette, [name withheld] has supplemented Engineering department with a shoreside fleet engineer [name withheld] a former engineer on the vessel, and Chief Engineer as needed to maintain work/rest. 1st Asst. Eng. is scheduled to return after 30 days from departure.

Whenever it is necessary to make decisions in matters not specifically covered by the regulations in this subchapter or by referenced requirements, the marine inspector shall clearly state the reasons which caused him to arrive at such decisions. The Oily Water Separator (OWS) on board the vessel does not operate in accordance with 33 CFR 151.10 and allows for an effluent greater than 15 parts per million to enter the navigational waters of the United States. Vessel is to make permanent repairs to the OWS prior to its operation to the satisfaction of the Coast Guard.

During a fireside inspection of the port inboard auxiliary boiler (#CHI-74-70) two insets/bulges were discovered in the combustion chamber tube approx 18" inward from the burner assembly end. These insets project approx 1 1/2" inward. Boiler shall be taken out of service and assessed by a certified boiler specialist. Provide service report to USCG MSU Duluth indicating whether, or not, boiler is safe for continued operation and whether, or not, repairs are required before resuming operations.

Vessel failed to make auxiliary boilers CHI-74-70 and CHI-74-69 ready for inspection during the 2023 winter lay-up season. Both boilers are overdue for Fireside Inspection, and CHI-74-70 is additionally overdue for Waterside Inspection. Complete all required boiler inspections by no later than 19 JUN 2023. Note: Automation and safeties were satisfactorily tested on both boilers during the 2023 fit-out exam.

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. The use of the elevator requires the opening of an electrical panel; it is not operating normally, as designed. The last state inspection of the elevator was in 2016. Elevator must be inspected safe for use by an appropriate local/state entity before continued use by shipboard personnel.

Acceptance of lifesaving appliances and arrangement will remain in effect unless the OCMI deems their condition unsatisfactory. Periodic maintenance prescribed in the lifeboat engine’s technical manual by 46 CFR 199.190 (b)(1)(iii), including seasonal check out and end of season servicing, of the lifeboat engine are not completed. Complete the seasonal check out and end of season servicing of the lifeboat engine within 30 days to the satisfaction of the attending marine inspector.

Electrical installations on vessels must ensure protection of the crew from electrical hazards. In bathroom #44, stateroom #34, and the laundry space, lights were found without appropriate protective covers. Replace/install lighting covers in identified accommodation spaces to the satisfaction of the attending marine inspector within 30 days.

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work except as modified by § 91.45-1. The general scope of the plans shall be as noted in § 91.55-5. Vessel changed loading program to Cargomax system, approved by ABS. The vessel also added an ABS approved addendum to the loading manual, stability letter, and loading software which permits the use of the inner bottom voids as ballast tanks, without approval from the Coast Guard. Submit plans to update the new loading program system on the stability letter and the addendum permitting use of the inner bottom voids as ballast tanks, to the Coast Guard Marine Safety Center for approval within 60 days.

Radiators and other heating apparatus must be so placed and shielded, where necessary, to avoid risk of fire, danger, or discomfort to the occupants. A portable heating apparatus was found operating in an crewmember’s unattended stateroom near combustible materials, posing an unacceptable fire risk. Such apparatus are not permitted onboard the vessel. Rectified on the spot.

At each inspection for certification and periodic inspection, the inspector shall examine the tank tops and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. There is a leaking fuel line on the starboard engine near the governor. The leaking fitting shall be repaired to prevent any combustible liquids from leaking.

VESSEL FAILED TO DISCHARD EXCESS OILY WATER BILGE WASTE AT THE MISTURSKY PORT OF CALL AS ISSUED UNDER ACTIVITY 7457704. PRIOR TO DEPARTURE FROM DULUTH, MN ALL OILY WATER BILGE WASTE SHALL BE REVMOVED FROM THE VESSEL TO THE SATISFACTION OF AN ATTENDING MARINE INSPECTOR.

