Vessel Master made notification to MSU Chicago extension of crew shortage due to the consent based discharge of an Able Seaman at Sault Sainte Marie, MI. A replacement has been found but is awaiting medical clearance to join the vessel. Master shall ensure 3-watch system is maintained, work/rest requirements are met, and all maintenance is performed in accordance with SMS. Master attests to be sufficiently manned and that work/rest requirements will be met for the voyage between Gary, IN and Two Harbors, MI. Prior to departing Two Harbors, MI a replacement with the same or greater credential shall be found for the vacancy. If no replacement is found, the master/operator must provide a new request to the cognizant OCMI with a new Temporary Manning Requests Checklist (D9-WI-DOM-009(01)-F(01)).

Non metallic fleixble hose must be in accordance with SAE J1942. The type of flexible fuel hoses used on E-GEN and other machinery must have fire sleeve IAW SAE J1942 (incorporated by reference).

Where fire extinguishing systems or equipment are installed but not required, the systems or equipment must meet the requirements of this part. The SCBAs malfunctioned during the fire drill. SCBAs must be replaced or inspected by certified 3rd party prior to departure.

Vessel Master notified Sector Eastern Great Lakes OCMI of a non-consensual crew shortage of the 1st assistant engineer. Master shall ensure a 3-watch system is maintained and work/rest requirements are met. Master attests to be sufficiently manned, that a 3-watch system will be maintained, and that work/rest requirements will be met. Prior to 8/01/2025 departure from Conneaut, OH with a tentative port schedule of Superior, WI a replacement with the same or greater credential of a 1st assistant engineer shall be found for the vacancy. If no replacement is found prior to departure from Conneaut, the master must provide notification to the cognizant OCMI and resubmit the District 09, Temporary Manning Request Checklist: D9-WI-DOM-009(01)-F(01). 46 CFR 15.515(a)

All piping, valves, and fittings shall meet the applicable requirements of subchapter "F" (Marine Engineering) of this chapter. The fire main piping on starboard side forward weather deck has a temporary flexible repair joint installed. Vessel shall have origional expansion joint repaired and installed removing the current temporary repair. The permanent repaired installation of rebuilt expansion joints shall be hydrostatic tested at 1 1/2 times the designed max allowable working pressure of the vessel fire main piping system prior to departure from next port in Superior, WI / Duluth, MN area. Testing shall be witnessed by the local OCMI. 46 CFR 95.10-15(a) 46 CFR 56.97-40(a)(7)

Each safety valve for a boiler must be tested at the interval specified by table 1 to § 61.05-10. The Safety Valve on the inboard boiler is not functioning properly. Inboard boiler is not to be used until USCG witnesses a successful test of the safety valve. 46CFR61.05-20

All rotating machinery capable of injuring personnel must be provided with adequate covers or guards. Guards on the front of the main engines have been removed for ease of adjusting the stretch of the new belts. 46CFR58.01-20

Exhaust systems must be suitably insulated or cooled, by means of lagging. The STB Inboard M/E lagging on the exhaust system is removed for ease of torquing down the new bolts. 46CFR58.10-15(c)(2)

The inspection shall be such as to insure that the vessel, as regards lifesaving appliances is in satisfactory condition and fit for the service for which it is intended. Unable to lower the lifeboat, or conduct a MOB drill on the vessel due to ice in the water. 46CFR91.25-10 46CFR97.15-35 46CFR199.45(b)(2)

All piping, valves, and fittings shall meet the applicable requirements of subchapter F. The Fire main on deck, both port and STB side had a leak from an installed expansion joint. 46 CFR 95.10-15(a)

Firehose must be connected to the outlets at all times. Fire houses throughout the house and engine room were not hooked up. 46 CFR 95.10-10(h)

Each section of firehose must be lined commercial firehose that conforms to Underwriters' Laboratories, Inc. Standard 19 or Federal Specification ZZ-H-451E. Two fire hoses were leaking and must be replaced. 46 CFR 95.10-10(n)(3)

Each vessel must carry a lifejacket for each person on board. Vessel's COI requires 44 lifejackets, and the vessel only had 26 ready. 46 CFR 96.06-1 46 CFR 199.70(b)

