The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel reported a pinhole leak on the no. 5 port ballast tank in way of the cargo hold. Vessel has been granted to transit to Calcite, MI, where it shall conduct temporary repairs to the satisfaction of the local [name withheld], vessel has been authorized to enact permanent repair during winter layup 2027 (no later than 01MAR2027). 46 CFR 91.30-1

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. However, in special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. Vessel has 4 fractures on the 1P/2p ballast water tanks corrugated bulkhead. One is close to the 1st inboard bottom girder, another one is on the most outboard bottom longitudinal by the bilge knuckle, and two are located up where the horizontal shelf meets the first outboard plane of the corrugated bulkhead. Vessel is authorized to conduct temporary repairs to the satisfaction of the attending marine inspector. Permanent repairs shall be completed during winter layup 2027 to the satisfaction of the local OCMI.

Alterations and repairs to inspected vessels must be done—(1) Under the direction of the Officer in Charge, Marine Inspection; and(2) Except as provided in paragraph (b) of this section, in accordance with the regulations in this subchapter, to the extent practicable. Vessel reported an ingress of water into the aft peak tank originating from a crack located on the side shell. Vessel also identified a leaking pipe within the tank associated with the "rudder stock void drain". Vessel shall identify exact location and extent of both material failures, and submit a repair proposal to the satisfaction of Class and Coast Guard. Satisfactory repairs shall be made prior to the vessel's departure from the Port of Duluth/Superior to the satisfaction of Class and Coast Guard.

There are multiple non-working bulbs (and/or bulb-sockets) on the PME and SME local control-displays, resulting in a lack of local visual display for critical alarm/shutdown conditions. Demonstrate proper operation of alarm/shutdown condition bulb indicators on both local control displays.

Vessel's PSTPs (#7 and #23) indicate that a high jacket-water temperature condition results in shutdown of the main engines. Shutdown for this conditon is not required as per the applicable ABS Steel Vessel Rules. Furthermore, onboard tests proved that the engines do not shutdown for high jacket-water temperature condition; they alarm only (as required). Submit PSTP revision request to USCG Marine Safety Center to remove this automation outcome from these 2 procedures.

Vessel crop and renew the wasted metal found on the pipe section and enact permanent repairs to the satisfaction of the local OCMI. 46 CFR 95.10-15

essels must be equipped with independently driven fire pumps in accordance with Table 95.10-5(a). Vessel's emergency fire pump shaft located in the aft section of the tunnel snapped while unloading iron ore. Vessel shall enact permanent repairs to the satisfaction of the local OCMI. 46 CFR 95.10-5

While underway on Lake St. Clair the vessel experienced a loss of propulsion, the starboard main engine had a high pressure alarm and shut down. The vessel was not able to clutch onto the port main engine to make way and anchored as a result. Vessel was dead ship towed to Waterfront Petroleum Detroit - Mistersky to evaluate extent of damage/repair. Vessel shall diagnose necessary repairs and make permanent repairs to starboard main engine and clutch prior to movement.

While under way in Lake Huron, the vessel crew discovered a leak in the starboard side fire main, on deck. The vessel shall make repairs to the satisfaction of ABS or the Coast Guard, and preform an operational test.

Vessel's crew found a pinhole leak on the starboard fire main piping deck aft by cargo hold 6. Vessel shall enact permanent repairs to the satisfaction of the local OCMI. 46CFR95.10-15

In the inspection of hulls, boilers, and machinery of vessels, the standards established by the American Bureau of Shipping shall be accepted as standard by the inspectors. Vessel identified a pinhole leak in the fire main deck piping around midship while offloading cargo at CSX dock in Toledo, OH. Vessel is to conduct repairs to the satisfaction of the local OCMI. 46CFR91.15-1

The minimum number of ring life buoys with water lights attached shall be in accordance with Table 94.43-10(a); numerous automatic water lights were inoperable. 46 CFR 94.43-109(a)

The "Test Procedure" block for PSTP #68 on Pg. 31 (Steering Gear No. 2 Breaker - Open) is annotated incorrectly (wrong breaker on main switchboard is listed). Make correction and resubmit PSTP for approval.