At each inspection for certification and periodic inspection, the inspector shall examine the tank tops and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. The lube oil purifier space has excessive oils and oily sludges accumulated in the bilges. Bilges are required to have all oily sludges and oils removed from the bilges.

At each inspection for certification and periodic inspection, the inspector shall examine the tank tops and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. There are several leaking oil/fuel fittings below the governor on the port engine. The leaking fittings shall be repaired to prevent any combustible liquids from leaking.

If the periodic inspection reveals deficiencies in your vessel's maintenance, you must make repairs any or all repairs or improvements within the time period specified by the OCMI. On the port engine there are two 1/4" tubing lines, air and/or oil that have temporary repairs that have failed and are leaking. Renew all sections of tubing necessary to prevent any leaking of air and/or oil.

At each inspection for certification and periodic inspection, the inspector shall examine the tank tops and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. The port engine room bilges contain an excessive amount of combustible material, sludges and liquids. All combustible materials, sludges and liquids shall be removed from the bilges.

At each inspection for certification and periodic inspection, the inspector shall examine the tank tops and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. At time of inspection vessel had 8000 gal of oily water in the bilge; mixture was removed and pressure washers were brought in to clean residual oil in the engine room. Bilges are now filled with more oily water mixture and pose a level of risk for fire. Vessel shall dispose of oily water at Mistursky port of call. 46CFR92.05-1(a)

Each self-propelled vessel must have a navigation light indicator panel in the navigating bridge to control side, masthead, and stern lights. The panel must visually and audibly signal the failure of each of these. The navigation light panel does not sound an audible alarm for the stern light and also needs bulbs replaced for stern and range light indicators. 46CFR111.75-17(b)

Each panelboard must meet section 17.1 of IEEE 45-2002 (incorporated by reference; see 46 CFR 110.10-1). Panel P-137 in galley hallway has open breaker spaces. Vessel shall replace the holes with blanks as necessary. 46CFR111.40-1

The scope of the periodic inspection is the same as that for the inspection for certification, as specified in § 91.25-10. The Officer in Charge, Marine Inspection will insure that the vessel is in satisfactory condition and fit for the service for which it is intended. At time of inspection vessel was unable to put the life boat in the water for a MOB drill. Vessel shall conduct a MOB drill and provide documentation to USCG. 46CFR91.27(b)(2)

It shall be the duty of the master and chief engineer to see that the vessel, and, in particular, the quarters are in a clean and sanitary condition. The chief engineer shall be responsible only for the sanitary condition of the engineering department. Two of the heating tables and some of the freezers in the galley were inoperable or malfunctioning. Conduct repair work as necessary to bring this equipment back to working conditions. 46CFR97.15-10(a)

Each battery-operated, relay-controlled lantern used in accordance with Table 112.05-5(a) must: Have rechargeable batteries; Have an automatic battery charger that maintains the battery in a fully charged condition; and not be readily portable. E-light fixture in the anchor windlass room has a broken lens and did not turn on when tested. Fix lantern as required. 46CFR112.39-1

Each muster station and embarkation station must be adequately illuminated by lighting with power supplied from the vessel's emergency source of electrical power. At time of inspection the lifeboat embarkation station lighting was inoperable. Vessel replaced fixture with a new one. 46CFR199.110(c)

The integrity of any deck in way of a stairway opening, other than a stairtower, shall be maintained by means of “A” or “B” class bulkheads and doors at one level. The integrity of a stairtower shall be maintained by “A” Class doors at every level. The doors shall be of self-closing type. Door from C deck to the bridge does not fully close. Adjust/clean as required to make door self closing. 46CFR92.07-10(d)(4)

At each inspection for certification and periodic inspection, the inspector shall examine the tank tops and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. Vessel had several 55 gal drums of oil stored in the E/R that needed to be moved to designated areas or off the vessel. Crew removed some of the drums and relocated the others. 46CFR91.25-45(a)