Each cargo vessel must carry an immersion suit approved under approval series 160.171 of an appropriate size for each person on board. Vessel's COI requires 44 immersion suits, and the vessel only had 34 ready. 46 CFR 96.06-1 46 CFR 199.273(a)

lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. The lights attached to the life rings listed battery type to be used as Lithium, and that Alkaline batteries must not be used. All batteries found in the lights were Alkaline batteries. 46 CFR 199.70(a)(3)(ii)

Lifeboat in Great Lakes Service must carry at a minimum 6 hand flares, and 4 rocket parachute flares. Vessel only had 6 had flares in their lifeboat. 46CFR199.175 46CFR199.620(j)

A Survey, either general or partial, according to the circumstances shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of the lifesaving appliances, firefighting or other equipment, or whenever any important repairs of renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this sub-chapter. The vessel self reported that both of the STBD main engines de-clutched from the reduction gear for an unknown reason resulting in a reduction of propulsion. Vessel may be required tug escort when transiting through restricted or congested waterways/ports and is to provide class report attesting to repair that was completed by ships crew.

All engine installations shall be suitable for the intended service, and designed and constructed in conformance with subchapter F. Vessel Master made notification of starboard inboard engine clutch de-clutching and having issues. Vessel allowed to proceed directly to next port of call in Conneaut, OH and shall rectify prior to departure from Conneaut, OH. 46 CFR 58.10-5(a) 46 CFR 58.10-10(a)

In addition to the emergency power source required under part 112 of this chapter, each self-propelled vessel must have at least two electric generating sources. Vessels reported the Port generator malfunctioned and is not currently operational. 46CFR90.25-1 46CFR111.10-3 17a, c

All fire main piping, valves, and fittings shall meet the applicable requirements of Subchapter F (Marine Engineering). While conducting a pressurized test of the fire main, several major leaks were identified throughout the system: on the main deck port side at the expansion joint and port side unloading tunnel.

Each cargo vessel must carry an immersion suit approved under approval series 160.171 of an appropriate size for each person on board and enough immersion suits stowed at the watch stations, work stations, or work sites to equal the number of persons normally on watch in, or assigned to, those locations at any time. Found immersion suits throughout the vessel to be wasted and out of service; condemned 11 immersion suits.

Each cargo vessel must carry the number of lifebuoys associated with the prescribed table. 02 lifebuoys were wasted and condemned.

Each vessel shall have an installed fire main, hoses, and hose accessories that shall be capable of delivering an adequate supply of water to all portions of the vessel. During pressure test of the fire hoses, 02 fire hoses ruptured and were condemned.

All fire main piping, valves, and fittings shall meet the applicable requirements of Subchapter F (Marine Engineering). While conducting a pressurized test of the fire main, several minor leaks were identified throughout the system: on the main deck in various locations at hydrant valves and expansion joints, and in the accommodation space at the hydrants. List of valves the need to be repaired or replaced: Location: Fire Station Engine Room: 21, 22 Accommodations: Galley STBD, Captain's Office STBD, Pilot [name withheld] Port: Valve IVO of CH #24, Expansion joint IVO of CH #24 aft Main Deck Stbd: Expansion joint IVO of CH #5, Valve IVO of CH #6 & 7, Valve IVO of CH #9 & 10, valve IVO of CH #24, and Valve IVO of CH #30.

There are a significant amount of improper or damaged cable penetrations in class A structural fire boundaries. Areas include stair towers, machinery space to accommodation boundaries, decks, overheads, etc. Cable penetrations in class A boundaries shall be of an approved type and meet class A standard. Many of the multi-cable transits have had the blocks drilled out and improperly modified. This vessel is to be fully survey for these failures. ALL cable penetrations are to be corrected with approved material IAW their manufactures instructions. Penetration seals must meet testing in FTP Code under approval series 164.138, or ASTM E-814. To be completed no later than 18MAY2024.

The stern thruster was found to have significant hydraulic leakage coming from the shaft seal. This creates numerous hazards and indicates that the system is not operating as designed. The cause and proper repair are to be identified and then corrected. The stern thruster may not be used until repairs are made. To be completed no later than 18MAY2024.

Each lifejacket must have a lifejacket light securely attached to the front shoulder area of the lifejacket. Numerous lights were found damaged and expired batteries. Renew.