The "Oil in Bilge Water Overboard" alarm located at the ECR main console is not working (mechanical components of system are confirmed working during fitout testing however). Make alarm functional.

The pressure gauge for the emergency fire pump is not functioning. Repair and/or replace, and demonstrate proper functioning.

Records of daily hours of rest must be maintained onboard the vessel; work rest logs did not reflect crew working during recorded drill times. Ensure drill hours are recorded as work hours; to be cleared at next topside.

Each panelboard overcurrent protection device must have a description of the served load of the circtuit (i.e., the rated load) listed on the circuit directory; no panelboard had the rated load listed. 46 CFR 111.40-1(h) (Old "J") 46 CFR 111.30-1(h) (Old "J")

An inspection shall be made whenever any important repairs or alterations are undertaken. Five 1-2" cracks were found in port ballast tanks. One on the bulkhead between #1 port ballast and the cargo tunnel, two in the bulkhead between #1 & #2 port ballast, and two between the #2 and #3 port ballast. Temporary repairs were completed to the satisfaction of class and [name withheld] repairs to be conducted during winter layup 2025. 46 CFR 91.45-5

Each generator must meet the applicable requirements for construction and testing in section 4-8-3 of the ABS Marine Vessel Rules (incorporated by reference; see 110.10-1 of this sub chapter). Vessel has self reported an issue with paralleling the two main service generators. Parts have been ordered and should arrive within a week.46 CFR 111.12-5

The construction of ballast systems shall be in accordance with the requirements of subchapter F (Marine Engineering). Vessel notified Sector Detroit OCMI of pinhole in bulkhead between starboard #1 ballast tank and tunnel, causing a small leak. Vessel shall affect permanent repairs at port of destination in Calcite, MI. 46 CFR 96.03-1

The Engine Order Telegraph system is giving a continuous alarm when any ahead order was given, regardless of if the bridge and the engine room show the same order. 46CFR113.35-5(c)

Tests and inspections of the lifesaving equipment shall be carried out during each inspection for renewal of certification and periodic inspection, and shall include, The proper condition and operation of the survival craft and rescue boat launching appliances at loads ranging from light load to full load. The vessel was not able to conduct the light load and full load tests today. 46CFR199.45(b)(1)

Each davit, winch, fall, and other launching appliance must be thoroughly inspected and repaired, as needed, once each year. Vessel had no record of servicing the davit for the rescue boat. 46CFR199.190(f)(2)

Vessel experienced a failure in its stbd engine. #8 Cylinder's Indicator Cock in the stbd bank blew out. Vessel may proceed to Calcite, MI to effect repairs. Transit within Detroit River and Sector Detroit COTP Zone is permitted under the following conditions: 1. Installed generators fully functional 2. functionla bow thruster 3. anchor watch provided through transit of restricted maneuverability/piloted zone. Prior to departure, vessel shall contact USCG COTP in Sault Ste Marie prior to departing Calcite, MI.

Provide Class Report to USCG Sector Detroit.

Vessel shall make repairs to to satisfaction of Class or USCG.

Crop and renew shell plate in forepeak in the starboard side at the turn, an area marked off by attending Surveyor and Marine Inspector. Repairs must be completed to the satisfaction of CG and/or Class prior to departure.

FIRE MAIN PIPING ON THE STARBOARD SIDE IS LEAKING. ALL PIPING SHALL MEET THE APPLICABLE REQUIREMENTS OF SUBCHAPTER F. MAKE PERMANENT REPAIRS AND PRESSURE TEST PIPING PRIOR TO DEPARTING BUFFALO, NY.

Vessel has extensive damage throughout its port ballast tanks #1, #2, #3, and #4. Vessel is permitted to proceed to dry dock at Fraser Shipyard, Inc.-Superior, WI. Permanent repairs shall be completed to the satisfaction of attending Coast Guard Marine Inspector or ABS Surveyor prior to departure from Fraser Shipyard in Superior, WI.

