An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs or alterations are undertaken. Vessel's Flex Camshaft Technology (FCT) box cover has sheared bolts that are accounted for and may have fallen inside the camshaft assembly. Vessel shall account for all missing bolt pieces and enact permanent repaits to the FCT box cover once all the pieces have been found to the satisfaction of the local OCMI prior to departure to Saginaw, MI. 46 CFR 91.45-5

The company shall provide the designated SIP support personnel w/training as required by the CAP. The CAP/VAP requires the Master enroll crew based on job description and enters quals into NS when complete. The SIP Pre-Training Information Form has only been completed for the Master and [name withheld] enrollement & enter form ISS-OPS-PR-011 for entire crew.

The vessel must comply with the Sulfur Oxide (SOx) emission limits specified in Regulation 14.4.3 of MARPOL Annex VI, as required under 33 USC 1903 (b)(2) by the required date (MARCH 2027). All conversion or modification work necessary to achieve compliance shall be conducted under the oversight of the Officer in Charge, Marine Inspection (OCMI), with plan review performed as appropriate.

When the main steering gear includes two or more identical power units, no auxiliary steering gear need be fitted, if- in a cargo vessel, the main steering gear is capable of moving the rudder as required by paragraph (b)(2). Vessel reported starboard main steering gear system unable to be started. Vessel authorized only to proceed from Bay City, MI to Stoneport, MI under the following conditions: 1) Vessel not to exceed 4 knots while on Saginaw River. 2) Steering gear space shall be manned while on Saginaw River. 3) Vessel shall not enter any other river systems or restricted waterways. 46 CFR 58.25-10(e(2) 46 CFR 58.25-10(b)2)

The port anchor chain had a detachable link separate from the chain. Affect repairs or replace the link to the satisfaction of ABS before the end of winter layup. 46 CFR 96.07-5(a) / ac / 705

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Dryer machine in laundry room adjacent to steering gear room has a bucket that is collecting all the lint and it is not properly exhausted as recommended by the manufacturer's installation manual. Dryer equipment shall be not used until properly exhausted and lint bucket is removed to the satisfaction of a CG marine inspector. 46 CFR 92.05-1

Where cables pass through fire boundaries, arrangements should be made to ensure the integrity of the bulkhead is not impaired. Cable penetration to conveyorman shop has an IT cable that runs through a cable gland which compromises the fire boundary. Enact permanent repairs prior 30NOV24 to the satisfaction of CG marine inspector 46 CFR 111.60-5(a)(1) Ref. IEEE 45-2002 Sect. 25.7

Combustible material should be avoided in the construction of electrical equipment. Deck cable penetration to main switchboard bus used do not appear to be flame resistant or retardant and appears to be of combustible nature. Conduct permanent repairs prior 30NOV2024 to the satisfaction of CG marine inspector. 46 CFR 111.01(b)

Existing structure arrangements and materials previously approved will be considered satisfactory so long as they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspection. Door to laundry room adjacent to steering gear room is missing. Installed original fire door required in the space prior 30NOV2024 to the satisfaction of a CG marine inspector. 46 CFR 92.07-90

These cords and cables should be used only for connection of portable lamps and appliances and other equipment not suitable for fixed wiring and should conform to the requirements of NFPA 70-2002, Article 400, for hard usage and extra hard usage. Washer and dryer equipment located in laundry room is using flexible cords to provide power to the equipment. Vessel shall have a permanent electrical installation to the satisfaction of a CG marine inspector prior to 30NOV2024. 46 CFR 111.60-13 Ref. IEEE 45-2002 Sect. 24.6.1

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel reported loss in fuel pressure to both main diesel engines reducing propulsion and maneuverability. Vessel is required to diagnose and effect necessary repair of associated components to ensure completeness of its machinery equipment. 46 CFR 91.30-01 - 705 ac

13101 - Auxiliary machinery vital to the main propulsion system must be provided un duplicate unless the system served is provided in independent duplicate, or otherwise provides continued or restored propulsion capability in the event of a failure or malfunction of any single auxiliary component. Port MDE shaft generator is currently inoperable and the vessel is running on starboard MDE alone. Vessel must make repairs. Vessel is permitted to transit on single engine until repairs are made. Vessel is required to operate thruster generators and maintain an anchor watch while transiting rive systems with limited maneuverability. 46 CFR 58.01-35, 15C

Periodic safety tests must be conducted at intervals specified by the USCG to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Company employee designated to conduct PSTP's on the generator does not follow the approved PSTP's. PSTP's must be utilized. Either follow or obtain approved amendment for PSTP's to reflect actual conditions for testing. Specifically, procedure B9-B and B9-C for the generator are not being followed. Submit CAR within specified time period. 46 CFR 61.40-6

Follow or obtain approved amendment to PSTP for Clayton boiler auto temp switch which reflects an operating temperature of 380, when actual conditions on board are 440 degrees. Submit CAR within specified time period. 46 CFR 61.40-6

Vessel does not have approved PSTP for the emergency generator. 46 CFR 61.40-6

nonmetallic flexible hose may be used in fuel oil systems only where flexibility is required and in lengths not exceeding 30". Fuel supply hose in emergency generator room exceeds 30". Submit a CAR within the specified time frame. 46 CFR 56.60-25(b)(4)

Vsl operating on Port Main Propulsion Engine, due to broken and unavailable parts with STBD fuel delivery Pumps. Vsl to make repairs to STBD fuel pumps as soon as practicable after overnight delivery on 05JUL22.

