No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Isolate leaking hydraulic fluid cooler on the conveyor belt tensioning motor to prevent contamination of water in the aft peak tank and ensure aft peak tank is free of oil prior to using aft peak tank for cooling water.

Vessel Captain reported that a crack was discovered in the starboard main engine (SME) block, rendering it inoperable. The extent of the damage is unknown at this time pending further investigation. Vessel is authorized to operate under reduced propulsion with the ship's port main engine (PME) until 07 JUL 2022 IAW USCG D9's "GUIDANCE FOR VESSELS OPERATING WITH REDUCED PROPULSION". Vessel is to make permanent repairs to the SME, and demonstrate proper operation. Repairs must be satisfactorily completed prior to resuming cargo operations (at the end of this 30-day period allowing operation under reduced propulsion). Vessel owner/operator to keep MSU Duluth notified of any changes in the vessel's operational status, and/or maneuvering characteristics. Use of assist tugs in restricted waters, as outlined by the company rep., has been approved by all applicable OCMI zones. Examination of repairs shall be verified by either a Class Surveyor or USCG Marine Inspector.

The material, design, construction, workmanship and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by ABS Steel Vessel Rules. Vessel experienced equipment failure rendering the starboard main diesel engine inoperative. Vessel is to make permanent repairs to the starboard main diesel engine, in accordance with ABS Steel Vessel Rules. Permanent repairs to be completed to the satisfaction of either Class or Coast Guard Marine Inspectors.

Heat sensor above prime mover on #1 SSDG not operational. Manning levels on COI are contingent on proper operation of automated monitoring systems, including fire detection. Make permanent repairs to designated heat sensor.

Port main engine has faulty overspeed sensor causing the engine to shut down. Replace sensor and test engine/sensor to the satisfaction of Coast Guard Marine Inspector.

Yielding rod on Port main engine was sticking causeing the rpms to hunt and not settle on the given input. CE was able to fabricating replacement part within 2 hours and make permanent repair.

Make repairs and demonstrate proper operation of ship's #3 service generator.

Voltage regulator for newly installed SSG (#1) is not operating properly, resulting in an inablility for the #1 SSG to be paralleled with the other 3 operational SSGs. Troubleshoot cause for voltage regulation issue and make repairs upon arrival in Port of Duluth, MN.