A crack was found on the bottom plate in Port Ballast Tank #4. IAW NVIC 7-68, the ends of the crack were "drill stopped", then the area was sealed with a cement box patch. The vessel must crop and renew the bottom plate, with an appropriately sized insert, at the next scheduled dry-dock (27MAR20).

Hull shall remain watertight below the deck line. Damage discovered at the forward anchor box, leading to water ingress in the space.

On 27MAR22 the JOHN J. BOLAND experienced a fire involving an oil fired furnace in the vessel's engine room. This failure presented a serious threat to the vessel and crew's safety. The cause of failure has not been determined, and the crew has reportedly made repairs to the furnace using spare parts onboard, which may not be in accordance with the manufacturer recommendations. Due to the unknown nature of the failure and the unverified repairs, the furnace must be immediately taken out of service and be locked and tagged out to prevent further use. The furnace may not be placed back into service until it is evaluated, serviced and properly repaired by personnel specifically trained in the installation and maintenance of HVAC equipment. Once properly serviced, a complete report from the servicing company shall be provided to the [name withheld] to place the furnace back into service must be granted by a qualified Coast Guard Marine Inspector after the installation and reports have been evaluated.

While conducting the general walk through of the engine room, MI discovered wires and cables above the port MDE shaft generator that were damaged; Chief Engineer was not aware at the time but believed it happened over the winter lay-up while maintenance was being conducted on the port generator. Wires and cables shall be installed so that the crew is protected from electrical hazards. Fix the damaged wires/cables and remove the dead-end wires in the same vicinity. 46CFR111.01-1(a)(2)

The PSTPs used for annual testing of automation are not accurately reflecting machinery controls, procedures or results that occur with the engine room. Of particular note is the "TL Services Propeller Pitch and Engine Load Control System". PSTPs shall be submitted to and reviewed by Coast Guard (Marine Safety Center) to verify arrangement, controls and procedures. Once Chief Engineer reviews the PSTPs and "pen & inks" required changes, submit the edited changes to MSC for review and "Ready for Shipboard Testing" letter. Further review and testing will be conducted by attending MI to complete PSTP regulatory requirement. *ongoing project from previous annual inspection* 46CFR62.20-1(a)(7)

The fire doors in the stair tower for the accommodation decks and stairway to the pilot house were not completely closing on their own. The integrity of a stairway shall be maintained by "A or B" class doors and stair tower integrity shall be maintained by "A" class doors at every level. Doors shall be of self-closing type. Fix or replace the self closing apparatus of each door identified. 46CFR92.07-10(d)(4)

While conducting the Year 2 Continuous Internal Structural Exam (CISE) on the JOHN J. BOLAND, MI's were not able to see structural connections due to the excessive amounts of mud and ice. The inspection standards established by American Bureau of Shipping (ABS) for hulls shall be accepted as standard for the Coast Guard Marine Inspector. ABS Rules Part 7.3.3 state all spaces are to be clean and free from water, scale, dirt, etc., to reveal corrosion, deformation, fractures, damages or other structural deterioration. Vessel must provide a written plan to MSD Sturgeon Bay addressing how completion of Year 2 and all remaining years of the CISE will be attained. 46 CFR 91.15-1

The arrangement of main propulsion machinery must be at least equivalent to the standards established by the ABS Steel Vessel Rules. Vessel reported a loss of variable pitch for the propeller. They did not report a loss of engines, just loss of pitch. Vessel was unable to duplicate the problem during trouble shooting. Captain was advised to have additional personnel in the Engine room, and ready to anchor for a safe transit should the problem happen again while underway. Service technician to attend. Repairs are to be made to the satisfaction of attending Marine Inspector or to the attending Class Surveyor.

Vessels piping shall be in accordance with Sub chapter F Marine Engineering. Vessels fire main has a leak. Effect permanent repairs and test to the saftisfaction of the local OCMI.