Each immersion suit must have a lifejacket light securely attached to the front shoulder area of the immersion suit. Numerous lights were found with broken lights or wasted batteries. Renew.

Retroreflective material must be capable of being attached to lifesaving equipment either by mechanical fasteners or by an adhesive. Numerous lifebuoys throughout the vessel were found with wasted, torn, or missing retroreflective tape.

A sufficient number of lifejackets must be carried for persons on watch and for use at remotely located survival craft stations. Storage for the forward lifejacket and immersion suit was used as a waste receptacle.

The engine room exhaust fan stbd 15 amp breaker's toggle is broken off. Replace the toggle in kind and demonstrate proper operations.

Alterations and modifications, such as re-engining, re-powering, or replacing extensive amounts of cabling, must comply with the regulations in this subchapter. Forward anchoring space has modifications made to distribution panel with no preventions made to prevent hazards to personnel.

All electrical equipment must be designed and installed to operate for the particular location and environment in which it is to be used. Deck lighting was found with broken covers, missing lights, and broken stanchions. Make permanent repairs to the deck lighting system.

Each lifeboat must be plainly marked with Type II retro-reflective, and placed on the boat to meet the arrangement requirements in IMO Resolution A.658(16). Retroreflective tape on lifeboat was wasted. Renew.

Each lifejacket and immersion suit container must be marked in block capital letters and numbers with the quantity, identity, and size of the equipment stowed inside the container. Containers were not marked with quantity.

Detectors and other notifying devices must be of approved types. Smoke detectors were not equipped with approved batteries per the manufacturer's instructions.

Closures for hull openings at or below the freeboard deck must be as strong as the structure to which they are attached and must be watertight. Gaskets on closure devices in the forward anchoring compartment were found wasted, soiled, and missing.

Manholes and flush scuttles within any superstructure other than an enclosed superstructure must have permanently attached covers. Forward entrance to the anchoring space is missing the gasket on the dead cover.

In the engine room near the changing room on the port side, the rotating general alarm beacon is not functionly correctly. Replace the beacon with a properly functioning rotating or flashing red light.

All electrical equipment must be designed and installed to operate for the particular location and environment in which it is to be used. Exposed wiring was found on the stbd side of main deck IVO cargo hold #21.

The life boat is due for weight testing. To include light weight, full weight, and a 1.1x B weight dynamic test of the davits and release mechanisms. At the conclusion of the testing, the life boat is to be lowered and operated by the crew to demonstrate a man overboard drill.

Each lifesaving appliance, including lifeboat equipment, must be inspected monthly using the checklists to make sure the appliance and the equipment are complete and in good working order. A report of the inspection, including a statement as to the condition of the equipment, must be recorded in the vessel's official logbook. As evidence by the above deficiencies, owner/operator shall ensure the implementation of monthly inspection program with associated records.

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required by the regulations in this subchapter is provided, maintained, and replaced. As evidence by the above deficiencies, owner/operator shall ensure the implementation of an inspection program with associated records.

Vessel reported a partial loss of propulsion to both port side main engines. Both starboard main engines remain fully operational. The port main engines would not go above "idle" level RPMs causing their inability to provide additional propulsion. It is suspected/reported that the selector valve (pneumatic) for CPP pitch to shaft RPM integrator failed allowing CPP to pitch as commanded but RPMs not to increase. Air leakage from the valve is suspected. Correct prior to departure from Indiana Harbor, IN.

Inboard auxiliary boiler #76-81 failed hydrostatic test as indicated by a leak in the lower inboard tube at the aft end of the boiler. Vessel to complete permanent repair (tube-replacement) and demonstrate satisfactory post-repair hydrostatic test. Inboard boiler to remain out of service until these conditions are met and approved by the USCG.

Both auxiliary boilers are overdue for safety relief valve testing. Complete above tests in the presence of a USCG Marine Inspector prior to bringing both boilers back into normal service.

A survey, either general or partial, according to the circumstances, shall be made every time a defect is discovered which affects the efficacy of its equipment. [name withheld], was notified of a reduction in propulsion due to the loss of the turbo on the port inboard main diesel engine, resulting in the shut down of the inboard shaft generator. Vessel shall submit class report to [name withheld] prior to departing Superior, WI. Vessel authorized to move to CN Dock in Duluth, MN for completion of repairs with tug assistance.

