Vessel was damaged by fire on or around 01 FEB 2021. Premanent repairs are to be made to the areas of the vessel that was damaged by fire, water, smoke, of fire fighting efforts on or around 01 FEB 2021.

Ensure compliance with the CG approved North Shore Environmental Construction, Inc. de-watering plan dated 03 FEB 2021. Approval from USCG Sector Lake Michigan is required prior to deviation from the plan. [phone withheld]

On December 10, 2019, vessel reported a failure in the port steering pump while conducting pre-departure checks in Two Harbors, MN. Vessel is authorized to transit to Superior-Duluth for repairs, while operating in dual pump mode only. Tug Assist is required inside the harbor breakwall of Superior-Duluth. Vessel shall submit a tech report attesting to the cause, repair, and satisfactory operation of the steering system prior to departure from the port.

Rudder response is slow when operating on stbd steering pump. Properly repair stbd steering pump and conduct operational test. Provide a statement from the Chief Engineer or attending technician attesting to the stbd steering pump's repair and operational testing, to the satisfaction of the [name withheld].

Renew MCT penetration fire stop material with approved material under the fwd most 450 volt distribution panel in the ECR, the penetrations in the bulkhead separating emergency steering from the engine room, and three penetrations in the aft inboard bulkhead of the generator room.

Starboard 42 inch butterfly ballast water (BW) sea chest (SC) valve electric motorized valve control mechanism (MRV) is not operational. Make proper repairs to existing or renew the MRV. When complete, submit a statement from Chief Engineer or 3rd party technician, with supporting documents to the attending CG MI, reflecting the repairs, with statement reflecting MRV is fully functional and able to fully closure and open the 42inch BW valve, both manually and remotely, per 46 CFR 56.50-95(b)(2). Pending repairs, when conducting BW operations and valve is open, an additional crewmember shall be present within the engine room. COI reflects manning for minimally attending machinery plants, per 46 CFR 62.50-20-(e)(4) flooding safety; controls must be provided to operate sea inlet and discharge valves. Current condition, engineer on watch unable close the stbd 42 inch BW valve from within the engine control room.

Make permanent repairs to sideshell hull damage in way of the port forepeak, between frames #2 and #3 to the satisfaction of ABS and the U.S. Coast Guard Officer In Charge, Marine Inspection.

Vessel AB discovered multiple broken welds (varying in length from 6" to 14") in way of the port and stbd anchor pockets as a result of ice damage. Make permanent repairs (drill-stop and V+W both-sides) and notify USCG when complete.

Several Ballast Tank hatch covers, 1 thru 10 Port & Stbd sides on Spar deck, inner ring in way of hatch cover gaskets were wasted. Where wasted, renew inner rings. Master shall submit a statement to attending CG MI when all wasted inner rings have been renewed.

Port Main Diesel Engine recently overhauled, unable to verify the over speed trip. Upon completing break-in period, Chief Engineer to test the engine over speed trip per Mfg. Colt-Piestick test procedure, PC2V Engine Control System for Over Speed Governor & Trip dated 2/75, and provide statement to CG MI reflecting the results.

While witnessing over speed alarm/shutdown on MDE, CE was unable to produce the Coast Guard approved procedure. None of the MDE automation procedures were available. Approved procedures must be provided and tests witnessed in accordance with these procedures. If updates are required, an additional 835 will be issued.

Due to MDE overhaul, break in period required prior to testing overspeed. Once break in period is satisfied perform overspeed in presence of class surveyor and/or MI.

For C32 SSG replacement, demonstrate periodic safety test procedures after approval from MSC.

Crop & renew damaged hull plating in the Forepeak, #1, 2, 3, & 4 Port Ballast Tanks, per 2 July 2016 damage survey.

Dry dock and make vessel available for full damage survey assessment of grounded area at nearest capable facility.

Make proper repairs to port anchor brake system to the satisfaction of an attending CG MI and ABS Surveyor.