As far as practicable, rescue boats must be launched with assigned crew and maneuvered each month, at least once every 3 months. Provide proof of completed drill.

New air conditioning unit was installed but does not have approved drawings from MSC. Unit is to be locked out/tagged out until approvals are properly obtained and verified by an attending Marine Inspector.

Sound powered phone at the emergency steering stations was found inoperable. Vessel to repair/replace and prove proper communication between emergency steering station and pilot house.

Rotating red light between main engines in lower engine room was found to not be rotating. There must be a red-flashing or rotating beacon where the emergency alarm can not be heard over background noise. Vessel to repair/replace as necessary to meet requirements.

Wire/cabling was found hanging in the forward end of #4 cargo hold. Vessel to secure the wire to prevent potential damage per IEEE Std.

Electrical equipment in locations requiring exceptional degrees of protection as defined in 46 CFR11.15-1 must be enclosed to meet at least the minimum degrees of protection in ABS Steel Vessel Rules, section 4-8-3, Table 2, or appropriate NEMA 250 type for the service intended. Newly installed distribution boxes for the AC unit near the wheelhouse do not appear to meet this standard, IP56.

Vessel reported fracture in shell plate port side engine room at frame 206-207 at the 28’09” level. Temporary repairs were accepted (drill stop ends, Vee and weld, tested for watertight integrity). Make permanent repairs to affected plate and associated structural members to the satisfaction of attending USCG Marine Inspector and/or ABS surveyor.

Vessel (while underway on Lake Superior) reported a leak in the fire-main located on the aft exterior bulkhead of the lower fantail, between the spar deck and boat deck, on vertical piping run supplying fire-station #29. Leak is due to a 1.5" crack caused by ice expansion. Repair fire-main as original to the satisfaction of the USCG prior to departure from the port of Cleveland, OH. Note: Vessel Captain reports they have applied a soft-patch to the leak that has satisfactorily held at 100 p.s.i.

During planned drydocking 2 small cracks (2-3" length) were discovered adjacent to each other in the #4P B/T bottom shell-plate approx. 9' and 12' respectively from the centerline keelson. Crop and renew damaged area as original and demonstrate watertight integrity via NDT.

Received notification that #2 cylinder on STBD M/E had a jacket-water leak requiring engine shutdown and subsequent repair. Make repairs to restore full functionality and notify MSU Duluth upon completion of repairs/satisfactory tests.

The vessel's stbd SSDG has suffered a mechanical failure due to a loosened machinery guard getting caught in the stator and subsequently damaging the windings. Repair stbd SSDG to restore full functionality. Upon completion of repairs and satisfactory testing provide service technician's report to MSU Duluth.

A 10-12 foot fracture is present under the rubrail along the #6 ballast tank due to damage from ice. Effect permanent repairs to hull plating prior to departure from next port of call.

02114- A bulkhead fracture was reported in hold #2, port ballast tank #3. Vessel is authorized to transit to Sandusky, OH. Vessel shall effect repairs prior to departure from Sandusky, OH to the satisfaction of the attending CG Marine Inspector, or RO. Master shall immediately notify the cognizant USCG OCMI if any conditions change. 46 CFR 170.005, 46 CFR 91.30-1

Vessel shall lower/launch and maneuver port side lifeboat IAW MSIB 04-18, to the satisfaction of the attending MI.

The diesel bunker tank has a 4 inch long fracture located at approximately tank mid level; weeping fuel into the bilge. The vessel is authorized to transit to Detroit for repairs if the following conditions are met: 1) reduce fuel level to at least 1 foot below tank fracture, 2) wave height of less than 5 feet for route, 3) daylight transit only, 4) 24 hour engine room watch with rounds.

Effect permanent repairs to portside forepeak hull plating fracture in vicinity of 25 foot mark. Repairs shall be completed prior to departure from NPOC.

Launch and exercise Port Lifeboat.

Complete repairs to #1 through 3 cargo holds as per USCG/ABS worklist issued 22 March 2018.

Make permanent repairs to wasted deck plating directly under aft anchor windlass prior to the end of winter 17/18 lay up period. Temporary repairs have been verified/approved by ABS/CG.

Main Fire detection pannel (Honeywell) fails to relay fire zone details to MAST automation pannels in the ECR, Bridge, Galley, Engineers' staterooms. Prove proper operation to the satifaction of the attending Marine Inspector.

Periodic Safety Test Procedures (PSTPs) have not bee finalized/examined without comments by Marine Safety Center (MSC). Upon MSC approval, provide MSU Duluth a copy of PSTPs and make vessel available for complete review/testing.

Emergency fuel shutoffs (group 1 of 4 only), remote operation failed to secure diesel oil day tank, diesel oil bunker tank and the heavy fuel oil overflow tank. Prove proper operation to the attending MI.

Forward archway bracket on starboard side within the #3 cargo hold is fractured at the cargo longitudinal bulkhead connection. Make permanent repairs by v+w.

Three seperate areas of 8-12" fracturing within the #3 cargo hold at approximate fr 115, 120, and 124 where the longitudinal bulkhead meets the slope. Areas have been temporarily repaired by clad weld on the cargo side. Permanent repairs are to be effected at next layup period 2017-18.

Make permanent repairs to a fracture within the #1 cargo hold at frame 48 on the port side at the long to slope connection. Temp repairs have been effected from the cargo hold side. Permanent repairs need to be effected at next layup in 2017-18.