Fire hazards are to be minimized insofar as is reasonable. The turbo chargers on the STBD Outboard MDE is missing the insulation. Must install new, fitted insulation within 90 days.

All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of sub chapter F (Marine Engineering) of this chapter. The starboard shaft generator overheated due to a faulty cooling pump and was shut down rendering the bow and stern thrusters inoperable. Effect permanent repairs prior to departing Conneaut, OH and ensure the bow is manned during transit in case of emergency anchor deployment. Utilize tug assist while conducting mooring evolution at next port of call.

Port shaft generator (generator side) apparently has suffered catastrophic ground failure. Effect permanent repairs or replace as necessary.

Vessel reported cylinder head defect/malfunction for port outboard main diesel engine (POMDE). Initial diagnosis and repair by engineering crew unsuccessful. Vessel intends to have shoreside maintenance facility effect repairs upon arrival in [name withheld]. Make necessary repairs to POMDE and provide objective evidence to [name withheld] that the POMDE is fully functional.

Due to issues encountered during attempted hydro testing and time constraints the operator of the vessel has asked to defer all required auxiliary boiler inspections until 1NOV23. Both aux boilers are due for fireside, waterside, hydro, automation testing, safety relief valve testing, and all boiler mounts (first off valves) must have their bonnets removed/opened for valve inspection. A fireside inspection was conducted during this lay-up period but credit was not given. The boilers may not be placed into service or operated in ANY way until all required inspections are completed for credit to the satisfaction of an attending CG Marine Inspector. The required inspections must also be completed NO LATER THAN 1NOV23.

Due to issues encountered during attempted hydro testing and time constraints the operator of the vessel has asked to defer all required auxiliary boiler inspections until 1NOV23. Both aux boilers are due for fireside, waterside, hydro, automation testing, safety relief valve testing, and all boiler mounts (first off valves) must have their bonnets removed/opened for valve inspection. A fireside inspection was conducted during this lay-up period but credit was not given. The boilers may not be placed into service or operated in ANY way until all required inspections are completed for credit to the satisfaction of an attending CG Marine Inspector. The required inspections must also be completed NO LATER THAN 01NOV23. Failure to complete the required inspections by the due date or placing the boilers into service or using them in any way prior to the required inspections will subject the operator to enforcement action. 46 CFR 61.05

The EPIRB must be stowed in its float-free bracket with controls set for automatic activation. Proper servicing of hydrostatic release is required. Replace 2 EPIRB hydrostatic releases as they are expired. 46 CFR 199.510 46 CFR 199.45(b)

Due to the mooring arrangement at the time of annual inspection the rescue boat could not be lowered, released or get underway. The crew is to conduct a full MOB drill and take video of the lowering, releasing and running of the rescue boat within 30 days. 46 CFR 199.160

Each vessel must have suitable pilot boarding equipment available on each side of the vessel. Repair the bottom step of the port side pilot's ladder. 46 CFR 96.40(b)

A survey shall be conducted every time a defect is discovered affecting the safety of the vessel and whenever important repairs are made. A raw water belt drive sheave bracket was found broken and leaking which caused the crew to shutdown the vessel's outboard STBD ENG. Complete troubleshooting and make permanent repairs. Provide either Class or technician's report attesting to satisfactory repairs and outboard STBD ENG operation.

Discovered a corroded pit in the upper surface of a boiler tube in the outboard auxiliary boiler, uppermost tube bank on left side (located in the upper waterside chamber approx 12" in from the access cover). Pit is approx 1/4" in diameter and approx 1/32" deep. Replace tube prior to 2022 op-season.

Noted cracked/loose refractory throughout the outboard auxiliary boiler (end doors, combustion chamber). Repair refractory prior to 2022 op-season.

There is a 3-4" long through-crack in the protective drip plate above the outboard auxiliary boiler control unit. Repair or replace drip plate prior to the 2022 op-season.

Vessel lost port SSDG (vessel has only 2 SSDGs) due to unknown mechanical failure. Vessel is required to have 2 generating sources for primary service electrical loads. Repair port SSDG and demonstrate full functionaliy via test IAW the approved PSTPs. Repairs/tests must be completed prior to departure from port of Duluth/Superior.

