No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Isolate leaking hydraulic fluid cooler on the conveyor belt tensioning motor to prevent contamination of water in the aft peak tank and ensure aft peak tank is free of oil prior to using aft peak tank for cooling water.

Vessel Captain reported that a crack was discovered in the starboard main engine (SME) block, rendering it inoperable. The extent of the damage is unknown at this time pending further investigation. Vessel is authorized to operate under reduced propulsion with the ship's port main engine (PME) until 07 JUL 2022 IAW USCG D9's "GUIDANCE FOR VESSELS OPERATING WITH REDUCED PROPULSION". Vessel is to make permanent repairs to the SME, and demonstrate proper operation. Repairs must be satisfactorily completed prior to resuming cargo operations (at the end of this 30-day period allowing operation under reduced propulsion). Vessel owner/operator to keep MSU Duluth notified of any changes in the vessel's operational status, and/or maneuvering characteristics. Use of assist tugs in restricted waters, as outlined by the company rep., has been approved by all applicable OCMI zones. Examination of repairs shall be verified by either a Class Surveyor or USCG Marine Inspector.

The material, design, construction, workmanship and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by ABS Steel Vessel Rules. Vessel experienced equipment failure rendering the starboard main diesel engine inoperative. Vessel is to make permanent repairs to the starboard main diesel engine, in accordance with ABS Steel Vessel Rules. Permanent repairs to be completed to the satisfaction of either Class or Coast Guard Marine Inspectors.

Heat sensor above prime mover on #1 SSDG not operational. Manning levels on COI are contingent on proper operation of automated monitoring systems, including fire detection. Make permanent repairs to designated heat sensor.

Port main engine has faulty overspeed sensor causing the engine to shut down. Replace sensor and test engine/sensor to the satisfaction of Coast Guard Marine Inspector.

Yielding rod on Port main engine was sticking causeing the rpms to hunt and not settle on the given input. CE was able to fabricating replacement part within 2 hours and make permanent repair.

Make repairs and demonstrate proper operation of ship's #3 service generator.

Voltage regulator for newly installed SSG (#1) is not operating properly, resulting in an inablility for the #1 SSG to be paralleled with the other 3 operational SSGs. Troubleshoot cause for voltage regulation issue and make repairs upon arrival in Port of Duluth, MN.

Submit plans for review and approval to add sufficient charging capacity and battery source for electronic controls of all ship service generators.

#2 Ship's service generator tripped while vsl was backing out of slip. Generator was brought back online and load tested but reason for failure could not be identified. Service technician will attend at next port of call.

Great Lakes Fleet, Inc. has elected to voluntarily deposit the Certificate of Inspection (COI) for the M/V EDGAR B. SPEER to Marine Safety Unit Duluth, and apply for temporary lay-up status due to low economic demand. The COI will be returned to Great Lakes Fleet, Inc. upon request without lapse with the caveat that all inspection requirements under SIP are satisfied prior to sailing.

During the course of the annual SIP Audit significant program element breakdowns were identified compelling remedial action by the coast guard in way of requiring a follow up audit to ensure system failures are identified and corrected. Make vessel available to the SIP agent for a follow on audit prior to the end of the 2008 shiping season.

Fire control plan requires revision to reflect location of response gear.

Port #4 Ballast tank had small & 7" crack 8ft aft of forward bulkhead at second stiffener.

Install blanks on exposed connections on electrical panel L-9. Submit SIP Correction report to [name withheld].

Life boat engine, failed to start during routine test. Fuel lift pump or injector pump failed.

#6 ballast tank Starboard side aft, 12"-18" crack at welded seam. Forward of starboard main engine

Make repairs to fractured gussets in the Port #3 void, frame 7, 8, 9, port void #5 frames 3 and 4 and stbd void #5 frame 5, 6, and 13. Cracks are at the base of the Gussets between the cargo slope and the ballast tank bulkheads.

Conduct permanent repairs to #1 web frame, in # 6 stbd ballast tank,between side longs #2-6 & web frame #2 IVO sidelong #2. Repairs to consist of inserting affected areas, temporarily gouged & re-welded. Permanent repairs to be completed to the satisfaction of the attending C.G. Marine Inspector.

Conduct permanent repairs to the 12th deck beam (from aft) inboard, in the #3 stbd ballast tank. Repairs to consist of inserting affected area. Temp repairs authorized to consist of drill stop, gouge and re-weld. Permanent repairs to be completed to the satisfaction of the attending C.G. MI

Starboard side, inboard group 1 pump, had a low pressure switch failure. Hydraulic fluid did leak from the switch, contained in containment.