It is mandatory that the design, fabrication and erection of piping constructed under the regulations in this subchapter demonstrate leak tightness. Diesel fuel lines in the starboard side generator space had fuel-soaked rags tied around pipe connections. Vessel crew removed rags, cleaned area and tightened leaking fittings. 46CFR56.97(b)

All piping, valves, and fittings shall meet the applicable requirements of Subchapter F(Marine Engineering) of this chapter. The respective piping systems installed on ships and barges shall have the necessary pumps, valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. While conducting the 2nd annual COI, MIs discovered fractures in the fire main system located in the the starboard walking tunnel, a doubler on the fire main in the port walking tunnel and fractures/holes in the connecting piping systems coming directly off of the emergency fire pump (deck wash down system). The lack of preparation or knowledge of these conditions prior to USCG attendance shows a lack of proactive management from the company. All identified deficiencies require permanent repairs prior to receiving credit for annual inspection. 46CFR95.10-15 46CFR56.01-1(b)

The installation of all lifesaving appliances and arrangements must be in accordance with Subchapter W (Lifesaving Appliances & Arrangements) of this chapter. Each launching station, or each two adjacent launching stations, must have an embarkation ladder as follows: (1) each embarkation ladder must be approved under approval series 160.117 or be a rope ladder approved under approval series 160.017. (2) Each embarkation ladder must extend in a single length from the deck to the waterline with the vessel in its lightest seagoing condition under favorable conditions of trim adn with the vessel listed not less that 15 degrees either way. While conducting the 2nd annual COI inspection, MIs found the embarkation ladder for the lifeboat to be unservicable and too short for the installed location. The lack of preparedness or knowledge of these conditions prior to USCG attendance shows a lack of proactive management from the company. An approved embarkation ladder shall be provided and installed prior to receiving credit for the annual COI inspection. 46CFR96.06-1 46CFR199.110(f)

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries and storerooms, other than small service lockers, shall be of “A” Class construction. The deck below the main distribution panel in the engine control room and several multi cable transits are missing structural fire protection. Vessel shall establish a plan to have structural fire protection installed in these spaces and submit that plan to USCG for approval. 46CFR92.07-10(c)

Each nonrising-stem valve, lever-operated valve, or other valve where, because of design, the position of the disc or closure mechanism is not obvious must be fitted with an indicator to show whether the valve is not obvious must be fitted with an indicator to show whether the valve is opened or closed. The reach rods for the fuel closure valves on deck did not have indicators of valve condition or system. Repair the open/close indicators and put valve identification on each. 46CFR56.20-9(a)

Closures for hull openings at or below the freeboard deck must be as strong as the structure to which they are attached and must be watertight. Port, aft port hole glass is cracked. Repair the port hole to maintain original integrity. 46CFR45.135

Periodic safety tests (PSTP) must be a step-by-step/check off list format and approved by the Coast Guard. Vessel has new equipment installed and it is not reflected in the PSTP. Review entire PSTP, make changes, and re-submit to the Marine Safety Center for approval. *705, allows a year to go through all procedures, submit to MSC and then conduct shipboard testing with USCG MI. 46CFR61.40-1

In reference to electrical engineering, repairs and replacements in kind must comply with subchapter J. Cable for lifeboat davit motor controller was damaged. Yard replaced cable and added junction boxes. 46CFR110.01-3(a)

Each vessel must have suitable pilot boarding equipment available for use on each side of the vessel. If a vessel has only one set of equipment, the equipment must be capable of being easily transferred to and rigged for use on either side of the vessel. Port side boarding ladder has damage in the railing, foot connections and steps. Vessel shall repair ladder or replace as required. 46CFR96.40-1(b)

At each inspection for certification and periodic inspection the marine inspector shall conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as he feels necessary to check safe operation. Port main diesel engine had an overhaul over winter and needed to go through a break-in period prior to testing shutdowns (specifically overspeed). Conduct testing when OEM standards allow and report results to Class and USCG. 46CFR61.20-3(a)