Attending CG Marine Inspector was unable to witness and verify a light load weight test on the vessel life boat (LB). Crew to conduct a light load weight test of the LB in the presence of a CG Marine Inspector iaw 46 CFR 199.45

Attending CG marine Inspector was unable to observe crew life boat drill due to vessel moored against another vessel. In the presence of a CG Marine Inspector crew required to lower, release, operate and retrieve the vessels LB iaw 46 CFR 199.40

Small fracture with water observed leaking in #2 Cargo hold stbd side fwd slope plate at frame 21 in vicinity of unloading tunnel/ballast tank/slope plate intersection. Temporary weld repairs made in slope plating. Temporary weld repair shall be cut and crop out, with fractured area of slope plating renewed. Permanent repairs shall be to the satisfaction of attending ABS Class Surveyor. Company shall notified attending CG Marine Inspector when permanent repairs are complete and accepted as required by 46 CFR 92.01-10

Small fracture with water observed leaking into the stbd unloading tunnel, at frame 21 in way of ballast tank longitudinal bulkhead at slope plating. Existing weld excavated, and re-welded. Attended CG MI believes repairs are temporary, and affected area steel plate needs to be cut and crop out renewed. Company required to make permanents repairs to the satisfaction of attending ABS Class Surveyor. Company shall notified attending CG Marine Inspector when permanent repairs are complete and accepted as required by 46 CFR 92.01-10

Emergency Diesel Generator (EDG) was unable to start and supply electrical power upon loss of the normal power. Make Repairs and demonstrate proper operation of the emergency generator in the presence of a CG Marine Inspector iaw 46 CFR 112.05-5 & 112.25-10

Observed excessive water leaking from the fwd stbd side main deck fireman piping expansion joint. Crew required to adjust. When corrected Master to provide a statement and photo to attending CG Marine inspector to reflecting expansion joint properly adjusted iaw 46 CFR 95.10-90

Observed several life ring buoys reflective tape was worn. Where required renew worn reflective tape. When corrected Master to provide a statement and photo to attending CG Marine inspector stating all worn reflective tape on ring buoy where noted were renewed iaw 46 CFR 199.176

Observed one electrical cable nylon stuffing tube, penetrating through an A-0 bulkhead; located above stbd side upper engine room entry door. Cable penetrations thru A boundary’s are required to be steel stuffing tubes or approved A Class boundary cable transits. Remove nylon stuffing tube and renew with a steel or approved penetration iaw 46 CFR 92.01-10 on or before 31MAR2022

Observed stbd side upper engine room entry door would not automatically close; closing mechanism removed and missing. Locate, renew, and or reattach the automatic closure mechanism iaw 46 CFR 92.07-10(d)(4).

Both Aux Boilers due for Credit Fire & water side examinations. Crew unable to make ready for examination during CG MI’s attendance. Prior to placing either boiler into service, CG Marine Inspector must attend and complete credit Fire & Water side examinations iaw 46 CFR 61.05 on or before 30NOV2021

Unable to verify the proper operation of either aux boilers controls during CG MI attendance. Prior to operating aux boiler CG MI must attend and verify all periodic safety tests (PSTP) and operation of each aux boiler iaw 46 CFR 61.35 on or before 30NOV2021

During testing of the outboard auxiliary boiler (S-4106) safety relief valve (SRV), it was discovered that the boiler's pressure could not be pressed high enough to pop the SRV due to an inoperable fuel solenoid (1 of 4). New solenoid has been ordered and will be replaced upon reciept. Re-test SRV upon replacement of solenoid and subsequent restoration of normal system pressure. Test to be witnessed by a USCG Marine Inspector.

Vessel has a small weep hole with water entering the engine room, coming from the hull side shell on the port side just forward of the sea chest. Effect repairs to the satisfaction of the Coast Guard Marine Inspector once the vessel arrives in Silver Bay, MN.

Did not conduct drills to maintain social distancing practice (COVID-19) during inspection. Fire and abandon ship drill to be conducted in presence of Coast Guard at a future date.

Make repairs to the Port Outboard Main Engine Turbo afer suffering damage from a fire. Submit technician report to marine inspector.

Vessel experienced steering motor failure on stbd inboard motor. Ensure proper operation of steering gear, provide a description of what caused the loss of steering, how it was corrected, and conduct follow up steering testing to the satisfaction of the attending marine inspector.