Vessel has a pin holeleak in the fire main, port side main deck. Temporary repairs have been made with a soft patch.

Small 5" crack noted on side-shell aft of frame 74 and just before frame 75 below first stringer in #7 STBD Ballast tank. Temporary Concrete patch fitted. Must be inspected annually during winter lay-up and permanent repairs must me made by next scheduled dry dock.

Suitable hand covers, guards, or rails shall be installed in way of all exposed and dangerous places. Damaged/warped flooring on B deck, STBD side poses a new tripping hazard;provide handrails until deck is repaired 46 CFR 92.25-15

Accommodations for officers and crew on all vessels must be securely constructed and deck coverings within accommodations must be of an approved type. Flooring on B deck, stbd side was bubbling up and caused a tripping hazard. Remove affected flooring section and provide repair proposal for the area. *705 due within 60 days 46 CFR 92.07-10

All piping, valves, and fittings shall meet the applicable requirements of subchapter F. Discovered two leaking expansion joints on port side fire main; adjust couplings to eliminate the leak. 46 CFR 95.10-5

Provide documentation of launching, release, operation and retrieval of rescue boat. 46 CFR 199.160

All systems of an electrical nature shall be installed in accordance with subchapter J. The construction of each lighting fixture for a non-hazardous location must meet UL 1598A or IEC 60092-306 Three light fixtures were found in substandard condition: (1) fwd anchor windlass room, (2) dunnage room, and (3) port side deck flood light. Repair or replace light fixtures as required. 46 CFR 96.05-1 46 CFR 111.75-20

Periodic safety tests (PSTP) must be a step-by-step/check off list format and approved by the Coast Guard. Vessel has new equipment installed and it is not reflected in the PSTP. Review entire PSTP, make changes, and re-submit to the Marine Safety Center for approval. 46 CFR 61.40-10 46 CFR 50.20-5 *705 within one year

Each vessel much have a current magnetic compass deviation table or graph in the wheelhouse. Vessel's last deviation was done in 2016; swing compass to get current deviations. 33 CFR 164.35

Provide liferafts for 150% of the vessels crew on each side of the vessel IAW D9 approval letter dated 17 NOV 2008.

Vessel suffered momentary loss of starboard steering pump while underway. Pump became operational shortly thereafter, but engineers were unable to recreate the original fault. Provide a technician's report indicating that the pump is operational and provide a statement from the vessel's Captain certifying that the steering system is fit for continued service.

Feedback sensor pitch control failed while vessel was transiting into Detroit River causing a loss of propulsion power alarm on the bridge. Vessel operated astern propulsion to anchorage to change the sensor.

Starboard engine inoperable due to auxiliary raw water coupling failure. Vessel is authorized to proceed to Conneaut, Ohio under reduced propulsion utilizing the port engine. Vessel shall conduct repairs and provide inspection/testing to the satisfaction of the local OCMI prior to departing Conneaut, Ohio. Vessel must keep local OCMI informed of any changes in propulsion capabilities during transit.

Demonstrate proper operation and deployment of fast rescue boat.

Vessel was unable to lower the open Life Boat, as well as get the life boat underway during the Periodic COI inspection due to location at the dock. Vessel is required to lower the life boat into the water and have crew maneuver the life boat in the water in the next 30 days.

Sector Lake Michigan received an NRC report (#1230104) of a discharge of 01 gallon of oil from the heat exchanger while transiting to Waukegan, IL. The cause of the release was a faulty operation of the vessels heat exchanger, which was being used to melt ice on the fire lines. Make permanent repairs to heat exchanger/thermal fluid heater. Conduct pressure test and prove proper operation to the satisfaction of an attending CG Marine Inspector.

Complete permanent repairs to stbd MDE to the satisfaction of the cognizant OCMI.

Ensure proper control of both engines from vessel's bridge.

Vessel could not conduct rescue boat weight tests as required. Conduct light weight and full weight tests within 14 days.

Deviation table is not current. Provide an updated deviation table within 7 days.