While conducting the internal structural exam (ISE) on the AMERICAN SPIRIT, MI's were not able to see structural connections due to the excessive amounts of mud and ice. The inspection standards established by American Bureau of Shipping (ABS) for hulls shall be accepted as standard for the Coast Guard Marine Inspector. ABS Rules Part 7.3.3 state all spaces are to be clean and free from water, scale, dirt, etc., to reveal corrosion, deformation, fractures, damages or other structural deterioration. Vessel must provide a written plan to MSD Sturgeon Bay addressing how completion of the required ISE will be attained. 46 CFR 91.15-1

Starboard rudder system is operating slower than normal. Vessel shall conduct repairs and provide inspection/testing to the satisfaction of the OCMI.

Vessel was unable to get lifeboat underway during the inspection due to its location at the dock. Vessel is required to lower the lifeboat and have the crew maneuver it to meet the requirements of 46CFR 199.80 (d).

Complete Fire, MOB and Abandon Ship drills to the satisfaction of an attending Coast Guard Marine Inspector. MOB drill shall incorporate the launching, operation and recovery of the rescue boat.

Prove proper operation of port and starboard propulsion engine safeties and shutdowns. Engineering department watches shall be manned with an Assistan Engineer, QMED and Wiper until full operation of engine automation is resotred. This authorization is valid for the voyage from Silver Bay, MN to Cleveland, OH on or about 3/27/2020 only.

Before the vessel leaves port and at all times through out the voyage, each life-saving appliance must be in working order and ready for immediate use. The lifeboat failed to immediately start during the lifeboat drill. The hydraulic pressure was depleted on a start attempt and pressure was unable to be immediately restored using the manual hydraulic pump.

Each life-saving appliance, including lifeboat equipment, must be inspected monthly using he checklist required under paragraph (b)(1)(i) of this section to make sure the appliance and equipment are complete and in good working order. The hydraulic starting system for the lifeboat is not being maintained and operated as prescribed in the manufacturer's operation and maintenance manual. The hydraulic system is unable to develop the necessary pressure to maintain the minimum prescribed pressure.

Complete hull plate renewal of inset plating on bow during winter lay up 2019

Significant damages to the port side of the stem bar x 2 frame bays horizontally, and to the starboard side of stem by 1 frame bay, extending approximately 20' vertically from the lower forepeak deck to just below the deck at the turn. There are a multitude of frames damaged IWO the inset. Make permanent repairs prior to commercial operations.

Vessel ran hard aground in the vicinity of the entrance to the Minnesota Slip in Duluth-Superior Harbor creating a risk to safe navigation and a potential threat to the vessel, crew, and/or marine environment, due to the grounding.

Properly secure or remove all dead in wiring found in the following locations: 1) elevator shaft/top of elevator 2) Port side deck hatch crane 3) Port side fan room, upper deck lighting fixture.

Vessel's #4 service generator was inoperable at the time of inspection and required repairs; testing of the generator shutdowns was not possible. Upon proper repair of the #4 service generator provide full serving report to include satisfactory shutdown testing; test generator in presence and to the satisfaction of CG Marine Inspector if practicable.

Testing of vessel's safety valves on both boilers showed blow-down (reset) of the valves occurred outside of acceptable parameters, by an excessive percentage of what the lift pressure was. Safety valves for both boilers shall be adjusted or replaced to ensure blow-down is between 2% and 4% (or 4 psi) of lift pressure.

Complete repairs to the leaking Stbd MDE #16 Cylinder Fuel Oil Supply Line.

Make satisfactory repair or replacement of faulty port engine jacketwater high temperature shutdown switch.

Make permenant repairs and prove proper operation of the main fire pump.

It appears that the local alternate control does not have an EOT indicator. Demonstrate to the satisfaction of the local OCMI, operation of sound powered phones between the pilothouse and the manual alternate control.

Prove proper operation of the bridge and emergency steering sound powered phones or obtain an alternative means approval from CG MSC IAW 46 CFR 58.25-15.