While witnessing an abandon ship drill and launching of the open lifeboat, MI's witnessed unsatisfactory performance by crew, including lack of familiarity of equipment and disorganization. Vessel shall demonstrate a complete abandon ship drill to the satisfaction of the attending marine inspector; to include mustering crew, operating davit, launching and maneuvering lifeboat no later than 19 July 2019.

Vessel was unable to get the Lifeboat underway during the annual COI inspection due to the location at the dock. Vessel is required to lower the Lifeboat into the water and have the crew maneuver the lifeboat in the water.

Vessel has cut/exposed/dead-end wires and cables on various places throughout the vessel (i.e. stbd-side of steering stand, chart table radio, all throughout crawl-space under the pilot-house, under stbd-side aft bridge-wing grating outside, etc.). All such wires and cables shall either be properly capped and enclosed or removed.

Vessel's lifeboat must be launched and operated by the crew / maneuvered in the water and recovered at least once every three months during abandon ship drill; at the time of inspection, the lifeboat was dock-side to. Vessel to get the lifeboat launched, maneuvered in the water with oars and recovered to the satisfaction of an attended USCG Marine Inspector (per D9 direction).

Vessel's auxiliary boilers were open for inspection and not able to be operated to test their safety relief valves. Proper operation of both auxiliary boilers safety relief valves shall be witnessed to the satisfaction of an attending USCG Marine Inspector.

Pressure indicating devices on the port-side fuel oil day tank were missing covers. Repairs shall be made to the satisfaction of an attending USCG Marine Inspector.

General alarm markings to identify the alarm system in the cargo tunnel are either missing or painted-over in multiple locations. Markings to identify the alarm system shall be installed or re-painted.

Emergency lighting in the cargo tunnel was not indicated as such in several areas. Emergency lighting in the tunnel space shall be marked or re-painted to indicate the locations of all emergency lighting.

Emergency generator fuel supply tank level indicator was not functioning properly. Indicator shall be made to function to the satisfaction of an attending USCG Marine Inspector.

Vessel's starboard ship's service diesel generator was not operational at the conclusion of the inspection, as it would not start. Prove proper operation of the generator prior to departure, to the satisfaction of the attending USCG Marine Inspector.

During post-repair hydrostatic test of inboard auxiliary boiler discovered 3 leaking fire tubes. Replace failed fire tubes and conduct satisfactory hydrostatic test in presence of a USCG Marine Inspector, prior to operationally using boiler. Repairs shall be performed by an ASME certified company holding an "R" stamp. All repairs shall be IAW Section IV of the ASME Boiler and Pressure Vessel Code.

Make weld repairs to the inboard auxiliary boiler to repair shell fracture under and in way of the burner. Repairs shall be preformed by an ASME certified company holding an "R" stamp and by and by ASME Certified welder(s). All repairs shall be IAW section IV of the ASME Boiler & Pressure Vessel Code and as modified by 46 CFR Part 53.

Make repairs to the outboard auxiliary boiler to remove and renew the two leaking fire tubes. Repairs shall be preformed by an ASME certified company holding an "R" stamp and by and by ASME Certified welder(s). All repairs shall be IAW section IV of the ASME Boiler & Pressure Vessel Code and as modified by 46 CFR Part 53.

Upon completing repairs on both aux boilers, CG Marine Inspector shall attended and verify repairs and witness hydrostatic tests on each boiler before placed into service.

In the presence of a CG Marine Inspector Lower, Releases, Operate, Retrieve and Stowage the Lifeboat.

Vessel could not conduct lifeboat weight tests due to mooring arrangements. Vessel must conduct lifeboat light/full weight test within 14 days.

Vessel was unable to demonstrate boiler automation and operation of safety relief valve on both aux boilers. Prove proper operation of boiler automation and demonstrate satisfactory operation of safety relief valves on both Aux boilers prior to putting them back into service.

While cropping high-floor connections in the #5P ballast tank a welder inadvertently cut too far into the bottom shell, resulting in a minor, but controllable, leak. The hole was filled with weld and ground smooth. A steel frame of approximately 17x29x10 was constructed around the temp repair. The frame will be filled with concrete and capped with a steel plate.
